메타분석을 통한 주·객관적 인지-언어 평가 간 상관성 연구 : 정상 노년층, MCI, 치매 환자를 중심으로 (Meta-Analysis of Correlation Between Subjective and Objective Cognitive-Linguistic Tests : Focused on Normal Aging, MCI, and Dementia)

  • 인지-언어 능력에 대한 노인들의 주관적 호소는 경도인지장애나 치매로의 진전을 예측하는 중요 요인이다. 그러나, 인지-언어 능력의 주 객관적 평가 간 상관성은 연구마다 상이하다. 특히, 양자 간 상관성을 체계적으로 살펴본 국내 연구는 매우 드물다. 본 연구에서는 2000년 이후 게재된 국내외 문헌 중 총 26개 논문을 대상으로 메타분석을 실시함으로써 정상 노년층, MCI, 치매 집단의 자기 보고형 및 정보제공자 보고형 평가와 객관적 평가 간 상관성을 알아보고자 하였다. 질적 분석 결과, 분석에 포함된 연구 대상자의 수는 26~657명이었다. 주관적 평가의 유형은 자기 보고형 75.4%, 정보제공자 보고형 24.6%였으며, 객관적 평가의 하위 영역은 기억력, 전반적 인지능력, 언어능력 등의 순으로 많았다. 메타분석 결과, 자기 보고형 평가와 객관적 평가 간 상관성은 치매 집단이 가장 높았고, 정보제공자 보고형은 세 집단 모두에서 효과적이었다. 객관적 평가의 하위 영역별로는, 정상 노년층에서 자기 보고형과 추론력, 정보제공자 보고형과 기억력 및 언어능력 간에 상관성이 높았다. MCI에서는 자기 보고형과 언어능력 등, 정보제공자 보고형과 전반적 인지능력 등의 영역 간에 높은 상관성이 있었고, 치매 집단에서는 두 유형 모두 기억력, 언어능력, 전반적 인지능력을 잘 반영하였다. 본 연구는 세 집단의 인지-언어 능력에 대한 두 유형의 주관적 평가와 객관적 평가 간 상관성을 체계적으로 분석함으로써 증거 기반적 자료를 제공할 수 있다.

Emerging evidence that ginseng components improve cognition in subjective memory impairment, mild cognitive impairment, and early Alzheimer's disease dementia

  • Ginseng is a traditional herbal medicine used for prevention and treatment of various diseases as a tonic. Recent scientific cohort studies on life prolongation with ginseng consumption support this record, as those who consumed ginseng for more than 5 years had reduced mortality and cognitive decline compared to those who did not. Clinical studies have also shown that acute or long-term intake of ginseng total extract improves acute working memory performance or cognitive function in healthy individuals and those with subjective memory impairment (SMI), mild cognitive impairment (MCI), or early Alzheimer's disease (AD) dementia who are taking AD medication(s). Ginseng contains various components ranging from classical ginsenosides and polysaccharides to more recently described gintonin. However, it is unclear which ginseng component(s) might be the main candidate that contribute to memory or cognitive improvements or prevent cognitive decline in older individuals. This review describes recent clinical contributors to ginseng components in clinical tests and introduces emerging evidence that ginseng components could be novel candidates for cognitive improvement in older individuals, as ginseng components improve SMI cognition and exhibits add-on effects when coadministered with early AD dementia drugs. The mechanism behind the beneficial effects of ginseng components and how it improves cognition are presented. Additionally, this review shows how ginseng components can contribute to SMI, MCI, or early AD dementia when used as a supplementary food and/or medicine, and proposes a novel combination therapy of current AD medicines with ginseng component(s).

The Reciprocal Causal Relationship between Social Activities and Health with Reference to the Cognitive Function Level among Community-dwelling Older Adults: A Cross-Lagged Panel Analysis

  • Purpose: The aim of this study is to examine the reciprocal causal relationship between social activities and health with reference to the cognitive function level among community-dwelling older adults. Methods: We conducted a longitudinal data analysis using the $4^{th}$ (Time 1) and $5^{th}$ (Time 2) waves from the Korean Longitudinal Study of Ageing adopting cross-lagged panel analysis (CLPA). Results: A total of 3,473 community-dwelling older adults were included in the analysis: 2,053 in the normal cognitive function group, 912 in the mild cognitive impairment group, and 508 in the moderate to severe cognitive impairment group. The CLPA revealed that higher levels of social activities at Time 1 significantly influenced better subjective health perception at Time 2 in all three groups. In addition, better subjective health perception at Time 1 significantly influenced higher levels of social activities at time 2 only in the normal cognitive function group. Conclusion: This study demonstrates that participating in social activities has positive effects on health regardless of the cognitive function level in community-dwelling older adults. This finding suggests that there is a need to develop strategies that can be used to encourage older adults with cognitive decline to maintain participation in social activities.

정상인을 통해 알아본 우울 증상과 불안 증상이 특정 인지 기능에 미치는 영향 (Effects of Depression and Anxiety Symptoms on Specific Cognitive Function by Evaluating Healthy Subjects)

  • 연구목적 본 연구는 우울증의 동반증상으로 알려진 인지 기능 저하가 우울증보다 선행될 수 있다는 점에 착안하여 정상인에게서 우울 증상과 불안 증상이 인지 기능에 미치는 영향을 관찰하고자 한다. 방 법 일반인 50명을 모집하여 인지 기능 및 임상 증상에 대한 평가와 임상 증상이 인지 기능에 미치는 영향을 알아보기 위해 피어슨 상관분석과 다변량 회귀분석을 시행하였다. 우울증에서 흔히 저하되는 인지 영역을 평가하는 검사 소항목간의 상관성을 알아보기 위해 위해 소항목간 상관분석을 시행하였다. 결 과 선로잇기검사(B형)는 우울 증상(r=0.300, p=0.03)과 나이(r=0.323, p=0.02)와 상관 관계를 보였고 이에 영향을 미치는 독립변수는 우울 증상(β=0.304, p=0.03)과 나이(β=0.335, p=0.01)이며 선로잇기검사(B형)에 대해 14.8%의 설명력을 보여주었다. 주관적 인지 기능 검사는 불안 증상(r=0.434, p=0.002)과 상관 관계가 있었다. 인지 기능 소항목간 상관분석에서 주관적 인지 기능검사는 주의력검사를 제외한 나머지 항목과 상관 관계가 있었다. 결 론 본 연구에서 우울 증상은 나이, 교육기간 같은 인구학적 특성 외에 독립적으로 실행기능에 기여하는 것을 알 수 있었다. 우울증에서 인지 기능 저하가 흔한 장기적인 임상 결과임을 감안할 때 인지 기능에 대한 적극적인 조기 개입과 평가가 도움이 될 것으로 기대한다.

Relationship Between Amyloid Positivity and Sleep Characteristics in the Elderly With Subjective Cognitive Decline

  • Background and Purpose: Alzheimer's disease (AD) is a neurodegenerative disease characterized by a progressive decline in cognition and performance of daily activities. Recent studies have attempted to establish the relationship between AD and sleep. It is believed that patients with AD pathology show altered sleep characteristics years before clinical symptoms appear. This study evaluated the differences in sleep characteristics between cognitively asymptomatic patients with and without some amyloid burden. Methods: Sleep characteristics of 76 subjects aged 60 years or older who were diagnosed with subjective cognitive decline (SCD) but not mild cognitive impairment (MCI) or AD were measured using Fitbit® Alta HR, a wristwatch-shaped wearable device. Amyloid deposition was evaluated using brain amyloid plaque load (BAPL) and global standardized uptake value ratio (SUVR) from fluorine-18 florbetaben positron emission tomography. Each component of measured sleep characteristics was analyzed for statistically significant differences between the amyloid-positive group and the amyloid-negative group. Results: Of the 76 subjects included in this study, 49 (64.5%) were female. The average age of the subjects was 70.72±6.09 years when the study started. 15 subjects were classified as amyloid-positive based on BAPL. The average global SUVR was 1.598±0.263 in the amyloid-positive group and 1.187±0.100 in the amyloid-negative group. Time spent in slow-wave sleep (SWS) was significantly lower in the amyloid-positive group (39.4±13.1 minutes) than in the amyloid-negative group (49.5±13.1 minutes) (p=0.009). Conclusions: This study showed that SWS is different between the elderly SCD population with and without amyloid positivity. How SWS affects AD pathology requires further research.

SNS 이용동기와 SNS 중독이 주관적 웰빙에 미치는 영향: 사회적 유대감의 조절효과 (The Effect of the Subjective Wellbeing on the Addiction and Usage Motivation of Social Networking Services: Moderating Effect of Social Tie)

  • 특정한 관심이나 활동을 공유하는 사람들 사이의 관계망을 기반으로 서비스를 제공하는 페이스북이나 트위터와 같은 SNS의 활성화로 정보공유나 의사소통이 활발해지면서 SNS에 대한 관심이 증가하였다. 본 연구는 관심의 대상이 되고 있는 SNS에 대한 연구를 수행할 것이며, 새로운 매체의 이용 동기를 설명하기 위한 이론인 이용과 충족이론을 기반으로 SNS 이용 동기를 살펴본다. SNS 이용 동기를 감정적 동기와 인지적 동기로 구분하고, 이용 동기와 SNS 중독 간의 관계를 알아보았다. 감정적 동기는 SNS에 대한 오락성과 환상으로 구분하고 인지적 동기는 SNS에서의 정보 부담과 시스템 사용 부담으로 살펴한다. SNS 중독은 시간적 내성, 금단 불안, 중단 실패, 생활 장애로 구분하였으며 SNS 중독과 주관적 웰빙과의 관계를 살펴본다. 마지막으로 사회적 자본 이론을 기반으로 SNS 특성인 사회적 유대감의 조절효과를 분석한다. SNS 사용자들을 대상으로 설문을 수행하였고, 가설을 검증하기 위하여 286부의 설문지를 분석에 활용하였다. 가설검증결과를 보면, 감정적 동기인 오락성과 환상은 SNS 중독에 정(+)의 영향을 미쳤고, 인지적 동기인 정보 부담과 시스템 사용 부담도 SNS 중독에 정(+)의 영향을 미쳤다. 또한 SNS 중독은 주관적 웰빙에 통계적으로 유의한 영향을 미쳤으며, 마지막으로 사회적 유대감에 따라 SNS 중독이 주관적 웰빙에 다른 영향을 미치고 있음을 알 수 있었다. 본 연구의 결과는 SNS 사용자와 SNS 관련 종사자들에게 유용한 정보를 제공할 수 있을 것이다.

Electroencephalography for Early Detection of Alzheimer's Disease in Subjective Cognitive Decline

  • Background and Purpose: Early detection of subjective cognitive decline (SCD) due to Alzheimer's disease (AD) is important for clinical research and effective prevention and management. This study examined if quantitative electroencephalography (qEEG) could be used for early detection of AD in SCD. Methods: Participants with SCD from 6 dementia clinics in Korea were enrolled. 18F-florbetaben brain amyloid positron emission tomography (PET) was conducted for all the participants. qEEG was performed to measure power spectrum and source cortical activity. Results: The present study included 95 participants aged over 65 years, including 26 amyloid PET (+) and 69 amyloid PET (-). In participants with amyloid PET (+), relative power at delta band was higher in frontal (p=0.025), parietal (p=0.005), and occipital (p=0.022) areas even after adjusting for age, sex, and education. Source activities of alpha 1 band were significantly decreased in the bilateral fusiform and inferior temporal areas, whereas those of delta band were increased in the bilateral cuneus, pericalcarine, lingual, lateral occipital, precuneus, posterior cingulate, and isthmus areas. There were increased connections between bilateral precuneus areas but decreased connections between left rostral middle frontal area and bilateral frontal poles at delta band in participants with amyloid PET (+) showed. At alpha 1 band, there were decreased connections between bilateral entorhinal areas after adjusting for covariates. Conclusions: SCD participants with amyloid PET (+) showed increased delta and decreased alpha 1 activity. qEEG is a potential means for predicting amyloid pathology in SCD. Further longitudinal studies are needed to confirm these findings.

유방암 환자에서 화학요법이 인지기능에 미치는 영향에 대한 메타분석 (A Meta-analysis of Chemotherapy related Cognitive Impairment in Patients with Breast Cancer)

  • Purpose: The purpose of this study was to evaluate the cognitive effects of chemotherapy in patients with breast cancer. Methods: Using several databases, prospective studies were collected up to August 2011. Of 2,106 publications identified, 12 met the inclusion criteria, and 8 studies were used to estimate the effect size of chemotherapy on cognitive impairment. Results: Twelve studies were done since 2005 and most of the research was performed in Europe or North America. Eight studies were used to generate effect size across the cognitive domains of attention/concentration, verbal and visual memory, executive function, visuospatial skill, language, and subjective cognitive function. Each of the cognitive domains showed small effect sizes (-0.02 ~ -0.26), indicating diminished cognitive function for the chemotherapy group compared with non-chemotherapy groups. Conclusion: Finding suggests that breast cancer patients who undergo chemotherapy may experience mild cognitive decline. Further study is needed to generate knowledge and guideline for interventions to address chemotherapy related cognitive impairment in these patients.

Facial Emotion Recognition in Older Adults With Cognitive Complaints

  • Background and Purpose: Facial emotion recognition deficits impact the daily life, particularly of Alzheimer's disease patients. We aimed to assess these deficits in the following three groups: subjective cognitive decline (SCD), mild cognitive impairment (MCI), and mild Alzheimer's dementia (AD). Additionally, we explored the associations between facial emotion recognition and cognitive performance. Methods: We used the Korean version of the Florida Facial Affect Battery (K-FAB) in 72 SCD, 76 MCI, and 76 mild AD subjects. The comparison was conducted using the analysis of covariance (ANCOVA), with adjustments being made for age and sex. The Mini-Mental State Examination (MMSE) was utilized to gauge the overall cognitive status, while the Seoul Neuropsychological Screening Battery (SNSB) was employed to evaluate the performance in the following five cognitive domains: attention, language, visuospatial abilities, memory, and frontal executive functions. Results: The ANCOVA results showed significant differences in K-FAB subtests 3, 4, and 5 (p=0.001, p=0.003, and p=0.004, respectively), especially for anger and fearful emotions. Recognition of 'anger' in the FAB subtest 5 declined from SCD to MCI to mild AD. Correlations were observed with age and education, and after controlling for these factors, MMSE and frontal executive function were associated with FAB tests, particularly in the FAB subtest 5 (r=0.507, p<0.001 and r=-0.288, p=0.026, respectively). Conclusions: Emotion recognition deficits worsened from SCD to MCI to mild AD, especially for negative emotions. Complex tasks, such as matching, selection, and naming, showed greater deficits, with a connection to cognitive impairment, especially frontal executive dysfunction.

Neuro-cognitive Ramifications of Fasting and Feeding in Obese and Non-obese Cases

  • Preliminary studies have claimed that short term fasting would negatively affect school performance and cognition. In contrast some other studies have reported not important decline in cognition and executive function as a result of fasting. Also limited attention was generally devoted to dietetic regimens, nutritional status and body weight. Yet neuroscience and neuro-cognitive aspects of acute hunger on the electroencephalogram and differences between obese and non-obese cases is not well understood. Hence, we decided to design and perform a case study in a more controlled situation similar to reality. Therefore, we performed several examinations including subjective tests (for eating status) and objective tests (cognitive tests such as Stroop effect and Sternberg search and electroencephalogram measures such as steady-state visual evoked potential and auditory steady-state responses) for an obese and a non-obese academic case before and after a simple breakfast. The results showed that the breakfast effects on the neuro-cognitive functions depend on either obesity status, nutritional status of the case or the type of cognitive task (visual or auditory). This paper would open a new insight to answer some important questions about the neuro-cognitive implications of fasting and feeding in obese and non-obese human cases.
