Radiological Evaluation of CT Contrast Medium Extravasation (CT 조영제 혈관외유출의 방사선학적 고찰)

  • Extravasation of contrast material is a not infrequent complication of enhanced imaging studies and large volume extravasation may result in severe damage. Subcutaneous extravasation of the radiographic contrast medium is one of the complications of the contrast medium-enhanced procedures. Automated power injectors enable the contrast material to be delivered at a uniform high-flow-rate and as a nonfragmented bolus, and this is essential for many contrast material enhanced CT(computed tomography) applications. The major risk associated with the use of automated power injectors is the well known complication of contrast material extravasation at the injection site. Automated injection of CT contrast material can produce the compartment syndrome. Selection of the nonionic contrast material after careful evaluation of the intravenous administration site and monitoring of the patient during the use of a mechanical power injector may help minimize or prevent extravasation injuries. Early identification is important and conservative management is effective in most cases. Prevention of these injuries with the education of radiological technologist remains the ultimate aim.

Influence of Joint Mobilization on Formalin Induced Pain Prevention of Rats (관절가동술이 흰쥐의 포르말린 유도 통증 예방에 미치는 영향)

  • Purpose: The purpose of this study was to determine the preventive effect of joint mobilization on biphasic pain response induced formalin test. Methods: Sprague-dawley rats(n=30) were ramdomly divided into the control group without intervention, sham control group with application of hand contact without mobilization, joint mobilization group with application of hand contact with mobilization. Joint mobilization of knee procedure involved an grade III extension mobilization basically with anterior-posterior gliding of the tibia on the femur. Formalin injection caused biphasic pain response which is lated for 60 minute. The first phase result from primary afferent sensory fiber, wheareas the second phase has been proposed to central sensitization in the central nervous system. Behavioral analysis was performed by digital camera after 5% formalin subcutaneous injection into the dorsal foot. Results: Pain response of joint mobilization group show significant lower than control gorup and sham control group. Conclusion: This result suggest that pre-application of joint mobilization may be effective intervention to prevent the formalin induced pain.

Quantitation of abamectin by HPTLC and its pharmacokinetics after intramuscular injection in pigs (돼지에서 근육주사한 Abamectin에 대한 HPTLC 분석 및 약물동태학)

  • We established a new method to analyze abamectin using HPTLC (high performance thin layer chromatography) in order to obtain its pharmacokinetic profiles in pigs. Recovery of abamectin in pig serum after fluorescence derivatization was $80.01{\pm}3.82%$ at 0.1ppm and $83.67{\pm}3.63%$ at 10ppm, respectively. Detection reproducibility in terms of coefficient variation (c.v.) was 3.09% and 2.74% (intra-day), and 3.71% and 51.7%(inter-day), for 0.1 and 10ppm, respectively. Pharmacokinetics of abamectin was studied in five Yorkshire-Landrace mixed bred male pigs ($35.0{\pm}2.7kg$) administered intramuscularly 0.3mg/kg b.w. Pharmacokinetic profiles of abamectin in pigs were described by the 1-compartment open model with first-order absorption and first-order elimination. AUC (area under the curve) was $262.65{\pm}16.44ng{\cdot}day/ml$ and the biological elimination half-life ($t_{1/2},\;k_e$) was $5.28{\pm}0.84$ days, indicating somewhat high bioavailability and long half-life by the intramuscular route. We suggest intramucular injection of abamectin could be also used in place of the recommended route of its subcutaneous administration so far.

  • Bee Venom acupuncture has anti-inflammatory and analgesic effect that facilitates natural healing. So it is used in treatment of several musculoskeletal diseases. Recently, we experienced a case of De Quervain's disease that was not responded to steroid injection. It was successfully treated with intra-synovial Bee Venom acupuncture for two weeks. It is thought that Bee Venom acupuncture can be used in tenosynovitis. And Bee Venom injection techniques like intra-joint and subcutaneous method are need to be developed.

Analysis of Medication Errors of Nurses by Patient Safety Accident Reports (환자안전사고 보고서를 통한 간호사 투약오류 분석)

  • Purpose: The purpose of this study was to identify and analyze the characteristics of nurses' medication errors during three years. Methods: Retrospective survey study design was used to analyze medication errors by nurses among patient safety accidents. Data were collected for three years from January, 2017 to December, 2019. Data were analyzed using frequency, percentage, 𝑥2-test, and logistic regression with SPSS 26.0 program. Results: Of a total 677 medication errors, 40.6% were caused by nurses. Among the medication errors, near miss (n=154, 56.0%), intravenous bolus injection (n=170, 61.8%), wrong dose (n=102, 37.1%) and carelessness for repetitive work (n=98, 35.6%) were the most common. Medication errors differed by department, and nurses' career, and patient safety accident type. The results of the logistic regression analysis showed that the risk factors of adverse events were medication of fluids (OR=3.93, 95% CI: 1.26~12.27), insulin subcutaneous injection (OR=39.06, 95% CI: 4.58~333.18), and occurrence of extravasation/infiltration (OR=7.26, 95% CI: 1.85~28.53). Conclusion: The simplest and most effective way to prevent medication errors is to keep 5 right, and a differentiated education program according to department and nurse career is needed rather than general education programs. Hospital-level integrated interventions such as a medication barcode system or a team nursing method are also necessary.

Influence of Acupuncture Stimulation on Electrical Resistance under Adiaphoresis Induced by Regional Anaesthesia (국소마취에 의한 발한 억제 상태에서 침자극이 인체 전기 저항에 미치는 영향)

  • Objectives : When sweating factor is excluded, to know the influence of acupuncture stimulation on electrical resistance. Methods : Regional anaesthesia(Lidocane 2%, 1ml, Subcutaneous injection) is applied to measured regions, 4cm round area including left Neiguan P6. So we can make adiaphoresis. Before and after acupucnture stimulation to Laogong P8, we measured the electrical resistance of anesthetized left Neiguan P6, anesthetized non-acupoint, normal left Ximen P4, and normal right Neiguan P6. For measurment, we use the 1.2V, DC system that has 100$k{\Omega}$ internal resistance Results : Under the adiaphoresis, Acupuncture stimulation makes the significant decrease of electrical resistance. In normal regions, so does it. Conclusions : When the acupuncture stimulation is applied to human boby, the decrease of electrical resistance is due not only to sweat secretion but also to other internal resistance.

Ear mite infestation in a lop-eared rabbit (Oryctolagus cuniculus) and successful treatment with ivermectin

  • A 2-year-old female lop-eared rabbit (Oryctolagus cuniculus) was presented to the veterinary clinic at a zoo with pruritus, alopecia, and crusting of the ear. Examination of skin scrapes revealed an infestation with the rabbit ear mite, Psoroptes cuniculi. Weekly subcutaneous ivermectin injection over a three-week period resulted in remission of the clinical signs and improvement of the overall conditions of the rabbit.

The Effects of Mercury and Cadmium Administered in Subcutaneous Tissue on Enzymatic Activity and lipidperoxidation (피하조직에 투여된 수은과 카드뮴의 효소활성과 과산화지질에 미치는 영향)

  • Heavy metals like Mercury and Cadmium cause various kinds of toxicities in the organs of Liver and Kidney. To observe the results of toxicity in the liver, kidney, and serum when the rats were injected subcutaneously with HgCl$_2$ and CdC1$_2$ and sacrificed after 24 hours and 72 hours from the last injection, we measured variation of lipidperoxide values in rat liver homogenate, variation of aspartate aminotransferase and alanine aminotransferase in rat serum. Variation of lipidperoxide values in rat kidney homogenate and variation of BUN in rat serum. It was found that Mercury and Cadmium administered subcutaneously to the skin in the air could cause the damages of liver and kidney.

A Case Report of Progressive Hemifacial Atrophy (진행성 반안면위축환자의 치험례)

  • A progressive hemifacial atrophy is characterized by progressive atrophy of subcutaneous fat and rarely muscle and bone. Its contour follows the underlying muscle. Unilateral involvement is common. The treatment goal has been focused on the augmentation of the soft tissue. Many materials such as implants, collagen, fat graft, fat injection, dermal fat graft, filler and vascualized autogenous graft have been used. Although these materials have been used, the best treatment hasn't been achived. In severe cases underlying soft tissue, muscle and bone may be atrophied and massive soft tissue graft, implant and orthognathic surgery must be used. The author used the dermal-fat tissue for the pupose of soft tissue augmentation. We can get the massive soft tissue by the dermolipectomy procedure through the mini-abdominoplsty. The facial augmentation was done by augmentation of the dermal-fat tissue. The progressive hemifacial atrophy is hard to treat by only one procedure and many modalites must be considered.

Influence of sodium selenite on oxygen free radical in mercuric chloride induced renal failure (Mercuric chloride 유발 신부전에서 산소유리기에 미치는 Sodium selenite의 영향)

  • Oxygen free radical have recently been found to mediate cell injury after ischemia in the kidney. The purpose of our study was to determine whether selenium had an effect on damge mediated by oxygen free radical in inorganic mercury induced renal failure, toxic model of renal failure. Toxic renal failure model was produced by subcutaneous injection of mercuric chloride (4mg/kg) once a day for 7 consecutive days. In additionally, coadministration of sodium selenite (1mg/kg) was performed by the same condition. As a consequence of this study, we were able to detect partially unequivocal role of selenium as below dipicted. The combination of sodium selenite showed that markedly inhibited production of superoxide radical in mercuric chloride alone. On the other hand, combined sodium selenite was unable to enhance against significantly lowered superoxide dismutase activity after mercuric chloride insult. However, simultaneous administration of sodium selenite was inclined to induce mitochondrial superoxide dismutase and catalase.
