A Shih Tzu Puppy had clinical onset of anotexia crying and progression of neurological sings when enlargement of the cranial vault at 1 month old and died after showing clinical signs during 2 months period. Radiological and pathological examinations were performed. Radiological findings were homogeneous appearance of the calvaria with cortical thinning, loss of the normal convolutional skull markings and persistent fontanelles. Grossly enlargement of the cranial vault thinning of the bone and defective closure of the fontanelles were also observed. The entire subcortical area of the cerebral hemispheres with severe, dilatation of ventricles and cerebrospinal fluid(CSF) wits absent. There was parenchyma atrophy affecting chiefly in the white mater and the cerebral cortices, axon degeneration and necrosis and gitter cell infiltration in the whiter matter and the subependymal area. Mononuclear perivacular cuffing in the cerebrum and the pons was shown. Based on the radiological, gross and histopathological findings, this case was believed to have congenital hydrocephalus with nonsuppurative encephahitis. Possible etiology on the case is also discussed.
Cerebral vessels are functionally and structurally specialized to provide adequate blood flow to brain which shows high metabolic rates. Cerebral hemorrhage or ischemic infarction due to cerebrovascular injury or occlusion can cause the immediate brain damage, and if not treated rapidly, can lead to serious or permanent brain damages, and sometimes life-threatening. Unlike these popular cerebrovascular diseases, there are diseases caused by genetic problems. Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is one of them. CADASIL does not show the high incidence, but it is considered to be significantly affected by regional obstructiveness such as islands and therefore, to be an important genetic disease in Jeju. This paper aims to summarize the possibility of animal model research that can provide preclinical data for CADASIL disease research and to evaluate its applicability in future research plans.
The case of a 66- year-old woman with coexisting idiopathic basal ganglia calcification(IBGC) and dementia was presented. The calcification was detected in bilateral basal ganglia, dentate nucleus, and thalamus by brain imaging. Serum calcium and phosphorus levels were normal. The underlying diseases of calcification of basal ganglia such as parathyroid dysfunction and other infectious, toxic, or metabolic illness were excluded. The patient had memory impairment and frontal executive dysfunction without aphasia, agnosia, apraxia, and visuospatial impairment in neuropsychological test. It suggested that the cognitive impairment might be due to the dysfunction of frontal-subcortical circuit.
Objectives: We investigated injury of corticostriatal (CStr) tract in patient with mild traumatic brain injury (mTBI), which was demonstrated by DTT. Method: A 44-year-old female with no previous history of neurological, physical, or psychiatric illness had suffered from head trauma resulting from a pedestrian car accident. She complained that could not quickly move the left hand with her intension. After three month's administration, her slowness movement of left hand recovered rapidly to the point that she was able to extend all fingers quickly. Results: On DTT configuration, the integrity of the left CStr tract was well-preserved, however the right CStr tract showed narrowing and partial tearing in the subcortical white matter on a DTT at 25 months after onset. Conclusion: Injury of the right CStr tract was demonstrated in a patient who developed mild motor control problems following mild TBI. We believe that the evaluation of the CStr tract from the secondary motor area for patients who showed unexplained motor control problem is necessary.
Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is a single-gene disease caused by mutations in the neurogenic locus notch homolog protein 3 (NOTCH3) gene. The spectrum of clinical manifestations is broad, ranging from asymptomatic to typical ischemic stroke, and mainly depends on the location of the mutations. We describe the case of a 76-year-old female without apparent neurological deficits. However, brain magnetic resonance imaging revealed confluent lesions in the white matter. Direct sequencing of the NOTCH3 gene revealed a novel pathogenic mutation, c.811T>A, which results in a mild phenotype. Therefore, this report will expand the current knowledge in regards to the mutations that can cause CADASIL.
Alzheimer's disease is one type of dementia, the symptoms can be treated by detecting the disease at its early stages. Recently, many computer-aided diagnosis using magnetic resonance image(MRI) have shown a good results in the classification of AD. Taken these MRI images and feed to Free surfer software to extra the features. In consideration, using T1-weighted images and classifying using the convolution neural network (CNN) model are proposed. In this paper, taking the subjects from ADNI of subcortical and cortical features of 190 subjects. Consider the study to reduce the complexity of the model by using the single layer in the Res-Net, VGG, and Alex Net. Multi-class classification is used to classify four different stages, CN, EMCI, LMCI, AD. The following experiment shows for respective classification Res-Net, VGG, and Alex Net with the best accuracy with VGG at 96%, Res-Net, GoogLeNet and Alex Net at 91%, 93% and 89% respectively.
Objective : This study was tried to investigate the specific relationships among cognitve function, neurbehavioral symptoms, and daily living functions, as well as provide the guidline of more proper clinical approches for patients with subcortical cerebrovascular disease. Objects and Methods Subjects were 85 patients whose diagnosis was confirmed by brain CT or MRI and controls were 195 normal persons matched by educational level with the subjects. The cognitive functions were evaluated by BNA(Benton neuropsychiatric assessment), subjective neurobehavioral symptoms by SCL-90-R(Sympton Check List-90-Revised), objective neurobehavioral symptoms by NRS(Neurobehavioral Rating Scale), and daily living function symptoms by NRS(Neurobehavioral Rating Scale), and daily living function by GERRI(Geriatric Evaluation by Relative's Rating Instrument) and IADL(Instrumental Activities of Daily Living Scale). Results: 1) Subjects showed significantly lower cognitive functions than controls in all tests of BNA except Lt-Rt Orientation Test(p=0.09) and facial Recognition Test(p=0.186). 2) In subjective neurobehavioral symptoms, subjects showed significantly lower scores in all symptoms except anxiety(p=0.059), hostility(p=0.159), and phobic anxiety(p=0.849). But in objects neurobehavioral symptoms, subjects showed significantly higher in scores in psychoticism (p=0.000) and neuroticism(p=0.025) of NRS. 3) The score of social functioning of GERRI(p=0.000) and that of IADL(p=0.000) were significantly higher in subjects than in controls. 4) for correlation between cognitive and daily living functions, there were significant correlations between the scores of all items on BNA and the score of cognitive or social function of GERRI and the socre of MDL in corntrols, whereas in subjects, there were significant correlations only between the scores of BNA and the score of IADL. 5) for correlation between neuroehavioral symptoms and daily living functions, there were significant correlatons between the socre of subjective neurobehavioral symptoms and the scores of all subscales of GERRI and the score of MDL in controls. On the contrary, in subjects, there were significant correlations between the score of social function of GERRI and the score of objective neurobehavioral symptoms such as psychoticism, agitiation-hostility, and decrease d motivation-emotional withdrawl. Conclusion : Above results suggest that disturbances in specific function of brain may play a role as a predictor of impairments with specific daily living functions and also suggest that specific correlations among various functions may be useful as clinical parameters for setting of the treatment goal and for assessing the ongoing process in the treatment and rehavilitation of the patients with subcortical cerebrovascular disease.
목적: 본 연구는 초기의 피질하 혈관성치매와 알쯔하이머병 환자에서 Tc-99m HMPAO 단일광자방출단층촬영술을 이용하여 국소 뇌 혈류 흐름의 특징적인 양상을 평가하고 두 질환에서의 차이를 비교하고자 하였다. 대상 및 방법: 피질하 혈관성치매 환자 16명, 알쯔하이머병 46명, 12명의 대조군이 포함되었다. 피질하 혈관성치매와 알쯔하이머병은 NINCDS-ADRDA 와 NINDS-AIREN 기준에 합당하였고, 이들의 연령, 교육연수, 한국형 간이정신상태검사 점수와 치매임상척도 점수를 일치시켰다. 세 군에서 국소 뇌혈류 흐름을 측정하고자 SPM 분석을 이용한 TG-99m HMPAO 단일 광자방출단층촬영술을 실시하였다. 피질하 혈관성치매와 알쯔하이머병에서의 단일광자방출단층촬영술 결과를 정상 대조군과 비교하였고, 이어서 두 질환 간의 차이를 비교하였다. 결과: 대조군과의 SPM분석 결과 피질하 혈관성치매에서는 우측 관자엽, 시상, 좌측 뇌섬엽, 상부 관자이랑, 양측 띠이랑, 이마엽에서 의미 있는 관류저하가 있었고, 알쯔하이머병에서는 좌측 모서리위이랑, 상부 관자이랑, 중심뒤이랑, 하부 마루소엽, 우측 뒤통수관자이랑, 양측 띠이랑에서 관류저하을 보였다(uncorrected p<0.01). 피질하 혈관성치매 환자에서 알쯔하이머병 환자에 비해 우측 해마이랑, 띠이랑, 좌측 뇌섬엽, 양측 이마소이랑 부위에 의미있는 관류저하를 보였다(uncorrected p<0.01). 결론: 본 연구는 피질하 혈관성치매와 알쯔하이머병 환자에서의 관류의 특징과 차이를 보여주었고, 이러한 결과는 질병의 초기단계에서 두 질환을 구별하는데 도움을 줄 수 있을 것으로 생각한다.
뱀 탐지 이론은 영장류가 천적인 뱀과 경쟁하면서 뱀을 효과적으로 탐지할 수 있는 시각 체계를 갖추게 되었다고 설명한다. 구체적인 가설 중 하나는 먼지세포 중심의 피질하 시각 경로가 사람으로 하여금 심적 자원을 사용하지 않고서도 자동적으로 뱀의 위협을 탐지할 수 있게 한다는 것이다. 이에 본 연구는 뱀 영상에 대한 인간 참가자의 반응을 공포 표정의 얼굴 및 꽃에 대한 반응과 비교함으로써 뱀 탐지 이론의 가정들을 검토하였다. 참가자들은 원본 영상을 관찰하거나, 원본 영상에서 색상, 밝기와 대비, 공간주파수 에너지 차이를 제거한 변환 영상을 관찰하였다. 실험 1의 참가자들은 각 영상에 대한 정서가와 각성 유발 정도를 평정하였고, 실험 2의 참가자들은 연속점멸억제 절차에서 표적 자극을 탐지하였다. 그 결과, 뱀에 대한 반응은 시각 요인의 영향을 가장 크게 받았다. 영상들의 시각적 차이를 제거했을 때, 뱀 영상은 덜 부정적이고 각성을 덜 유발하며 연속점멸억제에서 느리게 탈출하였다. 그에 비해, 다른 범주에 대한 반응은 영상 변환의 영향을 덜 받았다. 특히, 공포 표정의 얼굴은 일관적으로 영상 조건에 상관없이 위협적인 대상으로 평정되었으며 빠르게 탐지되었다. 또한, 실험 1에서 측정한 각성 평정의 변화량과 실험 2에서 측정한 연속점멸억제 탈출 시간의 변화량이 부적 상관을 보였다. 영상 변환 후 각성 평정 점수가 많이 감소한 뱀 영상일수록 탐지반응시간이 증가하였다. 이러한 결과는 뱀이 인간 관찰자의 위협 탐지 반응에 미치는 영향이 공포 표정의 얼굴에 비해 제한적이며, 연속점멸억제 탈출 반응과 의식적 평정 반응이 처리 기제를 공유할 가능성을 시사한다. 결론적으로 본 연구는 인간의 뱀 탐지가 무의식적 피질하 시각 경로의 산물이라는 가정에 의문을 제기한다.
Depression and executive dysfunction are common neuropsychiatric sequelae of stroke. Patients with stroke are more predisposed to depression and executive dysfunction compared to patients with similar degree of physical disability. Both depression and executive dysfunction are also associated with poor prognosis such as high mortality and delayed recovery after stroke. Complex neurobiological and anatomical mechanisms are associated with the development of depression and executive dysfunction after stroke. Activation of pro-inflammatory cytokines is thought to be associated with onset of depression, whereas injuries in frontal-subcortical circuit are thought to be a link between depression and executive dysfunction. Early detection of depressive symptoms and both pharmacological and non-pharmacological treatment would be helpful. In this review paper, the authors investigated 1) biological and neuroanatomical substrate for poststroke depression and executive dysfunction, 2) the relationship and common etiopathology for poststroke depression and executive dysfunction, and 3) pharmacological and non-pharmacological treatment for poststroke depression. The contents of the paper are as follows : the prevalence, clinical manifestation, and biological etiology for poststroke depression, neuroanatomical abnormalities as a common etiological factor for depression and executive dysfunction, pharmacotherapy and non-pharmacological approach.
