구조화된 상지운동이 혈액투석 환자의 동정맥루 협착에 미치는 효과 (Effects of Structured Arm Exercise on Arteriovenous Fistula Stenosis in Hemodialysis Patient)

  • Purpose: The purpose of this research was to develop and prove the effectiveness of structured arm exercise, which was used to reduce Arteriovenous Fistula (AVF) and Arteriovenous graft (AVG) stricture of hemodialysis patients. Methods: Quasi-experimental research design with non-equivalent control group was applied. 26 Subjects were participated in this study. 12 of hemodialysis patients who do not have a normal range of Static Intra Access Pressure Vein (SIAPV) score in the last three months were assigned to the experimental group and 14 patients who have a normal range of SIAPV score in the last three months to the control group. To analyze the collecting data after structured arm exercise, non parametric method with the repeated measures ANOVA by the Friedman test and Wilcoxon Signed Ranks Test for post-hoc test was performed. Results: Unlike the experimental group after three months, the control group's SIAPV data went over the normal range. The experimental AVF group showed a difference in data after month 2 and month 3. - In AVG group, there were clear differences in each month of the test. Conclusion: This study proved that structured arm exercise therapy could be a simple and effective intervention. It is suggested to be actively utilized for hemodialysis patients.

Effects of Combined Cognitive and Physical Exercise Program on Cognitive and Physical Functions in Older Adults with Mild Cognitive Impairment

  • Objective: This study aimed to investigate the impact of a 12-week combined cognitive and physical exercise program on cognitive and physical functions in older adults diagnosed with mild cognitive impairment (MCI). Design: A one-group pretest-posttest study. Methods: Twelve participants with MCI engaged in a weekly 60-minute session of combined cognitive and physical exercise program. Cognitive function was assessed using the Montreal Cognitive Assessment (MoCA), while physical function was evaluated through measures of muscle strength, postural balance, and walking capabilities. Muscle strength assessments included the arm curl test, handgrip strength, and the 5 sit-to-stand test. Postural balance was evaluated using the one-leg stance test, timed up-and-go test, functional reach test, and four square step test. Walking function was analyzed through a gait analysis device. Pre- and post-intervention measurements were compared to determine the effects of the exercise program. Results: The results demonstrated significant improvements in MoCA, arm curl test, timed up-and-go test, walking speed, and cadence following the 12-week intervention (p<0.05). MoCA scores revealed enhanced cognitive performance, while measures of muscle strength, including the arm curl test, exhibited significant changes. Improvements in timed up-and-go test scores indicated enhanced mobility, accompanied by increased walking speed and cadence, as evidenced by gait analysis. Conclusions: This study suggests that a structured 12-week program incorporating both cognitive and physical exercises can lead to meaningful improvements in cognitive and physical functions among older adults with MCI.

혈액투석환자를 위한 운동프로그램의 효과 (The effects of an exercise training in hemodialysis patients)

  • The purpose of this study was to investigate the effect of exercise training on work capacity, psychologic functions and quality of life in hemodialysis patients. Forteen hemodialysis patients, 3 males, 11 females, age ranged from $23{\sim}58$ years($42.3{\pm}0.4$) were selected and assessed using a modified Bruce protocol on a treadmill. The 3 months supervised exercise training consisted of 60 minites session thrice weekly on the treadmill, bicycle and arm ergometer at $40{\sim}60%$ of maximum $O_2$ consumption. The changes of maximum $O_2$ consumption, psychologic test and quality of life questionnaire in 14 patients before and after 3 months exercise training have been measured. The mean maximum $O_2$ consumption($VO_2$ max) of exercises increased by 23% after training(pre-and postexercise $26.3{\pm}4.6ml$/kg/min vs $29.8{\pm}4.9ml$/kg/min, p=0.013). There was significant reduction in anxiety score(p=0.004) and significant improvement in quality of life score(p=0.031) after training. The result of this study indicated that a structured exercise training for hemodialysis patients provides many benefits. These result suggest the exercise training can improve the work capacity, psychologic functions and quality of life outcomes in hemodialysis patients.

김해시 자동차 부품 생산직 근로자의 근골격계 증상 유병률과 관련요인 (Musculoskeletal Symptoms Prevalence and Its Related Factors of Workers in Manufacturing Industry of Automobile Parts in Gimhae City)

  • Background: This study was conducted to provide preventive measure for the musculoskeletal disorders in automobile parts manufacturing workers. Method: The author surveyed to the musculoskeletal symptoms prevalence and its related factors from 10th to 17th April 2011 with structured self administered questionnaires. 223 out of 225 collected questionnaires were used for final analysis, excluding 2 questionnaires with no valid response. Based on the diagnostic criteria of NIOSH (National Institute for Occupational and Health), an investigation was made into the prevalence of musculoskeletal symptoms as well as into the factors related to individual items. Results: The prevalence of musculoskeletal symptoms according to the criteria of NIOSH was the highest in the shoulder (52.9%), followed by the neck (39.%), the hand/wrist (35%), the waist (29.6%), the arm/elbow (24.7%), and the leg/food (23.8%). One-way analysis showed that among general characteristics, age was the musculoskeletal risk factor with the greatest effect. Whereas among work-related characteristics, significant risk factor didn't find. Yet it was shown that among ergonomic work postures, high degree of musculoskeletal risk was shown by the posture involving frequent and repetitive movement of the arm and the hand/wrist and also by the posture involving standing for a long time. Multiple regression analysis showed that musculoskeletal risk was 1.795 times higher in those age 50 and over than in those under age 50; 1.67 times higher in the high risk stress group than in the low risk stress group; and 1.131 higher in the group involving the repetitive use of the hand/arm than in the other groups (p<.05). Conclusion: The prevalence and stress score of automobile parts manufacturing workers were higher than other occupation workers. Among general characteristics, drinking and smoking were shown to be related to stress score; while age was shown to have significant effect on musculoskeletal risk.

일개 지역 농부에서 근골격계 통증에 대한 스트레칭 운동의 효과 (The Effects of Stretching Exercise to Musculoskeletal pain in the Community-Dwelling Farmers)

  • 본 연구는 농작업자에게 스트레칭 운동을 적용하여 근골격계 통증에 대한 개선 효과를 알아보고자 하였다. 연구 대상은 28명의 일개 지역 농촌거주자가 참여하였으며 무작위 추출법에 의해 스트레칭군(n=13, $59.67{\pm}4.77$세)과 대조군(n=15, $61.44{\pm}10.41$세)으로 나누었다. 본 연구는 구조화된 설문방식을 이용하였으며, 작업관련 근골격계 질환의 증상을 갖는 신체 6부위에서 통증의 강도와 하루시기 중 통증변화에 따른 통증의 강도를 시각적 상사척도를 이용하여 측정하였다. 스트레칭군에서는 4주 동안 전체 12회의 스트레칭 치료가 실시되었다. 인구통계학적 특성과 통증의 특성에서 두군 간 유의한 차이는 없었다. 스트레칭 적용 4주후, 스트레칭군에서 팔/팔꿈치관절을 제외한 거의 모든 관절부위와 하루 중 통증정도가 치료 전에 비해 그리고 대조군에 비해 각각 유의한 개선효과를 보였다. 따라서 스트레칭은 지역사회 주거 노인 농부에서 근골격계 통증의 조절을 위한 효과적인 중재 방법 중 하나로 생각된다.
