Clinical Observation on Current Status of Stroke Patients Admitted to Oriental Hospital (한방병원에 입원한 뇌졸중환자의 역학적 동향에 대한 임상적 관찰)

  • Objectives: When stroke occurs, Korean people usually hope to be treated by traditional therapy and to be admitted to an oriental hospital. So this observation was done to make a small contribution toward stroke studies at oriental hospitals. Methods: This observation was made on 189 cases of stroke patients who were admitted to Kang Nam oriental medicine Hospital of Kyung Hee University from May, 1999 to August, 2000. Results and Conclusions : 1. The incidence of cerebral infarction was 3.6 times that of cerebral hemorrhage. 2. The incidence in females was 1.25 times of that in males. The incidence of stroke increased with aging and more cerebral hemorrhage occurred in lower age group than cerebral infarction. 3. There was higher morbidity in October and May, during the changing of the seasons, than any other months. 4. Hypertension the most common preceding disease, followed by heart disease, diabetes mellitus, hyperlipidemia. 5. Alcohol was the highest risk factor, followed by smoking, then obesity. 6. In cerebral infarction, MCA territory was the most frequent lesion site. 7. The change of Modefied Bathel Index(MBI) between MCA territory and others in cerebral infarction had no significance for 4weeks.(P>0.05)

The Clinical Effect of Drinking Habit in Acute Stroke Patients (급성기 중풍환자에서 음주습관이 중풍의 발생양상에 미치는 임상적 영향)

  • Objectives : This study investigated the clinical effect of a drinking habit in acute stroke patients. Methods : 409 acute stroke patients were included from October 2005 to October 2006. Patients were hospitalized within 14 days after the onset of stroke at DongGuk University International Hospital, Kyungwon University In-cheon Oriental Medical Hospital, or Department of Cardiovascular and Neurologic Diseases (Stroke Center), Kyung Hee University Oriental Hospital. We investigated general characteristics, drinking habit, and stroke subtype by TOAST classification. Results : Among drinking subjects, hemorrhagic stroke was more frequent than ischemic stroke (odds ratio 3.04), and less in small vessel occlusion than others (odds ratio 1.84). Ischemic stroke was associated with a longer (30 yrs) drinking habit than hemorrhagic stroke. Conclusions : To acquire more concrete conclusions on this theme, we need further and larger scale research.

  • Objective : This study was aimed to investigated the warning signs and its relationship with the other characteristics in acute stroke patients. Methods : Three-hundred sixty three acute stroke patients were recuited in the oriental medical hospitals of 3 universities located in metropolitan region from October in 2005 to October in 2006. We evaluated their stroke type with brain MRI, their waring signs, and general characteristics such as age, sex, past history, risk factor, etc. Result : Of the 363 patient, 138(38%) patients were experience of warning signs. The frequency of Stress were found more in experience of warning signs than none of them. Warning signs were more common in Large Artery Atherosclerosis than in the other etiology. Conclusion : Although a concrete conclusion can hardly be drawn from this study, it reminds physicians of the importance of warning signs which their patients appeal. For the further investigation, it seems to be necessary to construct fundamental databases for stroke by increasing the number of patients and by examination into the etiology and location.

  • 1980년대 후반기부터 치주질환과 뇌경색(ischemic stroke)자료의 연관성을 모색하는 시도가 있어왔다. 이번 연구의 목적은 치주질환과 뇌경색 유무와의 어떤 관계가 있는지를 60세 이상의 노인을 대상으로 조사, 통계 분석하였다. 자료는 미국의 총 국민조사 격인 The Third Nation Health and Nutrition Examination Survey (NHANES III)를 이용하였다. 이번 연구에서 unadjusted logistic model 통계법을 이용하여 치아 상실수와 뇌경색 경험이 통계학적으로 유의한 수치의 상관성이 있음을 알게 되었다. 또한 나이와 흡연유무를 고려, 조정한 후 multiple logistic model 통계법으로 잔존치아가 적을수록 더욱 뇌경색에 걸릴 확률이 높음을 보였다. 그러나 두 질병에 동시에 선택된 중요한 위험인자 (risk factor)를 모두 고려, 조정 한 후에는 통계학적인 유의성을 찾지 못했다. 치은퇴축, 치주낭 깊이, 치석, 탐침시 출혈과 뇌경색 경험은 각각의 비교법에서 약간의 상관성을 보이나, 모든 통계법을 통해 일괄된 결과를 얻을 수는 없었다.

Medication Use as a Risk Factor for Falls among Hospitalized Stroke Patients (노인전문병원에 입원한 뇌졸중환자의 복용약물과 관련된 낙상 위험요인)

  • Purpose: To identify the relationship between medication use and falls among hospitalized stroke patients. Method: The medical records of 472 patients with strokes were reviewed using a questionnaire on falling developed by the authors. Frequencies, percentages, means, standard deviations, and t-test and ${\chi}^2$-test, multiple logistic regression analysis were done using the SAS program. Results: The rate for falls by the patients during their stay in the hospital was 14.0%. The length of stay was longer and the morbidity duration of stroke shorter in the fall group than in the non-fall group. The use of sedatives, laxatives, and antidepressants was a significant predictor of falls and was associated with increase likelihood of falling(1.82, 1.81, 1.75 times respectively). Conclusion: In hospitalized stroke patients, there was a significant association between the use of sedatives, laxatives, antidepressants and falls. The number and kinds of ingested drugs was also associated with falls. It is necessary to further analyze the causes of falls based on results of the present study.

  • Objective : The major risk factors of stroke are hyperlipemia, ateriosclerosis, hypertension, diabetis mellitus, smoking, drinking, age, personal history, family history etc. Hypertension, smoking and hyperlipemia are very important factor to promote stroke. We intended to observe the serum lipid density of 51 patients come to Joongpoong & Chronic Disease Center of Conmaul Oriental hospital, comparing with already reported study. Methods : We studied 51 stroke-patients( 38 cerebral infarction, 13 intracerebral hemorrhage) of Conmaul Oriental hospital, Department of oriental internal medicine, Joongpoong & Chronic Disease Center from January 1. 1999 to August 31. 2000, which examined by neurologic test, brain-CT, brain-MRI. . We used DOSHISA TBA-30FR(Japan) as instrument for measuring total cholesterol and triglyceride of serum. Results and conclusions : The results are as follows 1. The rate of hyperlipemia and presumed-hyperlipemia is higher in female patient-group than male patient-group. 2. The rate of hyperlipemia and presumed-hyperlipemia is higher in drinking group than non-drinking group. 3. The rate of hyperlipemia and presumed-hyperlipemia is higher in hypertension group than normotention group.

The Hematologic Study as risk factors on premature examination of stroke (PES) (중풍위험인자로서의 혈액학적 소견 연구 - 중풍조기검진 326명에 대한 분석 -)

  • 1.Purpose Stroke is the most frequent cause of mortality and morbidity rate in Korea with ischemic heart disease and cancer. Recent recurrence rate of stroke is high, but little attention had been given to the features of its' cause. The purpose of this studies is to examine the hematologic states in Korean normal adults through premature-examination of stroke(PES). 2.Method The study group consisted of 326 cases that they had no significant results by CT through premature examination of stroke(PES) in Cheonan oriental hospital of Daejeon university. We tested RBC, hemoglobin, hematocrit, WBC, PT, PTT, PLT, total protein, albumin, total bilirubin, direct bilirubin, GOT, GPT, $\gamma$-GTP, total cholesterol, triglyceride, HDL-C, glucose, BUN, creatinine, Na, K and evaluated by cross sectional study. 3.Result In the PES, there were many patients with low RBC(11%), shorten PT(26.7%), delayed PTT(19.3%), high TP, GPT, ${\gamma}$ -GTP(respective1y 16.9%, 14.4%, 13.5%), high cholesterol level and triglyceride level(respectively 18.7%, 28.2%). We didn't found significant cases in hemoglobin, hematocrit, WBC, PLT, albumin, total bilirubin, direct bilirubin, GOT, HDL-C, glucose, BUN, creatinine, Na, K. 4.Conclusion In this study, we demonstrated that short PT, high triglyceride level are special views in PES. Also prospective studies are needed continuously to search the preventing methods of the CVA.

The Comparison on the General Characteristics of Acute Stroke Patient between Heat pattern group and Cold pattern group (한열변증에 따른 중풍 환자의 제반특성 비교)

  • Object : This study was conducted as part of the national project to standardize stroke diagnosis in Korean medicine. In this study, we aimed to evaluate the characteristics of acute stroke between heat pattern group and cold pattern group. Methods : We recruited stroke patients from 5 universities(Kyung-Hee University oriental medical center, Kyung-Hee University East-West Neo medical center, Kyungwon university lncheon oriental medical center, Kyungwon university Songpa oriental medical center and DongGuk university llsan oriental medical center) from April, 2007 until February, 2010. We diagnosed them and selected 463 heat pattern patients and 182 cold pattern patients. Results : We find that the risk factor of smoking, alcohol, diet(prefer to meat) are more associated with the heat pattern group. On the other hand, inflammation history in recent 3 months and diet(prefer to sea food) are more associated with the cold pattern group. RBC, Hg, Hct, TG, CK and Cl are more related to the heat pattern group. CPT, total cholesterol and HDL are more related to the cold pattern group. Tae-eum type takes high distribution of the heat pattern group, and So-eum type takes high distribution of the cold pattern group. Conclusions : This study provides evidence that the heat pattern group have more risk factor than the cold pattern group.

Study on Characteristics of Abdominal Obesity among Acute Stroke Patients (뇌졸중 환자의 복부비만에 따른 제특성 연구)

  • Objectives : This study was aimed to assess characteristics in acute stroke patients according to abdominal obesity. Method : 1,506 subjects were included from the patients admitted to the Internal Medical Department at Kyunghee University Oriental Medical Center, Kyunghee University East-West Neo Medical Center, Kyungwon University Songpa Oriental Medical Center, Kyungwon University Incheon Oriental Medical Center, and D ongguk University Ilsan Oriental Medical Center from April 2007 to August 2009. Results : 1. Considering the demographic variables of the patients, the gender, older age, diabetes, hyperlipidemia and multiple infarction were significantly higher in the abdominal obesity group than in the control group. 2. Sasang constitution and oriental medical diagnosis showed significant difference in the abdominal obesity group. By oriental medical diagnosis, the D amp ness-Phlegm group was significantly higher in abdominal obesity group than in the control group. According to this analysis, we observed the general disposition of various characteristic distributions according to abdominal obesity in acute stoke patients These results can be utilized in the future as a basis material.

Tumor necrosis $factor-\;{\alpha}$, interleukin-6 and interleukin-10 polymorphisms in the Korean stroke patients

  • Objective: With the onset of stroke, white blood cells release several proinflammatory cytokines, including interleukin (IL)-6, IL-10, and tumor necrosis factor $(TNF)-{\alpha}$. It has been proven in previous studies that the release of these cytokines is related to the extent of damage to the brain and to overall prognosis. However, no studies have yet been performed to determine the connection with IL-6 and IL-10. Thus, this study is performed to see whether polymorphisms of IL-6, IL-10, and $TNF-{\alpha}$ genes that show increased serum concentration with the onset of stroke are related to stroke attack in Koreans. Methods : Peripheral blood samples derived from patients with stroke (n=100) and healthy controls (n=100) were taken under informed consent. In subjects with stroke, blood samples were obtained within 24 hours of stroke onset. Genomic DNA was isolated using the Wizard DNA Purification Kit (Promega, Madison, WI). Results : 1. Subjects with Heterozygote (GA) and Homozygote (AA) $TNF-{\alpha}$ gene types showed 2.433 and 20.457 times higher risks of being attacked by stroke, respectively, compared to subjects with wild type (GG) $TNF-{\alpha}$ gene type. The data was still statistically significant after adjusting for age, sex, history of smoking, and history of alcohol drinking. 2. Subjects with Homozygote (CC) IL-6 gene type showed 182.033 times higher risk of being attacked by stroke, compared to subjects with wild type (GG) IL-6 genes. This data was statistically insignificant (p=0.700). The data was still statistically insignificant after adjusting for age, sex, history of smoking, and history of alcohol drinking. 3. Subjects with Heterozygote (GA) and Homozygote (GG) IL-10 gene types showed 8.785 and 3.303 times higher risks of being attacked by stroke, respectively, compared to subjects with wild type (AA) IL-10 genes. The data was still statistically insignificant after adjusting for age, sex, history of smoking, and history of alcohol drinking. Conclusion : Our results suggest that the investigated $TNF-{\alpha}$ and IL-10 gene polymorphisms play an important role in stroke attack, but IL-6 gene polymorphism has not been found to associated with stroke.
