  • Purpose : The objectives of this study were to examine changes in gait parameters related to the COP and gait velocity resulting from the use of ankle foot orthosis, and to provide information for the prescription of ankle foot orthosis and gait rehabilitation training. Methods : We sampled a control group of 14 normal children of the same age as the spastic diplegia (spastic diplegia used ankle foot orthosis) and measured their COP at barefoot gait and their gait parameters at three gait velocities. Change in the COP according to the use of ankle foot orthosis measured one-way ANOVA and Gait parameters according to gait velocity used repeated measures ANOVA. Results : As a result of analyzing change in the COP it became close to that of normal children, and changes in gait parameters such as step width, stride length and stride time also became close to those of normal children. Conclusion : In conclusion, when spastic diplegia used ankle foot orthosis, their balancing ability was improved as a result of decreased change in the COP, and their changes in gait parameters also became close to those of the control group. These results show that the use of ankle foot orthosis improves spastic diplegia gait functions.

  • The purpose of this study was to investigate possibility of a forward gait with backward mechanism(dance gait) as rehabilitation and/or walking exercise by means of biomechanical variables. Thirteen professional women dancers(age, $21.1{\pm}1.3yrs$; height, $159.3{\pm}7.2cm$; body mass, $45.1{\pm}8.4kg$)participated in this study. We found that speed, stride length and double limb support time of a dance gait were more greater than backward gait, but stride width of dance gait less than a backward gait. Maximum RoMs, moments and powers of the lower limb joints on a dance gait were more frequent than a backward dance. These results were judged to be sufficient by the possibility of dance gait as rehabilitation and walking exercise.

  • Objective: The purpose of this study was to analyze the impact acceleration, shock attenuation and biomechanical variables at various running speed. Method: 20 subjects (height: 176.15 ± 0.63 cm, weight: 70.95 ± 9.77 kg, age: 27.00 ± 4.65 yrs.) participated in this study. The subjects ran at four different speeds (2.5 m/s, 3.0 m/s, 3.5 m/s, 4.0 m/s). Three-dimensional accelerometers were attached to the distal tibia, sternum and head. Gait parameters, biomechanical variables (lower extremity joint angle, moment, power and ground reaction force) and acceleration variables (impact acceleration, shock attenuation) were calculated during the stance phase of the running. Repeated measures ANOVA was used with an alpha level of .05. Results: In gait parameters, decreased stance time, increasing stride length and stride frequency with increasing running speed. And at swing time 2.5 m/s and 4.0 m/s was decreased compared to 3.0 m/s and 3.5 m/s. Biomechanical variables statistically increased with increasing running speed except knee joint ROM, maximum ankle dorsiflexion moment, and maximum hip flexion moment. In acceleration variables as the running speed increased (2.5 m/s to 4.0 m/s), the impact acceleration on the distal tibia increased by more than twice, while the sternum and head increased by approximately 1.1 and 1.2 times, respectively. And shock attenuation (tibia to head) increased as the running speed increased. Conclusion: When running speed increases, the magnitude and increasing rate of sternum and head acceleration are lower compared to the proximal tibia, while shock attenuation increases. This suggests that limiting trunk movement and increasing lower limb movement effectively reduce impact from increased shock. However, to fully understand the body's mechanism for reducing shock, further studies are needed with accelerometers attached to more segments to examine their relationship with kinematic variables.

  • Objective: The objective of this study was to investigate the feasibility and effects of balance training using a newly developed elastic band orthosis (aider) for improvement of mobility and balance in chronic stroke patients. Design: Cross-sectional study. Methods: Ten patients with chronic hemiplegia participated in this study. There were six males and four females; two patients had right hemiplegia and eight had left hemiplegia. This study investigated the effect of the elastic band orthosis on balance and gait ability compared with bare foot condition. Gait parameters were measured using the opto-gait system for analysis of the spatial and temporal parameters of walking in stroke patients. In addition, balance ability in stroke patients was evaluated using the Timed Up and Go (TUG) and Berg Balance Scale (BBS). Results: This study investigated the effect of the elastic band orthosis on balance compared with bare foot condition. The TUG and BBS showed significant improvement with use of the elastic band orthosis (p<0.05). Use of the Elastic band orthosis resulted in significantly improved velocity, cadence, less-affected step length, less-affected stride length, and less-affected single limb support in stroke patients (p<0.05). Conclusions: We demonstrated a significant improvement in dynamic balance and gait ability in chronic stroke patients using the elastic band orthosis. This orthosis may aid in prevention of spastic foot drop, leading to improvement of walking ability.

  • Objective: The purpose of this study was to determine the force of lower extremities, the change in walking ability on the ground by applying a walking training program based on perceptual learning to improve gait capacity of chronic stroke patients. Method: This study included Twenty-four patients with chronic stroke. Using a perceptual-based gait training, the experimental group trained twice a day for 30 minutes each time, 5 times a week, for a total of 8 weeks. The control group underwent ground gait training that excluded the element of a perceptual training for 30 minutes, 5 times a week for 8 weeks. Results: In the two groups, the maximum forefoot pressure after intervention was significantly different in both the LEPGT and GGT (p<0.05). The maximum midfoot pressure was significantly different in LEPGT (p<0.05). There was a significant difference in the maximum heel pressure after intervention between the two groups (p<0.05). As a result of comparing the change in step length and stride length after intervention in the two groups, there was a significant difference between the two groups (p<0.05). Conclusion: Both gait training programs was found that gait training based on perceptual learning and ground gait training were the training for improving the functional gait of stroke patient. Perceptual learning gait training utilizing intensive perceptual awareness was the training for improving gait capacity within the period than ground gait training.

  • Improvement in functional gait is one of treatment goals in treatment of cerebral palsy children. This study intended to examine the effects of insoles for postural correction on gait in spastic cerebral palsy patients by investigating changes in gait temporal spatial parameters. As the subjects, 15 spastic bilateral cerebral palsy patients participated in this study. Temporal spatial parameters of gait were measured using GAITRite system under three gait conditions. Bare foot gait, gait in shoes, and gait in insoles for postural correction were conducted. In order to look at differences in temporal spatial parameters according to three gait conditions, repeated one way analysis of variance was conducted. As post hoc test, Bonferroni was conducted. A significant level was set at ${\alpha}=.05$. According to the result of this study, gait velocity, cadence, step length, stride length of the left lower extremity significantly changed. When the subjects put on customized insoles for postural correction, the effect was greatest. There were no significant changes in stance time, single support time, double support time, swing % of gait, and stance % of cycle. Therefore, gait with insoles for postural correction positively influenced functional gait improvement and will be able to be usefully employed for spastic cerebral palsy children as one of gait assistance devices.
