뉴로피드백·뇌체조·한방차를 병행한 통합요법이 독거노인의 뇌기능 향상 및 삶의 질에 미치는 영향 (The Effect of the Integrated Therapy of Neurofeedback, Brain Gymnastics, and Oriental Herbal Tea on the Improvement of Brain Functions and the Quality of Life of Elders living alone)

  • 본 연구는 독거노인을 대상으로 뉴로피드백 훈련과 뇌체조 그리고 한방차를 병행한 통합요법을 적용하여 독거노인의 뇌기능 향상 및 삶의 질에 미치는 영향을 확인하기 위한 단일군 전후 설계인 실험 연구이다. 2014년 12월 22일부터 2015년 2월 28일까지 8주 동안, Y군에 거주하는 65세~90세의 독거노인 23명(남 10명, 여 13명)을 대상으로 뉴로피드백 훈련은 2 Channel Neurofeedback System을 이용하여 이완 집중 기억력을 중심으로, 뇌체조는 한국정신과학연구소에서 개발한 뇌체조 프로그램을 이용하여 각각 1회 30분, 주 2회, 8주간, 16회를 하였다. 그리고 한방차는 동의보감과 여러 선행연구를 토대로 개발하여 조제한 한방차를 8주간, 매일, 하루 3잔 음용하도록 하였다. 그 결과 통합요법이 뇌기능지수 중 긴장도와 항스트레스 지수가 유의하게 변화하여 뇌기능 향상에 긍정적인 효과가 나타났다. 또한 삶의 질에서는 혈압과 혈당이 낮아지고, 삶의 질, 일상생활, 우울증상 등에서 유의미한 결과가 나왔다. 결론적으로 통합요법이 독거노인의 뇌기능 및 삶의 질을 의미 있게 향상 시킬 수 있음을 보여주었다. 그러므로 뉴로피드백 훈련, 뇌체조 및 한방차를 병행한 통합요법이 독거노인들의 삶의 질을 높이는데 반드시 필요한 중재요법이며, 보다 실질적이고 체계적으로 심신을 치유하여 삶의 질을 향상시키게 할 것이다.

SLC25A13 유전자 돌연변이로 확진된 성인형 제 2형 시트룰린혈증 1례 (A Case of Adult-onset Type II Citrullinemia Confirmed by Mutation of SLC25A13)

  • 성인기에 나타난 운동으로 인한 스트레스와 과량의 단백질 투여에 의한 의식 변화, 기억 장애, 행동 장애등의 고암모니아혈증성 뇌병증 소견을 토대로 저자들은 성인형 제 2형 시트룰린혈증을 의심하였다. 검사 결과에서 고암모니아혈증, 혈중 시트룰린 상승, 요중 오로트산 경도 상승을 보였으며, 이를 통해 성인형 제 2형 시트룰린혈증을 진단하였다. SLC25A13 유전자 분석 결과를 통해 환자에게서 복합 이형 접합성 돌연변이(IVS11+1G>A, c.674C>A)를 확인하였다. 환자의 가족에서도 유전자 분석 검사를 진행하였고, 아버지, 어머니, 남동생에게서 성인형 제 2형 시트룰린혈증 보인자를, 언니에게서 성인형 제 2형 시트룰린혈증 환자임을 확인하였다. 그 동안 보고된 성인형 제 2형 시트룰린혈증의 임상 경과를 고려했을 때, 증상이 없던 언니에게서 질환을 발견하고 보존적 치료를 선제적으로 시작함으로써, 신경학적 장애 없이 일상 생활을 영위하고, 추가적인 뇌 손상을 방지하기 위한 간 이식 등 장기적인 치료 계획을 수립했다는 점에서 의의가 있다. 본 환자의 증례를 통해 고암모니아혈증성 뇌병증이 발생한 성인에서, 간질환 및 뇌질환의 증거가 없으며 다른 뚜렷한 원인이 없는 경우에는, 성인형 제 2형 시트룰린혈증을 고려해야 한다는 점을 인지하였다. 빈도가 드문 질환이지만 간성 혼수로 흔히 오인되고 있으며, 조기 진단 및 적절한 치료가 이루어지지 않으면 비가역적인 신경학적 후유증이 발생할 수 있다. 또한 다른 요소 회로 대사 질환 및 간성 혼수와는 달리, 고탄수화물 식이가 질병의 경과를 인위적으로 악화시킬 수 있기에 적절한 대사 이상 검사 및 유전자 검사가 시행되어야 하겠다. 본 증례는 반복적인 고암모니아혈증성 뇌병증 소견을 보인 37세 여성과 뇌병증 소견이 없었던 언니에게서 성인형 제 2형 시트룰린혈증을 진단하고, 즉각적인 치료를 통해 심각한 신경학적 장애 없이 일상 생활을 지속하고 있는 환자들을 문헌 고찰과 함께 보고하는 바이다.

한국에서 개발된 곤충유래 약용버섯인 누에동충하초의 생산기술개발 및 약리학적 특성 (Properties of the Silkworm (Bombyx mori) Dongchunghacho, a Newly Developed Korean Medicinal Insect-borne Mushroom: Mass-production and Pharmacological Actions)

  • 동충하초(Cordyceps)는 B.C. 2,000년 이후로부터 지금까지 중국, 한국, 일본 등 아시아 국가에서 잘 알려진 전통적인 불로장생의 비약으로 사용되어져 왔다. 곤충기생 곰팡이균으로서 원조 동충하초로 불리는 Cordyceps속에 속하는 Cordyceps sinensis는 우리나라 한반도에서는 발견되지 않고 있다. 그 이유는 해당 곰팡이균에 특정적인 기주곤충인 박쥐나방(Hepialus armonicanus)이 우리나라에는 존재하지 않기 때문으로 보인다. 따라서 차선책으로서 한국형의 동충하초, 즉 Paecilomyces tenuipes 동충하초균에 대한 곤충생체배지로서 누에(Silkworm, Bombyx mori)를 기주곤충으로 사용하는 누에동충하초의 인공적인 생산방법을 세계 최초로 개발하게 된 것이다. 따라서 본 총설은 누에동충하초를 개발하게 된 역사적 배경과 생산방법 및 약리학적 효능과, 비곤충유래 동충하초인 인공배지 배양동충하초(밀리타리스동충하초: Cordyceps militaris, 붉은자루동충하초: Cordyceps pruinosa 등)의 배양조건과 동충하초 품종육성, 배지배양조건, 동충하초의 식품에의 응용 가능성, 화학적 구성성분 및 약리적 효능 등에 대해 고찰하였다. 곤충유래의 누에동충하초 및 비곤충유래동충하초균사체 및 자실체동충하초의 주요 약리작용은 항종양, 면역증강, 항피로, 항스트레스, 항산화, 항노화, 항당뇨, 항염증, 항혈전, 항지질 및 해충방제 효능으로 요약된다. 동충하초의 주요 생리활성물질들은 단백다당체(헥소스, 헥소사민), 코디세핀, 만니톨, 산성다당체 등이다. 단백다당체와 부타놀 추출물은 매우 강한 항종양 효과를 보이나 세포독성은 나타내지 않았다. 만니톨은 종양유발쥐의 수명을 유의성 있게 연장하였다. 에르고스테롤은 항종양 활성은 보이지 않았으나 왕성한 식세포활성을 보였다. 따라서 우리가 높은 관심을 가지고 있는 누에동충하초의 항종양 활성은 세포독성효과보다는 면역력증강효과에 기인하는 것으로 사료된다[164].

자생식물 혼합 추출물이 SD 흰쥐에서의 행동양상 및 항산화 체계에 미치는 영향 (Effect of Natural Plant Mixtures on Behavioral Profiles and Antioxidants Status in SD Rats)

  • 주간졸음증은 업무수행 능력을 감소시키고 사고의 위험을 증가시키는 것으로 보고되어 있으며, 특히 교통사고의 주요 원인이 된다. 카페인은 전 세계적으로 가장 널리 이용되는 커피, 녹차, 콜라 등의 음료에 함유되어 있는 물질로써 정신적 스트레스를 완화시키고, 자살충동 및 우울증의 감소 등 특정 효과가 있는 것으로 보고된바 있으며 졸음을 억제하기 위한 수단으로 주로 이용된다. 그러나 산소 소모의 증가, 체내 에너지 손실 및 대사율 증가뿐 아니라 비장의 DNA 합성 감소, 체내 지질 산화 등의 많은 부작용을 가진다. 따라서 본 연구에서는 국화, 민들레, 두릅, 냉이, 석창포, 과라나 등의 자생식물들을 이용하여 행동학적 분석을 통한 각성 효과뿐만 아니라 인체의 생리활성에 안전한 물질을 탐색하고자 하였다. Open field test의 결과, 단회 투여 시 대조군에 비해 카페인 투여군에서 활동적인 행동 양상을 나타내었으며 NP군에서 카페인 투여군과 같은 수준의 활동 정도를 나타내었다. 또한 3주간 NP의 투여는 카페인의 투여에 비해 증가한 활동성의 결과를 보였다. 총콜레스테롤과 중성지방, LDL-콜레스테롤 농도의 경우 각 군 간의 유의적 차이가 나타나지 않았으며 HDL-콜레스테롤 농도의 경우 대조군에 비해 카페인군과 NP군에서 유의적 차이는 없었으나 카페인 투여군에 비해 NP 투여군에서 유의적으로 높은 수준을 보였다. 혈중 지질과산화 농도는 대조군(5.6 mM)에 비해 카페인군(11.7 mM)에서 유의적으로 높은 농도를 나타낸데 반해 NP군(6.4 mM)은 대조군과 같은 수준을 나타내었다. 간세포의 DNA 손상정도에서는 대조군($18.4{\pm}2.1$)에 비해 카페인 투여군($23.8{\pm}3.1$) 에서 유의적으로 손상이 증가한데 반해 NP군($15.9{\pm}1.7$)은 대조군에 비해 유의적 차이를 보이지 않음으로써 NP의 장기 투여는 DNA 손상에 아무런 영향을 주지 않았다. 본 연구의 결과, 국화, 민들레, 두릅, 냉이, 석창포, 과라나 등의 자생식물 혼합추출물이 카페인과 비슷한 각성효과를 보이면서도 장기복용 시 안전한 것으로 나타나 각성효능이 있는 기능성식품으로서의 가능성이 충분하다고 사료된다.

인도네시아의 메타내러티브와 동티모르의 로칼내러티브의 서술구조 비교 (A Comparison of the Metanarrative and East Timor's Local Narrative in Indonesia under the Suharto's Regime)

  • 본 논문은 인도네시아의 국민국가적 메타내러티브와 동티모르의 로칼내러티브의 서술 구조를 비교·분석한다. 이 두 내러티브는 모두 민족을 초역사한 민족주의적 역사서술 방식을 보여주고 있지만, 인도네시아와의 통합과 분리라는 각기 다른 정치적 지향점 속에서 역사적 사건에 대한 서로 다른 해석을 내리고 있다. 우선 메타내러티브에서는 로칼의 다양성을 민족의 도식 속에 흡수해서 바라본다. 이 내러티브에서는 원형민족의 상을 자바와 수마트라의 일부 왕국에서 찾는다. 마자빠힛과 스리위자야를 중심으로 한 이 왕국들은 힌두-불교문화를 중심으로 찬란한 과거 문명을 꽃피우고 영토적 통일성을 이룬 특징이 있다. 이러한 영광스러운 과거는 곧 식민시대의 도래와 함께 사라진다. 메타내러티브에서는 식민시기의 암울한 현실에서 종족들이 어떻게 일치단결하여 식민의 착취와 압제에 항거했는지를 부각시킨다. 그럼으로써 다양한 종족들은 "항쟁"이라는 공통의 목표로 가진 민족이라는 이름의 단일 정체성으로 규정된다. 이 과정에서 식민세력에 대해 우호적인 입장을 취한 로칼의 역사는 메타내러티브에서 누락되었고, 초국가적 역사서술 양상 속에서 로칼의 역사는 후진적인 것으로 은밀히 묘사되었다. 또한 다양한 분리주의 운동은 공산주의 등의 이데올로기에 의한 반통합적 소요로만 그려졌다. 한편, 동티모르의 자주적 역사관 속에서 역사서술의 목표는 동티모르의 민족을 창출하고 민족의식을 고취시키는 것으로, 그 궁극적 목표는 인도네시아로부터의 분리독립이었다. 이 역사에서는 영광스러운 과거의 흔적을 찾아낼 수 없는 한계점을 "가상의 영광스러움"에 대한 인식의 환기로 극복하고, 포르투갈 식민기억을 의도적으로 과장하여 네덜란드 식민지였던 인도네시아와의 단절을 강조한다. 또한 식민세력의 450년간의 수탈과 착취를 강조하고, 그 고통 속에서 동티모르 민족이 노예화되었음을 보여주며, 이와 같은 노예화가 인도네시아라는 또 다른 식민국에 의해 연속되고 있음을 상기시켜 이를 독립운동의 모티브로 삼고 있다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology. 30609 T00401030609 ^x To develop an artificial bone substitute that is gradually degraded and replaced by the regenerated natural bone, the authors designed a composite that is consisted of calcium phosphate and collagen. To use as the structural matrix of the composite, collagen was purified from human umbilical cord. The obtained collagen was treated by pepsin to remove telopeptides, and finally, the immune-free atelocollagen was produced: The cross linked atelocollagen was highly resistant to the collagenase induced collagenolysis. We propose a simple adaptive algorithm to increase capability of noise cancelation in QRS complex detection with two stage adaptive filter. Two adaptive filters can afford to keep track of the changes of both noise and QRS complex. Each adaptive filter consists of prediction error filter and FIR filter The impulse response of FIR filter uses coefficients of prediction error filter.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
