Treatment Methods for Functional Recovery after Total Knee Arthroplasty (슬관절 전치환술 후 기능 회복을 위한 치료법)

  • Total knee arthroplasty is performed widely in degenerative arthritis of the knee joint, and the frequency of use is increasing. Efforts should be made to achieve functional recovery, such as returning to daily life, and the recovery of strength and range of motion after surgery. The procedure should be approached from a range of perspectives, such as patient factors, surgical technique and rehabilitation. The patient's age, degree of obesity, sex, and strength of the quadriceps muscle can affect the functional recovery after surgery. In addition, the patient's mental state, such as expectation value and satisfaction, can also have an effect. For functional recovery, patient education, pain control, and strengthening of the quadriceps muscle can be performed prior to surgery. Postoperative physiotherapy, such as icing and compression, transcutaneous electrical nerve stimulation therapy, neuromuscular electrical stimulation therapy, low frequency low intensity magnetic field therapy, strengthening of quadriceps muscle, and range of motion exercise can also be applied. In recent years, hydrotherapy, which restores the strength and balance of the leg, is being performed increasingly. These treatments are not only performed shortly after surgery, but are also performed continuously. In addition, the surgeon should apply it appropriately considering the patient's condition, compliance, and social and psychological conditions.

The Effect of Aging Treatment on the High Temperature Fatigue Fracture Behavior of Friction Welded Domestic Heat Resisting Steels (SUH3-SUS 303) (마찰용접된 국산내열 강 (SUH3-SUS303 )의 시효열처리가 고온피로강도 및 파괴거동에 미치는 영향에 관한 연구)

  • It is well-known that nowadays heat resisting and anti-corrosive materials have been widely used as the components materials of gas turbines, nuclear power plants and engines etc. In the fields of machine production industry. And materials for engine components, like as the exhaust valve of internal combustion engine, have been required to operate under the high temperature range of $700^{\circ}C$-$800^{\circ}C$ and high pressured gas with repeated mechanical load for the high performance of engines. For these components, friction welding for bonding of dissimilar steels can be applied for in order to obtain process shortening, production cost reduction and excellent bonding quality. And age hardening recently has been noticed to the heat resisting materials for further strengthening of high temperature strength, especially high temperature fatigue strength. However, it is difficult to find out any report concerning the effects of age hardening for strengthening high temperature fatigue strength to the Friction welded heat resisting and anti-corrosive materials. In this study the experiment was carried out as the high temperature rotary bending fatigue testing under the condition of $700^{\circ}C$ high temperature to the friction welded domestic heat resisting steels, SUH3-SUS303, which were 10hr., 100hr. aging heat treated at $700^{\circ}C$ after solution treatment 1hr. at $1, 060^{\circ}C$ for the purpose of observing the effects of the high temperature fatigue strength and fatigue fracture behaviors as well as with various mechanical properties of welded joints. The results obtained are summarized as follows: 1) Through mechanical tests and micro-structural examinations, the determined optimum welding conditions, rotating speed 2420 rpm, heating pressure 8kg/mm super(2), upsetting pressure 22kg/mm super(2), the amount of total upset 7mm (heating time 3 sec and upsetting time 2 sec) were satisfied. 2) The solution treated material SUH 3, SUS 303, have the highest inclination gradient on S-N curve due to the high temperature fatigue testing for long time at $700^{\circ}C$. 3) The optimum aging time of friction welded SUH3-SUS 303, has been recognized near the 10hr. at $700^{\circ}C$ after the solution treatment of 1hr. at $1, 060^{\circ}C$. 4) The high temperature fatigue limits of aging treated materials were compared with those of raw material according to the extender of aging time, on 10hr. aging, fatigue limits were increased by SUH 3 75.4%, SUS 303 28.5%, friction welded joints SUH 3-SUS 303 44.2% and 100hr. aging the rates were 64.9%, 30.4% and 36.6% respectively. 5) The fatigue fractures occurred at the side of the base matal SUS303 of the friction welded joints SUH 3-SUS 303 and it is difficult to find out fractures at the friction welding interfaces. 6) The cracking mode of SUS 303, SUH 3-303 is intergranular in any case, but SUH 3 is fractured by transgranular cracking.

  • This study has investigated the effects of aging treatment on thc nlechanical propcrries of blg-iiAl-x%nix - 0.1.2) alloys fabricated by the squeeze castmg process. The microstructurcs of as-squeeze cast were composed of pro eurectic $\alpha$ magnesium solid solution, super saturated $\alpha$ solid solution and $\beta(Mg_{17}AI_{12}$) compund. Agcd at both $200^{\circ}C$ and $240^{\circ}C$, Mg--6AixZn alloys showed the peak hardness due to the formation of $\beta(Mg_{17}AI_{12}$) precipitates. The tiiscontinuous precipitates of the lamella r\.pe are predominant at $200^{\circ}C$ aging tredrment. tvhilc. the finely dispersed continu ous precipitates were major type at $240^{\circ}C$ treatment. Mg-- GAI-xZn a1loi.s fabricated hy the squeeze casting process had the hetter combination of tensile strength and elongation compared to the conventionally cast alloys.   • The Conservation Science Team of the National Museum of Korea has carried out the conservation on the mandolin-type bronze sword that was excavated from Weolnae-dong, Yeosu as had been requested by the Research Center of Dolmens in Northeast Asia. The mandolin-type bronze sword from Weolnae-dong, Yeosu is accounted to be the longest one among all the bronze swords of the same type that have ever been excavated until now and it was in a treated condition with the primary conservation treatment already achieved. Due to the corrosion in progress, it was in a very brittle condition being in two separate parts of the upper and lower parts. With the upper part exposed and the lower part with earth, they were urgently collected. The Conservation Science Team carried out the conservation treatment on them by connecting the lower part of the mandolin-type bronze sword to the upper part after making the lower part exposed, and then by using an estimated restoration method for lost portions. When carrying out the conservation treatment, the glass fibers of 10 wt% Paraloid B-72 (in Xylene) was used as a method for strengthening the brittle artifact, and a non-destructive analysis was carried out to identify the ingredients using the X-ray fluorescence spectrometer.

Tuberculosis Surveillance and Monitoring under the National Public-Private Mix Tuberculosis Control Project in South Korea 2016-2017

  • Background: The national Public-Private Mix (PPM) tuberculosis (TB) control project provides for the comprehensive management of TB patients at private hospitals in South Korea. Surveillance and monitoring of TB under the PPM project are essential toward achieving TB elimination goals. Methods: TB is a nationally notifiable disease in South Korea and is monitored using the surveillance system. The Korea Centers for Disease Control and Prevention quarterly generates monitoring indicators for TB management, used to evaluate activities of the PPM hospitals by the central steering committee of the national PPM TB control project. Based on the notification date, TB patients at PPM hospitals were enrolled in each quarter, forming a cohort, and followed up for at least 12 months to identify treatment outcomes. This report analyzed the dataset of cohorts the first quarter of 2016 through the fourth quarter of 2017. Results: The coverage of sputum, smear, and culture tests among the pulmonary TB cases were 92.8% and 91.5%, respectively. The percentage of positive sputum smear and culture test results were 30.7% and 61.5%, respectively. The coverage of drug susceptibility tests among the culture-confirmed cases was 92.8%. The treatment success rate among the smear-positive drug-susceptible cases was 83.2%. The coverage of latent TB infection treatment among the childhood TB contacts was significantly higher than that among the adult contacts (85.6% vs. 56.0%, p=0.001). Conclusion: This is the first official report to analyze monitoring indicators, describing the current status of the national PPM TB control project. To sustain its effect, strengthening the monitoring and evaluation systems is essential.

  • The following conclusions were obtained after bibliographic investigation on the therapy of lung cancer by western, oriental, and integrated oriental and western medicine. 1. Lung cancer is classified into small cell lung cancer(SCLC) or non small cell lung cancer(NSCLC) in the treatment by western medicine, and applied with the means of surgery, radiotherapy and chemotherapy alone or combined, depending on the stage and the symptom. 2. Treatment by oriental medicine includes the means of strengthening body resistance to dispel pathogenic factors(扶正祛邪), combined approach of reinforcement and expulsion(攻補兼施), and reinforcing both qi and blood(氣血雙補), depending on the initial, middle, and terminal stage. And also treatment based on differentiation of symptom(辨證施治) is applied according to the type of symptom; deficiency of qi of both lung and spleen(肺脾氣虛), heat symptom of lung by deficiency of yin(肺熱陰虛), stagnation of damp-phlegm and blood(濕痰瘀阻), stagnation of qi and blood(氣血瘀滯), deficiency of both qi and yin(氣陰兩虛). Single or combined herb drug is used according to the symptom. 3. Treatment by integrated oriental and western medicine improved survival rate and quality of life. It promoted recovery and improved survival rate in the patients receiving surgery. Integrated radiotherapy and oriental medicine treatment reduced adverse effect by radiotherapy and improved therapeutic effect and survival rate. Integrated chemotherapy with oriental medicine treatment reduced side effect by chemotherapy and improved quality of life and survival rate. These results suggest that therapy of lung cancer should be applied with integrated oriental and western medicine from diagnosis to treatment for promoting therapeutic effect. And further study on this therapy should be ensued.

Effect of an Application of Pelvic Compression Belt on the Strength of Hip Flexor in Healthy Young Adult (골반벨트 적용이 건강 성인의 고관절 굴곡근 근력에 미치는 영향)

  • Background : To assess the effect of a pelvic compression belt on the strength of hip flexor in healthy young individuals. Study design: Pre- and post-treatment measurement design on one factor was used. Methods : 30 healthy volunteers (male: 15, female: 15) participated in this study. Dynamometer was used to measure the strength of hip flexor, and measurements were performed before and after the application of the pelvic compression belt at neutral position of hip joint and at 30 cm raised position from floor with straight leg in supine. Results : After the application of the pelvic compression belt, the strengths of hip flexor measured at both positions were significantly increased when compared with before the application (p<.05). However, at neutral position of hip joint and at 30cm raised position from floor, there were significantly different in the changing patterns in the strengths of hip flexor between men and women (p>.05). Conclusion : The findings suggest that the pelvic compression belt is helpful in strengthening hip muscles. With easy application, it is sufficiently feasible for clinical use.

Effects of a Physical Therapy Program on Quality of Life, and Neck and Shoulder Disability in Patients With Head and Neck Cancer (두경부암 환자를 위한 물리치료 프로그램이 삶의 질, 목과 어깨의 장애 정도에 미치는 영향)

  • This study examined the effects of a physical therapy program on quality of life (QOL), and neck and shoulder disability in head and neck cancer patients. The program included neck and shoulder range of motion (ROM) exercises, massage, progressive strengthening exercises, and stretching exercises. Sixteen patients who were assigned to an experimental group performed physical therapy for 40 minutes three times a week for eight weeks. Fifteen other patients were assigned to a control group who did not performed the physical therapy program. The European organization for research and treatment of cancer (EORTC QLQ-C30) and head and neck (EORTC QLQ-H&N) instruments, and the Neck Disability Index (NDI) were assessed before and after the rehabilitation program. The 40-minute program consisted of a 10-minute ROM exercise for the neck and shoulder, a 10-minute massage and 15-minute of progressive resistance exercises, followed by a five-minute stretching exercises. Statistically significant differences were noted for changes in global health, physical function in the EORTC QLQ-C30 and cancer related symptoms in the EORTC QLQ-H&N35 (p<.05). The NDI also showed significant differences (p<.05). Physical therapy may therefore benefit the physical aspects and QOL and improve neck and shoulder disability in patients with head and neck cancer.

The Burden of Cancer in Member Countries of the Association of Southeast Asian Nations (ASEAN)

  • This paper presents the most recent data on cancer rates and the burden of cancer in the ASEAN region. Epidemiological data were sourced from GLOBOCAN 2008 and disability adjusted life years (DALYs) lost were estimated using the standard methodology developed within the World Health Organization's Global Burden of Disease study. Overall, it was estimated there were over 700,000 new cases of cancer and 500,000 cancer deaths in ASEAN in the year 2008, leading to approximately 7.5 million DALYs lost in one year. The most commonly diagnosed cancers were lung (98,143), breast (86,842) and liver cancers (74,777). The most common causes of cancer death were lung cancer (85,772), liver cancer (69,115) and colorectal cancer (44,280). The burden of cancer in terms of DALYs lost was highest in Laos, Viet Nam and Myanmar and lowest in Brunei, Singapore and the Philippines. Significant differences in the patterns of cancer from country to country were observed. Another key finding was the major impact played by population age distribution on cancer incidence and mortality. Cancer rates in ASEAN are expected to increase with ageing of populations and changes in lifestyles associated with economic development. Therefore, ASEAN member countries are strongly encouraged to put in place cancer-control health care policies, focussed on strengthening the health systems to cope with projected increases in cancer prevention, treatment and management needs.

Strength of CNT Cement Composites with Different Types of Surfactants and Doses (분산제의 종류 및 사용량에 따른 CNT 보강 시멘트 복합체의 강도변화)

  • This study was aimed to investigate the difference in strength of Carbon Nanotube (CNT) reinforced cement mortars with different types of surfactants and doses. In the experimental program, CTAB, SDBS and TX10 which were common surfactants adopted to improve CNTs dispersion in fabricating CNT composites in many industrial fields were included and superplasticizer which was revealed to be effective to disperse CNTs especially in CNT reinforced cementitious composites were added as well. Superplasticizer presented less strength reduction in cement mortar and more strength gain by adding CNTs among four types of surfactants. Higher dosage of superplasticizer caused lower strength of cement mortar. Adding CNTs of 0.4 wt.% or less to cement didn't show strength enhancement by adding CNTs but 0.8 wt.% of CNTs resulted in strengthening effect after all. Finally, a combination of 0.1 wt.% of CNTs, superplasticizer and sonication treatment could lead to strength improvement by adding CNTs in cement mortar.
