Objective : This study began with a recognition of the increase in internal disorders caused by functional impairment of the Spleen-Stomach(脾胃) due to modern dietary culture and environment. Method : Way of studying is select and analysis sentences which is include 'Stomach-Yin(胃陰)', 'Stomach-Jin(胃津)', 'Spleen-Stomach-Yin(脾胃陰)', 'Stomach-Yang(胃陽)', 'Spleen-Stomach-Yang (脾胃陽)' in Imjeungjinamuian(臨證指南醫案), and organize relevant paper. Result & Conclusion : In the history of the Spleen-Stomach Theory(脾胃學說), the theoretical mainstream was focused on On-yang(溫養), which was supplemented with methods of sweeten-cool-moisturize(甘凉濡潤) and communication-descent(通降法) by Ye, Tianshi(葉天士). Ye, Tianshi followed the academic theory of Li, Dongyuan(李東垣), putting emphasis on Stomach-gi(胃氣), along with some unique developments of his own. He specifically argued that the Spleen and Stomach be treated separately, and while accepting the methods of Li Dong-yuan in Spleen treatment, asserted that the Stomach-Yin be treated with medicinals with sweet/plain and sweet/cool/moist(甘平與甘凉濡潤) characteristics, and Stomach-Yang be kept firmly, as to prevent obstruction and reflux, by means of medicinals that communicate the Yang softly(通陽柔劑). As a result, the Yin-Yang(陰陽), Dryness-Dampness(燥濕) and Cold-Heat(寒熱) balance of the Yin-Jang (陰臟), Spleen and the Yang-Bu(陽腑), Stomach, the Stomach-Yin and Stomach-Yang was achieved both theoretically and in treatment by means of both communication and supplementation, adding accuracy to clinical application of the Spleen-Stomach Theory.
The aim of the present study was to investigate the development of theory of the deficiency of yin-yang in the spleen-stomach(脾胃). The spleen-stomach theory is a very valuable composition of the oriental medicine. Its first theoretical basis was established by Li Dong Yuan(李東垣) who wrote Piweilun("脾胃論"). He insisted the importance of spleen yang(脾陽) which is the "postnatal base of life" by transforming and transporting of food essence and fluids and raises the clear. After him, his theory had influenced many descendant medical men. one of them, Ye Tian Shi(葉天士) found out the differences of between stomach and spleen. Especially he focused on the stomach yin(胃陰) which is easy to be exhausted by dry-fire(燥火). And he also made another remedy, "the stomach yang should be moved well(宣通胃陽)", which emphasize on that cold and stagnant cause the deficiency of the stomach yang. After that, spleen yin(脾陰) theory was set up by Tang Zong Hai(唐宗海). His spleen yin(脾陰) was deeply related with digestive enzymes and pancreas, because his theory was established by comparing oriental and western medicine. These four theories became the theoretical basis of the deficiency of yin-yang in the spleen-stomach(脾胃), and similar symptom of the spleen-stomach(脾胃) could be categorized according to the pattern identification(辨證) which was developed from these four theory.
In the literatual study on the hiccup, the results were as follows; 1. Hiccup is usually named as Hae yek, Hyel yek, Yel yek. 2. The cause of hiccup are stomach cold, rising of stomach fire, stagnation of vital energy and stagnation of phlegm, yang deficiency of spleen and kideny, deficiency of stomach-yin, or mental disorder due to the stagnation of phlegm, dyspepsia, depressed vital energy. 3. The prescription of hiccup are frequently used Gamchogungangtang Gangwhalbujatang Leejungtang Guelpigungang-tang due to stomach cold, Sosihotang Daesihotang Sojaganggitang due to rising up of stomach fire, Sunbokdaejasuktang due to stagnation of vital energy an dstagnation of phlegm, Bojungikgitang Goakhwanganwyisan Samsoeum due to spleen and kidney yang deficiency, Jaeumganghawtang Daebohoan due to deficiency of stomach yin, Hwanglyenjuklyetang Leejintang Guelpitang due to the stagnation of phlegm, Daewhajungeum due to dyspepsia, Mokhwangjogisan due to depressed vital energy.
The basic principles in the "Spleen-stomach theory(脾胃論)" sets up the phases and roles of spleen-stomach (脾胃) by establishing Earth(地 坤 土) and exposing the reality of spleen-stomach(脾胃) of human body which has its own shape and form with Heaven's reality exhibited. The meaning of Earth is based on the constant meaning of Earth in 'Earth Original-Earth as extended and stable ground(坤元一正之土)' giving form and shape, and Earth's movement with circulation, then exposes itself as 'Earth as plowing land(耕種之土)' concerning both the application of Five Phases and the physical characteristics of Earth. The Yin-Yang recognition on Earth is revealed as Yin Earth(陰土)-Yang Earth(陽土). Spleen(脾) was established as Yin Earth(陰土) and Stomach(胃) as Yang Earth(陽土). The seasonal assignment of Earth is Indian Summer(長夏), which is divided from Summer, and becomes Heat(熱), and the Yin-Yang recognition of Earth comes to be the meaning of the center and border. According to the Five Phasic recognition, it becomes Earth(土) and gets to be Dampness(濕) in accordance with Six Qi(六氣). By examining the relationship between physiological character of stomach and true viscera pulse(眞臟脈) with "Huangdinejing(黃帝內經)", the study shows these fallowing results. Stomach is not only one of the six bowels(六腑), it represents them. It is called as yang brightness of foot(足陽明) because it has relation with function of earth among five phases(五行), great storage(倉廩) and root of five visceras and six bowels(五臟六腑), has close relationship with triple energizers(三焦). Stomach is an important organ that feeds acquired qi(後天之氣) based on food and stomach qi (胃氣) which is a transformed form by going through digestion, is significant for life conservation. In human body, the activation of stomach qi can be known by pulse. The true viscera pulse is a pulse which predicts the period of death. If stomach qi exsits, viscera qi can be led to greater yin of hand(手太陰), however, when it does not exist, five viscera qi can not be led to greater yin of hand and this kind of situation appears the true viscera pulse. Hence, by knowing the relation between stomach qi and five viscera qi, the condition of normality, disease, and death can be known. In "Hwangdineijing" it says, stomach qi, shortly string like pulse(微弦), shortly surging pulse(微鉤), shortly floating pulse(微毛), and shortly sunken pulse(微毛) shown in four seasons are normal pulse. And also it emphasizes, normal pulse is made when four seasons, five viscera pulse, and stomach qi are in harmony. So the dysfunction of spleen and stomach makes dampness-heat obstruction and then makes Internal heat at last. The syndrome of Eum deficiency is applicable to bojungikgitang. As contrasted with Eum deficiency, Eum exuberance is occurred cold-dampness obstruction, which we call 'Eum exuberance'. The syndrome of Yang exuberance is applicable to Ijungtang. In the light of "Somun Jogyongron", We explain that the syndrome of Yin deficiency and Yin exuberance, which was caused by dampness-heat, cold-dampness obstruction and internal damage based on disorder of the spleen and stomach.
This study searched the philosophical and medical thought of mutual understanding and human body. Mutual understanding is a fundamental problem in all branches of oriental studies. In other words, mutual understanding becomes the existential foundation of heaven-earth-human(三才). So human beings, heaven and the earth can't exist if there is no mutual understanding. It comes out the problem of self-consciousness in philosophy, and qi movement pattern like upward, downward, inward and outward movement in traditional korean medicine. Human beings have mutual understandings with heaven and the earth from a macroscopic standpoint, on the other side the human body from a microscopic standpoint. Qi movement is the mutual understanding and response of qi in human body, so with which the physiological functions and pathologic changes of viscera and bowels comes out. Therefore we want to present how to complete qi movement between viscera-bowels and organs to examine mutual understandings in human body closely. The results was summarized as follows; First, upbearing the clear yang and down bearing the turbid yin of spleen-stomach is main pivot of upward and downward of qi movement, and it is true form of mutual understandings between viscera and bowels, so upward, downward, inward and outward movement of whole viscera and bowels can be controlled by spleen-stomach. Second, by restraining relationships between downward heart fire and upward kidney water, heart fire and kidney water have close communications physiologically and pathologically as upper-lower, yin-yang and water-fire. Third, by restraining relationships between upbearing and effusion of liver and purification and down-sending of lung, liver and lung are outer circles of upward and downward movement, so they have antagonistic functions. Firth, by the relationships between upbearing the clear and down bearing the turbid of spleen-stomach and free coursing of liver, free coursing of liver-gallbladder is the essential requirement that it certify transportation and transformation of spleen-earth, so the disease of liver-gallbladder induce spleen-stomach disease pattern to affect its function of transportation and transformation. Fifth, by spleen and kidney, spleen yang is based on kidney yang, so the weakness of kidney yang can affect the function of transportation and transformation of spleen-stomach as it can't warm spleen earth. Sixth, by homogeny of liver-kidney, essence and blood of liver-kidney and ministerial fire of liver-kidney have mutal generation and limitation physiologically and they mutually are affected in pathologically.
Seopcheonsa(葉天士) regarded the cause of Stomach-Eum[胃陰] deficiency as the lack of fluid and humor induced the heat by Eum(陰, yin) deficiency constitution, external pathogen, overexertion and fatigue, misused medication etc. He said that the symptoms of Stomach-Eum deficiency was related to indigestion of food and that it induced the short supply of nutrition for the whole body and the symptoms by disabled Earth[土]-engenders-Metal[金]. He presented the treatment of clearing Stomach-Fire[胃熱] and nourishing Stomach-Eum[胃陰], and used Geumgwemaekmundongtang(金匱麥門冬湯). And he emphasized Stomach-Eum to supplement existing Spleen-Yang[脾陽] centered treatment. Dangjonghae(唐宗海) regarded Spleen-Eum[脾陰] as these, first, it was fluid and humor such as digestive enzyme. Secondly, it was nutritive substance itself. Finally, it contained plaster-oil and blood-agglutination which formed flesh with nutritive substance. Seopcheonsa and Dangjonghae both thought that fluid and humor was necessary to digest food. However, Dangjonghae's Spleen-Eum[脾陰] included nutritive substance as the result of digestion. So we can say that Dangjonghae integrated Spleen-Eum with Stomach-Eum and Spleen-nutrient.
