Prejunctional Modulation of Non-adrenergic Non-cholinergic Relaxation of the Rabbit Proximal Stomach by Potassium Channels (토끼 위 근위부의 비-아드레날린 비-콜린성 이완반응의 포타슘 체널에 의한 접합전 조절작용)

  • The effects of different $K^+$ channel blockers were investigated on the non-adrenergic non-cholinergic (NANC) relaxations in the circular muscle of the rabbit proximal stomach. Non-selective blockers of $K^+$ channels, 4-aminopyridine (4-AP, 3~30${\mu}M$) and tetraethylammonium (TEA, 100~1000${\mu}M$) significantly enhanced the NANC relaxations in a concentration-dependent manner. The enhancement was more prominent for the NANC relaxations induced by the electric field stimulation (EFS) with lower frequencies. Blockers of large conductance $Ca^{2+}$-activated $K^+$ channels, charybdotoxin and iberiotoxin, a blocker of small conduntance $Ca^{2+}$-activated $K^+$ channels, apamin and a blocker of ATP-sensitive $K^+$ channels, glibenclamide had no effect on the NANC relaxations, respectively. Exogeneous administration of nitric oxide (NO, 1~30${\mu}M$) caused concentration-dependent relaxations which showed a similarity to those obtained with EFS. None of the $K^+$ channel blockers had an effect on the concentration-dependent relaxation in response to NO. These results suggest that prejunctional $K^+$ channels regulate the release of NO from the NANC nerve in the rabbit proximal stomach as the inhibition of prejunctional $K^+$ channels increases the NANC relaxation induced by the EFS.

Effects of Prostaglandin $E_2$ on the Spontaneous Contractions and Electrical Activities of the Antral Circular Muscle in Guinea-pig Stomach

  • The spontaneous contractions of gastric smooth muscles are regulated by slow waves, which are modulated by both nervous system and humoral agents. This study was designed to examine the effects of prostaglandin $E_2$ ($PGE_2$) on the contractile and electrical activities of antral smooth muscles in guinea-pig stomach, using an intracellular recording technique. To elucidate the underlying mechanism for its effect on contractility, ionic currents were also measured using a whole-cell patch clamp method. The basal tone by $PGE_2$ was variable, whereas the magnitude of phasic contractions was reduced ($19.0{\pm}2.1%$, n=19). The resting membrane potentials were hyperpolarized ($-4.4{\pm}0.5%$ mV, n=10), and plateau potentials were lowered ($-2.9{\pm}0.5%$ mV, n=10). In most cases, however, the initial peak potentials of slow waves were depolarized more by $PGE_2$ than those of control. The frequency of the slows wave was increased from $5.7{\pm}0.2$ cycles/min to $6.5{\pm}0.2$ (n=22). Voltage-operated $Ca^{2+}$ currents were decreased by $PGE_2$ (n=5). Voltage-operated $K^+$ currents, both Ca-dependent and Ca-independent, were increased (n=5). These results suggest that $PGE_2$ plays an important role in the modulation of gastric smooth muscle activities, and its inhibitory effects on the contractility and activities of slow waves are resulted from both decrease of $Ca^{2+}$ currents and increase of $K^+$ currents.

A Study on Dietary Factors Related to the Incidence of Stomach Cancer and Colon Cancer in Korea (위암 및 대장암의 유발에 영향을 미치는 식이요인에 관한 연구)

  • This study was undertaken to investigate the dietary factors related to the incidence of stomach and colon cancers in Korean. The subjects were 139 stomach and 52 colon cancer patients recruited from 3 general hospitals in Seoul. Food intake, anthropometric measurement, and blood compositions were studied through personal interview and using medical records. Body weight, body mass index, triceps skinfold thickness, body muscle mass of the subjects were lower than reference values. The body weight was reduced after the onset of the illness, which suggests body waste due to the cancers. The patients showed the lower valuies of hemoglobin and hematocrit. Serum protein and calcium were at lower limit of the normal range. Therefore the untritional status assessed by anthropometry and blood composition should be said to be marginal. The average intake of most of the nutrients except Ca of the subjects met the RDA, but the range was wide and the nutrient intake of large proportion of the subjects feel below 75% of RDA. The food intake of egg, milk, legumes, and fruts were lower than national average, on the other hand the subjects had higher intake of meat, vegetables, and fats. The subjects drank and smoked heavily, stomach cancers being more severs. From this results, dietary risk factors for the stomach and colon cancers in Korean did not agree with the reports of western societies. Even though the intakes of meat and animal food of colon cancer patients can not be classified as high, it was higher than stomach cancer and national average. Therefore it could be concluded that with increasing consumption of animal food, the incidence of colon cancer would be increased in Korea.

The Antiulcer Effects of Alove vera on the Stomach ulcer Induced by Stress in rats (흰쥐의 스트레스성 위궤양에 대한 Alove vera의 항궤양작용)

  • This was performed to investigate the antiulcer effects of Aloe vera on the stomach ulcer induced by restraint and water-immersion stress in rats. For this experiment, 60 male Sprague-Dawley strain were used. The experimental groups were divided into five: a control(CA) and 4 aloe treatment groups. Each dose of aloe was 50 mg/kg BW(AA), 100mg/kg BW(AB), 200 mg/kg BW(AC), and 400 mg/kg BW(AD). The rats were allocated to each group by 12 and observed for 4 weeks. The results are as following. 1. The stomach surface pH in each group showed no significant difference, byt the values of aloe treatment groups were higher than the value of the control group. 2. The gastric wall mucus was significantly increased in all aloe treatment groups(p<0.05) compared with the control group. Especially in AC and AD group the differences were higher(p<0.01). 3. At shear rate rate 11.25, 45.0, 90.0, 225.0 sec-1, whole blood viscosity and plasma viscosity were measured. Most of the values of aloe treatment groups were significantly low compared with those of the control group(P<0.05). 4. The ulcer index of aloe treatment groups were significantly low campared with control group(p<0.05). Especially in AC and AD group the differences were more significant(p<0.01). 5. Less severe ulcers were observed in AA and AB group than in the control group. Tissues of AC and AD group had only slight ulcers and necrosis of tissue was not observed in these groups. Especially in AD group, there was more mucus than other groups and it seemed that alove vera stimulated the epithelial regeneration. From the results of this study, it can be concluded that the oral administration of Alove vera results in protection of stomach ulcer by stimulating the secretion of gastric mucus and the circulation of blood.

Properties of Spontaneous Activity in Gastric Smooth Muscle

  • Mammalian gastric smooth muscles generate spontaneous rhythmic contractions which are associated with slow oscillatory potentials (slow waves) and spike potentials. Spike potentials are blocked by organic $Ca^{2+}-antagonists,$ indicating that these result from the activation of L-type $Ca^{2+}-channel.$ However, the cellular mechanisms underlying the generation of slow wave remain unclear. Slow waves are insensitive to $Ca^{2+}-antagonists$ but are blocked by metabolic inhibitors or low temperature. Recently it has been suggested that Interstitial Cells of Cajal (ICC) serve as pacemaker cells and a slow wave reflects the coordinated behavior of both ICC and smooth muscle cells. Small segments of circular smooth muscle isolated from antrum of the guinea-pig stomach generated two types of electrical events; irregular small amplitude (1 to 7 mV) of transient depolarization and larger amplitude (20 to 30 mV) of slow depolarization (regenerative potential). Transient depolarization occurred irregularly and membrane depolarization increased their frequency. Regenerative potentials were generated rhythmically and appeared to result from summed transient depolarizations. Spike potentials, sensitive to nifedipine, were generated on the peaks of regenerative potentials. Depolarization of the membrane evoked regenerative potentials with long latencies (1 to 2 s). These potentials had long partial refractory periods (15 to 20 s). They were inhibited by low concentrations of caffeine, perhaps reflecting either depletion of $Ca^{2+}$ from SR or inhibition of InsP3 receptors, by buffering $Ca^{2+}$ to low levels with BAPTA or by depleting $Ca^{2+}$ from SR with CPA. They persisted in the presence of $Ca^{2+}-sensitive$ $Cl^--channel$ blockers, niflumic acid and DIDS or $Co^{2+},$ a non selective $Ca^{2+}-channel$ blocker. These results suggest that spontaneous activity of gastric smooth muscle results from $Ca^{2+}$ release from SR, followed by activation of $Ca^{2+}-dependent$ ion channels other than $Cl^-$ channels, with the release of $Ca^{2+}$ from SR being triggered by membrane depolarization.

  • Ghrelin is a newly discovered peptide, which is released from the stomach and neurons in the hypothalamic arcuate nucleus (ARC), and potently stimulates growth hormone release and food intake. In the present study, we investigated the effect of ghrelin on $[Ca^{2+}]_i$ in thyroid FRTL-5 cells. Ghrelin (5 nM) increased $[Ca^{2+}]_i$ and TSH (1 unit/l) had an additive effect on $[Ca^{2+}]_i$ when extracellular normal solution was 1.1mM $Ca^{2+}$ containing Coon's modified Ham's F12 medium. When $Ca^{2+}-free$ medium containing 2 mM EGTA replaced the above normal solution, ghrelin also induced a similar rise in $[Ca^{2+}]_i$. In the middle of $[Ca^{2+}]_i$ increment by ghrelin, nifedipine $(1\;{\mu}M)$, nickel $(100\;{\mu}M)$ and $La^{3+}\;(100\;{\mu}M)$ had no effect on $[Ca^{2+}]_i$. After endoplasmic reticulum was depleted by cyclopiazonic acid $(CPA;10\;{\mu}M)$, ghrelin caused no visible change on $[Ca^{2+}]_i$ in $Ca^{2+}-free$/2 mM EGTA solution. These results suggest that ghrelin can increase $[Ca^{2+}]_i$ through endoplasmic reticulum in thyroid FRTL-5 cells.

Clinical Experience of Acute Pericarditis with Effusion (삼출액을 동반한 급성심낭염의 임상적 고찰)

  • Clinical experience of 48 acute pericarditis with effusion was reviewed and presented. There were 28 male and 20 female patients ranging from 3 years to 77 years old. Malignant effusion; Twenty patients had underlying malignancy. These etiologies were lung ca[8 patients, 40%], breast ca[7 patients 35%], lymphoma[2 patients, 10%], esophageal ca[1 patients, 5%], stomach ca[1 patient, 5%], ovarian ca[1 patient, 5%]. Uremic effusion; 15 patients with renal failure required surgical intervention. Traumatic effusion; 7 patients had traumatic pericarditis. These etiologies were stab wound [5 patients, 71.4%] and aspiration[2 patients, 28.6%]. Pyogenic effusion: 6 patients had pyogenic pericarditis. These etiologies were empyema thoracis[3 patients, 50%], liver abscess[2 patients, 33.3%], pneumonia[1 patient, 16.7%]. The patients were treated by pericardiocentesis, subxiphoid tube drainage, pericardiectomy: 4 of them underwent pericardiocentesis; 37, subxiphoid tube drainage; 5, pericardiectomy. We conclude that subxiphoid tube pericardial drainage was effective for treatment of pericardial effusion.

The Excitatory Mechanism of Substance P in the Antral Circular Muscle of Guinea Pig Stomach

  • This study was carried out to elucidate the excitatory mechanisms of Substance P in the antral circular muscle, using isometric contraction recording, conventional microelectrode method and whole-cell patch clamp technique. Substance P produced tonic and phasic contractions in a dose-dependent manner and depolarized membrane potential with increased amplitude of slow waves in muscle strips. Voltage-dependent $Ca^{2+}$ currents were increased by the application of Substance P from a holding potential of -60mV to 50mV in 10mV steps and this effect was blocked by the addition of an antagonist. Also Substance P increased transient and spontaneous oscillatory $K^+$ outward currents. The enhanced outward currents were abolished by apamin in dispersed single cells. These results suggest that the depolarization of membrane potential by Substance P activates voltage-dependent $Ca^{2+}$ channels, which represents an excitatory response in the antral circular muscle and led to an increase in $Ca^{2+}\;activated\;K^+\;currents$.

Clinicopathological Significance of Elevated PIK3CA Expression in Gastric Cancer

  • Purpose: PIK3CA is often mutated in a variety of malignancies, including colon, gastric, ovary, breast, and brain tumors. We investigated PIK3CA expression in gastric cancer and explored the relationships between the PIK3CA expression level and clinicopathological features as well as survival of the patients. Materials and Methods: We examined PIK3CA expression in a tissue microarray of 178 gastric adenocarcinomas by immunohistochemistry and reviewed patients' medical records. Results: In our study, 112 of the 178 gastric cancer patients displayed positive PIK3CA expression. Overexpression of PIK3CA was correlated with low grade differentiation (P=0.001), frequent lymphatic invasion (P=0.032), and high T stage (P=0.040). Patients with positive PIK3CA staining were more likely to display worse overall survival rate than those with negative PIK3CA staining, as determined by Kaplan-Meier survival analysis with log-rank test (P=0.047) and a univariate analysis using the Cox proportional hazard model (hazard ratio=1.832, P=0.051). Conclusions: Elevated PIK3CA expression was significantly correlated with tumor invasiveness, tumor phenotypes, and poor patient survival.

  • Capsaicin, a major pungent ingredient in red hot pepper, has long been used in food additives and drugs. We have previously reported that capsaicin induces apoptosis in Korean stomach cancer cell line, SNU-1. In the present study, the mechanism of capsaicin-induced apoptotic cell death was investigated in SNU-1. Treatment of capsaicin to SNU-1 produced dose-dependent increase of apoptotic cell death and [Ca2+]$_{i}$ concentrations.(omitted)d)
