For pain relief my collegue and I used thin acupuncture needles as electrodes in electric stimulation. The needles were inserted into a trigger point and into another point located in the same muscle instead of meridian points of electro-acupuncture. Low frequency electric stimulation was given through the needles to 130 patients for 15 min. The results were as follows In 25 acute sprain patients electric stimulation was given $3.14{\pm}1.12$ times and the pain was reduced on the average by $83.00{\pm}6.77%$ (VAS). In 45 chronic sprain patients electric stimulation was given $5.51{\pm}1.38$ times and the pain was reduced on the average by $70.22{\pm}8.98%$ (VAS). In 28 myofascial pain syndrome patients electric stimulation was given $6.22{\pm}1.25$ times and the pain was reduced on the average by $66.48{\pm}8.75$(%). In 7 muscle contraction headache patients electric stimulation was given $4.14{\pm}1.57$ times and the pain was reduced on the average by $75.00{\pm}9.57%$ (VAS). In 25 radiculopathy patients electric stimulation was given $4.73{\pm}1.131$ times and the pain was reduced on the average by $21.37{\pm}9.31%$ (VAS). We he conclude that electric stimulation therapy using acupuncture needles is very effective in acute sprain, chronic sprain, myofascial pain syndrome and muscle contraction headache. Any doctor with knows anatomy and trigger points can practice this method without studying oriental medicine or difficult acupuncture techniques.

Purpose : The purpose of study was to better understand the effects of Interferential Current (IFC) and Ultrasound on the ROM and pain in patients with chronic back muscle pain. Methods : Twenty patients with chronic back pain were randomly assigned to IFC stimulation groups and Ultrasound stimulation groups. Both groups used the same method for 20 minutes each session, three times a week for 4 weeks at the same time point and with the same amount of treatment. Measurement items are visual analogue scale (VAS) and range of motion (ROM). Results : This study showed that the IFC stimulation group and the ultrasound stimulation group demonstrated significant improvement in ROM increase and pain reduction. Both groups showed a significant reduction in VAS; however, the ultrasound stimulation group decreased more than ultrasound stimulation group. And, both groups showed a significant increase in ROM. The ultrasound stimulation group increased significantly more than the IFC stimulation group in ROM of flexion and extension. The IFC stimulation group increased significantly more than the ultrasound stimulation group in ROM of lateral flexion. Conclusion : Pain relief was more effective in the ultrasound group. The ultrasound showed a more significant effect than IFC in ROM of flexion and extension. The IFC showed a more significant effect than ultrasound in ROM lateral flexion. The results of this study suggest that the treatment effects are different on patients with chronic back pain, according to treatment methods.
Purpose: The purpose of study was : 1) to analyse the trend of research on the sensory stimulation in newborn in Korea, 2) to suggest direction for further study on the sensory stimulation in newborn and contributing to the use of intervention in newborn. Method: Research studies on the sensory stimulation in newborn were selected from the dissertations, parents-child health. Academic Society Journal, and from Korean Journal of Pediatrcs which were conducted between 1984 and 2002. The total numbers of the studies were 15. These studies were analyzed for 1) time of publication or presentation 2) thesis for a degree or nondegree 3) characteristics of subjects 4) sample size 5) research design 6) type of sensory stimualtion 7) meaurement tool 8) result. Result : 1)The most year of studies were the 1996's~ 2000's. 2) The degree of studies were higher than nondegree. 3) The most characteristics of subjects were the premature infants. 4) The most frequent sample sizes were 11-20. 5) The all research design was nonequivalent control group pretest-posttest design in quasi experimental basis. 6) The most type of sensory stimulation was Field massage(tactile and kinesthetic). 7) The measurement tool used all objective tools. 8) The most research data were collected by the measure of the weight, heart rate, behavior state, respiration, stress hormon and positive results. Conclusion: 1) Human sensory stimulation did not seem to harm the infants. 2) several positive effects of sensory stimulation that have been reported previously must be reevaluated. 3) further research will be required large sample size, long-term outcome, individualized stimulation program, early parent-infant contact in order to enhance the development of attachment, to determine the underlying mechanism and theorical frame for these stimulation effect.
Sleep disorders, increasingly prevalent in the general population, induce impairment in daytime functioning and other clinical problems. As changes in cortical excitability have been reported as potential pathophysiological mechanisms underlying sleep disorders, multiple studies have explored clinical effects of modulating cortical excitability through non-invasive brain stimulation in treating sleep disorders. In this study, we critically reviewed clinical studies using non-invasive brain stimulation, particularly transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS), for treatment of sleep disorders. Previous studies have reported inconsistent therapeutic effects of TMS and tDCS for various kinds of sleep disorders. Specifically, low-frequency repetitive TMS (rTMS) and cathodal tDCS, both of which exert an inhibitory effect on cortical excitability, have shown inconsistent therapeutic effects for insomnia. On the other hand, high-frequency rTMS and anodal tDCS, both of which facilitate cortical excitability, have improved the symptoms of hypersomnia. In studies of restless legs syndrome, high-frequency rTMS and anodal tDCS induced inconsistent therapeutic effects. Single TMS and rTMS have shown differential therapeutic effects for obstructive sleep apnea. These inconsistent findings indicate that the distinctive characteristics of each non-invasive brain stimulation method and specific pathophysiological mechanisms underlying particular sleep disorders should be considered in an integrated manner for treatment of various sleep disorders. Future studies are needed to provide optimized TMS and tDCS protocols for each sleep disorder, considering distinctive effects of non-invasive brain stimulation and pathophysiology of each sleep disorder.
Background : This study focuses on the new acupuncture method of embedding method which inserts a substance on the acupuncture points for continuous stimulation. Clinical applications and cautions were examined through literary investigations. Results : Based on the literary consideration of embedding method, the following results were obtained : 1. Embedding method is a combination of traditional and embedding technique to provide longer duration of stimulation on the acupuncture points. 2. To administer the embedding method, one needs to utilize a embedding thread besides acupuncture apparatus. Sheep gut is commonly used in China and the surgical thread is the choice in Korea. 3. Embedding method may vary from the patient to patient, depending on the nature and location of the illness. Piercing, embedding, and tying are some of the possibilities. 4. Embedding method may have different arrangement of threads based on the choice of usage. 5. Embedding method is effective for various chronic illnesses such as aches, functional diseases, and the diseases of internal organs. 6. When using the embedding methods, cautions against infection and side effects due to strong stimulation are mandatory.
Objectives : We implemented the Moxa-Extract Moxibustion Method in order to improve the conventional moxibustion therapy. This method is aimed to eliminate burning wounds and smoke, which are the defects of conventional moxibustion therapy. And we performed to verify the efficiency by comparing the Moxa-Extract Moxibustion Method with the conventional Indirect Moxibustion Method. We measured the body heat and the lasting time of blood circulation improveent using thermography. We implemented the Moxa-Extract Moxibustion Method in order to improve the conventional moxibustion therapy. This method is aimed to eliminate burning wounds and smoke, which are the defects of conventional moxibustion therapy. And we performed to verify the efficiency by comparing the Moxa-Extract Moxibustion Method with the conventional Indirect Moxibustion Method. We measured the body heat and the lasting time of blood circulation improvement using thermography. Methods : The moxibustion therapy has two kinds of effects: The formers are pharmacological effects of the Moxa's vasodilators and antioxidants. The latters are thermal effects which cause improvement of the blood circulation. To remove the demerits without omission of above therapeutic effects, we extracted the vasodilators and antioxidant compounds from the Moxa-CH2Cl2 fraction Moxa-EtOA and composed the moxibustion kit with (Ba0.8 Sr0.2)0.996 Y0.004 TiO2 + 0.5WT SiO2% positive Temperature Coefficients Thermistor. The experimental demonstrations have been made by the stimulating the spot which is CV4(Kwan-Won), CV8(Shin-Guel), CV12(Jung-Wan) acupuncture points of the conception vessel meridian(CV). And stimulating time was one hour. We divided the subjects into 5 groups such as no stimulation group, conventional Indirect Moxibustion group, only Moxa-Extract stimulation group, only heat stimulation group, and Moxa-Extract Moxibustion group. In the different cases, we have measured the body heat in pre-stimulation, just after stimulation, 2 hours after, and 4 hours after. Results : he body heats of the group who were stimulated by the Moxa-Extract Moxibustion method were increased by over the $2^{\circ}C$. And the body heats of the group who were stimulated by the Indirect Moxibustion Method were increased by average the $1^{\circ}C$. We have evaluated that the Moxa-Extract Moxibustion Method is improvement on the Indirect Moxibustion Method by the increasing rate is 200% and increasing time is 150% with the body heat of the abdominal region. Conclusion : We have implemented the Moxa-Extract Moxibustion Method and evaluated the efficiency of the Moxa-Extract Moxibustion Method comparing with the Indirect Moxibustion Method.
Purpose : Transcranial electromagnetic stimulation(TMS) is a noninvasive method which stimulates the central nervous system through pulsed magnetic fields without direct effect on the neurons. Although the neurobiologic mechanisms of magnetic stimulation are unknown, the effects on the brain are variable according to the diverse stimulation protocols. This study aims to observe the effect of the magnetic stimulation with two different stimulation methods on the cultured hippocampal slices. Methods : We obtained brains from 8-days-old Spague-Dawley rats and dissected the hippocampal tissue under the microscope. Then we chopped the tissue into 450 µm thickness slices and cultured the hippocampal tissue by Stoppini's method. We divided the inserts, which contained five healthy cultured hippocampal slices respectively, into magnetic stimulation groups and a control group. To compare the different effects according to the frequency of magnetic stimulation, stimulation was done every three days from five days in vitro at 0.67 Hz in the low stimulation group and at 50 Hz in the high stimulation group. After N-methyl-D-aspartate exposure to the hippocampal slices at 14 days in vitro, magnetic stimulation was done every three days in one and was not done in another group. To evaluate the neuronal activity after magnetic stimulation, the $NeuN/{\beta}$-actin ratio was calculated after western blotting in each group. Results : The expression of NeuN in the magnetic stimulation group was stronger than that of the control group, especially in the high frequency stimulation group. After N-methyl-D-aspartate exposure to hippocampal slices, the expression of NeuN in the magnetic stimulation group was similar to that of the control group, whereas the expression in the magnetic non-stimulation group was lower than that of the control group. Conclusion : We suggest that magnetic stimulation increases the neuronal activity in cultured hippocamal slices, in proportion to the stimulating frequency, and has a neuroprotective effect on neuronal damage.

We studied the effects of vibration stimulation method on upper limbs spasticity in patients with brain lesion. 21 patients with spasticity of the upper limbs selected and divided randomly 3 groups. And then vibratory stimulation was applied to the triceps brachii muscle in group I(n=7), to biceps brachii muscle in group II (n=7), and to both muscles in group III (n=7). Using Neuro-EMG_Micro to investigate the changes in spinal neuronal excitability, F-waves were measured at before and directly after stimulation, and 10 minutes later and 20 minutes later after stimulation especially. MAS(Modified Ashworth Scale) test for muscle tone and MFT(Manual Function Test) for the upper extremity motor function were performed before stimulation and 20 minutes later after stimulation for the purpose of clinical evaluation. In our study, MAS was significant decreased in all groups, F wave and F/M ratio parameters were decreased in all groups and more decreased specially in group III. MFT was increased in group II and III, and more increased specially in group III. Vibration stimulation reduced the neuronal excitability of spinal cord and also muscle tone, and improved the motor function of the upper extremity. These results suggested that vibration stimulation giving to both muscles(triceps and biceps brachii muscle) at the same time was more efficiency in reducing the neuronal excitability of spinal cord and improving the motor function of the upper limbs.

Background: Spinal cord stimulation is a well-established method for the management of several types of chronic and intractable pain. This form of stimulation elicits a tingling sensation (paresthesia) in the corresponding dermatomes. The goal of this study was to establish a correlation between the spinal levels of the implanted epidural electrodes and the paresthesia elicited due to stimulation of the neural structures. Methods: Thirty five patients, who received trial spinal cord stimulation, were evaluated. After the insertion of the lead to the selected position, the areas of paresthesia evoked by stimulation were evaluated. Results: Seventy-one percent of cases showed paresthesia in the shoulder area when the tip of the electrode was located between the C2-C4 levels. At the upper extremities, paresthesia was evoked in 86-93% of cases, regardless of the location of the electrode tip within the cervical spinal segments. The most common tip placement of the leads eliciting hand stimulation was at the C5 level. The most common level of electrode tip placement eliciting paresthesia of the anterior and posterior thigh and the foot were at the T7-T12, T10-L1 and T11-L1 vertebral segments, respectively. Conclusions: Detailed knowledge of the patterns of stimulation induced paresthesia in relation to the spine level of the implanted electrodes has allowed the more consistent and successful placement of epidural electrodes at the desired spine level.

The purpose of this study was to investigate how the modulation method affects the effectiveness of eliciting tactile sensations by electrical stimulation. Two methods were employed and the results were compared and analyzed; pulse amplitude modulation (PAM) and pulse width modulation (PWM). Thirty-five healthy subjects participated in the experiments to measure the stimulation intensity that began to elicit a tactile sensation - activation threshold (AT). Constant-current monophasic rectangular pulse trains were employed, and the stimulation intensity was varied from zero until the subject felt any uncomfortable sensation. The step size of the stimulation intensity was 100nC/pulse. After each experiment, the subject described the sensation both quantitatively and qualitatively. The two modulation methods did not make a significant difference as far as the AT values were concerned, but most of the subjects showed 'intra-individual' consistency. Also, it was confirmed that our range of the stimulation parameters enabled us to obtain three major tactile sensations; tickling, pressure and vibration. The results suggested that the stimulation parameters and the modulation type should be selected for each individual and that selective electrical stimulation of the mechanoreceptors needs more diversified researches on the electrode design, multi-channel stimulation protocol, waveforms of the pulse train, etc.
