2-DG Autoradiographic Imaging of Brain Activity Patterns by Electroacupuncture Stimulation in Awake Rats (전침자극(電針刺戟)에 의한 흰쥐 중추신경계(中樞神經系)내 대사활성(代謝活性) 변화(變化)의 영상화(映像化) 연구(硏究))

  • Objective : Functional brain mapping study on acupuncture stimulation using the [14C]2-deoxyglucose([14C]2-DG) autoradiography provides quantitative data and visualized pathway in central nervous system(CNS). We aimed to investigate the neural pathway and spatial distribution of metabolic activity elicited in CNS on electroacupuncture stimulation using [14C]2-DG autoradiography. Methods : The study were divided into three groups by stimulation times. 45-mins stimulation group according to Sokoloffs method, 5-mins stimulation group according to Duncun's method, and 15-mins stimulation group. ;A venous catheter was equipped into right jugular vein. The rats (Sprague-Dawley rats, 230-260g) were kept fastened loosely on a holding platform without anesthesia. Electroacupuncture stimulation (5 ms, 2 Hz, 1~3 mA) were applied on the left Zusanli (ST36) acupoint and [14C]2-DG ($25{\mu}Ci/rat$) injection was performed through the catheter. After sacrifice, the brain and the spinal cord were made to sections for film image. The film images were digitalized as the isotope concentration based upon comparison of optical densities with that of the standards and normalized by the optical density of corpus callosum. Results : 1. 15-mins stimulation group was most effective among 3 experiments. 2. On 15-mins stimulation group, medial geniculate nucleus, intetpeduncular nucleus intermedius, ventral periolivary nucleus, caudal periolivary nucleus, medial superior olive, lateral paragigantocellular nucleus, including hypothalamic arcuate nucleus were increased by more than 25% (at least, p<0.05) by electroacupuncture stimulation. 3. Especially, the metabolism in hypothalamic arcuate nucleus was increased by 90% (p<0.05). 4. The fact that arcuate nucleus of hypothalamus might play a role of interconnection area between ascending and descending pathway of acupuncture stimulation was demonstrated visually. Conclusions : Advanced study on electroacupuncture stimulation elicited significant increase of metabolic activity in various nuclei of hypothalamus will provide the important experimental basis in research of the relationship between electroacupuncture stimulation and internal visceral functions.

Comparison of Skin Stimulation Method and Topical Anesthetic Cream on Pain and Heart Rate Variability during Arteriovenous Fistula Puncture in Hemodialysis Patients (혈액투석 환자의 동정맥루 천자 시 피부자극법과 국소마취크림 도포에 따른 통증과 심박변이도 비교)

  • Purpose: The purpose of this was to compare effects of application of the skin stimulation method and topical anesthetic cream on pain, heart rate variability and satisfaction according to nursing intervention methods during arteriovenous fistula puncture in chronic renal failure hemodialysis patients. Methods: This study was a crossover design. Participants were 36 patients with chronic renal failure receiving hemodialysis treatment. Two forms of intervention were applied to participants, and then pain and heart rate variability were measured during the puncture. Results: There were no statistically significant differences according to each treatment in vein pain and artery pain. Also, there were no statistically significant differences according to each treatment in stress index, sympathetic activity (LF), parasympathetic activity (HF) and sympathetic activity/parasympathetic activity (LF/HF ratio). Satisfaction with application of skin stimulation method was statistically higher than that of topical anesthetic cream application. Conclusion: This suggests that application of the skin stimulation method complements disadvantages of topical anesthetic cream application and demonstrates possibility of application as a nursing intervention method which can be conveniently used by nurses in clinical practice.

A Comparison of Muscle Contraction Using Functional Electrical Stimulation: Intermittent High Frequency Alternating Stimulation Versus Intermittent Low Frequency Synchronous Stimulation (기능적 전기자극기를 이용한 간헐적 고주파 교대자극과 간헐적 저주파 동시자극의 근 수축력 비교)

  • Functional electrical stimulation (FES) training of the knee extensors is a useful way to rehabilitate the ability to stand and walk. However, training using FES has not been able to solve the problem of fatigue; clinical application of FES quickly produces muscle fatigue, due to the continuous activation of the muscles of the lower extremity. Therefore, reduction of muscle fatigue is an important factor in increasing the effectiveness of FES training in paraplegia. Intermittent high frequency alternating stimulation is a method that combines the advantages of high frequency (leading to strong muscle contractions) and alternating stimulation (reducing muscle fatigue), thereby continuously strengthening muscles. It is not known whether low frequency simultaneous stimulation results in stronger muscle contraction than high frequency alternating stimulation. This study compared the effectiveness of high frequency alternating stimulation with low frequency synchronized stimulation. Muscle power using FES on the quadriceps of 20 normal subjects were compared. Intermittent high frequency alternating stimulation did not produce more powerful muscle contraction than intermittent low frequency synchronized stimulation, because the muscle characteristics differed individually. Significant individual variation according to muscle characteristics was founded when applying FES. Accordingly, when physical therapists use FES to treat patients, they must be aware of individual variation in muscle characteristics.

Physiological and Psychological Effects of Vibroacoustic Stimulation to Scapular and Sacrum of Supine Position

  • Objective: This research measured physiological and psychological effects of Vibroacoustic stimulation(VA) to scapular and sacrum of supine position on the mattress. Background: When vibroacoustic stimulation applies to human body, it has a positive influence on physiological and psychological effects by stimulating the organs, tissues and cells of whole body. Method: This experiment was conducted to 10 normal males in two conditions: no stimulation and vibroacoustic stimulation. No stimulation experiment was executed as a supine position for 30 minutes without any vibrational stimulus, while vibroacoustic stimulation was transmitted by the vibrational speaker, which uses 40Hz frequency. Subjects had a laser Doppler flowmeter probe in scapular, sacrum, and also had 8 channel electroencephalogram(EEG) measurement sensor in the scalp. Blood pressure and skin temperature were measured in two conditions with an underlying posture for 30 minutes. Additionally, blood flow rate and EEG were measured before and after for two minutes on two conditions. Results: According to the vibroacoustic stimulation, blood flow rate and skin temperature were increased, while blood pressure was decreased. When using vibroacoustic stimulation compared to no stimulation, blood flow rate went approximately two times higher, and skin temperature also higher 3~4 times. Furthermore, the relative alpha power of brain wave was significantly increased when we applied to vibroacoustic stimulation. Conclusion: This experiment tested the VAT embedded in mattress in two conditions. According to this experiment, VAT decreases blood pressure, improves not only a physiological effect on blood flow rate as well as skin temperature, but also psychological functions by increasing relative alpha power. Application: The results of the publishing trend analysis might help physiological and psychological effects of vibroacoustic stimulation.

The Change of the Vital Sign by the Variables of Stimulated Areas in Interferential Current Treatment (간섭 전류 치료의 자극부위에 따른 활력징후의 변화)

  • Background: The purpose of this study is to have examined the influence on the blood circulation by comparing the differences between stimulating the sympathetic ganglion and the muscle group among the stimulation variables in interferential current stimulation. Method: The object of the study is the twenties(M=8, F=12), who are in great condition and have no pathological report for the blood circulation influence. The intensity of the inferential current stimulation is the medium degree, 100 bps constant current, which is the comfort and degree to confirm the muscle contraction. The areas stimulated are the stellate ganglion area in the seventh cervical vertebrae and the forearm muscle area. Results: We have made sure that there is no change in blood pressure and pulse and that the change in the skin temperature occurred highly. Conclusion: In considering the change of the blood circulation in case of stimulation area by the inferential current stimulation, we have seen that stimulating the sympathetic ganglion area is more effective than stimulating muscle area directly.

The Analysis of the Muscle Fatigue Recovery Effect on LR9 by Compound Stimulation of the PEMFs and LED (음포혈(陰包, LR9)에서 펄스형 전자기장 및 가시광 LED 복합 자극이 대퇴부 근피로 회복에 미치는 영향 분석)

  • Objectives : The aim of this study was to develop the combined medical stimulus system consisted of the PEMFs (Pulsed electromagnetic fields) and LED which are able to stimulate local point such as acupoints and trigger points. Methods : To evaluate the therapeutic effect on the musculoskeletal disorders and the possibility of alternative method on manual acupuncture, we compared the fatigue recovery of two groups by analyzing the EMG and peak torque (non-stimulation and, stimulation group) after strenuous knee exercise. We chose the LR9 as a stimulation point. Results : The median frequency (MF) and fatigue index (F.I) of the stimulation group were recovered faster than those of the non-stimulation group. Also the peak torques of both groups were not restored until after 20 minutes. However, the peak torque of the stimulation group was regained higher than that of the non-stimulation group. Conclusions : We confirmed that the proposed combined stimulus system had useful effects as treatment instrument of musculoskeletal disorder using non-invasive method of PEMFs and LED.

The change of the peripheral blood circulation by the method of interferential current stimulation (간섭전류 자극 방법에 따른 말초혈류의 변화)

  • Purpose : The purpose of this study is to have examined the influence on the blood circulation by comparing the differences between stimulating the sympathetic ganglion and the muscle group among the stimulation variables in interferential current stimulation and to have found out the most effective stimulating mode for the improvement the peripheral blood circulation in the interferential current stimulation. Method : The subjects of the study is the men and women in the twenties, who are in great condition and have no pathological report for the blood circulation influence. The intensity of the inferential current stimulation is the medium degree, 100 bps constant current, which is the comfort and overt degree to confirm the muscle contraction. The areas stimulated by the interferential current stimulation are the stellate ganglion area in the seventh cervical vertebrae and the forearm muscle area. The stimulating time is twenty minutes long. After stimulating the two areas, the change of the blood circulation has been measured. Results : Both stimulating the sympathetic ganglion and stimulating the muscle, before and after interferential current stimulation, we have seen that the amount of blood circulation was increased significantly and statistically in both two groups(p<.01) and that the amount of blood circulation was increased significantly and statistically in the case of stimulating the sympathetic ganglion(p<.01). Conclusion : The conclusion we received that inferential current stimulation had the clear influence on increasing the peripheral blood circulation. And stimulating the sympathetic ganglion area is more effective than stimulating muscle area directly.

Effects of Microcurrent Stimulation on c-fos and Calcitonin Gene-Related Peptide Expression in the Spinal Cord on Rats Induced Pain (통증유발 백서에서 미세전류자극이 척수 분절 내 c-fos 및 CGRP 발현에 미치는 영향)

  • The purpose of study is that we will observe the change of c-fos and CGRP with the immunohistochemistry method and then we will study the effect of microcurrent stimulation following the frequency after inducing pain to rats with capsaicin. The experimental groups were divided by microcurrent application and pain induce. Normal control groups is used in experiment I, the group which we induce pain is used in experiment II, the application group which we induce pain and then the high frequency microcurrent stimulation is used in experiment III, the application group which we induce pain and then the low frequency microcurrent stimulation is used in experiment IV. c-fos was strongly expressed after pain induced 2 hours and positive neurons were decreased from 2 hours. At 7 days, positive neuron recovers to normal range, But c-fos positive neuron of microcurrent stimulation group were decreased from 2 hours. CGRP was strongly expressed after pain induced 24 hours, and positive neurons were decreased from 7 days. These results suggests that microcurrent stimulation therapy effect to control pain according to expression of c-fos and CGRP examined by immunohistochemistry. Also high frequency microcurrent stimulation is more effective than low frequency microcurrent stimulation for controling the pain.

  • The purpose of this study is to design electrical stimulation system fur stroke patients with pharyngeal dysfunction(dysphagia). Dysphagia is that the food has trouble in passing to the mouth from the stomach. Although we have many treatment methods for dysphagia, electrical stimulation system will be useful for stroke patients having dysphagia. Electrical stimulation system can be divided into body and electrodes. The body stimulation is composed to frequency counter, time control and current measurement part. These parts are to control the frequency, stimulating time and current intensity And they can be variable according to the patient's clinical assessment. The electrode plays a role to deliver the current from the system to the muscle. Also the position of the electrode can be variable according to the treatment method. We performed the clinical experiment with stroke patients who had swallowing disorder. The videofluoroscopy was used fur the observation. From the result of clinical experiment based on electrical stimulation, we expected that the dysfunction(in pharynx) level of the patient could be improved. However we could not have enough effectiveness of the treatment because of the number of patients, patient's adaptation and treatment period. We will design the optimized electrical stimulation system based on enough clinical experiment in the future.

The Characteristics on the Change of Cerebral Cortex using Alternating Current Power Application for Transcranial Magnetic Stimulation

  • A transcranial magnetic stimulation device is a complicated appliance that employs a switching power device designed for discharging and charging a capacitor to more than 1 kV. For a simple transcranial magnetic stimulation device, this study used commercial power and controlled the firing angle using a Triac power device. AC 220V 60 Hz, the power device was used directly on the tanscranial magnetic stimulation device. The power supply device does not require a current limiting resistance in the rectifying device, energy storage capacitor or discharge circuit. To control the output power of the tanscranial magnetic stimulation device, the pulse repetition rate was regulated at 60 Hz. The change trigger of the Triac gate could be varied from $45^{\circ}$ to $135^{\circ}$. The AVR 182 (Zero Cross Detector) Chip and AVR one chip microprocessor could control the gate signal of the Triac precisely. The stimulation frequency of 50 Hz could be implemented when the initial charging voltage Vi was 1,000 V. The amplitude, pulse duration, frequency stimulation, train duration and power consumption was 0.1-2.2T, $250{\sim}300{\mu}s$, 0.1-60 Hz, 1-100 Sec and < 1 kW, respectively. Based on the results of this study, TMS can be an effective method of treating dysfunction and improving function of brain cells in brain damage caused by ischemia.
