The purpose of this study is to design electrical stimulation system for pharyngeal dysfunction(dysphagia) in stroke patients. Pharyngeal muscle group activity is important, because contracting muscles provide the driving force at the initiation of the swallow and generate the pressure gradients necessary for bolus movement into the esophagus. Although we have many treatment methods for dysphagia, electrical stimulation system will be useful for stroke patients having dysphagia. Electrical stimulation can be divided into the body stimulation and electrodes. The body stimulation is divided again into frequency counter, time control and current measurement part. These parts are to control the current intensity, frequency and stimulating time. And they can be variable according to the patient's clinical assessment. The electrode plays a role to deliver the current from the system to the muscle. Also the position of the electrode can be variable according to the treatment method. We performed the clinical experiment with the stroke patient who has swallowing disorder. The videofluoroscopy was used for the observation. From the result of clinical experiment based on electrical stimulation, we expected that the dysfunction(in pharynx) level of the patient can be improved. However we could not have enough effectiveness of the treatment because of the number of patients, patient's adaptation and treatment period. We will design the optimized electrical stimulation system based on enough clinical experiment in the future.
Purpose : The purpose of this study was to compare the effect of muscle strength by stimulation of russian current and low frequency. Methods : The subjects were thirty young healthy volunteers who were divided into two groups including russian current group(n=15) and low frequency group(n=15). The intervention was applied totally 12 times (1 time, 10 minute) for 4weeks in each group. The peak torque and average power were measured and analysed using Biodex system 4 before the treatment, after 2 weeks, 4 weeks. Results : As a result, russians currents and low frequency stimulation increased significantly average power and peak torque with the lapse of time. However, there were not significant differences of the average power and peak torque between the groups in all periods. Conclusion : In conclusion, russians currents and low frequency stimulation had no differences in the increase of muscle strength.
Purpose : There is a lack of research on this field in the Republic of Korea, especially those that have seen the effect of interaction between transcranial direct current stimulation and computerized cognitive rehabilitation therapy. This study divided 30 dementia patients into a treatment group, which received transcranial direct current stimulation and computerized cognitive rehabilitation, and a control group, which received pseudo-transcranial direct current stimulation and pseudo-computerized cognitive rehabilitation. This study evaluated the effects of these treatments on the visual perception, cognition functions, and daily activities of dementia patients. Methods : Fifteen subjects were allocated to the treatment group and the other 15 subjects were allocated to the control group. Treatments were given at intervals of five sessions per week (30 minutes per session) for six weeks (30 times in total). This study used the Neurobehavioral Cognitive Status Examination (NCSE) to examine cognitive functions, MVPT to evaluate visual perception, and FIM to test daily living activities before and after applying the treatments. Results : The results of this study showed that cognitive functions, visual perception, and daily living activities significantly (p<.05) improved after the intervention in the treatment group and the control group. The changes in cognitive functions, visual perception, and daily living activities due to the treatments were significantly different between the groups (p<.05). Conclusion : The results indicated that transcranial direct current stimulation and computerized cognitive rehabilitation therapy improved visual perception and daily living activities by increasing cognitive functions. Consequently, it was found that the simultaneous application of transcranial direct current stimulation in conjunction with a computerized cognitive rehabilitation program was an intervention method that could positively affect the visual perception, cognitive function, and daily living activities of dementia patients. Based on the results of this study, the study of arbitration protocols for demential will have to be more active.
Purpose : This study examined the effect of transcranial direct current stimulation (tDCS) combined high intensity interval training (HIIT) on the aerobic exercise capacity of college soccer players. Methods : The subjects of this study were 30 college soccer players. They were divided into a high intensity interval training group combining transcranial direct current stimulation (Group I) and a high intensity interval training group (Group II). Each group had 15 subjects randomly assigned. After receiving general soccer training, each group additionally received high intensity interval training combined with transcranial direct current stimulation and high intensity interval training for 30 minutes 5 times a week for 8 weeks. Their VO2max and 20 meter shuttle run test, Yo-Yo intermittent recovery test were analyzed before the intervention. After 8 weeks of intervention, the above items were re-measured and an intergroup analysis was performed. Results : As a result of comparative analysis of VO2max intake between groups, 20 meter shuttle run test and Yo-Yo intermittent recovery test, a statistically significant difference was found. The high intensity interval training group (Group I) combined with transcranial direct current stimulation showed a significant difference in aerobic exercise capacity compared to the high intensity interval training group (Group II). Conclusion : These results showed that high intensity interval training group combined with transcranial direct current stimulation was more effective for aerobic exercise. Based on this study, this study proposes an effective program for patients as well as elite athletes. In the future, it is necessary to develop an effective transcranial direct current stimulation program and to study how to apply it for various patients.
Purpose : The purpose of this study is to investigate the effects of transcranial direct current stimulation and virtual reality program application on cognition and depression of patients with mild cognitive impairment, and to find an intervention method that can enhance active participation of patients with mild cognitive impairment. Methods : In this study, 50 mild cognitive impairment patients were divided into a treatment group (25 patients) and a control group (25 patients). The treatment group was applied with a transcranial direct current stimulation and a virtual reality program, while the control group received a placebo transcranial direct current stimulation and a virtual reality program. Both groups received five 50-minute sessions per week (one session per day) for six weeks (total of 30 sessions). NCSE was used to evaluate the cognitive functions of the patients before and after treatment intervention. Moreover, K-BDI was conducted to examine the depression of the patients. Results : As a result of the transcranial direct current stimulation and a virtual reality program intervention, the cognitive function of both treatment and control group significantly (p<.05) improved, and the depression of both treatment and control group significantly (p<.05) decreased. Moreover, the changes in cognitive functions and depression were significant between the two groups¸ treatment and control group (p<.05). Conclusion : The results of the study showed that the application of the transcranial direct current stimulation and virtual reality program significantly improved the cognitive function of mild cognitive impairment patients and decreased the depression of them. Therefore, it could be concluded that the transcranial direct current stimulation and virtual reality program was an intervention method which positively affects the cognitive function and depression of mild cognitive impairment patients.
목적 : 경두개 직류 자극과 강제 유도 운동 치료의 결합된 중재를 적용한 뇌졸중 환자의 회복과 관련된 연구 동향을 알아보고, 중재 계획 시, 도움이 될 수 있는 정보 및 근거를 제시하는 데 목적이 있다. 연구방법 : 전자 데이터베이스 PubMed, NDSL 등을 이용하여 2009년 1월부터 2018년 12월까지 10년간 온라인 데이터에 등록된 논문을 검색하였다. 주요 검색어로 'Transcranial direct current stimulation' or 'tDCS', 'Constraint-induced movement therapy' or 'CIMT', 'Upper extremity function', 'Upper limb', 'Stroke'을 혼용하여 사용하여 본 연구의 선정기준과 배제기준에 부합하는 6편의 논문을 최종 선정하여 분석하였다. 결과 : 경두개 직류 자극의 적용 전류의 세기, 적용시간 등과 강제 유도 운동 치료의 프로토콜은 다양한 방법으로 이루어져 있었지만, 경두개 직류 자극 직 후 CIMT를 시행하는 중재 절차는 동일하게 시행되었다. 중재 효과를 알아보기 위해 상지기능, 일상생활동작, 대뇌피질 활성도 평가 등이 사용되었다. 경두개직류 자극과 강제 유도 운동 치료의 결합된 중재는 뇌졸중 환자의 상지기능과 일상생활동작 개선에 효과적인 것으로 나타났으며, 대뇌피질의 활성화에도 유의미한 효과가 있는 것으로 나타났다. 결론 : 본 연구를 통해 경두개 직류 자극과 강제 유도 운동 치료의 결합된 중재 적용에 필요한 정보 제공 및 근거를 마련하였다. 추후 연구를 통해, 경두개 직류 자극의 자극시간과 전류의 세기, 전극의 부착 위치 등에 대한 일반화가 필요할 것이며 가장 적절한 강제 유도 운동 치료 프로토콜을 사용하여 큰 모집단을 대상으로 장기간 추적 관찰을 포함한 무작위 대조군 실험연구가 필요하다.
Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation method that delivers 1-2 mA of current to the scalp. Several clinical studies have been conducted to confirm the therapeutic effect of major depressive disorder (MDD) patients with tDCS. Some studies have shown tDCS's antidepressant effect, while the others showed conflicting results in antidepressant effects. Our aim of this review is to understand the biological bases of tDCS's antidepressant effect and review the results of studies on tDCS's antidepressant effect. For the review and search process of MDD treatment using tDCS, the US National Library of Medicine search engine PubMed was used. In this review, we discuss the biological mechanism of tDCS's antidepressant effect and the existing published literature including meta-analysis, systematic review, control trial, open studies, and case reports of antidepressant effects and cognitive function improvement in patients with MDD are reviewed. We also discuss the appropriate tDCS protocol for MDD patients, factors predictive of response to tDCS treatment, the disadvantages of tDCS in MDD treatment, and side effects.

A transcranial magnetic stimulation device is a complicated appliance that employs a switching power device designed for discharging and charging a capacitor to more than 1 kV. For a simple transcranial magnetic stimulation device, this study used commercial power and controlled the firing angle using a Triac power device. AC 220V 60 Hz, the power device was used directly on the tanscranial magnetic stimulation device. The power supply device does not require a current limiting resistance in the rectifying device, energy storage capacitor or discharge circuit. To control the output power of the tanscranial magnetic stimulation device, the pulse repetition rate was regulated at 60 Hz. The change trigger of the Triac gate could be varied from $45^{\circ}$ to $135^{\circ}$. The AVR 182 (Zero Cross Detector) Chip and AVR one chip microprocessor could control the gate signal of the Triac precisely. The stimulation frequency of 50 Hz could be implemented when the initial charging voltage Vi was 1,000 V. The amplitude, pulse duration, frequency stimulation, train duration and power consumption was 0.1-2.2T, $250{\sim}300{\mu}s$, 0.1-60 Hz, 1-100 Sec and < 1 kW, respectively. Based on the results of this study, TMS can be an effective method of treating dysfunction and improving function of brain cells in brain damage caused by ischemia.
High Voltage Pulsed Current Stimulation(HVPCS) and Microcurrent Electrical Neuromuscular Stimulation(MENS) have been used to promote the healing of decubitus ulcer and surgical wounds. The benefits of HVPCS and MENS are thought to include an inhibitive effect on bacterial growth. The purpose of this study was to compare the inhibitive effect of two different electrical stimulation techniques growth in vitro. Using agarose-based media, the two bacterial species Staphylococcus aureus, Esherichia coli - which are commonly isolated from open wounds were incubated in an incubator for 24 hours following exposure to HVPCS(400 V, 120 pps, $70{\mu}s$) and MENS($100{\mu}A$, 0.3 Hz). We then measured the zone of inhibition around each electrode. Both HVPCS and MENS produced an inhibitive effect on bacterial growth in this vitro study. However MENS was more effective than HVPCS.
