Wound Infection of Spinal Cord Stimulator: A Case Report (척수 신경 자극기 삽입부 감염의 치험례)

  • Purpose: Nowadays spinal cord stimulator is frequently used for the patients diagnosed as complex regional pain syndrome. The lead is placed above the spinal cord and connected to the stimulation generator, which is mostly placed in the subcutaneous layer of the abdomen. When the complication occurs in the generator inserted site, such as infection or generator exposure, replacement of the new generator to another site or pocket of the abdomen would be the classical choice. The objective of our study is to present our experience of the effective replacement of the existing stimulation generator from subcutaneous layer to another layer in same site after the wound infection at inexpensive cost and avoidance of new scar formation. Methods: A 50-year-old man who was diagnosed as complex regional pain syndrome after traffic accident received spinal cord stimulator, Synergy$^{(R)}$ (Medtronic, Minneapolis, USA) insertion 1 month ago by anesthetist. The patient was referred to our department for wound infection management. The patient was presented with erythema, swelling, thick discharge and wound disruption in the left upper quadrant of the abdomen. After surgical debridement of the capsule, the existing generator replacement beneath the anterior layer of rectus sheath was performed after sterilization by alcohol. Results: Patient's postoperative course was uneventful without any complication and had no evidence of infection for 3 months follow-up period. Conclusion: Replacement of existing spinal cord stimulation generator after sterilization between the anterior layer of rectus sheath and rectus abdominis muscle in the abdomen will be an alternative treatment in wound infection of stimulator generator.

Processing and Characteristics of Canned Seasoned Boiled Oyster Crassostrea gigas and Canned Seasoned Roasted Oyster Crassostrea gigas (조미 자숙굴(Crassostrea gigas) 통조림 및 조미 구운굴(Crassostrea gigas) 통조림의 제조 및 품질특성)

  • The purpose of this study was to collect basic data that could be used in the manufacture of two kinds of canned oyster Crassostrea gigas. The steamed oyster was prepared by shucking after boiling for 6 min at $105^{\circ}C$ and then washing and dewatering. The roasted oyster was prepared by baking steamed oyster at $140^{\circ}C$ for 20 min. The manufacturing methods of canned seasoned boiled oyster and canned seasoned roasted oyster were as follows. The boiled or roasted oyster (50 g) was added to a can (RR-90) along with a mixture of seasoning sauce 40 and then seamed using a vacuum seamer under 20 cm Hg after pre-exhausting at $90^{\circ}C$ for 20 min. The two kinds of canned oyster products produced under sterilization of Fo 12 min were tested for cultured bacteria, external appearance, proximate composition, pH, VBN (Volatile basic nitrogen), TBA (Thiobarbiuric aicd) value, amino-N, salinity, color value sensory evaluation, etc. Results showed that the canned seasoned roasted oyster had higher overall acceptability than the canned seasoned boiled oyster. The reason for this was judged to be that the process of roasting at $140^{\circ}C$ for 20 min influenced the sensory evaluation.

Factors Affecting the Incidence of Ectopic Pregnancy following Tubal Reversal (난관복원술후의 자궁외임신 빈도에 영향을 주는 인자들)

  • To evaluate the factors affecting the incidence of ectopic pregnancy following tubal reversal, the clinical characteristics of ectopic pregnancy group (N=28)and intrauterine pregnancy group (N=316)were compared in 344 cases of pregnancy following tubal reversal using microsurgical technique at the Department of Obstetrics and Gynecolgy from July 1980 to May 1989. We found no significant relationship between the incidence of ectopic pregnancy and the duration of sterilization. Although the incidence rate (11.2%) of ectopic pregnancy following laparoscopic cautery appeared to be the highest among tubal sterilization methods, none showed statistically significant differences. There was a fourfold difference in the incidence rate of ectopic pregnancy between post-reversal tubal length less than 7cm (12.7%)and greater than 7cm (3.5%). The mean interval from tubal reversal to conception was 15.7months in ectopic pregnancy group, and 9. 3months in intrauterine pregnancy group, the difference reaching statistical significance. While most(80%)of the pregnancies were achieved within the first 12months following tubal reversal in intrauterine pregnancy group, 50% of the pregnancies occured in the same period, and the cumulative pregnancy rate is related in a linear fashion to the duration after reversal in ectopic pregnancy group. The incidence rate of ectopic pregnancy was found to be higher after cornual-ampullary anastomosis and cornual-isthmic anastomosis.

  • Purpose: The purpose of this study is to investigate self management and subjective chewing ability and provide the basic information to research of healthy life of old people from development of education and method for dental health of denture patient. Methods: From 15th or August to 15th of September in Daegu and Gyeong-buk, denture patients were targeted for a survey. Data from 295 people were analyzed. Results: Adjusted $R^2$ of oral health and denture satisfaction of an object of study increased to 24.9%(F=20.460, p<.001). Adjusted $R^2$ of denture custody and method of sterilization was 37.8% and Adjusted $R^2$ of recognition denture satisfaction of chewing effect was 14.4% which affect to most(F=9.274, p<.001). Conclusion: In conclusion denture condition, denture custody, method of sterilization, chewing effect influence oral health, healthy life of patients who have denture.

Study on the Shelf Life of Sterilized Products according to Packaging Materials (포장재에 따른 멸균품의 유효기간에 관한 연구)

  • Purpose: The purpose of this study was to determine the most appropriate shelf life for sterilized products according to their packaging material. Methods: Samples were prepared to target six nursing units in one general hospital in Seoul. After steam and E.O gas sterilization, sterilized product, samples were supplied to wards. Data collection was conducted for 3 months, after the expiration date of 3 months had passed for samples packaged with crepe paper and nonwoven wraps. For samples packaged with paper-plastic pouches, data collection conducted for 3 months when the expiration date of 9 months had passed. The sterilized products were collected and tested for microbial contamination. Identification of the storage environment was done as samples were collected. Results: This study confirmed that the storage environment met international standards such as CDC, except for temperature. For steam sterilized crepe paper packaging samples and steam and E.O gas sterilized for nonwoven packaging samples no contamination in all products was found for 3 months past the expiration date. However, in the E.O gas sterilized paper-plastic pouch packaging sterile samples, Gram-positive bacilli were detected in one sample from a surgical intensive care unit at 45 weeks and another sample from an operating room at 47 weeks. Furthermore, the results did not show any microorganisms for up to 52 weeks in all products. Conclusion: According to the results of this study, sterilized product packaging made with crepe paper and nonwoven wraps is better able an extended shelf life from 3 months to 6 months, reducing unnecessary costs.

Safety of Temporary Use of Recycled Autoclaved Femoral Components in Infected Total Knee Arthroplasty: Confirming Sterility Using a Sonication Method

  • Background: The purpose of this study was to evaluate the usefulness of sonication technique for microbiological diagnosis and the sterility of the recycled autoclaved femoral components from infected total knee arthroplasty (TKA) using a sonication method. Methods: Nineteen femoral implants explanted from patients with infected TKA were sterilized with a standard autoclave method. Standard culture of the fluid before and after sonication of the sterilized implants was performed to detect pathogenic microorganisms. Additional experiments were performed to evaluate the sterility of the recycled implant by inducing artificial biofilm formation. Methicillin-resistant Staphylococcus aureus (MRSA) was inoculated into 10 implants and sterilization in a standard autoclave was performed, and then the fluid was cultured before and after sonication. Results: Two of the 19 sterilized implants were positive for growth of bacteria after sonication, whereas no growth was detected in the cultured fluid from the sterilized implants before sonication. The bacteria were Staphylococcus species in all two cases. In one of 10 implants inoculated with MRSA, the culture was positive for growth of bacteria both before and after sonication. However, Staphylococcus epidermidis was cultured from both occasions and thus this implant was thought to be contaminated. Conclusions: We found sonication for identification of pathogens could be helpful, but this finding should be interpreted carefully because of the possibility of contamination. Sterilization of an infected femoral implant with an autoclave method could be a good method for using the temporary articulating antibiotic spacer in two-stage revision arthroplasty.

Bactericidal Effect of Ultraviolet and Dry Treatment on Bacterial Contaminants in Facial Masks (Ultraviolet 및 건조 처리에 의한 마스크에 오염된 미생물 살균 효과)

  • Due to the pandemic caused by COVID-19, the demand for face masks is soaring and has often caused a shortage. The aim of this study was to evaluate the effect of ultraviolet (UV) and drying treatments on microbial contaminants in facial masks. To conduct this study, standard procedures were designed to develop samples contaminated by the control bacteria Escherichia coli, Staphylococcus aureus, and Pseudomonas aeruginosa. The contamination level of the standard samples was approximately 6.30 × 106 CFU/ml, and the UV light treatment was performed 1, 3, 5, and 7 times. To evaluate the effect of the UV and drying treatments, the masks were first treated with UV 1, 2, and 3 times, followed by the drying process. As a result, the mask contaminated with E. coli and P. aeruginosa showed a bacterial rate of approximately 99.9% after 1 UV irradiation, and in the case of the S. aureus-contaminated mask, it exhibited a bactericidal rate of approximately 99.9% after 7 UV irradiations. However, when the drying process was included after UV irradiation, all the samples contaminated with E. coli, S. aureus, and P. aeruginosa showed a bactericidal rate of 99.9% or more. The results of this study suggest that UV and drying treatments can effectively reduce the bacterial contaminants in facial masks. In addition, these results provide fundamental data and appropriate sterilization methods for reusing masks.

Status of Facilities, Equipment, and Environmental Hygiene Management of School Foodservices in Daiyue-district, Taian-city, China (중국 태안시 다이웨구 학교급식소의 시설·기구·환경 위생관리 실태)

  • The results of the inspection based on the sanitation checklist and the measurement of refrigerator/freezer temperature, illuminance, and ATP were conducted at 12 school foodservices in Daiyue-district, Taian-city, Shandong Province, China, at the same time, and the results were as follows. Five items of 'Use sterilized cutting boards', 'Use sterilized knives', 'Use worktable after sterilization', 'Use sink after sterilization', 'Sterilize the interior/exterior of the refrigerator regularly' in the checklist inspection were found not to be performed (0%). Two items of 'Disinfect hands after washing', 'Equipped with hand sterilizer' and 'Sterilize the kitchen areas regularly' were investigated as being performed only in one foodservice (8%). The average illuminance of the receiving stand were 373.08±106.35 Lux, 8% when the standard (540 Lux or higher) was complied with, the average refrigerator temperature were 7.06±0.82℃, 38.9% when the standard (-2 to 5℃) was observed 8% of cases complying with the standard (below -18℃) at -8.56±0.76℃ were investigated. As a result of ATP measurement, the surfaces of knife edge, cutting board, refrigerator door handle, worktable, and sink, which are not sterilized at all, were not suitable (accept ratio 0%), sterilized food plate (accept ratio 54.2%) and utensil for preserved food (accept ratio 75%) had a relatively high accept ratio. China's hygiene regulations should be revised in the direction of strengthening disinfection practices for facilities, equipment, and the environment, and hygiene measures such as education on disinfection and preparation of economical disinfection methods should be established.

Physical Seed Treatment Techniques for Germination Enrichment and Seed Sterilization (발아증진 및 소독을 위한 물리적 방법을 이용한 종자처리 기술)

  • Since seeds can be directly used as food resources as well as for crop cultivation or preservation of genetic resources, it is essential to develop high-quality seed processing technology to increase agricultural productivity. Seed treatment means processing technologies of seeds through physical or chemical treatment processes from after harvesting seeds to before sowing of seeds to improve germination and growth rate, durability, and immunity, etc. Since chemical seed treatment technology using pesticides or plant growth regulators has problems of environmental pollution and human toxicity, it is desired to develop an alternative technology. As a physical seed treatment method, various technologies such as ionizing radiation, plasma, microwave, and magnetic field are being developed, and some of them are being used practically. In this paper, I will summarize the mechanism of seed priming and disinfection, and the advantages and disadvantages of application, focusing on these physical seed treatment methods. Low dose or moderate intensity ionizing radiation, microwave, low-temperature plasma, and magnetic field treatments often promoted seed germination and seedling growth. However, effective removal of direct seed pathogens at these treatment intensities appears to be difficult. And it has been shown that relatively high-dose electron beam treatment using low-energy electron beams kills microorganisms on the seed surface and hull layer while not damaging the inner tissue of the seed, and is also effectively used for seed treatment on a commercial scale. In order to put the physical seed treatment technology to practical use in Korea, it is necessary to develop an economical scale treatment device along with the development of individual treatment technology to each crop.

A Study for Infection Control Standards for Medical Devices in NICU (신생아중환자실 의료기구의 소독방법, 소독주기, 교환주기에 대한 감염관리 표준화를 위한 연구)

  • Purpose: The purpose of this study was to identify the state of infection control in neonatal intensive care units (NICUs) including disinfection methods, disinfection cycles, and exchange cycles for medical devices and to suggest a basic framework which would help develop standardized infection control guidelines. Methods: From a list of NICU equipment developed from the NICUs in 4 tertiary hospitals, a structured questionnaire on 74 types of medical equipment was developed and sent to 31 hospitals by mail. The results were reviewed by panel of experts (56 persons), and analyzed for internal validity by a focus group (4 persons) using guidelines from the Centers for Disease Control and the Korean Hospital Nurses Association. Results: The results showed various methods, cycles, disinfectant levels for the disinfectants and exchange cycles in the medical equipment infection control of the 31 hospitals. The focus group developed a 66-item basic framework based on validity testing. Conclusion: From the results of this study, a framework of infection control standards for 66 types of medical equipment in the NICU was developed. It is suggested that further study be done to more precisely establish standard infection control guidelines for NICU medical equipment.
