Differences in Ankle Muscle Activity During Static Balance According to Age and Ankle Proprioception

  • Background: Older adults use different ankle muscle activation patterns during difficult static balance conditions. It has been suggested that this is related to a decline in proprioception with age, resulting in reduced postural balance. However, the association between proprioception and ankle muscle activity during quiet standing has not been directly assessed. Objects: This study aimed to investigate the effects of age and sensory condition on ankle muscle activity and the association between ankle proprioception and ankle muscle activity. Methods: We recruited 10 young women and 9 older women. Ankle proprioception was evaluated using joint position sense (JPS) and force sense (FS) divided by dorsiflexion and plantarflexion. The electromyographic activity of the tibialis anterior (TA) and gastrocnemius (GCM) muscles was collected during quiet standing. Results: Older women activated GCM muscle more than young during quiet standing and when performing difficult tasks. Older women had more errors in JPS dorsiflexion and FS plantarflexion than did young. The GCM muscle activity is related to JPS dorsiflexion and FS plantarflexion. Conclusion: Lower proprioception of the GCM with age leads to increased muscle activity, resulting in reduced postural balance. There was no difference in TA proprioception or muscle activity among older women with frequent physical activity.

고유수용성 신경근 촉진법의 통합 패턴에 따른 정적 균형의 변화 (Change of the Combined Patterns of Proprioceptive Neuromuscular Facilitation on Static Balance)

  • 고유수용성 신경근 촉진법의 통합 패턴이 정적 균형에 미치는지, 여자 대학생 20명을 대상으로 6주간 실험하였다. 고유수용성 통합패턴은 Dietz에 의해 고안된 스프린터와 스케이트 패턴을 이용하였으며, 훈련은 주3회 일 1회 실시하였다. Good Balance system을 이용하여 정상적인 기립자세, 눈을 감고 한발 들고 좌,우 기립자세, 눈을 뜨고 한발 들고 좌,우 기립자세에서 X축 속도, Y축 속도와 속도 모멘트 3항목의 변수를 측정한 결과 눈뜨고 서기자세, 눈감고 서기자세, 눈감고 우측외발서기 자세에서 X, Y 축의 평균 속도, 압력중심의 속도 모멘트 영역 모두에서 운동 전 후 감소하여 통계학적으로 유의하게 감소하였으며, 눈뜨고 좌측외발서기 자세에서는 X축의 평균 속도만 운동 전 후 감소하여 통계학적으로 감소하였다. 이 러한 결과 고유수용성 신경근 촉진법의 통합패턴이 정적 균형 능력에 영향을 미치며, 향후 본 연구의 운동방법을 임상에서 환자에게 적용할 수 있도록 더 쉽고, 다양한 형태의 기술이 포함된 연구가 진행 되어야 할 것으로 사료된다.

Changes of postural stability according to ankle fixation in healthy subjects

  • Objective: The purpose of this study was to examine the changes in postural stability according to ankle fixation in healthy university students. Design: Cross-sectional study. Methods: Thirty healthy subjects (15 males and 15 females, 20.13 years, 167.49 cm, 65.87 kg) were recruited on a voluntary basis. The BT4 system (HUR Laps Oy, Tampere, Finland) was used to measure the static (standing posture with eyes open and eyes closed) and dynamic (external perturbation and limits of stability (LOS) in the forward, backward, left, and right side) balance abilities. External perturbation was measured by the subject's postural sway velocity and area for 20 seconds after being impacted by a gym ball. Static and dynamic stabilities were measured with ankle joint fixation and non-fixation conditions. Ankle fixation was provided using Mueller tape on both ankle joints. Results: For static stability under the standing posture, there was no significant difference between standing with ankle joint fixation and non-fixation conditions. However, dynamic stability (external perturbation and LOS in the forward, backward, left, and right side) was significantly higher in the standing with the non-fixation condition compared to the standing with ankle joint fixation condition (p<0.05). Conclusions: Our results reveal that ankle joint fixation can influence dynamic stability during standing. Thus, we believe that this result provides basic information for making improvements in postural control and may be useful in balance training for fall prevention.

Can Functional Assessment Tools Reflect Balance Abilities at 3 Months after Total hip Arthroplasty?

  • PURPOSE: This study was conducted to determine if the Harrison hip score (HHS), a tool for assessing hip joint function, and the Burg balance scale (BBS), a general balance assessment tool, actually reflect the balance ability of total hip arthroplasty (THA) patients 3 months after surgery. In addition, this study investigated the initial weight distribution strategy for bilateral lower extremity after THA surgery to understand the balance of THA patients. METHODS: Fourteen 3-month THA patients performed static dual standing and sit-to-stand (STS) tasks. Ground reaction forces on each leg were collected to calculate the weight distribution symmetricity (SWD), and the HHS, functional HHS (f-HHS), and BBS were evaluated. Correlation analyses between SWD and the HHS (also f-HHS) and BBS were then applied to the THA patients. RESULTS: The correlations between functional evaluation tools (HHS, f-HHS, BBS) and SWD were weak strength for the static balance task, but moderate for the dynamic STS task. Among the evaluation tools used in the present study, f-HHS was most useful for evaluation of dynamic balance ability. CONCLUSION: The results suggest that use of HHS, f-HHS, and BBS as functional evaluation tools does not provide meaningful information regarding balance ability, but that they are useful for evaluating dynamic balance ability of THA patients. The dynamic balance ability at 3 months after THA seems to be under development.

요통환자의 자세동요와 동적기립균형에 관한 연구 (A Study of Postural Sway and Dynamic Standing Balance with Low Back Pain)

  • The purpose of this study was to compare difference of the static and dynamic balance in normal subject and LBP subject and recognizes about postural sway. The subjects of this study included 30 normal subjects and 30 LBP subjects. By using Active balance system, the static balance was measured by unit path length, circumference area, weight bearing, stabilometry length, while the dynamic balance was measured by step evaluation. Statistically analyzed using independent t-test to search static balance and dynamic balance difference in two groups. Postural sway appeared greatly in patient group than normal group in analysis result of static balance and postural sway was big in patient group of when closed eye and normal group and patient group did show statistical significance in unit path length, circumference area. Weight support of normal group was shared equally in weight bearing rate, but weight support of patient group stewed less to pain side. Anterior step time and Task achieve time of dynamic balance were spent more in patient group than normal group. Desire to be used to useful information to lay treatment policy about set-up and action of when quantification result of valued postural balance treats low back pain patient by objective tool, in the fixture, more researches in postural estimation desire that is enforced abuzz.

균형훈련이 뇌졸중환자의 발목 근육 활성도와 정적균형에 미치는 영향 (Effect of Balance Training on Ankle Muscle Activity and Static Balance in Stroke Patients)

  • Purpose: This study aims to understand the effect of balance training on ankle muscle activity and static balance in stroke patients. Methods: The subjects were composed of 12 patients who were admitted with chronic stroke in H, K, and R hospitals located in Gangwon-do from August to October 2014. This study conducted balance training on the subjects for 20 min a session, twice a day, five times a week for six weeks. This study measured the surface electromyogram (EMG) of the paretic side's tibialis anterior and medial gastrocnemius for muscle activity. Static balance was measured as a TETRAX stability index. The measurements were conducted before the experiment and six weeks after. The result was statistically analyzed using paired t-test at the significance level of 0.05. Result: Muscle activity of the paretic side's tibialis anterior and medial gastrocnemius did not show a significant difference in the comparison between pre- and post-balancing training. However, the stability index showed a significant difference (p<0.05) between standing with eyes open (NO) and standing on a sponge with eyes open (PO). Conclusion: Although balance training did not affect the muscle activity of chronic stroke patients, it had an effective influence on the stability index. That is, balance training was effective at the functional level, but it did not change the structure level in terms of the ICF evaluation standard. Balance training becomes more effective when it is applied in relation to task.

지지면에 따른 닫힌 사슬운동이 성인의 균형에 미치는 영향 (Effects of a Closed Chain Movement of the Support Surface on the Balance of Adults)

  • Purpose The purpose of this study was to investigate the change of balance ability by performing closed chain exercise on stable support surface and unstable support surface in twenties. Methods This study randomly selected 15 students in the closed chain exercise group on the stable support side and 15 on the closed chain exercise group on the unstable support side. Balance ability was measured before and after the start of exercise and static balance was measured by OLT(One Leg Standing Test) and FRT (Functional Reach Test). Result The changes of the function reach test of the closed chain movement according to the ground type were significant in the unstable and stable support surfaces and the change of function reach test after the intervention in the two groups was significantly improved compared with the closed chain movement respectively. The one leg standing test changes of the closed chain movement according to the ground type showed significant results on the unstable and stable support surfaces, but there was no significant difference in the one leg standing test changes after intervention between the two groups. Conclusion The effect of closed chain training on ground type is unstable. The change of function reach test and one leg standing test of the closed chain exercise group on the stable support surface resulted in significant changes after exercise, but there was a significant difference in the balance ability of function reach test change after intervention between the groups.

정적 균형능력 향상을 위한 하지 의류의 EMS 적용 효과 (EMS Application Effect of Lower Extremities to Improve Static Balance Capability)

  • The purpose of this study was to investigate whether there were improvements on balance when both ankle-jointed calf muscles and hip muscles, which affect balance capabilities, were activated through taping techniques and EMS. In this study, the One Leg Standing Test, a static balance test, was conducted by experimenting on a flat floor, foam pad, and a stretching board with a gradient of 20 degrees, respectively, to study static balance capabilities in different situations. Nine healthy men in their 20s were measured five times every five minutes considering muscle fatigue, and the difference between each variable was analyzed through post-test using nonparametric statistical analysis. Our results showed an equal increase in static balance capability was similar when EMS was applied only to calf muscles and only to hip muscles. Notably most improvements were seen when wearing calf supporters and taping technology pants, and applying EMS together. It was also found that the difference between EMS electric stimulation and balance capability was greater when wearing and applying supporters and taping technology pants. Based on the results of the present study, a muscle support band and EMS of taping techniques can improve balance capabilities. These findings are expected to form a basis for solutions Improving the balance capabilities

Characteristics of Static Balance in Patients With Adolescent Idiopathic Scoliosis

  • The purpose of this study was to compare the static balance of standing position between adolescent idiopathic scoliosis (AIS) and a normal group that were aged-matched. There were forty subjects included in this study. Twenty-seven healthy subjects (age, $13.9{\pm}1.2$ yrs; height, $161.9{\pm}7.5$ cm; weight, $52.2{\pm}7.7$ kg) and thirteen AIS subjects (age, $14.2{\pm}2.2$ yrs; height, $161.5{\pm}8.7$ cm; weight, $48.1{\pm}8.1$ kg) were participated in the study. The thirteen subjects in the AIS group had a major Cobb angle between $20.1^{\circ}$ and $49^{\circ}$. Each group was tested with the Balance Performance Monitor (BPM). The parameters for static balance were sway area, sway path, max velocity, mean balance, anterior-posterior angle, and left-right angle of each group with their eyes opened and again with their eyes closed. Both sides of the forward reach test and the lateral reach test were also performed on each group. Results from the BPM tested showed significantly increases in all parameters of static balance with those patients with AIS under the conditions where eyes were opened and closed. In the right and left forward reach test, there was no significant difference between normal and AIS groups. However, in the lateral reach test with right and left direction, there were significant differences between normal and AIS groups. For the normal subjects, there were significant differences in the parameters with sway path and anterior-posterior sway angle between the eyes opened and closed. However, there were no significant differences in the all parameters between eyes opened and closed for the AIS subjects. These results suggest that, balance programs could be used in the rehabilitation setting for intervention of AIS and evaluation of AIS. Further study is needed to measure many patients with AIS and other functional balance scales for clinical application.

The effect of focus of attention by electroencephalogram-feedback on balance in young adults

  • Objective: Electroencephalogram (EGG)-feedback is a training procedure aimed at altering brain activity, and is used as a treatment for disorders like attention. The purpose of this study was to determine the effects of external focus of attention by EGG-feedback on balance in young adults. Design: Cross-sectional study. Methods: Subject were students in Sahmyook University. Fifty young adults in their twenties and thirties. Subjects were performed both with and without external focus of attention by EEG-feedback on the posture of standing and tandem standing. Participants were educated effort to maintain static posture when they were under internal focus of attention. Good Balance System was used for measurement of postural consistency upon the following force platforms. Results: Body sway decreased significantly both normal standing and tandem standing with external focus of attention by EEG-feedback (p<0.05). Conclusions: The results demonstrate that the benefits of an external attentional focus are generalizable to young adults. The external focus of attention outperformed the internal focus of attention on the postural balance (p<0.05). It is showed that external focus of attention significant effects on balance by revoked automatic postural control of movement. Furthermore balance might be improved by training with an external focus. Further study is required to develop for training as a method of preventing fall in elderly peoples.
