Studies on the Administration of the Quality of Herbal Medicine (한약재 품질 관리에 관한 연구)

  • Objective: This study was conducted to suggest a proper administration system of the quality of herbal medicine. Methods: Related literature was carefully inspected and discussions among specialists in the field of herbal medicine's growth, production, circulation and administration were conducted. Results: 1. The administration of herbal medicine's quality should be done in each stage of its circulation; growing stage, harvesting stage, importing stage, production stage, quality examination stage, wrapping stage, circulating stage, and administration stage. 2. A safety standard should be established for items, parts and origins of each medicinal herb. A standard for the identification of forged products and objective standards for quality discrimination should also be established. 3. A national system for the training of herbal medicine quality administration personnel and herbal medicine discrimination personnel should be established. A data base of standardized product specimens should be established and a pharmacopoeia of herbal medicine should be published as a standard of quality administration in herbal medicine. 4. Research into the following preliminary areas should be conducted to investigate quality adminstration of herbal medicine: research related to herbal medicine's raw materials and herbal medicinal drugs; quality estimation of raw materials and drugs; quality administration in oriental medical hospitals; herbal medicine administration law; herbal medicine administration organization; herbal medicinal pharmacology; and policy-related research for the quality improvement of domestically grown medicinal herbs. Conclusion: For the proper administration of herbal medicine's quality, governmental support and intensive research among specialists should be done for the supply of good quality medicinal herbs and the improvement of the efficacy of herbal medicine.

Discriminating Factors of Stages of Change in Bone Mass Promoting Behaviors after Bone Mineral Densitometry (골밀도 검사를 받은 여성의 골량증진행위 변화단계 판별요인)

  • Purposes: This study was designed to explore the stage distribution of subjects according to stage of change for calcium intake and for exercise, and to identify factors that could discriminate among subjects in various stages. Methods: The sample consisted of 142 subjects who had taken bone mineral densitometry tests. The instruments used in this study were the Stage Placement Instrument for Calcium Intake and Exercise, the Osteoporosis Health Belief Scale and the Osteoporosis Knowledge Test, and the Osteoporosis Self Efficacy Scale. Data were analyzed using chi square, ANOVA, and discriminant analysis by using the SPSS 12.0 program. Results: For calcium stages, economic level, calcium knowledge, positive social norms for calcium intake, & educational level showed high standardized canonical discriminant function coefficients. For exercise stages, exercise efficacy, susceptibility, exercise benefit, educational level, positive social norm to exercise, educational level, and exercise barrier showed high standardized canonical discriminant function coefficients. Conclusion: This study implies that bone mass promoting program incorporating a stages of change model can be applied as useful nursing intervention.

Plan for the Development of a Standardized Dummy for Persons in Need of Rescue in a Confined Space (밀폐공간 구조 요구자를 위한 더미 표준화 개발 방안)

  • This study was conducted to develop a dummy in an environment similar to the human body, to prepare a standard for evaluation and to present the process of the production in order to evaluate the performance of the robot that can detect the persons needing rescue in a confined space, who are difficult for fire-fighting officials to rescue in case of fire and disaster. As a result, a standard for evaluation was developed and standardized into four parts 'Normal,' 'Risk Stage 1,' 'Risk Stage 2' and 'Risk Stage 3'based on the number of breath cycles, carbon dioxide concentration, core temperature and criteria for hearing to recognize the voice. In addition, in order to produce a dummy, fever, breathing capacity and voice output function were compared and analyzed. This study has significance that it built up basic data of the method of producing the actual dummy, by presenting characteristics and controlling methods using the waterproof insulation heating coil for the function, solenoid valve for the consecutive output of breathing capacity and USB program sound board for voice output.

The Validation of Standardized Nursing Care Plans Developed for Computerized Use in Clinical Practice (간호업무 전산화를 위해 개발된 표준화된 간호계획서의 타당성 검정)

  • Recognition of the usefulness and the importance of the nursing diagnosis is increasing. There is a prevailing opinion that nursing diagnosis should be used to improve the quality of nursing care. Developing standardized nursing care palns based on nursing diagnoses is therefore considered one of the most essential projects for professional growth and improvement in the nursing world of Korea. Consequently, in the first stage of this research project, the ten nursing diagnses used most frequently with patients on medical and surgical wards were determined and related nursing care plans were developed, implemented and evaluated. The application of the standardized nursing care plans raised the nurses' confidence and proved to be effective in enhancing the quality of nursing care. This study was initiated as the next stage, to develop, test, and determine the validity of nursing care plans for the remaining nursing diagnoses. Nineteen medical and surgical wards were selected for the study ; the 176 staff nurses working on those wards and 1211 patients hospitalized there (603 patients during the nursing care plan use) took part in the project. The following summarizes the results of the study : 1. After listing all the nursing diagnoses up to the 20th in frequency from each ward except the ten used in the first study, 22 nursing diagnoses were selected. Two related to ‘self care deficit’, were combined into one. Standardized nursing care plans were established for these 21 nursing diagnoses. 2. The first page of each nursing care plan lists the related factors and defining characteristics as supporting data. The application rate distribution revealed that the majority were recorded less than 50% of the time. For each nursing diagnosis, only one to three related factors were recorded more than 50% of the time regardless of the number of suggested related factors, and similarly, only one to five defining characteristics were recorded more than 50% of the time regardless of the number of suggested defining characteristics. Therefore, these factors and defining characteristics were proposed as the common related factors and the typical signs and symptoms for each nursing diagnosis. 3. The application rate distribution for the expected outcomes, and the nursing orders that were the main data of each nursing care plan occurred more than 50% of the time, unlike the related factors and the characteristics that occurred less frequently. These findings supported the clinical validity. 4. In an effort to evaluate indirectly the effect of the use of the standardized nursing care plans, nurses' job satisfaction and perceptions of their ability in the use of the nursing process were measured and compared. Scores after the use of the plans were significantly higher than those before. The experience in actually using the standardized nursing care plans with patients increased the nurses' professional and emotional satisfaction and their confidence in using the nursing process. Also when the nurses who actually used the nursing care plans were asked to rate their effectiveness, the highest score was given to ‘the ease of establishing the nursing goal’, followed by ‘improved professional advice and care for patients’, ‘the efficiency and systemization of charting’, ‘the definite recognition of the nursing problem’, and ‘the selection of effective and appropriate nursing interventions’ in descending order. The results indicate the nurses were very positive about the effect of the real clinical application of standardized nursing care plans, and that the objective of this study to utilize the nursing diagnosis to strengthen the nursing process was attained.

Recyclable Objects Detection via Bounding Box CutMix and Standardized Distance-based IoU (Bounding Box CutMix와 표준화 거리 기반의 IoU를 통한 재활용품 탐지)

  • In this paper, we developed a deep learning-based recyclable object detection model. The model is developed based on YOLOv5 that is a one-stage detector. The deep learning model detects and classifies the recyclable object into 7 categories: paper, carton, can, glass, pet, plastic, and vinyl. We propose two methods for recyclable object detection models to solve problems during training. Bounding Box CutMix solved the no-objects training images problem of Mosaic, a data augmentation used in YOLOv5. Standardized Distance-based IoU replaced DIoU using a normalization factor that is not affected by the center point distance of the bounding boxes. The recyclable object detection model showed a final mAP performance of 0.91978 with Bounding Box CutMix and 0.91149 with Standardized Distance-based IoU.

  • Despite the many advantages of peritoneal dialysis (PD) in children with end-stage renal disease, there exist redoubtable complications of PD that should be overcome. To prevent and manage these complications, a multidisciplinary team should provide support highly tailored for each child and family, based on the standardized practice guidelines for the management of pediatric PD. In this review, we summarize the clinical manifestations and management of several complications of PD.

A Study on Establishing a Standardized Process for the Development and Management of Food Safety Health Indicators in Korea (우리나라 식품안전보건지표의 개발 및 운용과정 정립에 대한 연구)

  • This study was conducted to establish a standardized process for developing food safety health indicators. With this aim, we proposed a standardized process, accessed the validity of the suggested process by performing simulations, and provided a method to utilize the indicators. Developing process for domestic environmental health indicators was benchmarked to propose a standardized process for developing food safety health indicators, and DPSEEA framework was applied to the development of indicators. The suggested standardized process consists of an exploitation stage and a management stage. In the exploitation stage, a total of 6 procedures (initial indicators suggestion, candidate indicators selection, data availability assessment, feasibility assessment, pilot study, and final indicator selection) are conducted, and the indicators are routinely calculated and officially announced in the management stage. The exploitation stage is operated by an interaction between a task force team who manages the overall process, and an advisory committee (minimum of 4 in academia, 2 in research, 4 in specialists of Ministry of Food and Drug Safety) who reviews and performs evaluations on the indicators. The standardized process was simulated with 45 initial indicators, and total of 4 indicators (17 detailed indicators) were selected: 'Proportion of domestic fruit/vegetable receiving 'acceptable' in the evaluation of pesticide/herbicide residues', 'Food-borne disease outbreaks', 'Food-borne legal infectious disease incidence', 'Salmonellosis incidence'. Synthetic food safety health index was derived by calculating percent difference with the data from 2010 to 2012. Results showed that when comparing the year 2010 to 2011, and 2011 to 2012, the overall food safety status improved by 10.37% and 9.87%, respectively. In addition, the contribution of indicators to the overall food safety status can be determined by looking into the individual indicators, and the synthetic index may be illustrated to enhance the ease of interpretation to the public and policy makers. In overall, food health safety indicators can be useful in many ways and therefore, attention should be drawn to conduct further studies and establish related legislations.

A Clinical Analysis of Thymoma (흉선종의 임상적 고찰)

  • Thymoma is defined as a neoplasm of the epithelialreticular framework cells of the thymus. The treatment of choice and the prognosis for patients with thymoma are still controversial subjects due to lack of a uniform histological classification and standardized criteria for typing thymic tumors. Between June 1985 and May 1989, eight patients underwent thymomectomy at the Department of Thoracic and Cardiovascular Surgery, Korea University Medical Center. A clinical analysis was performed and the following result was obtained. In histologic cell type, epithelial type was 2 cases, lymphocytic type 3 cases, and mixed type was 3 cases. The clinical stage was classified by Masaoka’s classification that Stage I was 4 cases, Stage II; 2 cases, Stage III; 2 cases and Stage IV was none. Myasthenia Gravis was associated with thymoma in 5 patients. In 8 patients, thymomectomy were performed. Adjuvant radiotherapy was performed in one patient with Stage I due to suspicious tumor remnant and in Stage II, III patients routinely. Combined chemotherapy was performed in one patient with Stage III due to local recurrence. We concluded that the most important prognostic factor of thymoma is extent of tumor invasion. And the presence of Myasthenia Gravis is no more prognostic factor. Surgical resection of thymoma is treatment of choice, If local invasion or distant dissemination is present, postoperative radiotherapy and /or combined chemotherapy is recommended.

Maximum standardized uptake value at pre-treatment PET in estimating lung cancer progression after stereotactic body radiotherapy

  • Purpose: This study aimed to identify the feasibility of the maximum standardized uptake value (SUVmax) on baseline 18F-fluorodeoxyglucose positron emission tomography-computed tomography (FDG PET/CT) as a predictive factor for prognosis in early stage primary lung cancer treated with stereotactic body radiotherapy (SBRT). Materials and Methods: Twenty-seven T1-3N0M0 primary lung cancer patients treated with curative SBRT between 2010 and 2018 were retrospectively evaluated. Four patients (14.8%) treated with SBRT to address residual tumor after wedge resection and one patient (3.7%) with local recurrence after resection were included. The SUVmax at baseline PET/CT was assessed to determine its relationship with prognosis after SBRT. Patients were divided into two groups based on maximum SUVmax on pre-treatment FDG PET/CT, estimated by receiver operating characteristic curve. Results: The median follow-up period was 17.7 months (range, 2.3 to 60.0 months). The actuarial 2-year local control, progression-free survival (PFS), and overall survival were 80.4%, 66.0%, and 78.2%, respectively. With regard to failure patterns, 5 patients exhibited local failure (in-field failure, 18.5%), 1 (3.7%) experienced regional nodal relapse, and other 2 (7.4%) developed distant failure. SUVmax was significantly correlated with progression (p = 0.08, optimal cut-off point SUVmax > 5.1). PFS was significantly influenced by pretreatment SUVmax (SUVmax > 5.1 vs. SUVmax ≤ 5.1; p = 0.012) and T stage (T1 vs. T2-3; p = 0.012). Conclusion: SUVmax at pre-treatment FDG PET/CT demonstrated a predictive value for PFS after SBRT for lung cancer.

  • Purpose: This study was designed to explore the stage distribution of subjects according to stages of change for exercise and to identify factors that could discriminate subjects in various stages. Methods: The sample consisted of 182 subjects who had metabolic syndrome. The instruments used in this study were the stage placement instrument for exercise, the metabolic syndrome knowledge and metabolic syndrome health belief scale, and the metabolic syndrome exercise self efficacy scale. Data were analyzed using chi-square, ANOVA, and discriminant analysis by using the SPSS 19.0 program. Results: For exercise stages, exercise efficacy, occupation, and exercise benefit showed high standardized canonical discriminant function coefficients. Subjects in precontemplation/contemplation stage for exercise were more likely to have occupations, had less exercise efficacy and exercise benefit than those in other stage. Conclusion: This study implies that the level of exercise efficacy and exercise benefit of subjects in precontemplation/contemplation stage need to be enhanced in developing exercise program incorporating the stage of change for metabolic syndrome patients.
