중소병원 임상간호사의 감염관리 지식과 표준주의 수행도 (Infection Control Knowledge and Standard Precaution Practice among Clinical Nurses in Small and Medium-sized Hospital)

  • 본 연구의 목적은 중소병원 간호사의 감염관리 지식, 표준주의 수행도를 파악하고 표준주의 수행도에 미치는 영향 요인을 규명하기 위함이다. 본 연구는 횡단적 서술적 조사연구 방법으로 7개 중소병원 간호사 258명을 대상으로 수행되었다. 감염관리 지식은 평균 7.25점, 표준주의 수행도는 평균 3.61점이었으며, 임상경력(r=.123, p=.047)과 직위(F=5.356, p=.005)에 따라 표준주의 수행도에 차이를 보였다. 감염관리 지식과 표준주의 수행도 간에는 통계적으로 유의한 양의 상관관계가 있었으며(r=.421, p<.001), 표준주의 수행도에 영향을 미치는 요인은 직위(β=-.187, p=.025)와 감염관리 지식(β=.408, p<.001)으로 나타났다. 중소병원 간호사의 의료관련감염 관리를 위한 표준주의 수행도를 증진시키기 위한 방안으로 일반간호사를 대상으로 감염관리에 대한 융복합 교육이 요구된다.

  • 이 연구는 임상실습을 실시하면서 병원감염에 노출되어 있는 응급구조과 학생들을 대상으로 병원감염관리 표준주의 지침의 인지도와 수행도를 파악함으로써 감염예방과 노출을 감소시킬 수 있는 효율적인 실천방안을 마련하는데 기초자료를 제공하고자 하였다. 연구결과 병원감염관리 표준주의 인지도에 비해 수행도가 낮은 결과를 보였고, 인지도와 수행도는 양의 상관관계가 있는 것으로 나타났으며 통계적으로 유의한 차이가 있었다(r=0.325, p=0.000). 향후 응급구조과 학생들의 병원감염관리 수행도를 증진 시킬 수 있도록 교과과정과 실습기관에서의 지속적인 교육프로그램이 수행되어야 할 것이다.

  • 본 연구는 간호학과에 재학중인 학생을 대상으로 의료관련감염관리를 위한 표준주의 지식, 수행의도, 수행에 대한 태도, 표준주의지침 수행을 위한 안전환경 및 수행도를 파악하고 이들 요인들이 간호대학생들의 표준주의 수행도에 어떠한 영향을 미치는 지를 규명하고자 시도되었다. 2018년 6월 15일부터 6월 30일까지 경기도 A대학교 간호학과 3~4학년 296명에게 설문지를 이용하여 자료수집을 하였다. 연구결과 대상자의 표준주의 수행도와 의료관련감염 관리지침 수행의도, 의료 관련감염 관리지침 수행에 대한 태도와 표준주의지침수행을 위한 안전환경은 각각 양의 상관관계로 나타났다. 대상자의 의료관련감염예방 수행도에 영향을 미치는 요인은 간호학생의 의료관련감염 관리지침 수행의도와 표준주의지침수행을 위한 안전환경으로 나타났으며 이 두 변수가 의료관련감염예방 수행도를 20% 설명하는 것으로 나타났다. 따라서 간호대학생들의 의료관련감염관리에 대한 표준주의의 수행도를 증진시키기 위해 수행의도를 높일 수 있는 교육프로그램의 개발과 보다 안전한 실습환경 조성이 필요할 것으로 사료된다.

  • This study considered as precautions in light of practical affairs related to a claim for damages focusing on CISG (1980) and PICC (2004). Given summarizing contents of this study, those are as follows. First, when exercising a claim for damages, proving the damages may be difficult and hard. Thus, there is necessity for stating the liquidated damages clause in contract given conclusion of contract. Second, as for the application of interest rate given a claim for interest, CISG is not covered interest rate. PICC is covered interest rate. However, there is possibility that PICC will not be applied as general principles. Thus, to remove this insecurity and uncertainty, there is necessity for stating this in contract by deciding on the detailed standard stipulation after fully discussing about interest payment with the counterpart given sale contract. Third, when a seller delivered non-conformity of the goods for contract, a buyer is desirable to exercise by discreetly judging the exercise method or limitation element on a problem of selecting and exercising remedy favorable to oneself out of a claim for damages and a right to reduce the price. Finally, There was suggestion that the contract parties are desirable to utilize by modifying and supplementing properly this in line with own business-based necessity and situation based on the ICC Model International Sale Contract, and to state CISG and PICC the governing law clause, in preparing contract. This study is expected to possibly become guideline in which the damaged party exercises a claim for damages or aims to cope with the counterpart's exercising a claim for damages.

Comparative Analysis of COVID-19 Infection Prevention Control Guidelines from Seven Countries: Implications on COVID-19 Response and Future Guidelines Development

  • Background: As prevention of coronavirus disease 2019 (COVID-19) transmission in healthcare settings has become a critical component in its effective management, COVID-19 specific infection prevention and control (IPC) guidelines were developed and implemented by numerous countries. Although largely based on the current evidence-base, guidelines show much heterogeneity, as they are influenced by respective health system capacities, epidemiological risk, and socioeconomic status. This study aims to analyze the variations and concurrences of these guidelines to draw policy implications for COVID-19 response and future guidelines development. Methods: The contents of the COVID-19 IPC guidelines were analyzed using the categories and codes developed based on "World Health Organization guidelines on core components." Data analysis involved reviewing, appraising and synthesizing data from guidelines, which were then arranged into categories and codes. Selection of countries was based on the country income level, availability of COVID-19 specific IPC guideline developed at a national or district level. Results: The guidelines particularly agreed on IPC measures regarding application of standard precautions and providing information to patients and visitors, monitoring and audit of IPC activities and staff illnesses, and management of built environment/equipments. The guidelines showed considerable differences in certain components, such as workplace safety measures and criteria for discontinuation of precautions. Several guidelines also contained unique features which enabled a more systematic response to COVID-19. Conclusion: The guidelines generally complied with the current evidence-based COVID-19 management but also revealed variances stemming from differences in local health system capacity. Several unique features should be considered for benchmark in future guidelines development.

EMC Safety Margin Verification for GEO-KOMPSAT Pyrotechnic Systems

  • Pyrotechnic initiators provide a source of pyrotechnic energy used to initiate a variety of space mechanisms. Pyrotechnic systems build in electromagnetic environment that may lead to critical or catastrophic hazards. Special precautions are need to prevent a pulse large enough to trigger the initiator from appearing in the pyrotechnic firing circuits at any but the desired time. The EMC verification shall be shown by analysis or test that the pyrotechnic systems meets the requirements of inadvertent activation. The MIL-STD-1576 and two range safeties, AFSPC and CSG, require the safety margin for electromagnetic potential hazards to pyrotechnic systems to a level at least 20 dB below the maximum no-fire power of the EED. The PC23 is equivalent to NASA standard initiator and the 1EPWH100 squib is ESA standard initiator. This paper verifies the two safety margins for electromagnetic potential hazards. The first is verified by analyzing against a RF power. The second is verified by testing against a DC current. The EMC safety margin requirement against RF power has been demonstrated through the electric field coupling analysis in differential mode with 21 dB both PC23 and 1EPWH100, and in common mode with 58 dB for PC23 and 48 dB for 1EPWH100 against the maximum no-fire power of the EED. Also, the EMC safety margin requirement against DC current has been demonstrated through the electrical isolation test for the pyrotechnic firing circuits with greater than 20 dB below the maximum no-fire current of the EED.

  • The standard of living has been getting higher by reason of industrialization and economic growth. It makes people have more cultural desire(wants). Because of reasons mentioned above, a number of entertainment place and performance with high-tech is increased. This has been brought also increasing a number of electrical accident such as electrical shock, electrical fires and accidents of electrical facilities. This paper suggests technical, operational and special precautions of actual electric main line systems in entertainment places and is also written on the purpose of being applied with related provisions to designing electrical facilities, construction, check-up and inspection and maintenance for preventing further electrical disasters in entertainment places.

간호대학생의 의료관련감염 표준주의지침 수행도에 영향을 미치는 요인 (Factors Influencing Nursing Students' Performance on Standard Precautions of Healthcare-associated Infection)

  • 본 연구는 간호대학생의 의료관련감염 표준주의지침 지식, 인지도와 수행도 간의 관련성을 확인하고 수행도에 영향을 미치는 요인들을 파악하고자 시도되었다. 본 연구결과 간호대학생의 의료관련감염 표준주의 지침 지식 정도는 총점 25점 만점에 평균 19.95±2.43점이었고, 의료관련감염 표준주의지침 인지도 정도는 5점 만점에 4.71±0.37점이었으며, 의료관련감염 표준주의지침 수행도 정도는 5점 만점에 4.55±0.49점이었다. 일반적 특성에 따른 의료관련감염 표준주의지침 수행도 정도는 학년(t=-3.627, p=.000), 감염예방교육 경험(t=3.239, p=.001)에 따라 통계적으로 유의한 차이가 있었고, 의료관련감염 표준주의지침 수행도는 지식(r=.201, p=.000), 인지도(r=.522, p=.000)와 통계적으로 유의한 양의 상관관계를 나타냈다. 의료관련감염 표준주의지침 수행도에 영향을 미치는 요인은 인지도(β=.507, p=.000), 학년(β=.169, p=.000), 감염예방교육 경험(β=.094, p=.043) 순이었고, 30.8%의 설명력을 나타냈다(F=50.520, p=.000). 따라서 간호대학생의 의료관련감염 표준주의지침 수행도를 향상시키기 위해서는 학년별 수준에 따른 적절한 의료관련 감염예방 교육방안을 모색하고 인지도를 향상시킬 수 있는 프로그램의 적용이 필요할 것으로 사료된다. 추후연구에서는 연구대상과 지역을 확대하고 신뢰도와 타당도 및 객관성을 가진 측정도구를 사용한 반복연구와 인지도를 향상시킬 수 있는 역할극이나 시뮬레이션교육프로그램의 적용 및 효과검증연구를 제언한다.

시뮬레이션을 적용한 임상추론 교과목의 적용효과: 일 대학의 예를 중심으로 (Evaluation of Effects of a Clinical Reasoning Course among Undergraduate Nursing Students)

  • Purpose: To evaluate undergraduate nursing students' ability in clinical competence, critical thinking, and problem solving following enrollment in a clinical reasoning course. Methods: A clinical reasoning course utilizing a human patient simulator and scenarios was offered to 22 senior students at a College of Nursing in Seoul. Students' clinical competence was measured with a checklist of 15 items by analyzing students' performance recorded on video tapes for eight scenarios. Critical thinking disposition and problem solving were measured by a self-administered questionnaire before and after the course. Data were analyzed using descriptive statistics and Wilcoxon signed-rank test. Results: The high scored items of clinical competence were: 'obtain relevant subjective/objective data', 'interpret vital signs', 'communicate with healthcare providers', and 'utilize standard precautions including handwashing.' Students' critical thinking and problem solving scores following the course were increased with statistical significance. Conclusion: A clinical reasoning course utilizing a human patient simulator creates a realistic clinical environment for nursing students and provides the opportunity to obtain clinical competence, critical thinking, and problem solving skills.

A Locally Acquired Falciparum Malaria via Nosocomial Transmission in Korea

  • A 57-year old man who was admitted to an emergency room of a tertiary hospital with hemoptysis developed malarial fever 19 days later and then died from severe falciparum malaria 2 days later. He had not traveled outside of Korea for over 30 years. Through intensive interviews and epidemiological surveys, we found that a foreign patient with a recent history of travel to Africa was transferred to the same hospital with severe falciparum malaria. We confirmed through molecular genotyping of the MSP-1 gene that Plasmodium falciparum genotypes of the 2 patients were identical. It is suggested that a breach of standard infection control precautions resulted in this P. falciparum transmission between 2 patients in a hospital environment. This is the first report of a nosocomial transmission of falciparum malaria in Korea.
