Constructing a Standard Clinical Big Database for Kidney Cancer and Development of Machine Learning Based Treatment Decision Support Systems (신장암 표준임상빅데이터 구축 및 머신러닝 기반 치료결정지원시스템 개발)

  • Since renal cell carcinoma(RCC) has various examination and treatment methods according to clinical stage and histopathological characteristics, it is required to determine accurate and efficient treatment methods in the clinical field. However, the process of collecting and processing RCC medical data is difficult and complex, so there is currently no AI-based clinical decision support system for RCC treatments worldwide. In this study, we propose a clinical decision support system that helps clinicians decide on a precision treatment to each patient. RCC standard big database is built by collecting structured and unstructured data from the standard common data model and electronic medical information system. Based on this, various machine learning classification algorithms are applied to support a better clinical decision making.

A Keyword Network Analysis of Standard Medical Terminology for Musculoskeletal System Using Big Data (빅데이터를 활용한 근골격계 표준의료용어에 대한 키워드 네트워크 분석)

  • The purpose of this study is to suggest a plan to utilize atypical data in the health care field by inferring standard medical terms related to the musculoskeletal system through keyword network analysis of medical records of patients hospitalized for musculoskeletal disorders. The analysis target was 145 summaries of discharge with musculoskeletal disorders from 2015 to 2019, and was analyzed using TEXTOM, a big data analysis solution developed by The IMC. The 177 musculoskeletal related terms derived through the primary and secondary refining processes were finally analyzed. As a result of the study, the frequent term was 'Metastasis', the clinical findings were 'Metastasis', the symptoms were 'Weakness', the diagnosis was 'Hepatitis', the treatment was 'Remove', and the body structure was 'Spine' in the analysis results for each medical terminology system. 'Oxycodone' was used the most. Based on these results, we would like to suggest implications for the analysis, utilization, and management of unstructured medical data.

  • In this study, a system to increase the expressiveness of existing standard terminology using three-dimensional (3D) data is designed. We analyze the existing medical terminology system by searching the reference literature and perform an expert group focus survey. A human body image is generated using a 3D modeling tool. Then, the anatomical position of the human body is mapped to the 3D coordinates' identification (ID) and metadata. We define the term to represent the 3D human body position in a total of 12 categories, including semantic terminology entity and semantic disorder. The Blender and 3ds Max programs are used to create the 3D model from medical imaging data. The generated 3D human body model is expressed by the ID of the coordinate type (x, y, and z axes) based on the anatomical position and mapped to the semantic entity including the meaning. We propose a system of standard terminology enabling integration and utilization of the 3D human body model, coordinates (ID), and metadata. In the future, through cooperation with the Electronic Health Record system, we will contribute to clinical research to generate higher-quality big data.

Research on a Valuation Standard and the Actual Condition About Security Management in PACS (PACS에서 보안관리 평가기준 연구와 실태조사)

  • This study is to prepare an evaluation standard about personal information protection and security management of a medical institution and to build up a grade standard of evaluation in PACS environment. We built up evaluation index based on 10 detailed items in four big categories (political security, technical security, data management security and physical security) by referring to ISO17799 (BS 7799), HIPPA (Health Insurance and Portability and Accountability Act of 1996) and domestic medical law. We have investigated at the thirty places where medical facility with the extracted security criteria and security evaluation index. Average score of physical security list, one of the big categories, was 18.5/20 (93%) at all medical institutions. Political security score was 18.5/30 (62%), data management security score was 12/20 (60%) and technical security score was 17.5/30 (58%). Therefore, security evaluation score was average 67 in 30 general hospitals, which was 4th level. The results showed that it is necessary to establish evaluation and management standard about personal information protection and security consciousness which are weak in PACS environment.

  • 본 논문에서는 원광대학교병원 의료정보시스템의 임상데이터를 OHDSI 가 제안하는 공통데이터 모델로 변환하여 표준화 시스템 구축에 대해서 기술한다. 또한, 검색속도 향상을 위해 인덱싱 기법을 적용한 성능평가 결과를 보인다. 구축된 표준화 시스템은 다양한 임상연구에 활용될 것을 기대하고 있다.

  • Objectives: The aim of this study is to explore and investigate the status of EMR currently used in Korean medical institutions and the suitability of the existing certification criteria for Korean medicine EMR certification. Methods: The survey was conducted using a related questionnaire from September to October 2022. The survey for current status and the suitability of the existing certification criteria was conducted separately between Korean medical institutions and Korean medicine EMR companies. Results: In a survey of Korean medical institutions on the current status of EMR, more than 80% answered that the imaging system and Korean medicine EMR could be linked. Most medical institutions did not exchange clinical information between institutions. When asked about the intention to develop standard EMR of Korean medicine in the future, 57% of institutions answered 'yes'. In future, if Korean medicine EMR certification criteria are developed, all EMR companies are willing to develop the EMR that satisfy them. Looking at the satisfaction survey of the existing EMR certification criteria of the Korean medicine EMR system, it was found that high/low satisfaction was shown in various areas, and in particular, the overall clinical information exchange function was insufficient. Conclusion: In order to introduce the Korean medicine EMR certification criteria, it must be considered of the current status of EMR and applicability of Korean medicine EMR for establishment of Korean medicine standard EMR certification criteria. By developing Korean medicine EMR certification criteria, high-quality medical services can be provided to medical consumers who want Korean medical treatment.

  • The data analysis system using Big-data is worthy to be used in various fields such as politics, traffic, natural disaster, shopping, customer management, medical care, and weather information. Particularly, the analysis of the momentum of an individual using an acceleration signal collected from a wearable device has already been widely used. However, since the data used in such a system stores only the data necessary for measuring the individual activity, it does not provide various analysis results other than the exercise amount of the individual. In this paper, I propose a system that analyzes collective behavior pattern and activity based on the acceleration signal that can be collected from personal smartphones for 24 hours a day and stored in big data. I also propose a system that sends acceleration signals and receives analysis results using standard messaging to use on various smart devices.

On statistical Computing via EM Algorithm in Logistic Linear Models Involving Non-ignorable Missing data

  • Many data sets obtained from surveys or medical trials often include missing observations. When these data sets are analyzed, it is general to use only complete cases. However, it is possible to have big biases or involve inefficiency. In this paper, we consider a method for estimating parameters in logistic linear models involving non-ignorable missing data mechanism. A binomial response and normal exploratory model for the missing data are used. We fit the model using the EM algorithm. The E-step is derived by Metropolis-hastings algorithm to generate a sample for missing data and Monte-carlo technique, and the M-step is by Newton-Raphson to maximize likelihood function. Asymptotic variances of the MLE's are derived and the standard error and estimates of parameters are compared.

Standard based Deposit Guideline for Distribution of Human Biological Materials in Cancer Patients

  • Background: Human biological materials from cancer patients are linked directly with public health issues in medical science research as foundational resources so securing "human biological material" is truly important in bio-industry. However, because South Korea's national R and D project lacks a proper managing system for establishing a national standard for the outputs of certain processes, high-value added human biological material produced by the national R and D project could be lost or neglected. As a result, it is necessary to develop a managing process, which can be started by establishing operating guidelines to handle the output of human biological materials. Materials and Methods: The current law and regulations related to submitting research outcome resources was reviewed, and the process of data 'acquisition' and data 'distribution' from the point of view of big data and health 2.0 was examined in order to arrive at a method for switching paradigms to better utilize human biological materials. Results: For the deposit of biological research resources, the original process was modified and a standard process with relative forms was developed. With deposit forms, research information, researchers, and deposit type are submitted. The checklist's 26 items are provided for publishing. This is a checklist of items that should be addressed in deposit reports. Lastly, XML-based deposit procedure forms were designed and developed to collect data in a structured form, to help researchers distribute their data in an electronic way. Conclusions: Through guidelines included with the plan for profit sharing between depositor and user it is possible to manage the material effectively and safely, so high-quality human biological material can be supplied and utilized by researchers from universities, industry and institutes. Furthermore, this will improve national competitiveness by leading to development in the national bio-science industry.

  • The research for coatings is one of the most popular and active research in the polymer industry. For the coatings, electronics industry, medical and optical fields are growing more important. In particular, the trend is the increasing of the technical requirements for the performance and accuracy of the coatings by the development of automotive and electronic parts. In addition, the industry has a need of more intelligent and automated system in the industry is increasing by introduction of the IoT and big data analysis based on the environmental information and the context information. In this paper, we propose an optimization model for the design of experiments based coating formulation data objects using the Internet technologies and big data analytics. In this paper, the coating formulation was calculated based on the best data analysis is based on the experimental design, modify the operator with respect to the error caused based on the coating formulation used in the actual production site data and the corrected result data. Further optimization model to correct the reference value by leveraging big data analysis and Internet of things technology only existing coating formulation is applied as the reference data using a manufacturing environment and context information retrieval in color and quality, the most important factor in maintaining and was derived. Based on data obtained from an experiment and analysis is improving the accuracy of the combination data and making it possible to give a LOT shorter working hours per data. Also the data shortens the production time due to the reduction in the delivery time per treatment and It can contribute to cost reduction or the like defect rate reduced. Further, it is possible to obtain a standard data in the manufacturing process for the various models.
