보행 시 노인의 상체 움직임에 대한 3차원적 분석 (The Three Dimensional Analysis of the Upper Body's Segments of the Elderly during Walking)

  • The purpose of this study was to investigate the kinematic variables of the upper part of the body for 8 elderly men during walking. For this study, kinematic data were collected using a six-camera (240Hz) Qualisys ProReflex system. The room coordinate system was right-handed and fixed in space, with righted orthogonal segment coordinate systems defined for the head, trunk, and pelvis. Based on a rigid body model, reflective marker triads were attached on the 3 segments. Three-dimensional Cartesian coordinates for each marker were determined at the time of recording using a nonlinear transformation(NLT) technique with ProReflex software (Qualisys, Inc.). Coordinate data were low-pass filtered using a fourth-order Butterworth with cutoff frequency of 6Hz. Three-dimensional angles of the head, trunk, and pelvis segment were determined using a Cardan method. On the basis of each segment angle, angle-angle plot used to estimated the movement coordinations between segments. The conclusions were as follows; (1) During the support phase of walking, the elderly people generally kept their, head the flexional and abductional posture. Particularly, the elderly displayed little internal/external rotation. (2) The elderly people showed extensional and external rotation postures in the trunk movement. Particularly, It showed the change from adduction into abduction at the heel contact event of the stance phase. (3) The elderly people showed almost same pelvis movement from the flexion into extension, from the abduction into adduction, and from internal rotation into external rotation at the mid stance and toe off of the stance phase.

보행 입각기(stance phase)시 거골하 관절의 운동 특성에 관한 연구 - CTA 및 시공간적 변수를 중심으로 - (A study of the movement characteristics of subtalar joint in stance phase)

  • The purpose of this study was to analyse the subtalar joint movement characteristics in human stance phase. The data of subtalar joint movement patterns are collected by CTA(calcaneus to tibia angle) measurements. CTA is defined as a angle formed between the bisect of the posterior shank and bisect of the posterior heel, as determined by placement of the retroreflective markers. The angle measured in degrees. The participants are 74 healthy individuals (37 men and 37 women) who have no orthopedic and neurological impairment, aged from 19 to 29 years(mean 22.95). Prior to participation, each subjecct informed the procedures of experiment from researcher and assistant researcher. The equipments of this study are walking grid, marking tapes, goniometer, video camera, monitor and ink for foot print are used in the study. In order to determine the statistical significance of result, the paired t-test and Pearson correlation were applied at the 0.05 level of significance. The results were as follows : 1. The reliability of measured CTA value are showed a high correlation, ranged from .86 - .94. 2. The mean value of step width are 7.67cm in men and 6cm in women. So, significant difference between men and women in 0.05 level of significance. 3. There was significant difference between man and women's TOA(toe out angle)(p<0.05). 4. The CTA of female's is more higher than male's one, however not statistical difference between man and women(p>0.05). 5. The CTA is reduced according to increasing TOA(p<0.05).

편마비 보행 시 족저압력중심의 이동특성과 동적균형능력의 상관관계 연구 (The Relationship between the Plantar Center of Pressure Displacement and Dynamic Balance Measures in Hemiplegic Gait)

  • The purpose of this study was to investigate how COP displacement of a hemiplegic foot in stance phase during gait is related to clinical balance measures and the recovery stage in hemiplegic stroke patients. Twenty-eight functionally ambulant hemiplegic patients who had suffered from strokes and thirty age-matched healthy subjects participated in this study. COP parameters were calculated. Clinical balance was measured using the Functional Reach Test (FRT) and Timed Up and Go Test (TUGT). The recovery stage, proprioception, and clonus of the ankles or lower extremities were also measured for physical impairment status. The COPx max-displacement in the medial-lateral side of the stroke patients was significantly longer than that of the normal group (p=.038). The COPy max-displacement in the anterior-posterior side of the stroke patients was significantly shorter than that of normal group (p<.001). Significant differences in the COPx and COPy displacement asymmetry index were found between the two groups (p<.01). The FRT was correlated with the COPx displacement (r=.552) and COPy displacement (r=.765). The TUGT was correlated with the COPy displacement (r=-.588) only. The recovery stage of the lower extremities was correlated with COPy displacement (r=.438). The results of the study indicate that the characteristic of COP displacement in hemiplegic feet in stance phase during gait is related to balance ability and recovery in stroke patients. COP parameters acquired by the mapping of foot pressure in stance phase during gait will provide additional useful clinical information. This information can be used by clinicians to assess objectively the pathologic gait with other diseases and to evaluate the therapeutic effects on gait in stroke patients.

하지 근육의 피로상태 동안 높은 굽 신발에 적용한 전면접촉인솔이 젊은 여성의 보행 특성에 미치는 영향 (The Effect of High-Heeled Shoes With Total Contact Inserts in the Gait Characteristics of Young Female Adults During Lower Extremity Muscle Fatigue)

  • This study investigated gait characteristics, kinematics, and kinetics in the lower extremities between two different shoe conditions (high heeled shoes (7 cm), and high heeled shoes with a total contact insert (TCI)) after lower extremity muscle fatigue. Although TCI shave been applied in high heeled shoes to increase comfort and to decrease foot pressure, no study has attempted to identify the effects of TCI in fatigue conditions. The purpose of this study was to determine the effects of walking in high heeled shoes with TCI after lower extremity muscle fatigue was induced. This study was carried out in a motion analysis laboratory at Hanseo University. A volunteer sample of 14 healthy female subjects participated. All in fatigue conditions, the subjects were divided into two groups. The muscle fatigue was induced by 40 voluntary dorsi- and plantar-flexion exercises and 40 heel-rise exercises of the dominant foot. Surface electromyography was used to confirm the localized muscle fatigue using power spectral analysis of three muscles (tibialis anterior, gastrocnemius medialis and lateralis). The results were as follows: (1) In muscle fatigue conditions, the use of TCI decreased the peak flexion angle of the hip joint significantly in the early stance phase (p<.05) and increased the peak hip flexion moment in the terminal stance phase (p<.05). (2) In muscle fatigue conditions, the application of TCI also increased peak hip power generation in the early stance phase and peak hip power absorption in the terminal stance phase (p<.05). (3) In muscle fatigue conditions, the use of TCI reduced the impact force significantly and increased the secondary peak vertical GRF. These findings suggest that the TCI may provide beneficial effects when muscle fatigue occurs for a high heeled shoe gait. Future research employing the patient population and various types of TCI materials are required to clarify the effects of TCI.

Effects of Action Observation Training Combied with Auditory Cueing on Gait Ability in Patients with Stroke: a Preliminary Pilot Study

  • Purpose: New therapeutic approaches have emerged to improve gait ability in patients with brain damage, such as action observation learning (AOT), auditory cueing, motor imagery etc. We attempted to investigate the effects of AOT with auditory cueing (AOTAC) on gait function in patients with stroke. Methods: The eighteen stroke patients with a unilateral hemiparesis were randomly divided into three groups; the AOTAC, AOT, and control groups. The AOTAC group (n=8) received training via observing a video that showed normal gait with sound of footsteps as an auditory cue; the AOT group (n=6) receive action observation without auditory stimulation; the control group (n=5) observed the landscape video image. Intervention time of three groups was 30 minutes per day, five times a week, for four weeks. Gait parameters, such as cadence, velocity, stride length, stance phase, and swing phase were collected in all patients before and after each training session. Results: Significant differences were observed among the three groups with respect to the parameters, such as cadence, velocity, stride length, and stance/swing phase. Post-hoc analysis indicated that the AOTAC group had a greater significant change in all of parameters, compared with the AOT and control groups. Conclusion: Our findings suggest that AOTAC may be an effective therapeutic approach to improve gait symmetry and function in patients with stroke. We believe that this effect is attributable to the change of cortical excitability on motor related to cortical areas.

Altered Ground Reaction Forces in Individuals with Chronic Ankle Instability Compared to Lateral Ankle Sprain Copers and Healthy Controls during Walking

  • Objective: Few studies have investigated alterations of ground reaction force (GRF) in individuals with chronic ankle instability (CAI) compared with lateral ankle sprain (LAS) copers and healthy controls during walking. This study aimed to investigate differences in GRF variables among the CAI, LAS coper, and control groups. Method: Eighteen individuals with CAI, 18 LAS copers, and 18 healthy controls were recruited for this study. All participants walked on 8-m walkway with a force plate three times. GRF data during stance phase were extracted and analyzed. The analysis of variance and ensemble curve analysis were used for statistical analyses of discrete points and time-series data respectively. Results: The CAI group showed a greater loading rate (LR) and a shorter time to impact peak force than the other groups, as well as decreased vGRF from 56% to 65% in the stance phase than the control group. No significant differences were noted in the other variables. Conclusion: Based on these findings, individuals with CAI should enhance their ability to create propulsion during the push-off phase and spend more time absorbing GRF to decrease the LR, which is considered one of risk factors for overuse injury and ankle osteoarthritis.

3차원적 동작 분석기를 이용한 건강한 여자 노인의 하지 정렬 상태와 슬관절 내전 모멘트의 상관 관계에 관한 연구 (3-Dimensional Gait analysis and the relationship between lower limb alignment and knee adduction moment in elderly healthy women)

  • Gait analysis can provide a better understanding of how the alignment of the lower limb and foot can contribute to force observed at the knee. Anatomic and mechanical factors that affect loading in the knee pint can contribute to pathologic change seen at the knee in degenerative pint disease and should be considered in treatment plan. The purpose of this study is to present the gait analysis data and to determine whether there is any relationships between alignment of the lower limb, foot progression angle and knee pint moments in elderly healthy women with 3-dimensional motion analyzer. The results were as follows; 1. Cadence showed 114.8 steps/min, gait speed showed 1.05 m/s, time per a stride showed 1.06 sec, time per a step showed 0.53 sec, single-supporting phase was 0.41 sec, double-supporting phase was 0.24 sec, stride length was 1.04 m, Step length was 0.56 m. 2. According to the parameters of kinematics, the maximal knee flexion angle through swing phase showed left $46.82^{\circ}$, right $40.19^{\circ}$ and the maximal knee extension angle showed left $-1.32^{\circ}$, right $2.01^{\circ}$. knee varus showed left $26.90^{\circ}$, right $30.93^{\circ}$. 3. Moment, one of kinetic parameters of knee pint the maximal flexion moment showed left 0.363. Nm/kg, right 0.464 Nm/kg and maximal extension moment showed left 0.389 Nm/kg, right 0.463 Nm/kg. The maximal. adduction moment showed left 0.332 Nm/kg, right 0.379 Nm/kg and the maximal internal rotatory moment showed left 0.13 Nm/kg, right 0.140 Nm/kg. 4. On sagittal plane, the maximal power of knee joint showed left 0.571 J/kg, right 0.629 J/kg. On coronal plane, the maximal power of knee joint showed left 0.11 J/kg, right 0.12 J/kg. On transverse plane, the maximal power of knee joint showed left 0.058 J/kg, right 0.072 J/kg. 5. The subject who had varus alignment of the lower extremity had statistically higher in knee adduction moment in mid stance phase. 6. The subject who had large foot progression angle had statistically lower in knee adduction moment in late stance phase. A relationship was observed between the alignment of the lower extremity and the adduction moment of the knee joint during stance phase. Hence, we need some research to figure, out the change of adduction moment according to the sort of knee joint osteoarthritis and the normal geriatrics as well. And we also require more effective, specific therapeutic program by making use of those background of researches.

진동자극 저항운동이 허리통증 경험자의 균형과 보행에 미치는 영향 (The Effect of Resistance Exercise with Vibration Stimulation on Balance and Gait of Experienced Back Pain Adults)

  • 본 연구는 진동자극 저항운동이 허리통증 경험자의 균형 및 보행에 미치는 영향을 연구하였다. 30명의 히리통증 경험이 있는 성인 대학생을 대상으로 8 Hz 진동자극 저항운동(n=10), 30 Hz 진동자극 저항운동(n=10), 허리 안정화운동(n=10)으로 무작위로 나누었다. 각 그룹의 중재는 6주동안 주 3회, 1회에 30분을 적용하였다. 실험 전에 정적균형과 보행패턴을 측정하였고, 6주 후에 실험전과 동일한 방법으로 측정하였다. 연구결과 세 그룹 모두에서 운동전과 후의 시간에 따른 정적균형, 디딤기율, 흔듦기율, 한걸음 길이, 한발짝률의 변화는 통계적으로 유의한 차이가 있었다(p<.05). 허리 안정화운동이 정적균형과 디딤기율에서는 그룹 간의 유의한 차이가 있었고(p<.05), 흔듦기율 및 한걸음 길이와 한발짝률은 그룹 간의 차이가 없었다. 결론적으로 진동자극 저항운동이 근수축 유발 기전에 의해 다리근육의 근력을 강화하였고, 강화된 다리 근력은 균형능력에 긍정적인 영향을 미쳤으며, 향상된 균형능력은 신체가 움직이는 동안 안정적으로 자세를 조절할 수 있게 하여 보행능력의 긍정적 측면에 영향을 미친 것으로 판단된다.

내리막 보행시 발 전족부 부착형 아웃솔의 각도 변화가 척추기립근과 하지근의 근활성도에 미치는 영향 (The Influence of Angle Change of the Forefoot's Adhesive Outsole Designs on the Electromyographic Activity of the Erector Spinae and Selected Lower Limb Muscles during Downhill Walking)

  • The purpose of this study was to evaluate the effect of angle change of forefoot's adhesive outsole on the electromyographic activity (EMG) of the erector spinae and selected lower limbs muscle during downhill walking over $-20^{\circ}$ ramp. Thirteen male university students (age: $25.4{\pm}3.9$ yrs, height: $176.2{\pm}5.1$ cm, weight: $717.4{\pm}105.0$ N) who have no musculoskeletal disorder were recruited as the subjects. To assess the myoelectric activities of selected muscles, six of surface EMG electrodes with on-site pre-amplification circuitry were attached to erector spinae (ES), rectus femoris (RF), biceps femoris (BF), tibialis anterior (TA), lateral gastrocnemius (LG), and medial gastrocnemius (MG). To obtain maximum EMG levels of the selected muscles for normalization, five maximum effort isometric contraction were performed before the experimental trials. Each subject walked over $0^{\circ}$ and $20^{\circ}$ ramp with three different forefeet's EVA outsole (0, 10, $20^{\circ}$) in random order at a speed of $1.2{\pm}0.1$ m/s. For each trial being analyzed, five critical instants and four phases were identified from the recording. The results of this study showed that the average muscle activities of MG and LG decreased in $20^{\circ}$ shoes compared to $0^{\circ}$ and $10^{\circ}$ ones in the initial double limb stance (IDLS). In initial single limb stance (ISLS) phase, the average muscle activities of ES increased with the angle of forefoot's adhesive outsole, indicating that the increment of shoes' angle induce upper body to flex anteriorly in order to maintain balance of trunk. In terminal double limb stance (TDLS) phase, average muscle activities of TA significantly increased in $20^{\circ}$ outsole compared to $0^{\circ}$ and $10^{\circ}$ ones. There was no external forces acting on the right foot other than the gravity during terminal single limb stance (TSLS) phase, all muscles maintained moderate levels of activity.

태권도 주춤서 지르기에서 Stretch-Shortening Cycle 이 관절파워에 미치는 효과 (The Effect of Stretch-Shortening Cycle on the Joint Power of the Jireugi in the Taekwondo Juchumseogi Stance)

  • The purpose of this study was to investigate the muscle mechanical properties of the pelvic axial pre-rotational movement for the Jireugi in the Taekwondo Juchumseogi stance. Eleven elite Taekwondo Poomsae athletes participated. Each participant performed 5 right hand Jireugi in Juchumseogi stance as fast and strong as possible while their motion was recorded by a 3D motion analysis system and the ground reaction forces by two force plates. The power and work of the muscular group surrounding the waist were analyzed to verify the effect of the stretch-shortening cycle (SSC) theory. The cause of the greater power seems to be the application of the SSC by the muscles surrounding the waist during the preparation phase of the pre-rotation group. For the none pre-rotation group, they only used the concentric contraction of the muscles surrounding the waist. Because the pre-rotation group used the SSC theory, they had the effect of shortening of the range of movement, creating a fast and more powerful rotation, thus anticipating the increase the magnitude of impact.
