"동의수세보원(東醫壽世保元)" 소음인(少陰人) 표병(表病)의 병기(病機) 및 치법(治法)에 대한 고찰(考察) - 익기(益氣)와 승양(升陽)을 중심으로 - (A Study on The Disease Mechanism and Treatment of The So-eumin Exterior Disease of "Dong-uisusebowon(東醫壽世保元)" -On Ikgi(益氣) and Seung-yang(升陽)-)

  • This paper studies the main treatment methods, Ikgi(益氣) and Seung-yang(升陽) applied frequently by Lee Jema(李濟馬) on the exterior disease of So-eumin(少陰人), one of the four constitutions. The methodology of this paper is to understand the pharmacology of "Dong-uisusebowon(東醫壽世保元)" by examining the formulas applied by Lee Jema. I have examined the organization of formulas in "Dong-uisusebowon(東醫壽世保元)" beforehand to categorize the formulas applicable in this study according to an objective standard. I have analyzed the prescriptions applied to So-eumin exterior disease. As a result, I could see that in the case of Ulgwang(鬱狂) syndrome, Ikgi(益氣) and Seung-yang(升陽) methods were mainly applied, but as the disease progressed, Seung-yang(升陽) was withdrawn while Ikgi(益氣) was stressed. Likewise, in the case of Mang-yang(亡陽), both methods were mainly adopted, but as the patients got worse, the level of Seung-yang(升陽) was maintained and that of Ikgi(益氣) was elevated with the addition of Buja(附子). Through this process, we could verify the overall action of Ikgi(益氣), Seung-yang(升陽) and Buja(附子). Originally, the two methods of Ikgi(益氣) and Seung-yang(升陽) are intimately related, but by analyzing the overall functions of the two methods, we could see that Seung-yang(升陽) and Buja(附子) support the Yang gi of the interior of So-eumin, while Ikgi(益氣) resolves inner stagnation of Yang gi and emits the cold pathogen of the exterior. Also, in the course of treatment, Ikgi(益氣) could only be realized after securing Seung-yang(升陽).

  • A Literature study on the language disturbance, the results are as follows; 1. Utterance was closely concerned not only the vocal organs(pharynx, larynx, epiglottis, lips, tongue, vocal cord etc,) but also five viscera{especially heart, lung, kidney etc.) in The Yellow Emperor's Canon of Internal Medicine. It is very like the vocal mechanism in Medical science. 2. In the language disturbance, It is classified with dysarthria and dysphasia in Medical science. But in Oriental medicine, it is expressed the language disturbance as coma-speech lessness, stiff tongue-speechlessness, frightening-speechlessness etc. Especially in Oriental medicine, Non-utterance is called aphasia in literature study. 3. In the concern of the language disturbance and five viscera, $Heart{\cdot}Lung{\cdot}Kidney$ are counted of first importence. In differential diagnosis, It is divided sthenia-syndrome and asthenia-syndrome. Sthenia-syndrome is classified with wind-cold, fire-evil, adverseness of vital energy, stagnation of phlegm, is easy to cure. Asthenia-syndrome is classified with sexual desire, anxiety-meditation, fear, is hard to cure. 4. The pathogenesis of dysphasia originated from two factors; The first internal damages are consumption of body fluid caused by lung-dryness and yin-dificiency of lung & kidney. The second disease caused by exogenous evjls is sluggishness of lung-energy. 5. In many using points of acupuncture of the language disturbance, the order is LI-4(合谷), H-7(神門), K-l(湧泉), L-3(太衝), K-3(太谿), S-6(三陰交), H-5(通里), G-15(아門), C-23(廉泉), S-40(豊降), K-6(照海), L-7(列缺), S-36(足三里) etc.

선체반류 중에서 작동하는 프로펠러에 의한 방향타 유입유동 PIV 계측 (PIV Measurements of Rudder Inflow Induced by Propeller Revolution in Hull Wake)

  • In the present study, the flow fields in between the propeller and the semi-spade rudder are investigated by using PIV technique to find out the influences of both simulated hull wake and propeller wake on the incident flow to the rudder. The velocity fields are measured at the propeller rotation angle of $180^{\circ}$ and the rudder deflection angles of $0^{\circ}$. Flow fields measured at each rudder deflection angle are analyzed in terms of angle-of-attack against the rudder leading edge. The hull wake increases the angle-of-attack more than that in the uniform inflow condition, forming the angle-of-attack of about $20^{\circ}$ at 0.7R(R=propeller radius) position. The distribution of the angle-of-attack is strongly affected by the stagnation point around the leading edge and camber effect of the rudder. These effects provide asymmetric distribution of angle-of-attack with respect to the leading edge of the rudder.

앞날 및 뒷날 유동 특성을 고려한 표면양력판 이론에 의한 2차원수중익 단면해석 (A Surface Panel Method for the Analysis of Hydrofoils wih Emphasis on Local Flows around the Leading and Trailing Edges)

  • 2차원 날개 단면 주위 유동문제를 포텐셜장에서의 표면양력판이론에 의하여 해석하였고 수치해석 효율을 증대시키기 위한 방법을 제시하였다. 날개 뒷날에서 유동이 쐐기 주위의 유동과 유사하다는 특성을 이용하여 계산효율을 증대시키기 위한 쐐기형 쿠타 조건(wedge type Kutta condition)을 제시하였다. 또한 쐐기형 계산효율을 증대시키기 위하여 세부 분할 방법을 적용하였다. 즉 날개 뒷날 부근의 4개의 양력판을 세분하고 세분된 양력판에서의 다이폴세기는 쐐기 주위 유동특성을 따르게 하였다. 세부분할 방법에 의한 쐐기형 쿠타조건을 2차원 날개단면 문제에 적용하였을 경우 수치계산 효율이 증가됨이 보였다. 날개 앞날에서의 유동은 앞날 반경(leading edge radius)을 갖는 원에 접하는 포물선(osculating parabola) 주위의 유동과 유사하다는 특성을 이용하여 비교적 적은 양력판 갯수에 의한 계산결과로부터 날개 앞날 주위의 유동을 정확히 계산하였다. 날개 앞날 주위의 급격한 유동변화를 정확히 계산함으로써 캐비테이션 발생 문제 및 날개 주위 경계층 문제를 계산하기 위한 정도 높은 입력자료를 제공할 수 있게 되었다.

  • Objectives & methods : This study aimed to investigate and compare the traditional acupuncture treatment and Sa-am acupuncture treatment of stroke. We investigated ${\ll}$Sa-am chimgujeongjeon(舍岩鍼灸正傳)${\gg}$ for Sa-am acupuncture treatment, and ${\ll}$Dongeuibogam(東醫寶鑑)${\gg}$ and ${\ll}$Chimgudaesung (鍼灸大成)${\gg}$ for traditional acupuncture treatment. Results & Conclusion : 1. In the traditional acupuncture treatment, acupoints on CV, GV, GB, LI, ST meridians to remove pathogens such as fire, damp, phlegm, blood stagnation and Ashi points (nearby points) are often used rather than acupoints according to the diagnosis of excess & deficiency in organs and meridians. 2. In ${\ll}$Sa-am chimgujungjeon(舍岩鍼灸正傳)${\gg}$, symptoms of stroke are classified into 21 and each symptoms are analyzed according to the diagnosis of excess & deficiency of organs and meridians, consequently treated using tonification & sedation of corresponding meridians. 3. For the treatment of stroke in ${\ll}$Sa-am chimgujungjeon(舍岩鍼灸正傳)${\gg}$, tonification & sedation of the acupoints on related meridian is often omitted, using only the acupoints on targeted meridian. 4. In ${\ll}$Sa-am chimgujungjeon(舍岩鍼灸正傳)${\gg}$, empirical points are preferably used for the treatment of stroke.

경항통(頸項痛)의 변증(辨證)에 관한 문헌고찰(文獻考察) (The Study on the Books of Oriental Medicine Which Deal with Variation in Diagnosis on the Neck and Nuchal Pain)

  • Objectives : The studies on variation in diagnosis on the neck and nuchal pain has not been done thoroughly as we can use it in clinical practice of these days. For this reason, I examined the variation in diagnosis mentioned in the classics of Oriental Medicine as the preceding study on standardization of variation in diagnosis on neck and nuchal pain. Methods : I gathered the twenty kinds of classics of Oriental Medicine that were computerized, the textbooks on Oriental Medicine which are being used these days, and the theses on current clinical research. After gathering these data, I analyzed these according to the variation in diagnosis. Results : The classics of Oriental Medicine on the neck and nuchal pain mentioned very much about the neck and nuchal pain occurred by the pathogenic factor of Wind, Cold, and Dampness, disharmony created by deficiency of Liver and Kidney, and pathogenic state of Meridians of Taiyang. According to the texts of these days, the differentiation of syndromes can be divided into four kinds of items such as Wind-Cold pathogen, Wind-Dampness pathogen, Phelgm-Heat, and disharmony between Qi and Blood. The theses of these days rarely mentioned about variation in diagnosis on the neck and nuchal pain. Conclusions : The differentiation of syndromes on the neck and nuchal pain can be divided into four kinds items as affection by exopathogen like Wind, Cold, Dampness, Heat, and so on, stagnation of Qi and the coagulation blood, deficient syndrome of Liver and Kidney, and deficient syndrome of Qi and Blood.

대기예보모형과 진단모형 결합을 통한 복잡지형 바람장 해석능력 평가 (Skillful Wind Field Simulation over Complex Terrain using Coupling System of Atmospheric Prognostic and Diagnostic Models)

  • A system coupled the prognostic WRF mesoscale model and CALMET diagnostic model has been employed for predicting high-resolution wind field over complex coastal area. WRF has three nested grids down to from during two days from 24 August 2007 to 26 August 2007. CALMET simulation is performed using both initial meteorological field from WRF coarsest results and surface boundary condition that is Shuttle Radar Topography Mission (SRTM) 90m topography and Environmental Geographic Information System (EGIS) 30m landuse during same periods above. Four Automatic Weather System (AWS) and a Sonic Detection And Ranging (SODAR) are used to verify modeled wind fields. Horizontal wind fields in CM_100m is not only more complex but better simulated than WRF_1km results at Backwoon and Geumho in which there are shown stagnation, blocking effects and orographically driven winds. Being increased in horizontal grid spacing, CM_100m is well matched with vertically wind profile compared SODAR. This also mentions the importance of high-resolution surface boundary conditions when horizontal grid spacing is increased to produce detailed wind fields over complex terrain features.

간호사의 이직경험에 관한 현상학적 연구 (A Phenomenological Study on Nurses' Experiences of Turnover)

  • Purpose: The purpose of this study is to examine nurses' turnover experiences and to provide basic information for developing turnover intervention program for effective human resources management. Methods: This study is an inductive, descriptive phenomenological research that uses profound interviews and observations. The subjects of the study were nurses retired from a hospital. Results: The themes were classified into the three theme clusters. The first theme was about 'deepening inside wound' which was described as experiencing stresses due to introversion, feeling a heavy burden about responsibilities as experienced nurses, feeling guilty about limits of performance and hurt about reproaches by seniors and having conflicts with physicians. The second theme cluster focused on 'powerlessness as time goes', which was stated as having difficulty in adaptation to the new rotated wards, feeling tired and having no spare time and difficulty in night duty, experiencing stagnation and powerlessness and feeling disappointed about inadequate rewards. The third was about 'can't find the exit' which were expressed with wanting to change the speciality because of concerns about future, having affection to motherhood, feeling conflict over the nursing profession itself, concentrating on the matter of turnover and facing related personal situations, and regretting turnover. Conclusion & Suggestions: Considering all these experiences, field managers in nursing should build up a support system that helps nurses overcome risks related to decision-making about turnover. In addition, nurse managers need to develop a human resources development program in which competent nurses are continuously won over through education and counseling.

The Effects of Alkali Sulfate on the Hydration of a C3A−CaSO4⋅2H2O System

  • The hydration mechanism of the $3CaO{\cdot}Al_2O_3-CaSO_4{\cdot}2H_2O$ system in the presence of alkali sulfates has been investigated. The early hydration rate of $3CaO{\cdot}Al_2O_3$ was accelerated by the addition of $Na_2SO_4$ and $K_2SO_4$. This is closely related to the formation of syngenite $(CaSO_4{\cdot}K_2SO_4{\cdot}H_2O)$, and the U-phase added $K_2SO_4$ and $Na_SO_4$ in the $3CaO{\cdot}Al_2O_3-CaSO_4{\cdot}2H_2O$ system, respectively. The formation of the rigid syngenite and U-phase structure led to rapid setting and decreases the sulfate content in the liquid phase of the hydrating cement to the extent that it cannot adequately retard the hydration of $3CaO{\cdot}Al_2O_3$. In case of the alkali sulfate not added to the $3CaO{\cdot}Al_2O_3-CaSO_4{\cdot}2H_2O$ system, the ettringite was transformed to monosulfoaluminate immediately after the consumption of gypsum. However, when the alkali sulfates were added to this system, the ettringite did not transform to monosulfoaluminate immediately even though the gypsum was completely consumed. There was a stagnation period to transform to the monosufoaluminate after the consumption of gypsum because the syngenite and U-phase remained as the sulfate source.

천궁으로부터 멜라닌 생성억제 물질 분리 (Isolation of Melanogenesis Inhibitors from Cnidii Rhizoma)

  • 천연물로부터 치은에 생성되는 멜라닌의 양을 효과적으로 조절하여 치은 미백을 목표로 하는 물질을 분리하고자 문헌에 소개되고 있는 미백 효과를 가진 전통한약을 비롯하여, 민간에서 사용되고 있는 약 100여 종의 고등식물을 대상으로 B-16 mouse melanoma cell lines에서 melanin 생성 억제 효과를 screening하였다. 그 결과 천궁의 methylene chloride 분획에서 활성을 나타내어 실험재료로 선택하여 2개의 화합물을 분리하였으며 각종 spectral data를 검토하여 linoleic acid methyl ester(1), 1,3-dilinoleoyl-2-stearoyl glycerol(2)로 구조를 규명하였다. 이들 화합물은 B-16 mouse melanoma cell lines에서의 melanin 생성억제 활성을 Kojic acid를 비교 물질로하여 측정하였다. 2종의 화합물 중 1,3-dilinoleoyl-2-stearoyl glycerol은 높은 활성을 보이지 않았으나 linoleic acid methyl ester는 Kojic acid에 비하여 강한 활성을 나타내었다.
