Neuropsychiatric Symptoms in Patients with Mild Cognitive Impairment and Dementia of Alzheimer's Type (경도인지장애 및 알쯔하이머형 치매 환자에서의 신경정신증상)

  • Objectives : We investigated the prevalence and composite score of the neuropsychiatric symptoms in patients with mild cognitive impairment(MCI), and dementia of Alzheimer's type(AD). The aim of this study is to analyze the correlation between the result of Korean Neuropsychiatric Inventory(K-NPI) and cognitive function. Methods : A total of 163 patients diagnosed with MCI or AD was divided into three groups(55 MCI patients, 56 dementia patients with mild stage, and 52 dementia patients with moderate, severe stage). We examined neuro-psychiatric symptoms by K-NPI and compared the prevalence and composite score of each subdomain in K-NPI among three groups. Results : The most common symptoms in the MCI group were depression/dysphoria, sleep/night-time behavior, anxiety, and irritability/lability. In mild AD group, the most frequent disturbance was agitation/aggression, depression/dysphoria, anxiety, apathy/indifference, and sleep/night-time behavior. In moderate to severe AD group, the most frequent disturbance was apathy/indifference, depression/dysphoria, agitation/aggression, and delusion. The frequencies of delusion, hallucination, agitation/aggression, apathy/indifference, aberrant motor behavior, appetite/eating change were statistically significant. The total NPI score showed a negative correlation with MMSE-KC and a positive correlation with GDS. Conclusions : Neuropsychiatric symptoms are common features of MCI and AD. These symptoms observed in MCI are similar to those of mild AD. Psychosis is most common in moderate to severe AD, leading to a faster rate of cognitive decline. Therefore, proper management according to the neuropsychiatric symptoms of MCI and three stages of dementia is needed.

Time Series Observations of Outcome Variables and the Factors Associated with the Improvement in the Patient Outcomes of Cataract Surgery (백내장수술환자 결과 변수들의 시계열적 관찰과 진료결과 향상에 영향을 주는 요인)

  • Objectives : To compare the multiple outcomes of patients with cataract surgery at perioperative time,3-4 months and 12 months after surgery and to assess patient outcomes associated with visual improvement(visual acuity of operated eye, visual function-14(VF-14), symptom score). Methods : For this assessment, a prospective study was conducted with 389 patients who had undergone cataract surgery for either one eye or both eyes. The surgery was peformed by 20 ophthalmologists who were practicing at university hospitals and general hospitals. Patients were interviewed and clinical data were obtained. Doctors were questioned with self-reported questionnaire forms. Medical records were examined in order to measure variables related to the surgical process such as surgical methods and ocular comorbidity. The survey was 'conducted at 4 stages' : preoperative time(389 cases), perioperative time(344 cases, 88.4%), postoperative 3-4 months (343 cases, 88.2%), and postoperative 12 months (281 cases, 72.2%). After excluding cases with incomplete data, 198 cases were enrolled in the study. Patient outcomes was measured for any improvement in the functional outcomes(visual acuity of operated eye, visual function, symptom score) at postoperative 3-4 months. Results : The visual acuity(operated, weighted average), symptom score, VF-14 score, satisfaction with vision, and subjective health status were shown to be improved at the perioperative time, postoperative 3-4 months and 12 mouths. An improvement in the Snellen visual acuity score was observed in 190 patients(96.0%), whereas improvements of the VF-14 score and cataract symptom sure were observed in 151 patients(76.3%) and 179 patients(90.4%), respectively. All three outcome measures demonstrated improvement in 137 patients(09.2%). The improvement of the three functional outcomes at 3-4 months after receiving surgery was associated with a lower level of visual function and a higher level of cataract symptom score at perioperative time, as well as a greater experience level of the surgeon. Conclusions : In this study, the estimates of the proportion of patients benefiting from cataract surgery varied with the outcome measure of benefit. Preoperative VF-14 score, a measure of functional impairment related to vision, and symptom score may be better measures of the benefit derived from cataract surgery than the change in visual acuity.

The Development of the Hantan River Basin, Korea and the Age of the Sediment on the top of the Chongok Basalt (한탄강유역의 발달과정과 전곡현무암 위의 퇴적물의 연대)

  • The development of the Hantan river basin can be divided into three stages. The first stage include the ancient Hantan channel system prior to the Chongokni basalt which yield dates of about 0.6 mya from the K/Ar dating method. During this period the Baekuyri formation was formed. The Baekuyri formation is widely observed under the Chongokni basalt along the current river system. The second stage is the period when stream channels stayed on the top of the basalt plateau. Aggradation and deggradation were continued by meandering and braiding channel systems until major stream channel was formed. The currently remaining deposit on the top of the basalt was formed by lacustrine and fluvial systems in this period. During this period Pleistocene hominid was present on edge of water and flood plain and left Paleolithic material. This period was begun at the time of the final basalt flow dated about 300,000 BP. The third stage is designed for the time when the Hantan river channel was dropped down to a level from which the channel could not influence the top of the basalt any more No more deposit could be formed but erosion by surface water has been continued on the top of the basalt since then. The dropping of the Hantan river channel was probably not very long after the final flow of the basalt. Because of frost action and heavy concentrated precipitation in the basin area along with blocky and clumnar joint structure of the basalt, erosional process of the basalt is believed to have been carried out within a relatively short time. The lowering of the Hantan river channel was probably completed in a cycle of major fluctuation of world cimate. Also, the redclay on the top of the basalt is believed to have been formed during a warm period around 200,000 BP, which accords with the climatic change suggested above fair1y well. The Paleolithic materials in tile deposits fell accordingly into approximately same time period.

Development of Bachelor Nursing Programme (일 대학 간호학과 교육 과정 개발 연구)

  • The educational preparation of nurses has been the focus of considerable debate globally. It is needed the change of the basic nursing education for professional nurse to prepare the trends of the health care needs of clients for coming new generation. And also it is believed that educational preparation for being nurses is to be responsible to clients' needs. The purpose of the study was to develop a baccalaureate program in nursing. This study were implemented through three stages from April 2001, to April 2002.: preparation stage which were consisted of reviewing of the literatures, interviewing the fellow nurses in a variety workplace, and consulting professors who were concerned with the nursing education to identify the present educational problems, and analyzing the contents of seven major courses in nursing education, evaluation stage about a new developed curriculum with the directors of the five nursing schools, and confirmation stage of new developed education programme. The developed education programme was designed as a four year program with 126 credit hours including 39 credit hours of liberal arts, 34 credit hours of supplementary courses, and 53 credit hours of nursing major based on the four basic nursing dimensions of human, health, environment and nursing. Nursing majors were consisted of Nursing Ⅰ(oxygenation), Nursing Ⅱ(nutrition elimination), Nursing Ⅲ(activity rest), Nursing Ⅳ(neurologic- endocrine protection), Nursing Ⅴ(fluids electrolytes/sex reproductive), Nursing Ⅵ(psycho-social), Community Health Nursing, Fundamental Nursing, School Health, and Emergency care. This new nursing programme was focused on the nursing education for prevention and rehabilitation nursing care as well as the acute and chronic nursing care at hospital, on the integrated nursing programme to become effective, and the nursing process to encourage the critical thinking. The new education programme focused on the professional nurses who are responsible the nursing ethics, communication skills, and professional beliefs to suit the future trends in health. And also it will be needed for faculties to manage the integrated curriculum, to analyze the contents of each subject, and to communicate with each other before a new education programme apply to their education programme in future.   • DNA methyltransferase 1 (Dnmt1) gene contains three different isoform transcripts, Dnmt1s, Dnmt1o, and Dnmt1p, are produced by alternative usage of multiple first exons. Dnmt1o is specific to oocytes and preimplantation embryos, whereas Dnmt1s is expressed in somatic cells. Here we determined that porcine Dnmt1o gene had differentially methylated regions (DMRs) in 5'-flanking region, while those were not found in the Dnmt1s promoter region. The methylation patterns of the porcine Dnmt1o/Dnmt1s DMRs were investigated using bisulfite sequencing and pyrosequencing analysis through all preimplantation stages from one cell to blastocyst stage in in vivo or somatic cell nuclear transfer (SCNT). The Dnmt1o DMRs contained 8 CpG sites, which located in -640 bp to -30 bp upstream region from transcription start site of the Dnmt1o gene. The methylation status of 5 CpGs within the Dnmt1o DMRs were distinctively different at each stage from one-cell to blastocyst stage in the $in$ $vivo$ or SCNT, respectively. 55.62% methylation degree of the Dnmt1o DMRs in the $in$ $vivo$ was increased up to 84.38% in the SCNT embryo, moreover, $de$ $novo$ methylation and demethylation occurred during development of porcine embryos from the one-cell stage to the blastocyst stage. However, the DNA methylation states at CpG sites in the Dnmt1s promoter regions were hypomethylated, and dramatically not changed through one-cell to blastocyst stage in the $in$ $vivo$ or SCNT embryos. In the present study, we demonstrated that the DMRs in the promoter region of the porcine Dnmt1o was well conserved, contributing to establishment and maintenance of genome-wide patterns of DNA methylation in early embryonic development.

Effects of Temperature, Salinity, and Diet on the Growth and Survival of the Freshwater Rotifer Brachionus angularis

  • We explored the possibilities of using the freshwater rotifer Brachionus angularis as a live food for small fishes cultured in fresh- or brackish waters. Brachionus angularis were collected from a reservoir for isolation and laboratory culture. Length and width of the lorica were $102.3{\mu}m$ and $76.6{\mu}m$, respectively, and those of amictic eggs were $64.4{\mu}m\;and\;47.9{\mu}m$, respectively. When their growth rates were examined at six different temperatures, i.e., 15, 20, 25, 30, 35, and $40^{\circ}C$, the highest daily growth rate of 0.801 was observed at $35^{\circ}C$, and growth was lower with decreasing temperature. Adaptation to salinity change was evaluated with two different modes of salinity increase: step-wise elevation lasting for short durations of 5 to 30 min or a long duration of 24 h. With the short duration modes, no individuals survived salinity higher than 10 psu, and the number of live individuals did not increase throughout the experiment. However, in the 24-h elevation, the number of individuals increased when salinity was elevated by 1 to 2 psu per day for the first 2 or 3 days, while no increase in number occurred at salinity increments higher than 3 psu per day. In addition, to assess the effect of different diets, four single-component diets (Chlorella vulgaris, Nannochloris sp., baker's yeast, or dry yeast) and three combination diets (C. vulgaris + Nannochloris sp. + baker's yeast + dry yeast; C. vulgaris 70% + baker's yeast 30%; C. vulgaris 30% + baker's yeast 70%) were used. The specific growth rates of B. angularis fed combination diets were higher than those of rotifers fed any single-component diet, with the highest rate of 0.648 in B. angularis fed a mixture of C. vulgaris, Nannochloris sp., baker's yeast, and dry yeast, and the lowest rate of 0.200 in those fed dry yeast only. Our results indicate that the freshwater rotifer B. angularis can be used for seedling production of both freshwater and brackish-water fishes that require small (less than about $120{\mu}m$) live food during their early stages.

Initial Change of Coleopteran Insect Community Affected by Anthropogenic Disturbances within Pine tree Forest (소나무림내 인위적 간섭에 따른 딱정벌레류 곤충 군집의 초기 변화에 관한 연구)

  • This study was carried out to investigate the changing pattern of forest insect communities in 2013 after harvesting of forest in 2012, and provide a basic data for the method of efficient management for preservation of forest biodiversity. Five types of cutting method in pine tree forest were selected to compare the coleopteran insect communities at Hajang-myeon, Gangwon-do, Korea, in 2013. In this study, we investigated the effects of timber harvest with green-tree retention on coleopteran insects. They were sampled with Lindgren funnel traps for five experimental sites during July to October, 2013. A total of 1,112 individuals in 156 species of 40 families by Lindgren funnel traps. Also, the analysis of coleopteran insects community was performed. Although the difference of density is not prominent among the survey areas in the early stages, it was appeared a tendency to increase in density and abundance of insects in the experimented sites excluding the control areas. Among them, strip clear-cutting area showed a increase of density and abundance in this study.

A Study on Psychological Responses and Evaluation of Degree of Satisfaction for Drivers to Traffic Informations Using Driving Simulator (차량시뮬레이터를 이용한 교통정보제공 유형에 따른 이용자 심리적 반응 및 만족도 평가에 관한 연구)

  • At present, the characteristics of drivers are not properly considered when informations are provided via VMS even though the Variable Message Signs(VMS) are one of the key information provision medias on the roads. In this study, the driver's psychological response(stress index) was evaluated using a driving simulator for the position of the VMS provided, the contents of the VMS provided, the size of characters on VMS, and types of VMS expression. It was appeared that the stress is the least when the position of the VMS provided 1.5km proceeded from exit, the contents of the VMS provided are time information and types of VMS expression are character form respectively and the size of characters on VMS is appeared not important. In addition, the change of the users' satisfaction level was measured when increasing the numbers of VMS on unit distance and the size of characters. The size of character was almost not effected in similar to the result of stress index and the satisfaction level increased when increasing the number of VMS on the unit distance. The results of this research can be utilized as basic data for the ITS system design and operation stages.

The Effects of In-service Education of Newborn's Behavior Response on the Nurse-Newborn Play Interaction (신생아의 행동반응에 대한 실무교육이 간호사와 신생아와의 유희적 상호작용에 미치는 영향)

  • The purpose of this study was to find out the effect of in-service education on the nurse-newborn play interaction. The research design adopted the pre-experimental design applied only for one pretest-posttest group and Barnard's mother-infant interaction model was used as a conceptual framework. The subjects were 26 nurses who were selected from 2 nurseries in general hospital in Pusan, and 52 healthy newborns who were after 4 days from birth during data collection period at the same hospital. The data were collected from June 1st to October 5th in 2001, by video-taping for the interactive behaviors between the nurse and the newborn, played for 5 minutes in nurseries 2 weeks before and after in-service education. The in-service education consisted of the newborn's behavior responses focused on the newborn baby's states, behavior, cues and state modulation, 3 times per 1 week, 90 minutes per 1 time, lecture, demonstration and hand out project. The experimental tool used for this research was Ha Young-Soo's Korean translation of the Maternal-Infant Play Interaction Scale by Thompson, Jody Baird, Sara Gordman, Bryant(1982), some parts of which were adapted to be suitable for the purpose of this research. Mother and baby scale by Wolke & James-Roberts(1987) was also modified and used as the criterion of nurse's perception of a newborn baby. The results of this study were as follows: 1. This result supported the major hypothesis : After the intervention of in-service education, the nurse and newborn play interaction was promoted. 2. After intervention of in-service education, the nurse perception of newborn, especially the criteria of newborn's reaction was promoted., The results of this research confirm that in-service education on the newborn's behavior responses such as states, behavior, cues and state modulation is an effective way to improve the interaction between the nurse and the baby. It elevated the nurse's sensitivities to the baby's needs. Therefore, in-service education can change the conditions of current nursing practice mostly centered on physical care into a better one in which nurses consider the emotional, social, and intellectual development stages of babies. Accordingly, in-service education contributed to promoting the effective nurse-newborn play interaction so positively.

Effects of Rice Straw on the Microflora in Submerged Soil -II. Relation to the Decommposition of Organic Matter (볏짚시용(施用)이 논토양(土壤)의 미생물상(微生物相)에 미치는 영향(影響) -II. 유기물대사(有機物代謝)에 관여(關與)하는 미생물(微生物)과 유기물(有機物)의 분해(分解))

  • These studies were carried out to investigate the effects of rice straw on microflora in relation to the decomposition of organic matter, and the rate of rice straw decomposition. The number of total bacteria was increased in the first stage, and the number of microorganisms in upper layer was generally larger than lower layer. The number of fungi tended to decline as rice plant grew. Aerobacter among cellulose decomposition bacteria decreased with time, and the number of microorganisms in lower layer was higher than upper layer. The number of glucose decomposition bacteria and sulfate reducing bacteria increased in the submerged soil to which rice straw was applied, but decreased by percolation. the change of manganese oxidizing bacteria seemed not to be affected by rice straw application while they tend to increase as the rice plant grew. The aspect of microorganisms in the percolated water was same that of lower layer, but the number was low as much $10^{-1}$ during the whole stages. The decomposition rate of rice straw applied to submerged soil was about 40 per cent during the rice grew. The decomposition rate of cellulose contained rice straw was about 30 per cent, and lignin was about 60 per cent. The 70-80 per cent of nitrogen remained in the rice straw applied to soil.
