폐암에서 혈중 Cyfra 21-1, SCC 항원 및 CEA의 진단적 유용성 (Diagnostic Usefulness of Serum Level of Cyfra 21-1, SCC Antigen and CEA in Lung Cancer)

  • 연구배경: Cyfra 21-1은 상피종양세포의 세포질에 존재하는 cytokeratin 19의 분절로서 상피종양세포의 파괴시 혈중내로 유리되므로 그 혈중 농도를 측정하여 종양표지자로 이용할 수 있는 것으로 알려져 있다. 이에 저자들은 폐암, 폐결핵, 기타폐질환 및 정상대조군 환자들의 혈청내 Cyfra 21-1, SCC 항원 및 CEA의 농도를 측정하여 폐암의 종양표지자로서 Cyfra 21-1과 SCC 항원 및 CEA의 진단적 효용성을 비교 관찰하고자 하였다. 또한 편평상피세포암에서 Cyfra 21-1과 평상펴셰포암의 특이 종양표지자로 알려진 SCC 항원과의 진단적 민감도와 특이도의 차이를 비교하고 그 병기 진행에 따른 Cyfra 21-1의 혈중농도의 증가 여부를 관찰하고자 하였다. 방법: 1992년 12월부터 1993년 6월까지 서울중앙병원에 입원하여 조직생검으로 초진단된 원발성 폐암 79예(편평상피세포암 41예, 선암 18예, 기타의 미분화 비소세포양 14예, 소세포암 6예)와 폐결핵 32예, 기타폐질환 23예, 정상대조군 23예를 대상으로 하였다. Cyfra 21-1과 ELSA2-CEA를 사용하였고, SCC 항원은 방사계수측정 kit인 ABBOTT SCC RIABEAD를 사용하였다. 결과: 1) Cyfra 21-1의 혈중농도는폐암군이 평균({\pm}표준편차) $18.38{\pm}3.65\;ng/mL$로서 비교군 $1.16{\pm}0.53\;ng/mL$보다 유의하게 높았다(p<0.0001). SCC 항원은 폐암군에서 $3.53{\pm}6.06\;ng/mL$로서 비교군 $1.19{\pm}0.5\;ng/mL$보다 유의하게 높았다(p<0.01). CEA는 폐암군에서 $35.03{\pm}13.9\;ng/mL$로서 비교군 $2.89{\pm}1.01\;ng/mL$ 보다 유의하게 높았다(p<0.0001). 2) 폐암군내에서는 Cyfra 21-1의 혈중농도가 편평상 피세포암군에서 $31.52{\pm}40.13\;ng/mL$로서 선암군 $2.41{\pm}1.34\;ng/mL$(p<0.0001) 및 소세포암군 $2.15{\pm}2.05\;ng/mL$(p=0.007) 보다 유의하게 높았다. SCC 항원의 혈중농도는 편평상피세포암군에서 $5.1{\pm}7.68\;ng/mL$로서 선암군 $1.36{\pm}0.69\;ng/mL$(p=0.009) 및 소세포암군 $1.1{\pm}0.24\;ng/mL$(p=0.024)보다 유의하게 높았다. 3) 편평상피세포암군에서 폐암의 병기 진행에 따른 Cyfra 21-1의 혈중농도의 증가는 없었다. 4) Cyfra 21-1의 진단양성 기준치를 3.3 ng/mL로 하였을때 편평상피세포암의 민감도가 83%로 선암의 22%, 소세포암의 17%보다 높게 산출되었다. SCC 항원의 민감도가 편평상피세포암에서 39%, 선암에서 11%, 소세포암에서 0% 이었다. CEA의 민감도가 편평상피세포암에서 20%, 선암에서 39%, 소세포암에서 33%이었다. 5) ROC 곡선 분석상 폐암의 진단에서 Cyfra 21-1의 민감도와 특이도가 SCC 항원 및 CEA 보다 우수한 것으로 나타났다. 결론: Cyfra 21-1은 폐임에서 SCC 항원 및 CEA에 비하여 민감도 및 특이도가 높은 종양표지자이며, 특히 편평상피세포암에서 그 민감도와 특이도가 높아 편평상피세포암의 특이 종양표지자로 알려진 SCC 항원보다 우수한 종양표지자로 사료되었다.

Effects of Preoperative Biopsies on Recurrence in Head and Neck Skin Cancer

  • Background Skin cancer is the most common malignant tumor in humans. Basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) are the two most common types of skin cancers. When skin cancer is clinically suspected, preoperative biopsies are recommended for a definite diagnosis. However, despite a concern over potential increased risk of metastasis associated with mechanical manipulation, there have been few investigations into whether a preoperative biopsy affected the recurrence of BCC and SCC. Methods Primary BCC or SCC patients who underwent standard surgical excision from 1991 to 2010 were reviewed and a retrospective analysis was performed. Ultimately, 45 BCC patients and 54 SCC patients, who did not meet the exclusion criteria, were analyzed. To identify whether a preoperative biopsy affected the recurrence of BCC and SCC, the recurrence rates of each with and without biopsy were compared. Results Preoperative biopsy had no statistically significant effect on recurrence (BCC, P=0.8680; SCC, P=0.7520). Also, there was no statistical significance between the interval from initial biopsy to first operation and recurrence (BCC, P=0.2329; SCC, P=0.7140). Even though there was no statistical significance, the mean interval from the biopsy to the operation among the BCC patients who underwent preoperative biopsy was 9.2 months in those who had recurrence and 2.0 months in those who had no recurrence. Conclusions There was no statistically significant relationship between preoperative biopsy and recurrence of BCC and SCC. However, there was a tendency toward recurrence in patients with a longer interval between the biopsy and the corrective operation in BCC.

비정형 편평세포 증식으로 오인된 진성대의 원발성 선편평세포암종 (A Case of Primary Adenosquamous Carcinoma of the True Vocal Fold Mimicking Atypical Squamous Cell Proliferation)

  • Adenosquamous carcinoma (ASC) of the larynx is very rare malignancy which has well defined two distinctive pathological features, an adenocarcinoma and a squamous cell carcinoma (SCC). Diagnosis of ASC by endoscopic biopsy is challenging due to small amount of harvested tissue. ASC has a worse prognosis than SCC with an early lymph node metastasis and a distant dissemination. We experienced a rare case of vocal fold ASC which was initially misdiagnosed as atypical squamous cell proliferation at frozen biopsy. We reported this case with a literature review.

반대측 림프절 전이를 동반한 비강의 편평 세포암 1예 (Primary Sinonasal Squamous Cell Carcinoma with Contralateral Lymph Node Metastasis)

  • Sinonasal malignant tumors comprise less than 1% of all cancers and 3% of all malignant tumors of the head and neck, which explains a lack of large series addressing the clinical characteristics and management of these tumors. Neck node metastasis occurs in only about 7% to 15% of malignant tumors compared with other head and neck cancers. A 90-yr-old woman presented with left palpable neck mass and right nasal mass occupying nasal cavity. Fine needle aspiration biopsy of left neck mass results in metastatic squamous cell carcinoma(SCC). PET/CT shows intense FDG uptake in right nasal cavity with bone invasion. Histopathologic examination of excised lesion in the right nasal cavity revealed SCC. We report here on a primary sinonasal SCC with contralateral lymph node metastasis.

Mechanism underlying Chios gum mastic-induced apoptosis on SCC25 human tongue squamous cell carcinoma cell line

  • Chios gum mastic (CGM) is a resin produced from the stem and leaves of Pistiacia lentiscus L var chia, a plant which grows only on Chios Island in Greece. CGM has been used for many centuries as a dietary supplement and folk medicine for stomach and duodenal ulcers in many Mediterranean countries and is known also to induce cell cycle arrest and apoptosis in some cancer cells. In this study, we further investigated the induction and mechanisms underlying the apoptotic response to CGM treatment in the SCC25 human tongue squamous cell carcinoma cell line. The viability of SCC25 cells, human normal keratinocytes (HaCaT cells) and human gingival fibroblasts (HGF-1 cells), and the growth inhibition of SCC25 cells were assessed by MTT assay and clonogenic assay, respectively. Staining with Hoechst and hemacolor dyes and TUNEL assays were employed to detect SCC25 cells undergoing apoptosis. SCC25 cells were treated with CGM, and this was followed by western blotting, immunocytochemistry, confocal microscopy, FACScan flow cytometry, MMP activity and proteasome activity analyses. CGM treatment of SCC25 cells was found to result in a time- and dosedependent decrease in cell viability, a dose-dependent inhibition of cell growth, and apoptotic cell death. Interestingly, CGM showed a remarkable level of cytotoxicity in SCC25 cells but not in normal cells. Tested SCC25 cells also showed several lines of apoptotic manifestation. Taken together, our present findings demonstrate that CGM strongly inhibits cell proliferation by modulating the expression of G1 cell cycle-related proteins and induces apoptosis via the proteasome, mitochondria and caspase cascades in SCC25 cells.

Keratinization of Lung Squamous Cell Carcinoma Is Associated with Poor Clinical Outcome

  • Background: Although the World Health Organization (WHO) classification of lung squamous cell carcinoma (SCC) was revised in 2015, its clinical implications for lung SCC subsets remain unclear. We investigated whether the morphologic characteristics of lung SCC, including keratinization, were associated with clinical parameters and clinical outcome of patients. Methods: A total of 81 patients who underwent curative surgical resection of diagnosed lung SCC, were enrolled in this study. Attributes such as keratinization, tumor budding, single cell invasion, and nuclear size within the tumor, as well as immunohistochemistry of Bcl-xL and pS6 expressions, were evaluated. Results: The keratinizing and nonkeratinizing subtypes did not differ with respect to age, sex, TNM stage, and morphologic parameters such as nuclear diameter, tumor budding, and single cell invasion at the tumor edge. Most patients with the keratinizing subtype (98.0%) had a history of smoking, whereas the nonkeratinizing group had a relatively higher proportion of never-smokers relative to the keratinizing group (24.0% vs. 2.0%; p=0.008, chi-square test). Expression of pS6 (a surrogate marker of mammalian target of rapamycin complex 1 [mTORC1] signaling that regulates keratinocyte differentiation), and Bcl-xL (a key anti-apoptotic molecule that may inhibit keratinization), did not correlate significantly with the presence of keratinization. Patients with the keratinizing subtype had a significantly shorter overall survival (85.2 months vs. 135.7 months, p=0.010, log-rank test), and a multivariate analysis showed that keratinization was an independent, poor prognostic factor (hazard ratio, 2.389; 95% confidence interval, 1.090-5.233; p=0.030). Conclusion: In lung SCC, keratinization is associated with a poor prognosis, and might be associated with smoking.

구강 편평세포 암종에서의 CD44 발현 (CD44 EXPRESSION IN ORAL SQUAMOUS CELL CARCINOMA)

  • The cell surface glycoprotein CD44 is a kind of adhesion molecule, which binds hyaluronic acid, type I collagen and fibronectin. Although there have been numerous reports on the expression and the function of CD44 in lymphocytes and macrophages, very little is known about its distribution and definite role in epithelial tissue, especially in oral epithelial one. The present study was performed to investigate the distribution and expression of the CD44 in human gingiva and squamous cell carcinoma(SCC) arising in human gingiva. And the authors compared CD44 expression with histopathologic grade of SCC. The results were as follows: 1. The CD44 was strongly expressed in granular, spinous and basal layers of normal marginal and attached gingiva, in spinous and basal layers of normal sulcular gingiva, and in all epithelial layers of normal junctional gingiva. 2. In SCC of gingiva, the CD44 was expressed in all but one case. In most of the cases the CD44 was expressed at cell membrane and the degree of expression was relatively strong. 3. In low-grade SCC of gingiva, the CD44 was strongly expressed, especially at the basal and spinous layers of abundantly keratinized cancer nests. In high-grade SCC of gingiva, the CD44 expression tended to be weak but was strong at cells showing individual keratinization. This study suggest that the CD44 expression of normal and cancerous gingival epithelium is associated with the degree of proliferation and differentiation of epithelial cells.

Pseudomonas aeruginosa exotoxin A(PEA)가 사람혀 편평암종세포에서 나타나는 세포자멸사 작용 (Apoptotic effect of Pseudomonas aeruginosa exotoxin A in human tongue squamous cell carcinoma(SCC) 25 cells)

  • Objectives : The purpose of the study is to examine the apoptotic effects of Pseudomonas aeruginosa exotoxin A(PEA) in squamous cell carcinoma(SCC) 25 cells. Methods : Cell growth reduction and apoptosis induced by PEA were confirmed by WST-1 assay, Hoechst 33258 staining, flow cytometry analysis, and Western blot assay. Results : The PEA treatment decreased the cell viability in a dose and time dependent manner: control; $100{\pm}0^e$(p<0.01), 0.1875 nM; $87{\pm}4.36^d$(p<0.01), 0.375 nM; $82{\pm}0.58^d$(p<0.01), 0.75 nM; $72{\pm}1.67^c$(p<0.01), 1.5 nM; $51{\pm}1.53^{bc}$(p<0.01), 7.5 nM; $31{\pm}1.20^{ab}$(p<0.01), 15 nM; $26{\pm}0.67^a$(p<0.01), control; $100{\pm}0^a$(p<0.05), 24 h; $51{\pm}1.53^b$(p<0.05), 48 h; $16{\pm}0.5^c$(p<0.05), 72 h; $12{\pm}1.67^d$%(p<0.05). The PEA was observed on SCC 25 cells with the half maximal inhibitory concentration(IC50) value of 1.5 nM at 24 hours. The PEA treated SCC 25 cells demonstrated several types of apoptotic indications, such as nuclear condensation, the increase of sub G1, and the cleavage of PARP-1 and DFF 45. Conclusions : PEA showed anti-cancer activity against SCC 25 cells via apoptosis. PEA may potentially contribute to human oral cancer treatment.

Lack of Utility of Cytokeratins in Differentiating Pseudocarcinomatous Hyperplasia of Granular Cell Tumors from Oral Squamous Cell Carcinoma

  • Granular cell tumor (GCT) of the oral cavity is a benign lesion. Half of oral GCTs demonstrate pseudocarcinomatous hyperplasia (PCH) of the mucosa which can mimic invasive islands of oral squamous cell carcinoma (SCC). Such similarity can be confusing when diagnosing or evaluating the two conditions, potentially leading to misdiagnosis or misclassification. Indeed, several misdiagnosed cases of oral GCT have been reported in the literature as OSCC or malignant oral GCT that resulted in unnecessary aggressive treatment for the affected patients. The aim of this study was to investigate if the cytokeratin pattern of the PCH can help in differentiating GCT from oral SCC. To distinguish between these two entities, we examined 12 patient specimens of oral GCT-PCH and oral SCC histologically and via immunohistochemistry (IHC) for CK13, CK17 and P75. The results suggest that the cytokeratin profile of PCH is similar to that of oral SCC. Therefore, consideration of IHC findings for epithelial markers alone may lead to erroneous diagnosis; thus, the presence of the granular tumor underneath the PCH and its immunopositivity for P75 or other neural definition markers can be essential to identify the underlying tumor and exclude oral SCC. Finally we recommend more studies on the molecular biology of PCH to understand how it can mimic oral SCC histologically without harboring its malignant phenotype clinically, which could have significant translational potential for understanding invasive oral SCC.

HLA-A, HLA-B, HLA-DRB1 Polymorphisms and Risk of Cervical Squamous Epithelial Cell Carcinoma: A Population Study in China

  • Cervical cancer is the second most common cancer in women. HLA class I and II alleles polymorphisms have been shown to be associated with cervical cancer risk, but results have varied among different populations. In this study, the HLA-A, -B, and -DRB1 alleles among 100 southern Chinese women with cervical squamous cell carcinoma (SCC) were compared to 254 controls. Our results showed that $B^*51$:01:02 allele frequency was significantly higher in patients with SCC than in healthy controls ($P=3.17{\times}10^{-5}$, $P_c$=0.005, OR=26.7). Statistical analysis also revealed a significantly decreased frequency of $B^*51$:01:02 ($P=7.01{\times}10^{-4}$, $P_c$=0.03, OR=0.12) in patients with SCC when compared with healthy controls. These results indicate that HLA-$B^*51$:01:02 may confer susceptibility to SCC and HLA-$B^*51$:01:02 may contribute to resistance to the development of SCC in Chinese women. None of the HLA-A-B or HLA-A-B-DRB1 haplotypes were significantly different in cases and controls after multiple testing corrections, indicating the individual allele associations to be independent of the identified haplotypes. These results support the hypothesis that some HLA-B alleles could be involved with susceptibility for developing SCC.
