Scaling and Root Planing with Concomitant Subgingival Curettage

  • Non-surgical therapy is still an important technique in periodontal treatment. In this study, scaling and root planing(SRP) with or without concomitant subgingival curettage were compared clinically and microbiologically. 14 moderate adult periodontitis patients were included in this study. After 2 weeks from screening visit, with split mouth design, one quadrant was treated by SRP, and the opposite side was treated by SRP with subgingival curettage. Clinical measurement and microbiological analysis was taken at baseline, 1 month, 3 month post-treatment. Clinical parameters used in this study was probing depth, gingival recession, gingival index, bleeding on probing, plaque index, tooth mobility(Periotest Value). Microbiological analysis consisted of determination of the percentages of 4 bacterial groups according to morphologic type with phase-contrast microscope and measuring Black-pigmented Bacteroides after anaerobic culture. 1. There were significant changes in probing depth and gingival recession at 1 month(P<0.05), and these changes remained through 3 month. However, no significant differences were observed between two groups(P<0.05). 2. There were also significant reductions in gingival index and bleeding on probing at 1 month(P<0.05),and these reduced levels were maintained through 3 month with no significant differences between two groups(P<0.05). 3. In both groups, motile bacteria decreased significantly at 1 months(P<0.05), but increased nearly to baseline level at 3 month. 4. The percentages of Black-pigmented Bacteroides, in both groups, decreased significantly at 1 month(P<0.05), and in the subgingival curettage group, significant more reductions were observed than in the root planing group(P<0.05). At 3 month, significant reduction was found in subgingival curettage group only(P<0.05). According to these results, we surmised that concomitant subgingival curettage and root planing give some advantageous effect on bacterial recolonization.

A Comparative Study of Algorithms for Estimating Land Surface Temperature from MODIS Data

  • This study compares the relative accuracy and consistency of four split-window land surface temperature (LST) algorithms (Becker and Li, Kerr et ai., Price, Ulivieri et al.) using 24 sets of Terra (Aqua)/Moderate Resolution Imaging Spectroradiometer (MODIS) data, observed ground grass temperature and air temperature over South Korea. The effective spectral emissivities of two thermal infrared bands have been retrieved by vegetation coverage method using the normalized difference vegetation index. The intercomparison results among the four LST algorithms show that the three algorithms (Becker-Li, Price, and Ulivieri et al.) show very similar performances. The LST estimated by the Becker and Li's algorithm is the highest, whereas that by the Kerr et al.'s algorithm is the lowest without regard to the geographic locations and seasons. The performance of four LST algorithms is significantly better during cold season (night) than warm season (day). And the LST derived from Terra/MODIS is closer to the observed LST than that of Aqua/MODIS. In general, the performances of Becker-Li and Ulivieri et al algorithms are systematically better than the others without regard to the day/night, seasons, and satellites. And the root mean square error and bias of Ulivieri et al. algorithm are consistently less than that of Becker-Li for the four seasons.

  • In the treatment of chronic adult periodontitis, scaling and root planing have been generally performed prior to periodontal flap surgery. The purpose of this study was to evaluate the clinical significance of nonsurgical periodontal therapy prior to periodontal flap surgery in patients with chronic adult periodontitis. Fifty six molars showing bilateral bony defects and 4-6mm periodontal pocket in twelve patients with chronic adult periodontitis were selected. By randomized split-mouth design, in one side, flap operation was performed 4 weeks after scaling and root planing, in the other group, flap operation was only performed without scaling and root planing. Probing pocket depth, gingival recession, clinical attachment level, gingival index, plaque index, and tooth mobility were measured at baseline, before flap operation and post-operation

Effects of scaling and root planing with or without a local drug delivery system on the gingival crevicular fluid leptin level in chronic periodontitis patients: a clinico-biochemical study

  • Purpose: The present split mouth study evaluates the effect of nonsurgical periodontal treatment on the gingival crevicular fluid (GCF) leptin level in chronic periodontitis. Methods: Ninety sites from 30 nonobese chronic periodontitis patients were selected and divided as follows: group I, 30 healthy sites receiving no treatment; group II, 30 periodontitis sites receiving scaling and root planing (SRP); and group III, 30 periodontitis sites receiving SRP with tetracycline local drug delivery. At baseline, after GCF sampling and clinical parameter recording, the assigned treatment was performed for the study groups. During recall visits, GCF sampling followed by clinical parameter recording was done for groups II and III. Results: Reductions in the probing depth and the clinical attachment level (CAL) were highly significant at different time intervals (except between day 0 and 45) in both groups II and III. Upon comparison, group III showed significant gain in CAL between day 0 and 15 and between day 0 and 45. After treatment, the reduction in the GCF leptin level was more significant in group III than in group II at day 15 but re-elevated almost to the pretreatment levels at day 45. Conclusions: Nonsurgical periodontal therapies were not effective in maintaining stable reduction in the GCF leptin level during the study period.

재생술식을 이용한 치근단 병소를 동반한 백악질 열리의 치료 (Treatment of cemental tear associated with periapical lesion using regenerative surgery; A case report)

  • Purpose: Cemental tear is a specific type of root surface fracture characterized by a complete separation of a cemental fragment along the cementodentinal junction or a partial split within the cementum along an incremental line. It is suggested to be a factor for periodontal or periapical tissue destruction. The aim of this study is to present a diagnosis and treatment of cemental tear associated with periapical lesion with root canal treatment and regenerative periodontal surgery. Treatments: A 60-year-old male who had a history of sports trauma on the mandibular right central incisor about 10 years ago presented with apical cemental tear. Clinical examination showed a slightly dark yellowish discoloration and sinus tract that was located on the apical labial mucosa. The mobility and percussion were also assessed on the diseased tooth and recorded as $Miller^{\circ}{\phi}s$ Class II and tenderness to percussion. The probing depth was within the normal limit (<3 mm). Radiographic examination revealed a radiolucent lesion at the apical area and extended to distal aspect of the tooth along the fragment of cemental tear. After root canal treatment, periapical surgery was performed. The bony defect was exposed and then the detached root fragment was removed. Apical root resection and retrograde filling with Mineral Trioxide Aggregate (MTA) were accomplished and the bony defect was filled with deproteinized bovine bone mineral (DBBM) and covered with biodegradable collagen membrane. Results: After 9-month follow-up, healing of the mandibular right central incisor was uneventful and no swelling, purulence or pain was revealed in the associated area. Probing pocket depth was favorably stable, and the tooth mobility was decreased to the Miller's Class I. Conclusions: Apical cemental tear associated periapical lesion could be successfully treated with removal of the detached cementum in combination with apical surgery and GTR procedure.

Effects of seed sources and shade on vigor of Brant's oak seedling

  • The use of local seed provenance is often recommended in forest restoration. Early vigor is a combination of the performance of seed germination and emergence after planting. The ability of young Brant's oak plants to grow and develop after emergence and its dependence on local habitat conditions was investigated in this study. The effects of seed source and shade on early growing seedlings of Brant's oak (Quercus brantii L.) were determined in field measurements. Seeds of Quercus brantii L. were collected from 4 forest areas (seed sources) in southern Zagros (Provinces of Kohkilouyeh-Bouyer Ahmad and Fars) at altitudes of 850, 1,100, 1,500, 2,100 m a.s.l., and planted in a nursery constructed in southwestern Iran. According to a split-plot design consisting of four blocks, each containing two main treatment plots (no shading, partial shading), each main plot was sub-divided into four sub-plots (for elevations of 850, 1,100, 1,500 and 2,100 m). Results showed that shade treatments had significant effects on emergence percentage and rate, shoot length, shoot dry weight (SDW), root dry weight (RDW), leaf area (LA), and chlorophyll content. Ecological factors also had an effect on seed performance. Altitude of seed source had a very significant effect on root length, LA, SDW, and RDW. The seeds collected from 850 m a.s.l. elevation showed the highest performance, especially in leaf area, root length, shoot dry weight, and root dry weight. Our results showed that the altitude of 850 m a.s.l. was the best for collecting Brant's oak seeds.

  • The purpose of this study was to investigated the effect of root planing and decalcified freeze dried allografts on the resorption of transplanted roots and the healing of preveously diseased recipient extraction sockets. The experimental chronic periodontitis was induced by elastic ligatures on the 2nd and 3rd mandibular premolars of 4 adult dogs, and after 8 weeks, crowns were removed and the teeth extracted. The extracted roots were split in half along the long-axis, and the extednt of plaque exposure was morked on the root surfaces with burs. The roots were either root-planed(Test group), or left uninstrumented(Control group), and transplanted in the extraction sockets with decalcified freeze-dried allografts filling the void. The flaps were sutured to cover the sockets completely. The animals were sacrificed after 12 weeks of healing, and the specimens were examined histologically. The results were as follows : 1. No signs of inflammation or disease activity were observed in either groups. 2. Replacement root resorption was observed in both groups. 3. More connective tissue attachments and less ankylosis were observed in the test groups compared to the control. 4. The unresorbed remains of DFDB particles were observed in both groups. 5. DFDB particles in the apical portion of the alveolar sockets were encased in newly-formed bone, while those in the coronal areas were seen encapsulated with connective tissue. 6. No significant difference was found between root-planed and uninstrumented roots relative to the healing and the bone fromation in the recipient extraction sockdets. From the present study, there seemed to be no significant benefits in root planing the transplanted roots or grafting the sockets with DFDB in order to curve the replacement resorption, although the root-planed roots showed more connective tissue attachments. There was also no significant benefits in root transplantation and DFDB for and enhanced healing and bone formation in alveolar extraction sockets.

Periodontal biotype modification using a volume-stable collagen matrix and autogenous subepithelial connective tissue graft for the treatment of gingival recession: a case series

  • Purpose: The purpose of this study was to propose a technique for periodontal biotype modification through thickening of the entire facial aspect using a volume-stable collagen matrix and autogenous subepithelial connective tissue graft (CTG) for the treatment of gingival recession. Methods: Four systemically healthy patients showing Miller class I or class II gingival recession in the mandibular incisor area were included in this study. Full-mouth scaling and root planing procedures were performed at least 4 weeks prior to periodontal plastic surgery. A split-thickness flap with a horizontal intrasulcular incision and 2 vertical incisions was used in cases 1-3, and the modified tunnel technique was used in case 4 for coronal advancement of the mucogingival complex. After the exposed root surfaces were debrided thoroughly, double-layered volume-stable collagen matrix was placed on the apical part of the recession and a subepithelial CTG harvested from the palatal area was placed on the coronal part. The amount of root coverage at 3 months postoperatively was evaluated in cases 1-3, and facio-lingual volumetric changes were analyzed in cases 1 and 2. Results: Healing was uneventful in all 4 cases and complete root coverage was shown in cases 1-3. In case 4, reduction of gingival recession was observed at 3 months after surgery. In cases 1 and 2, a comparison of stereolithographic files from the preoperative and postoperative time points demonstrated that the entire facio-lingual volume had increased. Conclusions: The surgical technique suggested herein, using a volume-stable collagen matrix and autogenous subepithelial CTG, may be an effective method for periodontal biotype modification through thickening of the entire facial aspect for the treatment of gingival recession.

  • This study was conducted to evaluate the cleansing effect of RC-Prep (10% Urea-peroxide, 15% EDTA) on apical third of root canal. Thirty single rooted human teeth were divided into three groups, and ten canals in each group were all enlarged three sizes greater than their original diameter with K-type files and irrigated with each of three irrigants. The three used irrigants were RC-prep (Premier Co.) in combination with 3.5% Sodium hypochlorite, Normal saline, 3.5% Na OCL. In each group, one of three irrigants were used in conjunction with instrumentation as they would be during clinical condition. After final irrigation, the canals were dried with paper points and the teeth were split longitudinally. The cleanness of canal walls iii the apical region were examined with Scanning Electron Microscope. The following results were drawn. 1. The use of Rc-Prep in combination with 3.5% NaOCl showed more clean canal surface than the use of other two irrigants. 2. The canals used RC-Prep in combination with 3.5% NaOCl revealed remnants of pulp tissue and smeared layer, but the openings of dentinal tubules were relatively clean and wide. 3. There was no significant difference in the debridement effect of 3.5% NaOCl and Saline solution. 4. The use of Saline solution showed great amount of remnants of pulp tissue which couldn't find in the use of 3.5% NaOCl.

K-FILE과 초음파 기구의 도말층제거효과에 대한 주사전자 현미경적 연구 (A SCANNING ELECTRON MICROSCOPIC STUDY ON THE REMOVING EFFICIENCY OF SMEAR LAYER BY K-FILE AND ULTRASONIC INSTRUMENT)

  • The purpose of this study was to evaluate the smear layer removing efficiency of two root canal preparation techniques. Twelve single-rooted teeth were used in two groups of six each. Group 1 was biomechanically prepared by hand using a K-file with a high volume of normal saline irrigation. Group 2 was. prepared by using ultrasonically activated K-file with a constant high volume of normal saline irrigation. After the experimental procedures, each root was split saggitally. The removing efficiency of the preparation methods were assessed in terms of surface condition of the canal walls at three levels, those coronal, middle, and apical thirds. On the basis of remaining debris, presence of smear layer, and patency of dentinal tubules, each canal was evaluated according to a scale form 0 to 2. A statistical analysis was used to indicated any significant differences in surface condition between the two methods. There was no statistical significance between hand instrumentation and ultrasonic instrumentation at the cervical third but removing efficiency of ultrasonic instrumentation was superior. No statistically significant differences were obhserved for middle or apical third.
