Effects of Taping the Lower Back on the Lumbopelvic Region and Hip Joint Kinematics During Sit-to-Stand

  • Excessive lumbar flexion during sit-to-stand (STS) is a risk factor for lower back pain. Postural taping can prevent unwanted flexion of the lumbar spine. This study aimed to demonstrate the effect of taping the lower back on the lumbopelvic region and hip joint kinematics during STS. Sixteen healthy subjects participated. All subjects performed the STS with and without taping of the lower back. A three-dimensional motion analysis system was used to measure the kinematics of the lumbar spine, pelvis, and hip joint during STS. The angle of the peak lumbar flexion, pelvic anterior tilting, and hip flexion and angular displacement of the lumbar spine between starting position and maximal lumbar flexion were collected. Paired t-tests, or Wilcoxon's rank-sum test for non-parametric distribution, were used to assess differences in the measurements with and without taping. A p-value <.05 was taken to indicate a significant difference. Significant differences were observed in the angle of the peak lumbar flexion, pelvic anterior tilting, hip flexion and angular displacement of the lumbar spine (p<.05). Taping was associated with a significant decrease in the angle of peak lumbar flexion and angular displacement of the lumbar spine between the starting position and maximal lumbar spine flexion. In addition, the peak angle of pelvic anterior tilting and hip flexion were significantly increased with taping. The findings of this study suggest that taping the lower back can decrease excessive lumbar flexion, and increase the pelvic anterior tilting and hip flexion motion during STS.

Effect of Palm Facing Forward Posture on SpinoPelvic Parameters on the Whole Spine Lateral Radiography (척추 전장 측면 방사선검사 시 손바닥 정면 자세가 척추골반지표에 미치는 영향)

  • The purpose of this study was to investigate the effect of cross arms and palms facing forward on spinopelvic parameters during the whole spine lateral radiography. In addition, we would like to present the usefulness of a posture with the palm facing forward during whole spine lateral radiography of the spine using EOS. The subjects of this study were images of a total of 50 patients (18 males, 32 females) who whole spine lateral radiography using the conventional method and the EOS method from October 2020 to March 2021. The posture used in this study was set as 'CAP' for cross arms and 'PUSH' for posture with palms facing forward. In this study, among the spinal stability factors, thoracic kyphosis (thoracic vertebrae 4 to 12), lumbar lordosis (lumbar vertebrae 1 to sacrum 1), sagittal vertical axis, sacral slope, and shoulder flexion angle were compared on average. The mean thoracic kyphosis was 34.52±12.46° for CAP and 28.46±10.81° for PUSH (p<0.01). The lumbar lordosis of CAP was 42.45±17.45°and that of PUSH was 40.56±16.14°(p>0.57). The sagittal vertical axis was 26.59±34.34 mm in CAP and 21.21±35.41 mm in PUSH (p>0.44). In CAP, the sacral slope was 30.96±10.29°, and in PUSH, it was 31.01±10.19° (p>0.98). shoulder flexion angle was 38.31±8.24° for CAP and 26,08±6.71° for PUSH(p<0.01). As a result of this study, the PUSH posture is considered to be a posture that can minimize the shoulder flexion angle and can perform a stable examination while minimizing changes in spino-pelvic parameter.

Correlation between Pelvic Angle and the Tibiofemoral Angle in the Genu Varum (내반슬로 인한 대퇴경골각의 변화가 골반경사각에 미치는 영향)

  • Objectives : The purpose of this study was to investigate the Correlation between Pelvic angle and the Tibiofemoral angle in the Genu varum. Method : Age of All subjects were 20-37 years. 33(male 10, female 23) subjects were assessed by whole spine x-ray and orthogram. The collected data consisted of items on general characteristics, Pelvic Incidence(PI), Pelvic Tilt(PT), Sacral Slope(SS) and Tibiofemoral Angle(TFA) of the subjects. Results : The PI, PT and TFA had a relationship significantly. However, there was no relationship between SS and TFA. Conclusion : The results suggest that TFA is related to PI, PT. Internal rotation due to genu varum of femoral at hip influence that PI and PT increase.

Comparative Effectiveness of MUSP Therapeutic Exercise Versus Electrical Therapeutic in Flexibility, Balance, Cobb's Angle and Chest Expansion in Patient with Scoliosis (MUSP 운동치료와 전기치료가 척추측만증환자의 Cobb's angle, 유연성, 균형능력 및 흉곽확장에 미치는 효과 비교)

  • PURPOSE : The purpose of this study to suggest the exercise programs for improving the function and structure by applying the Masan university scoliosis program exercise method and electrical therapy method to the scoliosis patient. METHODS : The subjects were 18 patients who were diagnosed with scoliosis. They were randomly assigned either to a Masan university scoliosis program exercise group (n=11) that received Masan university scoliosis program exercise program or to a electrical therapy group (n=7). Flexibility, static balance, dynamic balance, and spinal angles were measured by using one leg standing with closed eyes, functional reach test, and Cobb's angle, respectively. The chest expansion were calculated using differences of chest circumference between maximum inspiration and maximum expiration. RESULT : Masan university scoliosis program group before and after the intervention there was a difference in the static balance, spine angle(p<.05). Masan university scoliosis program group before and after the intervention there was a difference in the flexibility, static balance, spine angle, chest expansion (p<.05). CONCLUSION : These results revealed that Masan university scoliosis program exercise program improved flexibility, static standing balance, spine angle, used as scoliosis management and intervention. Therefore, it is expected to be used as a method for the treatment and prevention in the process of rehabilitation of patients with scoliosis.

The Study on Pillow Height for Arm Paresthesia Patients (한방병원에 내원한 팔 저림 환자들의 베개 높이 분석)

  • Objectives This study was performed to analyze relationship between pillow height and patients of arm paresthesia. Methods The data were collected from December 2013 to April 2014. The subjects who visited in Korean Medical hospital for Arm paresthesia were 65. The pillow height was measured from the surface to occipital protuberance. And the angle of cervical spine was measured from the tragus to the acromion. Results 1. The average height of pillow was $5.36{\pm}2.37cm$, and the average of the angle of cervical spine was $18.62{\pm}8.94^{\circ}$. 2. Correlation and regression analysis were performed on the pillow height, the angle of cervical spine and paresthesia. There was a significant correlation between the pillow height and paresthesia (R=0.389). And it had correlation between the angle of cervical spine and paresthesia (R=0.269). 3. The regression analysis was used to determine the influence of elements. The pillow height can influence to the arm paresthesia. Conclusions Through this study, we found out significant relationship between pillow height, cervical angle and patients of arm paresthesia. This result suggests that the higher pillow height and cervical angle can cause the more painful arm paresthesia.

Adult Idiopathic Scoliosis Treated by Acupuncture : A case report (침 치료로 호전된 특발성 척추 측만증 환자의 증례보고)

  • 목적: 이 증례 보고는 성인 특발성 측만증 환자에 있어서 등의 통증과 척추 만곡에 대한 침 치료의 효과를 보고하기 위한 것이다. 방법: 증례의 환자는 25세 여자 환자로 9년 동안 우측 흉추 측만증과 등과 허리의 통증이 있었다. 1주일에 3번, 3주간 침 치료를 시행했고, 치료 전후로 Whole spine x-ray를 촬영하였으며, 환자의 통증 정도를 Visual Analog Scale(VAS)로 평가하였다. 내원시 Cobb angle은 $22^{\circ}$였고, 관상면 균형은 -3.0cm, 쇄골각은 $-3.5^{\circ}$였다. VAS 8의 통증을 호소하였다. 결과: Cobb angle은 $5^{\circ}$ 감소하였고, 관상면 균형은 1.4cm, 쇄골각은 $1.7^{\circ}$ 회복되었다. 등과 허리의 통증은 VAS 8에서 VAS 3으로 감소하였다. 결론: 침 치료는 특발성 척추 측만증 환자에 있어서 등과 허리의 통증뿐만 아니라 척추 측만을 개선시키고 관상면 균형 회복에도 효과적이다.

Physiologic Cervical Alignment Change between Cervical Spine X-ray and Computed Tomography

  • Objective : The purpose of this study was to investigate the correlations among various radiological parameters used to determine cervical alignment from cervical spine radiographs (X-CS) and cervical spine computed tomography (CT-CS), both within and between modalities. Methods : This study included 168 patients (≤60 years old) without a definite whole spine deformity who underwent CT-CS and X-CS. We measured occipital slope (O-s), C1 slope, C2 slope, C7 slope, sella turcica - C7 sagittal vertical axis (StC7-SVA), spino-cranial angle, T1 slope, and C27-SVA. We calculated the O-C2 angle, O-C7 angle, and C2-7 angle from the measured parameters and conducted correlation analyses among multiple parameters. Results : The intrinsic correlation features among multiple cervical parameters were very similar for both X-CS and CT-CS. The two SVA parameters (C27-SVA and StC7-SVA) were mainly influenced by the upper cervical slope parameters (r=|0.13-0.74|) rather than the lower slope cervical parameters (r=|0.08-0.13|). The correlation between X-CS and CT-CS for each radiological parameter was statistically significant (r=0.26-0.44) except for O-s (r=0.10) and StC7-SVA (r=0.11). Conclusion : The correlation patterns within X-CS and CT-CS were very similar in this study. The correlation between X-ray and CT was statistically significant for most radiological parameters, and the correlation score increased when the horizontal gaze was consistently maintained. The lower cervical parameters were not statistically associated with translation-related parameters (C2-7 SVA and StC7-SVA). Therefore, the upper cervical segment may be a better predictor for determining head and neck translation.

Effects of Sling Exercise Program on Muscle Activity and Cervical Spine Curvature of Forward Head Posture (슬링 운동 프로그램이 머리전방자세의 근 활성도와 목뼈 배열에 미치는 영향)

  • The purpose of the study was to evaluate the effects of sling exercise program on muscle activity and cervical spine curvature of person with forward head posture. The subjects, 25 students, were divided into two groups. This experiment was intended to examine the muscle activity and cervical spine curvature while the subjects had performed the exercises to do for 2 times/week and 4 weeks. The factors of FHP were measured cranial vertical angle, cranial rotation angle, muscular activity and cervical spine curvature. Cervical spine curvature measured craniovertical, craniocervical, cervicohorizontal and upper cervical angle. Collected data were statistically analyzed by SPSS 10.0. After experiment for 4 weeks period, results were as following: Both group was significant difference of the results according to the CVA. Exercise group are more effective to increase muscle activity (p<.05), but cervical spine curvature was no significant difference. This result, we could find out there was a significant correlation between sling exercise and muscle activity, CVA and CRA.

A Case report of Idiopathic Scoliosis Patient : Comparison of X-ray and Digital Infrared Thermographic Imaging (특발성 척추측만증 환자의 X-ray와 적외선 체열촬영의 비교 1례)

  • Purpose : The purpose of this study is to report the comparison of X-ray and Digital Infrared Thermographic Imaging (DITI) of a patient with idiopathic scoliosis. Method: The patient in this case was a 25-year-old female. Her chief complaint was a pain in lumbar, scapular and shoulder regions. We examined her with DITI and X-ray. And then We compared DITI and X-ray. Results: Cobb's angle of thoracic spine was $24.78^{\circ}$ as a primary curve. Cobb's angle of thoracolumbar spine was $17.63^{\circ}$. Temperature on convex side of the thoracic spine was $0.3^{\circ}C$ higher than the other side. Temperature on convex side of cervical spine was $1.4^{\circ}C$ higher than the other side. There was no correlation curvature degree with temperature difference. Conclusion : There was a tendency that the temperature on convex side of the spine is higher than the other side of the spine. DITI is a useful assessment tool when it is used with X-ray as a diagnostic tool of idiopathic scoliosis. Further studies are needed.

A Case Study on Cervical Alignments Affecting by Activator (액티베이터가 경추의 정렬에 미치는 영향에 관한 사례 연구)

  • This study was performed to assess the efficacy of Activator for the patient with chronic neck pain. Mail, 26 years old, is treated with Activator, which was applied for 15 minutes every two days during the 4 weeks. The results of this study are as follows: 1. The treated case compared with the previous case showed that R.O.M particularly increased in flexion and extension. 2. The atlas/skull angle compared with the previous case showed that the angle increased from $0.8^{\circ}$ to $1.5^{\circ}$. In the atlas/skull, normal angle is $5.6^{\circ}$. 3. The angle of cervical curve lordosis compared with the previous case showed that the angle increased from $38.3^{\circ}$ to $48.1^{\circ}$. In the healthy spine, the angle is $42.0^{\circ}$. 4. The anterior curvature compared with the previous case showed that curve radii decreased from 7,220mm to 200mm. In the spine, normal curvature is 165mm. 5. Rush Jackson's angle compared with the previous case showed that the angle increased from $14.8^{\circ}$ to $29.1^{\circ}$. In the healthy spine, normal angle is $39.9^{\circ}$. We can not find significant level for Activator, but it is considered that activator method of chiropractic technique is effective.
