Bone Scintigraphic Findings in Leukemic Patients (백혈병 환자의 골스캔 소견)

  • The leukemias represent diffuse lesions of the bone marrow and significant bony abnormalities are associated with the more aggressive varieties of leukemias and with the younger age groups. On plain film, several roentgenographic findings are observed such as diffuse osteopenia, radiolucent metaphyseal bands, osteolytic lesions and periostitis. We evaluated bone scintigraphic findings using $^{99m}Tc-methylene$ diphosphonate $(^{99m}Tc-MDP)$ in ten patients with proven leukemia. The scan is abnormal in 90%. We classified abnormal scintigraphic findings to 3 types and these are increased diffuse juxta-articular uptake, focal increase of uptake and combined type. The common sites of focal uptake were femur, humerus, tibia, spine, ribs, calvarium, scapula and mandible. We concluded that $(^{99m}Tc-MDP)$ bone scintigraphy is sensitive imaging modality in leukemic patients with bone pain.

  • A thorax is consisted of a heart, great vesseles, lungs, ribs, sternum and thoracic spine etc. The quality of chest radiogram is very important in order to find out abnormality in the lung field. The image has two major characteristics; density and contrast which directly influence the diagnostic quality of the roentgenogram. It is very hard to make excellent film image in the lung field because of overlapping bones and other soft tissues. To take a good radiogram of lung field, we studied the condition of chest P-A projection in adult and obtained results as follows: 1. The average chest radiographic condition is resulted as 62KVP, 16 mAs in hospitals around Kyung Ki-Do, Korea, 2. The density of the chest 20cm in thickness, is equal to the water phantom 8cm in thickness. 3. The best quality of chest radiogram is achieved in the condition of the lung field at 100KVP, 9.6mAs by use of Grid 8:1.

  • A red grafted cactus of Gymnocalycium mihanovichii 'Red King' was bred by the Cactus Research Institute, Gyeonggi-do ARES. in 2007. A crossing between 'Redjewelry' and 'GG024824' in 2005 was obtained 135 seedlings. 'Red King' was finally selected through the test of specific growth and coloring characteristics in 2007. The color of body and tubercles is red. The shape of glove is flat round with 8~9 ribs. Spine is short length and light brown color. Growth is fast as 6 month after planting to reach 36.8 mm in diameter. Propagation ability is good as to set as many as 14.1 tubercles.

  • A yellow grafted cactus of Gymnocalycium mihanovichii 'Yellow King' was bred by the Cactus Research Institute, Gyeonggi-do ARES. in 2007. A crossing between 'GG0214319' and 'GG021295' in 2005 was obtained 338 seedlings. 'Yellow King' was finally selected through the test of specific growth and glove coloring characteristics in 2007. The color of glove and tubercles is yellow. The shape of glove is round with 8~9 ribs. Spine is short and brown color. Growth is fast as 6 month after planting to reach 34.1 mm in diameter. Propagation ability is good to set as many as 12.4 tubercles.

Investigation of Tube Voltage Range using Dose Comparison based on Effective Detector Exposure Index in Chest Radiography (흉부 X-ray 검사 시 선량 비교를 활용한 유효 Detector Exposure Index 기반의 적절한 관전압 범위 제안)

  • This study is to confirm the range of tube voltage for Chest X-ray in DR system by comparing with dose area product (DAP) and effective dose in efficient detector exposure index (DEI) range. GE definium 8000 was used to for the phantom study. The range of tube voltage is 60~130 kVp and of mAs is 2.5~40 mAs. The acquired images were classified into efficient DEI groups, then calculated effective dose with DAP by using a PC-Based Monte Carlo Program 2.0. The signal to noise ratio (SNR) was measured at 4 regions, including the thoracic spine, the lung area with the ribs, the lung area without the ribs, and the liver by using Picture Archiving and Communication System. The significance of the group for each tube voltage was verified by performing the kruskal-wallis test and the mann-whitney test as a post-test. When set to 4 groups dependned on the tube voltage, DAP showed significant differences; 60 kVp and 80 kVp, and 60 kVp and 90 kVp (p= 0.034, 0.021). Effective dose exhibited no statistically significant differences from the all of the group (p>0.05). SNR exhibited statistically significant differences from the all of the group in the liver except compared to 80 kVp and 90 kVp (p<0.05). Therefore, high tube voltages of 100 kVp or more need to be reconsidered in terms of patient dose and imaging in order to represent an appropriate chest X-ray image in a digital system.

Early Osteological Development of Larvae and Juveniles in Red Spotted Grouper, Epinephelus akaara (Pisces: Serranidae)

  • We observed the osteological development of larval and juvenile red spotted grouper (Epinephelus akaara) in order to generate data for the assessment of skeletal deformities and to inform phylogenetic systematics research. Larvae and juveniles were obtained from a aquafarm in Muan-gun, Jeolla-namdo Province, Korea. The average water temperature at the time of breeding was $23.0^{\circ}C$ and average water salinity was 33.0 psu. Freshly hatched fish larvae had not undergone any ossification, but ossification of the parasphenoid bone, which forms the base of the cranium, occurred as the juveniles reached an average body length (BL) of 2.49 mm. At the same time, ossification of the preopercle and opercle occurred in the operculum, and ossification of the maxilla, which forms the upper jaw, and the dentary bones, which form the lower jaw, began. In addition, ossification of the vertebra occurred by formation of 7 vertebral centra and the neural spine in the abdominal vertebra. When the juveniles reached an average (BL) of 5.22 mm, ossification of the nasal, lateral ethmoid, and alisphenoid bones occurred in the cranium; ossification of the endopterygoid and metapterygoid bones began in the palatine region; and ossification of the hypohyal and interhyal bones occurred in the hyoid arch. At an average (BL) of 20.9 mm, ossification of the basisphenoid bone in the cranium and the suborbital bone in the orbital region occurred. Ossification of the vertebra then occurred by the formation of long pairs of ribs from the third to the ninth abdominal vertebrae, completing osteological development.

Nineth Rib Syndrome after 10th Rib Resection

  • The $12^{th}$ rib syndrome is a disease that causes pain between the upper abdomen and the lower chest. It is assumed that the impinging on the nerves between the ribs causes pain in the lower chest, upper abdomen, and flank. A 74-year-old female patient visited a pain clinic complaining of pain in her back, and left chest wall at a 7 on the 0-10 Numeric Rating scale (NRS). She had a lateral fixation at T12-L2, 6 years earlier. After the operation, she had multiple osteoporotic compression fractures. When the spine was bent, the patient complained about a sharp pain in the left mid-axillary line and radiating pain toward the abdomen. On physical examination, the $10^{th}$ rib was not felt, and an image of the rib-cage confirmed that the left $10^{th}$ rib was severed. When applying pressure from the legs to the $9^{th}$ rib of the patient, pain was reproduced. Therefore, the patient was diagnosed with $9^{th}$ rib syndrome, and ultrasound-guided $9^{th}$ and $10^{th}$ intercostal nerve blocks were performed around the tips of the severed $10^{th}$ rib. In addition, local anesthetics with triamcinolone were administered into the muscles beneath the $9^{th}$ rib at the point of the greatest tenderness. The patient's pain was reduced to NRS 2 point. In this case, it is suspected that the patient had a partial resection of the left $10^{th}$ rib in the past, and subsequent compression fractures at T8 and T9 led to the deformation of the rib cage, causing the tip of the remaining $10^{th}$ rib to impinge on the $9^{th}$ intercostal nerves, causing pain.

Immediate Effect of Serratus Posterior Inferior Muscle Direction Taping on Thoracolumbar Junction Rotation Angle During One Arm Lifting in the Quadruped Position

  • Background: The serratus posterior inferior (SPI) muscle originates from the spinous process of T11-L2 and inserts at the lower border of the 9-12th ribs. This muscle is involved in thoracolumbar rotation and stability. Several positions can be used to improve trunk stability; the quadruped position is a good position for easily maintaining a neutral spine. In particular, during one arm lifting, various muscles act to maintain a neutral trunk position, and the SPI is one of these muscles. If trunk stability is weakened, uncontrolled trunk rotation may occur at this time. Tape can be used to increase trunk stability. There have been no studies on the effect of taping applied to the SPI muscle on thoracolumbar junction (TLJ) stability. Objects: This study compared the TLJ rotation angle between three different conditions (without taping, transverse taping, and SPI muscle direction taping). Methods: Thirty subjects were recruited to the study (18 males and 12 females). The TLJ rotation angle was measured during one arm lifting in a quadruped position (ALQP). Two taping methods (transverse and SPI muscle direction taping) were applied, and the TLJ rotation angle was measured in the same movement. Results: SPI muscle direction taping significantly reduced TLJ rotation compared to that without taping (p < 0.001) and with transverse taping (p < 0.001). There was a significant difference in the TLJ rotation angle between transverse taping and SPI muscle direction taping (p < 0.017). Conclusion: SPI muscle direction taping reduces the TLJ rotation angle during ALQP. Therefore, SPI muscle direction taping is one method to improve TLJ stability and reduce uncontrolled TLJ rotation during ALQP.

Cerebral Infarction Mimicking Skeletal Metastases on Tc-99m MDP Bone Scintigraphy

  • A 6000-year-old male with carcinoma of the prostate and cerebral infarction underwent a Tc-99m MDP bone scintigraphy for the evaluation of skeletal metastases. Bone scintigraphy (Fig. 1) showed multiple areas of increased uptake of Tc-99m MDP in the skull, spine, and ribs representing skeletal metastases. Two different patterns of uptake occurred in the skull region (Fig. 1A-C); one represents bony metastasis and the ether represents cerebral infarction. The shape, size, location, intensity, and border of the increased uptake differed between the two lesions. An oval-shaped pattern with smaller size, greater intensity and more sharply defined border in the frontal region was consistent with bony metastasis. A rectangular-shaped pattern with larger size, lesser intensity and relatively indistinct border in the temporo-parieto-occipital region was consistent with cerebral infarction. Increased uptake of bone-seeking radiotracers in cerebral infarction has been reported previously.$^{1-4)}$ A suggested mechanism by which bone-seeking radiotracers accumulate in the necrotizing cerebral tissue is an alteration of the blood-brain barrier induced during cerebral infarction, which results in entry of the radiotracers into the extracellular space of the brain.$^{4)}$ Brain CT (Fig. 2) performed 7 days before and one month after the bone scintigraphy revealed lesions on the right temporo-parieto-occipital region consistent with acute hemorrhagic and chronic cerebral infarction, respectively.

Image Quality Enhancement for Chest X-ray images (흉부 엑스레이 영상을 위한 화질 개선 알고리즘)

  • The initial X-ray images obtained from a digital X-ray machine have a wide data range and uneven brightness level than normal images. In particular, in Chest X-ray images, it is necessary to improve naturally all of the parts such as ribs, spine, tissue, etc. These X-ray images can not be improved enough from conventional image quality enhancement algorithms because their characteristics are different from ordinary images'. This paper proposes to eliminate unnecessary background from an input image and expand the histogram range of the image. Then, we adjust the weight per frequency band of the image for improvement of contrast and sharpness. Finally, jointly taking the advantages of global contrast enhancement and local contrast enhancement methods we obtain an improved X-ray image suitable for effective diagnosis in comparison with the existing methods. Experimental results show quantitatively that the proposed algorithm provides better X-ray images in terms of the discrete entropy and saturation than the previous works.
