Effectiveness of Breathing Exercises on Spinal Posture, Mobility and Stabilization in Patients with Lumbar Instability

  • PURPOSE: This study was conducted to monitor the performance of breathing exercises by patients with lumbar instability who had altered breathing patterns. METHODS: To investigate the effects of breathing exercises on spinal posture, mobility, and stabilization in patients with lumbar instability with altered breathing patterns, 30 adult participants were enrolled on the basis of the selection criteria and randomly assigned to the breathing exercise group (BEG) or trunk stabilization exercise group (SEG). A pre-test was performed prior to the intervention exercise program. The intervention exercise program consisted of 15 sessions (three sessions per week for 5 weeks) between August and September of 2016. The post-test was performed on the 6th week of intervention. RESULTS: Pre- and post-test comparisons of BEG and SEG revealed significant improvements in all tested items in the SEG, except for spinal mobility, while significant improvements in spinal postures 1 and 2, spinal mobility, and stabilization were found in the BEG. Between-group comparisons revealed that there were no significant differences in spinal posture 1, spinal posture 2, spinal mobility, or stabilization, whereas significant differences were found in spinal posture 2 and spinal mobility, with the BEG showing greater improvements than the SEG. CONCLUSION: Based on the findings in the present study, it is believed that breathing exercises have important effects on spinal posture, mobility, and stabilization in patients with lumbar instability who have altered breathing patterns.

인대 결손과 협부 결손이 요추 불안정성에 미치는 영향 (Influence of Ligament Deficits and Isthmic Defects on Instability in Lumbar Spine)

  • 척추 불안정성은 요통과 관련되어 있다고 알려져 있다. 하지만 척추 불안정성과 관련된 임상적 연구가 충분하지 않아서 불안정성의 정의가 정량적으로 확립되어 있지 않다. 또한 추간판 퇴행 및 손상, 인대 결손, 협부 결손 등이 불안정성을 발생시키는 요인으로 알려져 있으나, 척추 불안정성에 미치는 영향이 명확하게 분석되어 있지 않다. 본 연구에서는 요추 단분절의 3 차원 유한요소 모델을 이용하여 인대 결손과 협부 결손이 단독 또는 복합적으로 척추 불안정성에 미치는 영향을 정량적으로 분석하였다. 여기서 굽힘 및 신전 시 척추의 시상각과 전위거리가 척추 불안정성을 나타내는 지표라고 가정하였다. 본 연구의 결과는 척추 불안정성의 원인과 기전을 이해하는데 도움이 될 수 있을 것이다.

Lying Down Instability Undetected on Standing Dynamic Radiographs

  • It is well known that spinal instability should be evaluated in the standing lateral position. Standing dynamic flexion and extension radiographs are usually used to assess spinal instability. Here, we report a patient who experienced distraction instability while in the supine position rather than the standard standing position. To our knowledge, this is the first report of lying-down instability undetected on standing dynamic flexion and extension radiographs. We discuss the pathophysiological mechanism of this uncommon but possible entity and provide a review of the literature.

  • The purpose of this study was designed to find out the effectiveness of vertebral segment instability, muscle fatigue response on lumbar spine after apply lumbosacral stabilization exercise program to 4 patients with chronic low back pain and for 12 weeks. In this study, the lumbar spine motion with blind by MedX test machine and the difference of instability to lumbar vertebra segments in flexion, extension test of standing position and spinal load test(Matthiass Test) by Spinal Mouse. The stabilization exercise program was applied 2 times a week for 12 weeks in hospital and 2 times a day for 20 minutes at home. The results of the present study were as follows: 1. Instability test of lumbar vertebra segment is 2 type differential angle test between vertebrae segment and loading test of spine(matthiass) by Spinal Mouse. It appeared to improve stability of segments in sagittal plane after applying program. So lumbar spine curve increased lordosis toward anterior and was improved of the lumbar spine flexibility in flexion and extension. Specially, in matthiass test, (-) value was increased between lumbar vertebra segment when was the load on spine. And so applying stability improved after program. 2. Fatigue response test(FRT) results, in male, was raised muscle fatigue rate during increase weight, on the other hand female appeared lower than male. As a results, lumbosacral stabilization exercise was aided to improvement of lumbar spine vertebra segments stabilization. Spine instability patients will have a risk when in lifting a load or working with slight flexion posture during the daily of living life and it is probably to increase recurrence rate. Thus, not only lumbar extension muscle strength but also stability of vertebra segments in lumbar spine may be very important.

Removal of Intradural-Extramedullary Spinal Cord Tumors with Unilateral Limited Laminectomy

  • Total laminectomy for the removal of intradural-extramedullary spinal cord tumors has been used widely, but postoperative complications often develop, such as kyphosis, spinal instability, and persistent back pain. In this study, we evaluated seven patients with intradural-extramedullary spinal cord tumors with respect to the value of unilateral limited laminectomy. Our cases included six schwannomas, and one meningioma. The cervical region was involved in four cases, the thoracolumbar region in two cases, and the lumbar region in one case. The rationale for choosing a unilateral approach is to preserve musculoligamentous attachments and posterior bony elements as much as possible. The patients were mobilized on the third postoperative day and preoperative neurological symptoms were recovered within a few weeks. We did not observe any complication relating to unilateral limited laminectomy and at follow-up evaluation (at 3 and 12 months postoperatively), none of the patients showed spinal deformity or spinal instability. We think that the unilateral limited laminectomy is a safe and efficient technique for the treatment of intradural-extramedullary spinal cord tumors. We suggest that this technique is one of the best treatments for these tumors.

Is the Spinal Instability Neoplastic Score Accurate and Reliable in Predicting Vertebral Compression Fractures for Spinal Metastasis? A Systematic Review and Qualitative Analysis

  • Spinal metastases can present with varying degrees of mechanical instability. The Spinal Instability Neoplastic Score (SINS) was developed as a tool to assess spinal neoplastic-related instability while helping to guide referrals among oncology specialists. Some previous papers suggested that the SINS was accurate and reliable, while others disagreed with this opinion. We performed a systematic review regarding the SINS to evaluate its accuracy and precision in predicting vertebral compression fractures (VCFs). The 21 included studies investigated a total of 2118 patients. Thirteen studies dealt with the accuracy of SINS to predict post-radiotherapy VCFs, and eight dealt with the precision. Among 13 studies, 11 agreed that the SINS categories showed statistically significant accuracy in predicting VCF. Among eight studies, body collapse was effective for predicting VCFs in six studies, and alignment and bone lesion in two studies. Location has no statistical significance in predicting VCFs in any of the eight studies. The precision of SINS categories was substantial to excellent in six of eight studies. Among the six components of the SINS, the majority of the included studies reported that location showed near perfect agreement; body collapse, alignment, and posterolateral involvement showed moderate agreement; and bone lesion showed fair agreement. Bone lesion showed significant accuracy in predicting VCFs in half of eight studies, but displayed fair reliability in five of seven studies. Although location was indicated as having near perfect reliability, the component showed no accuracy for predicting VCFs in any of the studies and deleting or modifying the item needs to be considered. The SINS system may be accurate and reliable in predicting the occurrence of post-radiotherapy VCFs for spinal metastasis. Some components seem to be substantially weak and need to be revised.

요추부 불안정증의 방사선학적 소견 (Lumbar Spinal Instability and Its Radiologic Findings)

  • Objective : Lumbar spinal instability occurs when normal biomechanics support in lumbar vertebrae interrupted. Despite the recent enthusiastic studies, the precise radiological assessment has not been fully established, yet. Therefore, we carefully studied our cases to analyze the radiologic findings in lumbar spinal instability. Patients and Methods : We have put together radiological analysis and assessment based on 38 patients who have been diagnosed and treated for lumbar spinal instabilities from June 1994 to December 1998, Patients who have been diagnosed and treated for trauma were excluded from study. Results : The outcomes are as follows : 1) Lumbar lordotic curve was statistically significant in unstable group by 23.7, compared to the control group ($17.0^{\circ}$). 2) According to the resting x-ray, sagittal plane angulation measured on unstable group was $21.1^{\circ}$, control group $18.0^{\circ}$. Therefore unstable group was noticeably higher(p<0.01). 3) According to the resting x-ray sagittal plane displacement, unstable group had 4.3mm, the comparison had 1.2mm. Therefore measurement from the unstable group were significantly higher(p<0.01). 4) According to stress view, sagittal plane translation was 4.1mm for the unstables and 2.7mm for the comparisons. Therefore unstables were noticeably higher(p<0.01). 5) According to stress view, sagittal plane rotation was $15.1^{\circ}$ at L3-4, $22.0^{\circ}$ at L4-5, $27.9^{\circ}$ at L5S1 for the unstable group and $11.3^{\circ}$, $18.1^{\circ}$, $21.0^{\circ}$ each for the comparison. 6) Facet angle for unstable group, left $29.3^{\circ}-61.5^{\circ}$, right $24.4^{\circ}-63.2^{\circ}$ and the mean for each are $43.1^{\circ}$, $47.2^{\circ}$. The difference between left and right facet angle was $3.5^{\circ}-20.7^{\circ}$ and the mean value $15.3^{\circ}$. Facet angle for the comparisons for the left was $29.3^{\circ}-59.5^{\circ}$, right was in between $25.7^{\circ}-64.5^{\circ}$ range and the each mean are $44.9^{\circ}$ and $47.6^{\circ}$. Also, the difference between left and right facet angle was $4.1^{\circ}-9.3^{\circ}$ and the average was $17.1^{\circ}$. The average and the difference between the left and right angle are found not to have statistic necessity for both unstable and stable measurements(p>0.01). 7) 19 patients were found to have vacuum facet phenomenon among unstable group etc. results were collected. Conclusion : According to above results, we attempted to prepare the application to the patient of radiological analysis and assessment for lumbar spinal instability early checkup.

Cervical Fusion as a Protective Response to Craniovertebral Junction Instability: A Novel Concept

  • The author reviews the various types of cervical fusion that are associated with instability of the craniovertebral junction. Assimilation of the atlas, C2-3 fusion, the Klippel-Feil abnormality, and pancervical fusion are amongst the more common types of bone abnormalities. It is conceptualised that these types of cervical fusion are not related to any kind of embryological dysgenesis or fault, but instead emerge due to longstanding muscle spasms of the neck in response to atlantoaxial instability. Such bone fusions could be secondary protective responses to longstanding atlantoaxial instability.

후방불안정성을 동반한 요추 추간판 탈출증에서 Graf 고정술후 발생한 합병증 분석 (The Complications of the Graf Stabilization for Lumbar Disc Herniation with Posterior Instability)

  • 본 포항성모병원 정형외과에서는 1991년 5월부터 1995년 12월까지 척추불안정성과 관련된 요추부질환 중 연성고정술을 시행한 145례 중 문제점이 발생된 22례를 분석하여 다음과 같은 결론을 얻었다. 1. 총 145례 중 22례(15.1%)에서 연성고정술로 유발되는 문제점을 발견하였다. 2. 문제점으로 주위분절의 불안정성 증가 10례, 추간판 간격의 감소 8례, 척추경 나사못의 해리 3례, Graf band의 파열이 1례였다. 3. 추간판 간격의 감소 8례 중 초기 질환의 원 인으로 추간판이 파열된 경우가 6례로 의의있는 소견을 보였다. 이상과 같은 문제점이 관찰되었으나 주위분절의 불안정성 증가 및 척추경 나사못의 해리는 견고한 고정술에 비해 그 빈도가 낮은 것으로 보이며, 추간판 간격의 감소의 경우 역시 단순 수핵 제거술후 발생된 추간판 간격의 감소 보다 빈도가 적었으나, 보다 장기간의 추시 및 계속적인 관찰과 연구가 필요할 것으로 사료된다.
