황금(黃芩)이 척수압박손상 흰쥐의 척수신경세포 자연사에 미치는 영향 (Effects of Scutellariae Radix on Neuronal Apoptosis of Spinal Cord Contusion Injury in Rats)

  • Objectives : This Study was performed to evaluate the effects of Scutellariae Radix(SR) water-extract on the tissue and neuronal apoptosis of the spinal cord injury(SCI). Methods: SCI was induced by mechanical contusion following laminectomy of 10th thoracic vertebra in Sprague-Dawley rats. SR was orally given once a day for 7 days after SCI. Neuronal apoptosis was examined with terminal deoxynucleotidyl transferase-mediated dUTPnick-end labeling(TUNEL) assay. Bax (Bcl-2-asociated X protein), Bcl-2(B-cell blastoma 2), c-Fos(FBJ osteosarcoma oncogene) expressions were examined using immuno-histochemistry. Individual TUNEL and immuno-labeled cells expressing Bax, Bcl-2 and c-Fos were counted on the same level in peri-damaged region and in ventral horn. Results: 1. SR significantly reduced number of TUNEL labeled apoptotic cells induced by the spinal cord contusion injury. 2. SR significantly reduced Bax positive cells expression on the motor neuron in the ventral horn induced by the spinal cord contusion injury. 3. SR strengthened Bcl-2 expression on the motor neuron in the ventral horn induced by the spinal cord contusion injury. 4. SR reduced c-Fos expression on the motor neuron in the ventral horn induced by the spinal cord contusion injury. Conclusions : These results suggest that SR plays an inhibitory role against neuronal apoptosis and has significant effects for locomotor disfunction induced by SCI.

척수타박손상 후 트레드밀운동이 근 위축지연과 기능적 회복에 미치는 효과 (The Effects of Treadmill Exercise on Inhibition of Soleus Muscle Atrophy and Improvement Functional Recovery after Spinal Cord Contusion in the Rats)

  • 본 연구는 흰쥐의 흉추 9번을 타박 손상시키고 1주간의 휴식 후 스트레스를 유발하지 않는 저강도 트레드밀운동을 7주동안 수행하여, 가자미근 위축지연 변화를 western blotting과 H&E 염색 그리고 면역형광염색법으로 관찰하였으며, BBB-검사를 이용하여 기능적 회복을 관찰하였다. 형태학적 변화의 결과 척수손상 후 트레드밀 운동에 의해 하지부위 가자미 근과 근섬유의 횡단면 크기가 증가하였으며, 근섬유 세포에서 IGF-I 단백질 발현이 증가하였다. 또한 생화학적인 결과 척수손상 후 트레드밀 운동에 의해 하지부위 골격근 IGF-I 단백질 발현이 증가하였으며, 행동학적 검사를 통해 트레드밀 운동그룹이 비 운동그룹보다 기능적 회복이 개선된 것을 확인하였다. 이러한 결과는 근 비대에 직접적으로 관여하는 IGF-I 단백질 발현량이 운동에 의해 증가하여 가자미근크기 증가와 같은 근 위축 지연을 확인하였으며, 이러한 변화를 통해 기능적 회복에 긍정적인 결과를 보여주었다. 따라서 운동은 척수손상 환자에게 효과적인 기능재활 치료방법이 될 수 있으며, 장애를 최소화하여 자발적인 일상생활을 도모할 수 있을 것이라 생각된다.

척수손상 후 발생한 중추성 통증의 치험 -증례 보고- (The Treatment of Central Pain after Spinal Cord Injury -Case reports-)

  • Central neuropathic pain may occur in 10~20% of the patients after spinal cord injury. The central pain syndrome include spontaneous continuing and intermittent pain as well as evoked pain. The pain is evoked by non-noxious stimulation of the region (allodynia) and repeated stimulation (wind-up phenomenon). Four patients were referred suffering from severe pain, allodynia and hyperaesthesia after spinal cord injury. They had received conventional treatment with non-steroidal anti-inflammatory drugs, steroid, anticonvulsant, antidepressant and rehabilitation which failed to provide pain relief. We administered combination of low doses of morphine and ketamine (10 mg) through the epidural catheter with other conventional therapy. Satisfactory pain relief was achieved in each patient. The reduction of pain was not associated with severe side effects. The most bothersome side effect of ketamine was dizziness in one patient, only caused by bolus injection (ketamine 10 mg with normal saline 10 ml). This suggests synergy from this combination that provides an alternative treatment for central pain.

Effectiveness of Combined Korean Medicine on Traumatic Spinal Cord Injury: A Case Report

  • Objectives: This study aims to evaluate the clinical effectiveness of a Korean medicine treatment on a traumatic spinal cord injury. Metohds : A 44-year-old male patient who had suffered a traumatic spinal cord injury was treated for two months. The effectiveness of complex Korean medicine treatments was measured using the Numeric Rating Scale (NRS) and the Manual Muscle Test (MMT). Result : We found that the NRS of pain caused by numbness was decreased from 7 to 0-1, and the overall motor measured by the MMT was also improved. The patient's walking state was changed from wheelchair ambulation to walker ambulation. Conclusion : Based on this finding, complex Korean medicine treatments may be effective in treating patients' pain, controlling paresthesia, and aiding the recovery function of activities in daily living. However, because of the limitation of our study, further high-quality research should be conducted.

Improvement of Tetraplegia and Respiratory Symptoms with Complex Korean Medicine Treatment After Traumatic Cervical Spinal Cord Injury: A Case Report

  • This study reports the effectiveness of traditional Korean medicine in treating tetraplegia and respiratory symptoms (including dyspnea) after traumatic cervical spinal cord injury surgery. The patient was treated with complex Korean medical treatment including electroacupuncture, pharmacopuncture, and herbal medicine. The manual muscle test (expanded Medical Research Council system) was used to measure the patient's muscle strength. The Korean version of the modified Barthel index and the functional independent measure were used to evaluate the patient's independence in performing daily activities. Following 2 hospitalizations and treatments, the patient's muscle strength improved > 4+ score for all joints, and the Korean version of the modified Barthel index and functional independent measure scores increased from 26 to 79 and 56 to 95, respectively. Symptoms of dyspnea, coughing, and sputum gradually improved and finally disappeared during hospitalization. This study suggests complex Korean medicine treatment may be effective in the rehabilitation of post-surgical cervical spinal cord injury patients.

척수손상환자에서 보행 기능 평가도구인 WISCI II의 타당도와 신뢰도 (Validity and Reliability on the Walking Index for Spinal Cord Injury in the walking assessment of the SCI patient)

  • The purpose of this study was to determine the validity and the reliability of the WISCI II to ascertain its value in the walking function evaluation of spinal cord injury patient. The WISCI II consists of 20 variables with a total valus ranging from 6 to 20 score. A group of 23 spinal cord injury patient were included in this study. To determine the validity, kappa statistics between the WISCI II and SCIM II were measured. The result of this study are as follows: 1) In the validity study, the kappa statistics between the WISCI II and SCIM II were 0.79 and 0.84 for an initial total score and a discharge total score respectively, indicating a reasonable agreement between the two test. 2) In the reliability study, the Cronbach's alpha coefficient was 0.83 and 0.95 for total score indicating a good internal consistency. The finding suggest that the WISCI II demonstrated an acceptable validity and reliabilit for the evaluation of walking function capacity of spinal cord injury patient in clinical practice.

체중지지 트레드밀 후방 보행 훈련이 불완전 척수 손상 환자의 보행과 호흡에 미치는 효과: 단일사례연구 (The Effects of Gait Performance and Respiratory of Backward Walking in Body Weight Supported Treadmill on Incomplete Spinal Cord Injury: A Case Study)

  • Purpose : The aim of study was to assess the effect of backward walking combined with body weight supported treadmill training for patients with spinal cord injury. Methods : Forced vital capacity(FVC), forced expiratory volume at one second(FEV1), peak expiratory flow(PEF) and FEV1/FVC ratio(FER) were measured with a spirometer for on subject of T12 spinal cord injury. 10 meter walking test(10MWT), timed up & go test(TUG) and 6-Minute walking test(6MWT) were the measured. Intervention consisted of backward walking combined with body weight supported treadmill training five times a week for 12 weeks. Results : The date of 10MWT, TUG, 6MWT were improved. Furthermore, the date of PEF, FEV1, FVC, FER showed a improvement. Conclusion : Backward walking combined with body weight supported treadmill training may be a better and more effective method for gait performance and respiratory on incomplete spinal cord injury.

Isolated Spinal Accessory Nerve Palsy from Volleyball Injury

  • Spinal accessory nerve (SAN) palsy is typically a result of posterior triangle surgery and can present with partial or complete paralysis of the trapezius muscle and severe shoulder dysfunction. We share an atypical case of a patient who presented with SAN palsy following an injury sustained playing competitive volleyball. A 19-year-old right hand dominant competitive volleyball player presented with right shoulder weakness, dyskinesia, and pain. She injured the right shoulder during a volleyball game 2 years prior when diving routinely for a ball. On physical examination she had weakness of shoulder shrug and a pronounced shift of the scapula when abducting or forward flexing her shoulder greater than 90 degrees. Manual stabilization of the scapula eliminated this shift, so we performed scapulopexy to stabilize the inferior angle of the scapula. At 6 months postoperative, she had full active range of motion of the shoulder. SAN palsy can occur following what would seem to be a routine volleyball maneuver. This could be due to a combination of muscle hypertrophy from intensive volleyball training and stretch sustained while diving for a ball. Despite delayed presentation and complete atrophy of the trapezius, a satisfactory outcome was achieved with scapulopexy.

Activation of Matrix Metalloproteinases-9 after Photothrombotic Spinal Cord Injury Model in Rats

  • Objective : Matrix metalloproteinases (MMPs), especially MMP-2 and MMP-9 have been known to play an important role in secondary inflammatory reaction after spinal cord injury (SCI). The aim of this study was to investigate the expression and activity of MMP-2 and MMP-9 and to determine their relationship with disruption of endothelial blood-barrier after photochemically induced SCI in rats. Methods : Female Sprague-Dawley rats, weighing between 250 and 300 g (aged 8 weeks) received focal spinal cord ischemia by photothrombosis using Rose Bengal. Expressions and activities of MMP-2 and MMP-9 were assessed by Western blot and gelatin zymography at various times from 6 h to 7 days. Endothelial blood-barrier integrity was assessed indirectly using spinal cord water content. Results : Zymography and Western blot analysis demonstrated rapid up-regulation of MMP-9 protein levels in spinal cord after ischemic onset. Expressions and activities of MMP-9 showed a significant increased at 6 h after the photothrombotic ischemic event, and reached a maximum level at 24 h after the insult. By contrast, activated MMP-2 was not detected at any time point in either the experimental or the control groups. When compared with the control group, a significant increase in spinal cord water content was detected in rats at 24 h after photothrombotic SCI. Conclusion : Early up-regulation of MMP-9 might be correlated with increased water content in the spinal cord at 24 h after SCI in rats. Results of this study suggest that MMP-9 is the key factor involved in disruption of the endothelial blood-barrier of the spinal cord and subsequent secondary damage after photothrombotic SCI in rats.
