All the papers in the Journal of Korean Association for Disability and Oral Health of 2005 to 2018 were analyzed. A total of 181 papers were classified according to its type, field of the study, school, the number of authors, references, and written language. According to the type of the paper, 38 (21.0%) were original articles, 12 (6.6%) were review articles, 124 (68.5%) were case reports and 7 (3.9%) were others. In relation to the field, the most were pediatric dentistry. According to author's school, some schools showed preponderances. As to the number of researchers, the most common number of researchers was four (39 articles, 21.5%), and the number of the articles written by six (37, 20.4%) was similar to that of the articles written by three and five (28, 15.5%). As for the number of references, this number ranged from minimum 0 to maximum 40, and the average was 14.8. Regarding the language of the articles, English was used in 5 articles (2.8%), and Korean was used in 176 articles (97.2%). Thirty-nine (21.5%) papers were done in collaboration with workers of the other field. The efforts of this study was to provide useful information of the journal and the development of research in Special care dentistry.

  • Background: This study retrospectively investigated outcomes following dental implantation in patients with special needs who required general anesthesia to enable treatment. Method: Patients underwent implant treatment under general anesthesia at the Clinic for the Disabled in Seoul National University Dental Hospital between January 2004 and June 2017. The study analyzed medical records and radiographs. Implant survival rates were calculated by applying criteria for success or failure. Results: Of 19 patients in the study, 8 were males and 11 were females, with a mean age of 32.9 years. The patients included 11 with mental retardation, 3 with autism, 2 with cerebral palsy, 2 with schizophrenia, and 1 with a brain disorder; 2 patients also had seizure disorders. All were incapable of oral self-care due to serious cognitive impairment and could not cooperate with normal dental treatment. A total of 27 rounds of general anesthesia and 1 round of intravenous sedation were performed for implant surgery. Implant placement was performed in 3 patients whose prosthesis records could not be found, while 3 other patients had less than 1 year of follow-up after prosthetic treatment. When the criteria for implant success or failure were applied in 13 remaining patients, 3 implant failures occurred in 59 total treatments. The cumulative survival rate of implants over an average of 43.3 months (15-116 months) was 94.9%. Conclusion: For patients with severe cognitive impairment who are incapable of oral self-care, implant treatment under general anesthesia showed a favorable prognosis.

  • Glossectomy combined with radiotherapy causes different levels of tongue function disorders and leads to severe malocclusion, with poor periodontal status in cancer survivors. Although affected patients require regular access to orthodontic care, special considerations are crucial for treatment planning. This case report describes the satisfactory orthodontic management for the correction of severe dental crowding in a 43-year-old female 6 years after treatment for tongue cancer with total glossectomy combined with radiotherapy, to envision the possibility of orthodontic care for oral cancer survivors. Extraction was performed to correct dental crowding and establish proper occlusion following alignment, after considering the possibility of osteoradionecrosis. Orthodontic mini-implants were used to provide skeletal anchorage required for closure of the extraction space and intrusion of the anterior teeth. The dental crowding was corrected, and Class I occlusal relationship was established after 36 months of treatment. The treatment outcome was sustained after 15 months of retention, and long-term follow-up was recommended.

  • Objectives: This study aimed to review the definition, qualification conditions, accreditation field, and system of Japanese recognized dental hygienists. It is expected that it will be used as basic data for the system introduction of the Korean advanced dental hygienists. Methods: From May to November 2020, a literature review was conducted on Japanese dental hygienists and Japanese certified dental hygienists. Results: The lifelong education system training courses to become Japanese certified dental hygienists consisted of basic training, special training, and specific training. Each training session lasted for 15 h, and the number of training hours required to complete the course was 30 h. The training items for the field of recognition A were prevention of lifestyle disease, home care, oral function management, rehabilitation for dysphagia, prevention of diabetes, and oral management by medical and dental partnership. The training items for the field of recognition B were dentistry for the disabled, dentistry for the aged, community oral health, and oral health care. Conclusions: The Japanese recognized that dental hygienist system is valuable as a demonstration model in introducing the Korean advanced dental hygienist system.

  • Clinicians must be able to recognize post-extraction complications and treat them in a timely manner; complications that may potentially be life-threatening require special attention. Although arteriovenous malformation (AVM) is a very rare disorder, it may induce life-threatening hemorrhage during surgical intervention in the pertinent site. The present article examines the diagnosis and treatment modalities of AVM based on the case of a patient who was diagnosed with AVM with continuous bleeding after tooth extraction and who was successfully treated.

  • Spinal muscular atrophy (SMA) is an autosomal recessive, severe neuromuscular disorder in which degeneration of alpha motor neurons in the spine progressively weakens and ultimately paralyzes the proximal muscles. It occurs in one per 6,000-10,000 infants, and is a genetic disorder with the second-highest mortality rate worldwide. An 18-year-old male patient with SMA was referred for general anesthesia for difficulty in performing dental treatment due to limited mouth opening caused by temporomandibular joint (TMJ) pain. However, the patient had a high risk of general anesthesia complications, so TMJ pain during mouth opening was reduced through local anesthesia of the TMJ. Fortunately, the anesthesia was successful in reducing pain during mouth opening, enabling the patient to receive dental treatment with an adequate mouth opening.

  • Silver diamine fluoride (SDF) is an alkaline topical solution and it derives from the conjunction of silver nitrate and fluoride. It reduces the growth of cariogenic bacteria, inhibits degradation of dentinal collagen, impedes demineralization and enhances remineralization. It is inexpensive due to the low cost of materials and its application to dental surface is very simple and requires relatively short chair time. Previous studies have shown that the dental caries prevention effect of SDF is superior or similar to topical fluoride application. The main disadvantage of SDF is its esthetic result, and it permanently blacken carious enamel and dentin. The use of SDF has not yet been approved in Korea, but it may be helpful to prevent and treat dental caries in patients with special health care needs and uncooperative young patients.

  • Dental treatment for the disabled should be a customized that considers the characteristics and degree of cooperation of each disabled patient. There are additional considerations during implant treatment and tooth extraction in disabled patients. Since some brain lesion disorder or cardiac disease patients may be taking antiplatelet or anticoagulant medications, it is necessary to evaluate whether these medications should be discontinued before an invasive procedure. Precautions should be taken for patients with heart valve disease considering the risk of infective endocarditis, especially during invasive dental procedures. Moreover, disabled patients may have difficulty in following instructions and cautions. There are specific considerations for each stage of implant treatment in disabled patients. In the case of patients who are in the pre- or post-transplant state, it is necessary to assess their general condition and oral disease due to the risk of infection. Since disabled patients with various systemic diseases may visit the dental clinic, it is important to understand their characteristics and treatment process in order to flexibly adjust the dental treatment plan accordingly.
