Extravasation Injury and Pressure Sore in Brain Damage Patient with Stiffness of the Limbs

  • Extravasation injury refers to leakage of corrosive liquids from veins, resulting in tissue damage. The authors report on a case of extravasation injury to the left hand after administration of fluid to the antecubital area in a patient with brain damage. In order to minimize the effects of extravasation injury, rapid diagnosis and management are needed. In patients with stiffness, pressure sores can develop requiring more careful management by the medical staff.

  • 기관지내의 삽관은 전신마취나 인위적인 기도확보를 위해 흔히 시행되며, 그것의 유용성은 재론의 여지가 없다. 그러나 기관내 삽관후에 후두종창 궤양 그리고 육아종이 합병증으로 가끔 생기며 또한 성대마비도 드물게 온다. 최근에 저자들은 충수절제술을 위한 기관삽관마취후에 애성을 동반한 좌측 성대마비를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

가철성 의치의 유지관리 (Maintenance care for removable denture)

  • There are many kinds of maintenance care services for removable denture patient. Adjustment of the denture base and occlusion should be performed regularly not only for pain relief but also for maintaining the denture function. Direct and indirect relining are needed frequently in specific cases including mandibular distal extension case or non-symmetric residual teeth situation. Surface treatments for metal and resin are essential in the relining procedure. Clinical process for the denture repair is similar to indirect relining which needs inter-occlusal registration. Especially, the peridontal maintenance care and caries prevention are most important way to preserve the abutment teeth in partial edentulism. Moreover, the caring method for the denture and the tissue should be instructed to the denture patient.

Relationship of Somatic Cell Count and Mastitis: An Overview

  • Mastitis is characterized by physical, chemical and bacteriological changes in the milk and pathological changes in the glandular tissue of the udder and affects the quality and quantity of milk. The bacterial contamination of milk from the affected cows render it unfit for human consumption and provides a mechanism of spread of diseases like tuberculosis, sore-throat, Q-fever, brucellosis, leptospirosis etc. and has zoonotic importance. Somatic cell count (SCC) is a useful predictor of intramammary infection (IMI) that includes leucocytes (75%) i.e. neutrophils, macrophages, lymphocytes, erythrocytes and epithelial cells (25%). Leucocytes increase in response to bacterial infection, tissue injury and stress. Somatic cells are protective for the animal body and fight infectious organisms. An elevated SCC in milk has a negative influence on the quality of raw milk. Subclinical mastitis is always related to low milk production, changes to milk consistency (density), reduced possibility of adequate milk processing, low protein and high risk for milk hygiene since it may even contain pathogenic organisms. This review collects and collates relevant publications on the subject.

  • Infectious mononucleosis is Epstein-Barr virus (EBV) inducing a self-limiting clinical syndrome characterized by fever, sore throat, hepatosplenomegaly, and generalized lymphadenopathy. Gastrointestinal symptoms of EBV infection are nonspecific and occur rarely. EBV inducing acute gastrointestinal pathology is poorly recognized without suspicion. Careful consideration is needed to diagnose gastric involvement of EBV infection including gastric lymphoma, gastric cancer, and gastritis. A few recent cases of gastritis associated with EBV infection have been reported in adolescents and adults. However, there is no report of EBV-associated gastritis in early childhood. We experienced a rare case of 4-year-old girl with EBV gastritis confirmed by in situ hybridization.

  • This paper reports the application of field tests to weak ground. In this study, field and laboratory tests were performed in the west seashore of KOREA(Ildo, Sore, Kunsan, Youngam). Applied tests are standard penetration test, cone penetration test, dilatometer test, field vane test, unconsolidated undrained test, oedometer test and other fundametal material properties tests. The results of in-situ test are used to estimate soil types and undrained shear strengths of five clay local deposits. SPT results showed low reliability and FVT war also demonstrated the best field test to weak clayey deposits.

Nurse-perceived Patient Adverse Events and Nursing Practice Environment

  • Objectives: To evaluate the occurrence of patient adverse events in Korean hospitals as perceived by nurses and examine the correlation between patient adverse events with the nurse practice environment at nurse and hospital level. Methods: In total, 3096 nurses working in 60 general inpatient hospital units were included. A two-level logistic regression analysis was performed. Results: At the hospital level, patient adverse events included patient falls (60.5%), nosocomial infections (51.7%), pressure sores (42.6%) and medication errors (33.3%). Among the hospital-level explanatory variables associated with the nursing practice environment, 'physician-nurse relationship' correlated with medication errors while 'education for improving quality of care' affected patient falls. Conclusions: The doctor-nurse relationship and access to education that can improve the quality of care at the hospital level may help decrease the occurrence of patient adverse events.

Phenotypes of Integrase-Mutated Human Immunodeficiency Virus Type-1(HIV-1)

  • Point mutations in a highly conserved central region of the HIV-1 integrase protein were analyzed for their effects on viral replication and virion morphogenesis. Conservative amino acid replacements of two amino acid residues invariant un retroviral integrases, D116 and E152 of HIV-1, as well as the highly conserved amino acid S147, completely blocked viral replication in two CD4$\^$+/ human T cell lines. Mutation of four other highly conserved amino acids in the region had no detectable effect on viral replication, while Mutations at two positions, N117 and Y143, resulted in viruses with a delayed replication phenotype. Characteristic and reproducible defects id virion core structure were observed by electron microscopic analysis of sore of the replication defective integrase point mutants, indicating that mutant integrase proteins can interfere with the process of virion core maturation.

기도협착이 초래된 재발성 다발연골염 1례 (A Case of Relapsing Polychondritis)

  • Relapsing polychondritis is an uncommon disorder of unknown etiology. That involves multisystem characterized by recurrent inflammation, degenerative change of multiple cartilaginous structures. The most common clinical manifestation is an auricular chondritis. Other involving systems are joint, nose, eye, larynx, trachea, aorta, heart and kidney. A 26-year-old man who complained of hoarseness and sore throat was referred to the ENT department from internal medicine department. On initial video-laryngoscopic and radiologic examination, there was no abnormal finding. Treatment with high doses of corticosteroid and methotrexate was not effective. Symptoms were aggrevated with dyspnea. Three months later, on video-laryngoscopic examination, bilateral cord fixation with swelling was noted. The radiologic finding showed subglottic stenosis due to collapse of anterior wall of cricoid cartilage. The condition was managed with tracheotomy. We report a case of relapsing polychondritis involving larynx and trachea with bilateral vocal cord palsy which was managed with tracheotomy.
