Preliminary Study to Establish a Decision Support System in Sasang Constitutional Medicine with Clinical Data (사장체질 의사결정시스템 구축을 위한 체질 진단 자료를 이용한 예비연구)

  • The need for the study of the revealing Sasang constitution at scientific term is increasing as the application of this discipline to the patient produces more accurate result. To obtain scientific evidence of Sasang constitution, it is crucial to analyze accumulated clinical information and associate them to the biological indices that may classify Sasang constitution. Thus, the analysis of clinical information is the most important stepping stone to go toward to the stage of developing model and decision support system (DSS) for classifying Sasang constitution. This study is a preliminary analysis of 1,109 samples collected with 171 clinical indices. To find meaningful clinical indices for classifying Sasang constitutional medicine, we applied decision tree model for them. The skin of 66.5% within whole Taeeumin is thick and non feeble. In the case of 69.8% within whole Soyangin, the skin is non feeble and slippery. In the case of 64.4% within whole Soeumin. they have feeble skin. Therefore, the property of skin can be suggested to be more important than any other index for the classification of Sasang constitution.

Influence of Oxygen to Population Pharmacokinetics/Pharmacodynamics of Alcohol in Healthy Volunteers (건강한 성인에서의 알코올의 집단 약물동태/약물동력에 미치는 산소의 영향 연구)

  • Objective: To develop a population pharmacokinetics (PK)/pharmacodynamics (PD) model for alcohol in healthy volunteers and to elucidate individual characteristics to affects alcohol's PK or PD including dissolved oxygen. Methods: Following multiple intakes of total 540 mL alcohol (19.42 v/v%) to healthy volunteer, blood alcohol concentration was measured using a Breathe alcohol analyser (Lion SD-400 $Alcolmeter^{(R)}$). A sequential population PK/PD modeling was performed using NONMEM (ver 7.3). Results: Eighteen healthy volunteer were included in the study. PK model of alcohol was well explained by one-compartment model with first-order absorption and Michaelis-Menten elimination kinetics. $K_a$, V/F, $V_{max}$, $K_m$ is $8.1hr^{-1}$, 73.7 L, 9.65 g/hr, 0.041 g/L, respectively. Covariate analysis revealed that gender significantly influenced $V_{max}$ (Male vs Female, 9.65 g/hr vs 7.38 g/hr). PD model of temporary systolic blood pressure decreasing effect of alcohol was explained by biophase model with inhibitory $E_{max}$ model. $K_{e0}$, $I_{max}$, $E_0$, $IC_{50}$ were $0.23hr^{-1}$, 44.9 mmHg, 138 mmHg, 0.693 g/L, respectively. Conclusion: Model evaluation results suggested that this PK/PD model was robust and has good precision.

Antifungal Activities of Insect Against the Plant Pathogenic Fungi (곤충의 식물병원성 진균에 대한 항균활성)

  • The hemolmyph and whole body of insect, Bombyx mori, Allomyrina dichotoma and Neotocia brevitarsi, conspicuously inhibited the mycelial growth of several plant pathogenic fungi. The hemolymph of 1087 strain among the 16 strains of B. mori has inhibition activities against the 3 species of fungi, Alternaria panax, Collctotrichum gloeosporioides, and Pyricularia oryzae. The whole body of B. mori was more effective than the hemolymph as a inhibitor on fungi growth. The antifungal activity of B. mori was variable to the fungi species. In addition, A. dichotoma and N. brevitarsi showed antifungal activities against the same fungi as did B. mori. These data showed that the insect has potent antifungal activity. Whereas, the level of activities were differ according to the fungal species. This finding underlines that the possibility of the insect can be use of the agent as a inhibitor against the plant pathogenic fungi.

Volumetric-Modulated Arc Radiotherapy Using Knowledge-Based Planning: Application to Spine Stereotactic Body Radiotherapy

  • Purpose: To evaluate the clinical feasibility of knowledge-based planning (KBP) for volumetric-modulated arc radiotherapy (VMAT) in spine stereotactic body radiotherapy (SBRT). Methods: Forty-eight VMAT plans for spine SBRT was studied. Two planning target volumes (PTVs) were defined for simultaneous integrated boost: PTV for boost (PTV-B: 27 Gy/3fractions) and PTV elective (PTV-E: 24 Gy/3fractions). The expert VMAT plans were manually generated by experienced planners. Twenty-six plans were used to train the KBP model using Varian RapidPlan. With the trained KBP model each KBP plan was automatically generated by an individual with little experience and compared with the expert plan (closed-loop validation). Twenty-two plans that had not been used for KBP model training were also compared with the KBP results (open-loop validation). Results: Although the minimal dose of PTV-B and PTV-E was lower and the maximal dose was higher than those of the expert plan, the difference was no larger than 0.7 Gy. In the closed-loop validation, D1.2cc, D0.35cc, and Dmean of the spinal cord was decreased by 0.9 Gy, 0.6 Gy, and 0.9 Gy, respectively, in the KBP plans (P<0.05). In the open-loop validation, only Dmean of the spinal cord was significantly decreased, by 0.5 Gy (P<0.05). Conclusions: The dose coverage and uniformity for PTV was slightly worse in the KBP for spine SBRT while the dose to the spinal cord was reduced, but the differences were small. Thus, inexperienced planners could easily generate a clinically feasible plan for spine SBRT by using KBP.

Clinical Implementation of 3D Printing in the Construction of Patient Specific Bolus for Photon Beam Radiotherapy for Mycosis Fungoides

  • Creating individualized build-up material for superficial photon beam radiation therapy at irregular surface is complex with rice or commonly used flat shape bolus. In this study, we implemented a workflow using 3D printed patient specific bolus and describe our clinical experience. To provide better fitted build-up to irregular surface, the 3D printing technique was used. The PolyLactic Acid (PLA) which processed with nontoxic plant component was used for 3D printer filament material for clinical usage. The 3D printed bolus was designed using virtual bolus structure delineated on patient CT images. Dose distributions were generated from treatment plan for bolus assigned uniform relative electron density and bolus using relative electron density from CT image and compared to evaluate the inhomogeneity effect of bolus material. Pretreatment QA is performed to verify the relative electron density applied to bolus structure by gamma analysis. As an in-vivo dosimetry, Optically Stimulated Luminescent Dosimeters (OSLD) are used to measure the skin dose. The plan comparison result shows that discrepancies between the virtual bolus plan and printed bolus plan are negligible. (0.3% maximum dose difference and 0.2% mean dose difference). The dose distribution is evaluated with gamma method (2%, 2 mm) at the center of GTV and the passing rate was 99.6%. The OSLD measurement shows 0.3% to 2.1% higher than expected dose at patient treatment lesion. In this study, we treated Mycosis fungoides patient with patient specific bolus using 3D printing technique. The accuracy of treatment plan was verified by pretreatment QA and in-vivo dosimetry. The QA results and 4 month follow up result shows the radiation treatment using 3D printing bolus is feasible to treat irregular patient skin.

The value of prophylactic cranial irradiation in limited-stage small cell lung cancer: should it always be recommended?

  • Purpose: Prophylactic cranial irradiation (PCI) is a standard treatment for limited-stage small cell lung cancer (LS-SCLC) showing a response to initial treatment, but many patients do not receive PCI due to comorbidities or refusal. This study aims to define the patient group for whom PCI can be omitted with minimal risk. Materials and Methods: Patients with LS-SCLC who underwent radiotherapy with curative aim at our institution between January 2004 and December 2015 were retrospectively reviewed. Patients who did not receive PCI were evaluated for brain metastasis-free survival (BMFS), progression-free survival (PFS), overall survival (OS), and prognostic factors for survival, and treatment outcomes were compared with a patient cohort who received PCI. Results: A total of 350 patients achieved a response following thoracic radiotherapy, and 190 of these patients did not receive PCI. Stage I-II and a complete response (CR) to initial therapy were good prognostic factors for BMFS and OS on univariate analysis. Patients with both stage I-II and a CR who declined PCI showed comparable 2-year BMFS to those who received PCI (92% vs. 89%). In patients who achieved CR, PCI did not significantly improve OS or PFS. Conclusion: There should be less concern about omitting PCI in patients with comorbidities if they have stage I-II or a CR, with brain metastasis control being comparable to those patients who receive PCI.

A Study on Okso, Keun Seob's Hakchangeui(학창의)-Examining Data about Hakchangeui Written in $\lceil$Oksoko(옥소고)$\rfloor$- (옥소 권섭의 학창의에 관한 연구 - "옥소고"소재 학창의 관계자료를 대상으로-)

  • Okso, Keun Seob($1671\sim1759$) was descended from a noble family of Noreun(노론) and learned from his uncle, Keun Sang-ha who is a disciple of Song Si-Yeol, Kim Chang-hyeub and Kim Chang-heub. His $\lceil$Oksoko$\rfloor$ remains in the form of a transcription or a lithographic printing, and documents on 'Hakchangeui' are recorded in jabeui(잡의), Euijedogi(의제도기), which is a part of it. He left Seoul in 1714, settled in Chungpung, journeyed to many places, including the Hwang river area and Jecheon and led a literary and artistic life as Sadaebu(사대부), which was written in a collection of his works where the record about 'Hakchangeui', the most suitable clothes for the life of a retired scholar remains today to express one part of his life. This study introduces data about Hakchangeui written in $\lceil$Oksoko$\rfloor$, pursues the reason why Okso, Keun Seob told about it and intends to embody the figure of Hakchangeui based on the system, measure, material, and sewing method according to the records in a collection. Hakchaneui described in $\lceil$Oksoko$\rfloor$ reveals a structure that is different from old Hakchaneui, showing characteristics like the following. It is a Sadaebu's costume for field amusement in which the length of a former part is longer than that of a later part and the side seam of a garment rips. Black Yeon is attached to the white texture. Yeon widths of Git(collar), Sugu and Gil are varied, respectively. Yeon's figure attached to Gil and Somaejindong is a mountain. Git is Bangryeong and link parts between Git and Gil is connected with Sangchim. This Hakchangeui is considered to playas a gown because it has no string or support to adjust his dress. Especially we can identify that Hwayanggun(화양건) was used in this Hakchangeui.

냉음극형 X-선 튜브의 제작을 위한 CNT 페이스트의 무기 충전제에 대한 연구

  • X-선 튜브는 의료 영상 및 치료, 산업용 제전 장치, 비파괴 X-선 영상 장치 등에서 사용되는데 기존의 열전자원을 이용한 X-선 튜브와는 달리, 냉음극형 X-선 튜브는 빠른 속도의 디지털 구동이 가능하며 전력 소비가 낮은 장점이 있다. 따라서, 최근 많은 연구자들에 의해서 냉음극형 X-선 튜브에 관한 연구가 활발하게 진행되고 있다. 냉음극형 X-선 튜브는 전계 방출을 이용한 전자원을 사용하며, 탄소나노튜브 (CNT), Si, 다양한 종류의 나노선 등이 대표적이다. 그 중에서 CNT는 높은 종횡비로 인해 전계 방출 특성이 우수하여 가장 대표적인 물질이다. CNT를 이용한 전자원을 제작하기 위해서는 직접 성장법, 전기영동법, 스크린 프린팅법, 디핑법 등 다양한 방법이 존재한다. 직접 성장법을 제외한 방법들은 모재료인 CNT와 용매, 금속재료들을 섞어 페이스트나 수용액의 상태를 제작하여야 한다. 이 때, 금속 재료는 기판과 CNT간의 접착 및 전자 전도 통로의 역할을 하는 무기 충전제이며 일반적으로 나노 혹은 수 마이크로미터 크기의 상태로 존재하는 것을 주로 사용한다. X-선 튜브 제작은 일반적으로 외벽을 유리 혹은 세라믹을 주로 사용하는데 아노드 전극 및 캐소드 전극 등과 결합하여 진공 밀봉된 형태가 되어야 한다. 브레이징 방법은 금속과 세라믹을 결합하는데 매우 유용한 방법이며, 그 중에서도 진공 브레이징 방법은 다량의 부품을 한 번에 결합시킬 수 있다. 하지만 진공 브레이징 공정의 온도는 약 $700{\sim}1,000^{\circ}C$이며 이는 금속 재료가 충분히 증발할 수 있는 온도가 된다. 본 발표에서는 고온 진공 상태에서의 무기 충전제의 증발에 대한 현상을 관찰하고 고온진공 상태에서 증발없이 무기 충전제로의 역할을 할 수 있도록 다양한 금속 및 합금에 대한 연구를 수행하였다. 또한, 이 연구를 통해 선택된 무기 충전제를 포함하여 CNT 페이스트를 볼밀링 방법을 이용하여 제작하였으며, 이를 이용한 CNT 에미터가 X-선 튜브의 전자원으로 사용될 수 있는지 확인하기 위해 전계 방출 실험을 함께 실시하였다. 제작된 CNT 에미터가 우수한 전계 방출 특성을 가지고 있음을 확인하였으며, 이는 본 연구를 통해 선택된 금속 및 합금 재료가 무기 충전제로의 역할을 잘 수행하고 있음을 보여준다.

A Study on Architectural Characteristics Based on the Building Composition, Structure, and Construction Background of Okcheon Ijidang (옥천 이지당(二止堂) 건립배경과 건물구성 및 구조를 통해 본 건축특징)

  • Ijidang(二止堂) is a private village school (seodang, 書堂) established in Okcheon, north Chungcheong province where Jungbong Jo, Heon(重峯 趙憲, 1544-1592 AD) had trained for his sound mind and body(yusangcheo, 遊賞處). Jo, Heon was a notable Neo-Confucian scholar of the Kiho School and also a righteous general leading soldiers in Joseon Dynasty. According to Ijidanggi(二止堂記), Ijidang was constructed to honor Jo, Heon and to train local talents in 1647 with the support of Song, Si-yeol(宋時烈, 1607-1689 AD),a representative scholar belong to Kiho School and other local Confucian scholars. Ijidang currently consists of Main Body(本體) in the middle along with the East(Dong-lu 東樓) and the West Pavilions(Seo-lu 西樓) attached to each side. The Main Body and the Dong-lu firstly constructed were to give lectures and to rest within. Ijidang has undergone several changes in its form so far. The surrounding nearby Ijidang shows characteristics of the Confucian architecture for training(J eong-sa, 精舍) and the building itself was to be built in a scenic place apart from the secular world in which scholars stayed, cultivated their body and mind or taught disciples within. The lecture space of Ijidang is positioned next to the main hall(Dae-cheong 大廳) unlike other typical forms of a three-bay building(samganjije 三間之制) at that period. West lu, a two-story building added afterwards in the early 18th century representing characteristics of the Gate Pavilion(Mullu, 門樓) of Neo-Confucian Academies(Seo-won, 書院) in Joseon Dynasty was typically located where the entrance and the ground floor of the main building are visible simultaneously.

Changes in Rice Yield and Soil Properties under Continued Application of Chemical Fertilizer for 50 Years in Paddy Soil (화학비료 50년 연용에 따른 벼수량과 논토양 특성 변화)

  • This study was conducted to investigate the effect of long-term continuous application of fertilizers for rice cultivation. Changes of physical and chemical properties of paddy soil and the rice yield by continuous application of NPK fertilizers, ammonium sulphate, and urea over fifty years, from 1954 to 2003, were investigated. The rice yield index of each treatment were 100 of NPK plots, 84 of ammonium sulphate plots, 81 of urea plots, and 62 of no fertilizer plots. The variance of yield was large according to the quality of irrigation water. Nutrient uptakes by rice plants in ammonium sulphate and urea plots were significantly smaller than those in NPK plots; 86 and 75% in T-N, 79 and 82% in $P_2O_5$, 64 and 58% in $K_2O$, and 94 and 90% in $SiO_2$, respectively. Bulk density of soil in NPK plots significantly decreased compared to those in no fertilizer, ammonium sulphate, and urea plots, whereas CEC in NPK plots increased compared to other plots. Soil pHs of all plots were higher than that before experiment which was 5.2; 6.0 in no fertilizer, 5.9 in urea and NPK, and 5.4 in ammonium sulphate plots. The available phosphate in soil increased by $2.5mg\;kg^{-1}\;yr^{-1}$ when $70kg\;ha^{-1}$ of P fertilizer applied for rice cultivation, and decreased by $1.8mg\;kg^{-1}\;yr^{-1}$ when no P fertilizer applied.
