A Clinical Study on Outpatients in Oriental Neuropsychiatry Clinic of an Oriental Medicine Hospital (서울소재 동서한방병원 한방신경정신과 외래 환자의 임상고찰)

  • The aim of this study was to show the clinical aspects in outpatients who visited Oriental Neuropsychiatry Clinic. Method : We studied 419 patients who visited Oriental Neuropsychiatry Clinic of Dong-Sea Medical Center from march, 2007 to August, 2007. Results & Conclusion : 1. The ratio of male to female patients was 1:2 and age distribution was most frequent in 50s. 2. From onset, 32% of patients visited Oriental Neuropsychiatry Clinic in a week, 58% of patients visited in a month. 3. Oriental Neuropsychiatry diseases were 80%, Non-Oriental Neuropsychiatry diseases were 20%. The most common Oriental Neuropsychiatry diseases were Somatoform Disorders. 4. The most common diagnosises of the Oriental Neuropsychiatry diseases were Psychosomatic diseases. 5. The most common cheif corn1ains were Headache. 6. The most commonly used herbal medicine was Gamiondam-tang and Guibi-tang

Diagnostic Distribution of Psychiatric Disorders among Korean Young Adults

  • Objectives: The prevalence of psychiatric disorders among young adults is different from that among younger or older age groups because of biological and environmental changes. The purpose of this study was to analyze the diagnostic distribution of psychiatric disorders in 19-30-year-old Koreans based on their age and gender using data from the Korean National Health Insurance Service (NHIS). Methods: From the 2011 medical claims sample data of NHIS of 1,375,842 people, we extracted the data of 221,038 people aged 19-30 years, including 106,232 (48.1%) men and 114,806 (51.9%) women. We evaluated the overall changes in the diagnostic distribution of psychiatric disorders over a 3-year period. Results: The diagnostic frequency in women was 13,627 (59.0%), which was significantly higher than that in men. "Other anxiety disorders" was the most common psychiatric disorder in both genders, followed by depressive episodes, somatoform disorders, "other neurotic disorders," and nonorganic sleep disorders. In men, attention-deficit/hyperactivity disorder or intellectual disabilities were not among the top 10 disorders. In women, no significant changes in major psychiatric disorders were seen over the 3-year period. Conclusion: These results reveal the trends of diagnostic distribution of mental illnesses depending on the development, particularly in young adulthood. It is necessary to identify whether such trends are due to biological or environmental factors, aging processes, or complex influences.

Correlation of Testosterone and Pain Threshold in the Patients with Somatoform Disorder (신체형 장애에서의 Testosterone과 통각 역치의 상관관계)

  • Thirteen female patients of somatization disorder, undifferentiated somatoform disorder and somatoform disorder, NOS, diagnosed by DSM-IV were studied for their pain threshold and serum testosterone and the results were compared with the respective data of 20 control females. The results are as follows : 1) The pain threshold as measured by Variable Weight Pressure Algometer was significantly lower in the patient group(153.8${\pm}$39.5 gm/$0.05mm^2$) as compared to the control group(197.5${\pm}$66.7 gm/$0.05mm^2$)(p<0.05). 2) There was no significant difference of serum testosterone between the patient(0.175${\pm}$0.081 ng/ml) and the control(0.174${\pm}$0.108 ng/ml) groups. 3) A significant positive correlation was noted between the pain threshold and serum testosterone in the patient group(r=0.632, p<0.05, two tailed, Pearson's correlation test), but not in the normal control group(r= -0.405). From these results, it was suggested that the role of testosterone in endogenous pain control system might be more important in somatoform disorders than normals.

  • An association between psychological factors and temporomandibular disorders (TMD) has been extensively explored for over 40 years, and a high prevalence of depression and somatization has been consistently reported in patients with TMD. Current evidence suggests that patients' somatic awareness can serve as a primer for TMD incidence and further contribute to the transition to chronic pain. However, the current understanding of somatization from a medical perspective is limited. The best way to address patients with TMD pain who have somatization is also unclear. Therefore, this paper aims to provide an overview of somatization in the context of pain psychology and address its clinical implications in the context of TMD pain.

  • Objective : This study examined benzodiazepine prescription patterns of outpatients visiting the psychiatry department in a single general hospital in Korea. Methods : A retrospective descriptive analysis of benzodiazepine prescriptions was performed on a database from 2014 in a general hospital in Korea. We analyzed the following factors of adult outpatients: demographic factors such as sex and age, amount of benzodiazepine prescribed, treatment duration, and diagnosis based on the ICD-10. Results : In 2014, benzodiazepines were prescribed to 46.4% of the outpatients. Percentage of benzodiazepine prescription increased with age and was highest in the age group 40-59 years. Prescription was more prevalent in women and the prescription percentage increased by treatment duration. Patients with the F4 diagnosis (neurotic, stress-related and somatoform disorders) were the most highly prescribed group. For all diagnosis groups, prescription was more prevalent in females or similar for both sexes except for patients with F5 diagnosis (behavioral syndromes associated with physiological disturbances and physical factors), with males being more predominant. Conclusion : Despite the concern regarding the rate of benzodiazepine prescription and administration to geriatric patients, long-term prescription and usage among older patients is still prevalent.

  • Psychogenic neurological conditions account for 1% to 9% of admissions at neurological unit. They are considered manifestations of underlying psychiatry disorders such as somatoform disorders and factitious disorders or malingering. Diagnosis for psychogenic tremor is difficult because mimic tremor is occurred at the secondary of the organic disorders or coexist in a patient with tremors of organic origin. A patient in this case report was diagnosed as psychogenic tremor, and she also complained the severe anger. A patient did not improved with the chemical medication for a long time. The patient received relaxation training, herbal medication and acupuncture for 20 days and showed short-term alleviation on tremor and decreased anger problem and other somatic problems.

  • This study researches the effect of oriental psychotherapy treatments for various somatization symptoms by marital conflict. We treat Acupuncture, Herb-medication and oriental psychotherapy, and follow up the result for 3 times. Through an understanding of marital, The symptoms of the patient decreased. Oriental psychotherapy treatments is effective for Somatoform Disorders by Marital Conflict.

  • Objectives : In the psychiatry, structured interview is very important tool to be used in epidemiological or psychopharmacological studies. However, investigators often find that the administration of comprehensive diagnostic interviews is time-consuming and expensive to be used in clinical or research settings. Considering these points, Sheehan and his colleagues developed MINI (Mini International Neuropsychiatric Interview) to meet the need for a brief, reliable, and valid structured diagnostic interview for psychiatric disorders. The MINI has been translated into many languages and used in many countries. Therefore, we translated the MINI into Korean and determined its validity in this study. Methods : Twohundred seventy patients and normal subjects participated in the validation of the MINI versus an expert's professional opinion. Schizophrenia and other psychotic disorders, mood disorders including major depressive disorder and bipolar disorder, anxiety disorders including panic and other phobic disorders, and alcohol dependence were included in this study. The validity was obtained by examination whether MINI based diagnoses were compatible with diagnoses by expert psychiatrists. Results : The range of Kappa values was 0.22 (somatoform disorder) to 0.93 (bipolar disorder - past). Overall agreement between MINI and expert's diagnoses were good. The Kappa values for anxiety disorders seemed to be higher than those for other disorders. Conclusion : The MINI Korean version has the good validity. It also has potential applications as a diagnostic tool for psychiatric disorders.

  • The behaviors associated with illness are different depend upon individuals even if Patients complaints same symptoms and have same disease. Understanding the patterns of those behaviors become one of the important elements in determining the diagnostic and treatment approaches and treatment compliance. The sick role plays a essencial part in abnormal illness behavior. The characteristics of abnormal illness behavior ran be applied to many parts in medicine. In case of the various kinds of functional disorders whose organic foundations are obscure and in applying the cognitive therapy, rehabilitation program and occupational therapy, the assessment and evaluation of the abnormal illness behvior is known to be beneficial. For improving the comprehensive psychiatric treatments which could be applied to the Koreans more effectively in patients with somatoform disorders and other various kinds of neurotic disorders further researches especially on the medico-historical and socio-cultural aspects of the illness behavior should be followed. And understanding the abnormal illness behavior would be helpful in enhancing the medical cost effectiveness.
