개심술 126례의 임상적 고찰 (Clinical Experiences of Open Heart Surgery - A Report of 126 Case -)

  • Since we first performed open heart surgery on December 30, 1986, 126 cases were operated on up to August 31, 1989. Among the 126 cases, 65 cases were congenital heart disease of which 63 were acyanotic disease, and 61 cases were acquired heart disease, most of which were valvular heart disease. The age distribution of congenital heart disease was from 1 years 2 months to 48 years, and males had a slightly higher incidence. The age of acquired heart disease was from a minimum of 15 years to a maximum of 68 years, and the male to female ratio was 1;1.5. Midsternotomy was performed in all cases, and the aortic cannula was inserted through ascending aorta and the venous cannula inserted into the SVC and IVC through the right atrium. Vent was inserted through the right superior pulmonary vein. Cardioplegia solution was used in all cases; it was composed of sodium bicarbonate 3.5 ampule, KCL 14 mEq, 2% lidocaine 2.5 ml, 20 % albumin 50 ml and heparin 1000 units mixed to 950 ml with Hartman solution, and was made to 4oC and infused 10 ml per Kg every 20 minutes. The congenital heart disease had a variety of VSD in 32 cases, ASD 23 cases, PS 6 cases, PDA 2 cases, and one case each of Ebsteins anomaly and tricuspid atresia. The operations performed for acquired heart disease were 4 cases of OMC, 33 cases of MVR, and 5 cases of AVR, and 1 case of AVR with CABG. DVR was perfomed in 13 cases, and triple valve replacement was done in 1 case. Other than these, excision of LA myxoma was 2 cases, and repair of traumatic VSD and removal of a pulmonary embolism were one case each. The surgical mortality was 5 cases[4%], all of which occurred in valve replacement cases. Follow-up study revealed 2 late deaths. One died after a traffic accident and one died due to sepsis after he had received a gastrectomy for ulcer bleeding. The remaining patients were in good condition.

미세 댓잎분말의 개발 및 색의 안정화 (Development of Fine Bamboo Leaf Powder and Its Color Stability)

  • To develop a color stable and fine bamboo leaf powder (BLP) as a functional green biomaterial, bamboo leaf (BL) purchased from Sasa borealis and cultivated in Damyang, Jeonnam was treated with different conditions and BLP was evaluated. The four treatments comprised of boiling in water, in zinc chloride, sodium bicarbonate, and vinegar solutions, BLP4 was treated with 2% $ZnCl_2$ for 1 h, BLP5 was treated with 1% $ZnCl_2$ for 2 h, and BLP6 was treated with 1% $ZnCl_2+10%$ NaCl for 1 h. The particle size distribution, ash content, water binding capacity, and color change after heating in acidic solution were compared to commercial fine green tea (GTP) and bamboo leaf powders (CBLP). The particle size (cumulative 90%) of BLP was finest in BLP4 followed by BLP6 < BLP5 < GTP < CBLP. The water binding capacity of GTP was the highest and that of BLP was negatively correlated with particle size. After heating in acidic solution, the color of commercial GTP and CBLP changed from bright green to olive green, but the treated BLPs remained bright green. Especially, the -a (greenness) values for the commercial powders decreased from 11.2-13.6 to 3.1-3.8, while those of the treated BLPs did not change.

  • Present study was conducted to evaluate the performance of Anaerobic Hybrid Reactor (AHR) combined with two types of anaerobic attached growth reactors at mesophilic temperature ($37^{\circ}C$). The reactor was operated at the influent substrate condition of 19,400 mg/L soluble chemical oxygen demand (sCOD). The organic loading rate (OLR) and flow rate were varied in the range of $9.5{\sim}22.5kg/m^3$. day and 10.6 ~ 26.0 L/day respectively since start-up was done. The COD removal efficiency of 93 % was measured at the OLR of $14kg/m^3$. day in AHR. However a reduction in removal efficiency to as low as 85 % could have been related to a combined effect of high concentration suspended solids (SS) concentration over 3,800 mg/L. On the other hand the total COD removal efficiencies were measured to be 96.3 % and 96.2 % for AHR+APF and AHR+ADF respectively. The pH of the POME was adjusted to neutral range by using sodium bicarbonate at the initial stages of the reactor feed, later stages pH adjustment was not required as the pH was maintained in the desired neutral range due to self-buffering capacity of the reactor. The reactor proved to be economically acceptable and operationally stable. The biogas was measured to have $CH_4$ and $CO_2$ with a ratio of 35:65, and methane gas production rate was estimated to be $0.17{\sim}10.269L\;CH_4/g\;COD_{removed}$.

  • "Phosphotriester Method"에 따라 모형 oligodeoxyribonuclotide로서 EcoRI인식자리를 지닌 육량체 GpApApTpTpCp를 화학적으로 합성하면서, 이 과정에 포함되는 반응물의 보호, 축합 보호기의 제거, 생성물의 정제, 정량 및 확인작업의 전반에 걸쳐 기존 방법에 대한 평가와 개선을 수행하였다. 소량체합성의 기본 단위는 이량체로 함이 효유적이며, 착합단계의 수득율은 이량체>사량체>육량체의 순으로 감소함을 알았다. UV/IR분광법 및 hplc/tlc크로마토그래피법을 이용함으로써 미량의 시료를 사용하여 Oligonucleotide를 합성함에 수반되는 정량/확인의 난점을 해결하였다. 또한 5'-OH가 노출된 nucleotide 중 전자를 과량 사용하여 반응시키고 산처리하여 탄산수소나트륨 수용액으로 세척하는 것이 양호한 방법임을 알았다. 아울러 합성에 관련된 nucleotide와 그 유도체를 분광학적 크로마토 그래피자료를 수록하였다.

국내 응급의료센터의 해독제 보유 현황 (Survey of the Antidote Stocking in the Emergency Medical Centers in Korea)

  • Purpose: Inadequate hospital stocking and unavailability of essential antidotes is a worldwide problem. The purpose of this study was to determine the adequacy of antidote stocks in Korean hospitals based on a modified version of US antidote stocking guidelines. Methods: Both written and telephone surveys to collect information on hospital demographics and antidote stocking were given to the emergency departments or hospital pharmacies of 117 hospitals with emergency medical centers. The twenty antidotes included in the survey were taken from year 2000 US antidote stocking guidelines, except for activated charcoal and black widow spider antivenin. Antidote stocks were compared by hospital size, geographic location, and type of emergency medical center. Results: Complete responses were received from all hospitals. A mean of $12.4{\pm}2.9$ antidotes were adequately stocked per hospital. All hospitals stocked atropine, $CaCl_2$ naloxone, and sodium bicarbonate. However, digoxin Fab fragment (16%), cyanide kits (15%), EDTA (10%), BAL (9%), and fomepizole (1%) were not uniformly stocked. Large and teaching hospitals were significantly more likely to stock greater numbers of antidotes. Conclusions: Korean hospitals as a group do not have adequate antidote stocks. Korean stocking guidelines and an antidote management system are recommended in order to correct these deficiencies.

중탄산소듐 용액에서 납의 부식방지제인 자연산물질들 (Natural Occurring Substances as Corrosion Inhibitors for Tin inSodium Bicarbonate Solutions)

  • 0.1 M 중탄산소듐 용액에서 납전극부식에 대한 로소니아, 감초뿌리 및 카로브 추출물수용액의 부식방지작용에 대한것을 정전류극성방법을 사용해 연구하였다. 이들 물질들의 부식방지작용을 지적해주는 것으로 이들 물질들이 존재할 때 부식속도가 감소하는 것이 발견되었다. 부식방지효율은 추출물농도가 증가하면 증가한다. 이들 추출물의 부식방지작용은 납표면에 이들 추출물이 흡착되어 물질 및 하전이동의 장벽을 만들어 나타내는 것으로 설명되었다. 이들 추출물의 납표면에 흡착은 자발적으로 일어나며, Freundlich 등온흡착을 따르는 것으로 발견되었다. 또한, 변전위양극분극법을 사용해 이들 추출물은 염소를 포함한 용액에서 납의 움푹패이게 하는 부식으로부터 잘 보호해주는 것이 발견되었다.

Effect of Preconditioning Ischemia on Endothelial Dysfunction Produced by Ischemia-Reperfusion in Rabbit Coronary Artery

  • This study was designed to test whether or not 1) ischemia-reperfusion attenuates endothelium-dependent relaxation of coronary arteries and 2) preconditioning protects the arterial endothelium from ischemia-reperfusion injury. In anesthetized open chest rabbits, branches of the left circumflex artery were exposed to different combinations of the experimental conditions; ischemia (15 minutes), ischemia (15 minutes)-reperfusion (10 minutes), preconditioning ischemia, and pre-conditioning fellowed by ischemia-reperfusion. Preconditioning consisted of 3 occlusions of 2-min duration, each followed by n 5-min reperfusion. Rings of the artery exposed to the experimental condition and of normal left anterior descending coronary artery were prepared and suspended for isometric force measurement in organ chambers containing Krebs Ringer bicarbonate solution. The rings were contracted with 29.6 mM KCI. Ischemia alone did not attenuate endothelium-dependent relaxation by acetylcholine. However, ischemia-reperfusion significantly impaired endothelium-dependent relaxation. Endothelium-independent relaxation by sodium nitroprusside was not impaired by ischemia-reperfusion and the constrictive response to acetylcholine was not altered in reperfused rings without endothelium, compared with control rings. Arterial rings exposed to preconditioning followed by ischemia-reperfusion exhibited impaired endothelium-dependent relaxation by acetyl-choline. However, although preconditioning not fellowed by ischemia-reperfusion, attenuated endothelium-dependent relaxation at low concentrations of acetylcholine, the magnitude of the impairment by preconditioning followed by ischemia-reperfusion was significantly less than that of the impairment by ischemia-reperfusion alone. These data demonstrate that ischemia-reperfusion significantly attenuates endothelium-dependent relaxation by producing endothelial dysfunction and preconditioning Protects the endothelium of coronary arteries from ischemia-reperfusion injury.

Biotransformation of flavonoid-7-O-glucuronides by $\beta$-glucuronidases

  • $\beta$-Glucuronidases (E.C. 3.2.1.31) from Escherichia coli, Helix pomatia, and bovine liver activity have been investigated on 7-O-glucuronides (baicalin, wogonoside, and luteolin-7-O-glucuronide) and 3-O-glucuronides (quercetin-3-O-glucuronide and kaempferol-3-O-glucuronide). Bovine liver enzyme was not active on any of these substrates. E. coli and H. pomatia enzymes were active on 7-O-glucuronides, however, 3-O-glucuronides were resistant to $\beta$-glucuronidase hydrolysis. These results suggest that glucuronic acid at 7-position is more susceptible to E. coli and H. pomatia $\beta$-glucuronidases than that at 3-position. In addition, the subtle difference of aglycone structure on 7-O-glucuronides affected the preference of enzyme. E. coli enzyme was favorable for the hydrolysis of baicalin, however, H. pomatia enzyme was found to be efficient for the hydrolysis of wogonoside. Both enzymes showed the similar hydrolytic activity towards luteolin-7-O-glucuronide. When the Scutellaria baicalensis crude extract was subjected to enzymatic hydrolysis, baicalin and wogonoside were successfully converted to their aglycone counterparts with H. pomatia at 50 mM sodium bicarbonate buffer pH 4.0. Accordingly, the enzymatic transformation of glycosides may be quite useful in preparing aglycones under mild conditions.

감마선 조사에 의하여 가교된 폴리에틸렌 발포 시트의 제조 (Preparation of Polyethylene Foam Sheets Crosslinked by Gamma-ray Irradiation)

  • In this study, crosslinked polyethylene (PE) foam sheets were prepared through gamma-ray irradiation. PE foam sheets were prepared through a foaming process using sodium bicarbonate as a blowing and foaming agent. The prepared PE foam sheets were then crosslinked through gamma-ray irradiation. The crosslinking degree was increased to 86.0% with an increase in the absorption dose. The tensile strength of the crosslinked PE foam sheets was increased with an increase in the absorption dose. However, the elongation-at-break of the crosslinked PE foam sheets was decreased. The thermal decomposition temperature of the crosslinked PE foam sheets was increased to $421.2^{\circ}C$ with an increase in the absorption dose. The SEM analysis revealed that the morphology was not changed significantly after the crosslinking through gamma-ray irradiation.

Biochemical toxicity of Corexit 9500 dispersant on the gills, liver and kidney of juvenile Clarias gariepinus

  • Corexit 9500 is a dispersant commercially available in Nigeria that is used to change the inherent chemical and physical properties of oil, thereby changing the oil's transport and fate with potential effects on the environment. The aim of this study was to assess the biochemical (enzymes and electrolyte) toxicity of Corexit 9500 dispersant on the gills, liver and kidney of juveniles of Clarias gariepinus after exposure for 21 days. One hundred sixty fish were used without gender consideration. Range-finding tests were conducted over a 96-h period after acclimatisation of the test organisms in the laboratory. The test organisms (10/treatment) were exposed to Corexit 9500 in the following concentrations-0.00, 0.0125, 0.025 and 0.05 ml/l in triplicate. Twenty-one days later, fish was dissected. 0.5 g from each of the following organs-gills, liver and kidney tissues-was removed, homogenised and tested for enzymes [superoxide dismutase (SOD), catalase (CAT), alanine aminotransferase (ALT), aspartate transaminase (AST), alkaline phosphatase (ALP)], urea, creatinine and electrolytes (sodium ($Na^+$), potassium ($K^+$), chloride ($Cl^-$), bicarbonate ($HCO_3{^-}$)) following standard methods. In the gills, SOD and ALT to AST ratio were significantly lower than in control while the creatinine was significantly higher in the toxicant. In the kidney, creatinine was significantly higher in fish exposed to the toxicant. In the liver, ALP increased in the toxicant while urea was decreased. The mean electrolyte concentrations ($Na^+$, $K^+$, $Cl^-$ and $HCO_3{^-}$) increased significantly in the concentration of the toxicant (P < 0.05). The alterations observed in the activities of these electrolytes and enzymes indicated that Corexit 9500 interfered with transamination and metabolic functions of the fish.
