  • Microscopic surgery of the larynx normally have a low recurrence rate. However, once they do recur, it is uneasy to manage recurrent diseases. Recurrence is often the result of inappropriate postoperative care and voice management. Out of the 764 patients operated for benign inflammatory laryngeal diseases, we have conducted a clinical analysis of 54 cases who have had at least one episode of recurrence after the initial treatment with laryngomicrosurgery (7.1% of recurrence rate). High risk groups for recurrence were determined, and they were related to cigarette smoking, alcohol intake, laryngopharyngeal reflux. From our experience and literature review, we conclude that pre- and postoperative patient education concerning these risk factors and voice usage Is of critical need to prevent recurrence of inflammatory laryngeal lesions after laryngomicrosurgery.

  • Purpose: This study was to explore gender differences on presenting patients with acute myocardial infarction in the emergency department. Methods: The survey was done with 143 emergency medical charts presented to the emergency department and diagnosed with acute myocardial infarction between January 2005 and December 2006. The collected data were analyzed with frequency, chi-square, and t-test. Results: Significant gender differences were apparent in age, route to the emergency department, elapsed time from onset of symptoms to arrival, and initial heart rate. Women were significantly more likely to report hypertension, diabetes, and congestive heart failure than men, but men were significantly more likely to report smoking. Chest pain was the most common initial symptom in both men and women. Women were significantly more likely to report dyspnea and nausea/vomiting than men. Conclusion: Although similarities exist in the associated symptoms of acute myocardial infarction, women might experience different symptoms, compared to men. These findings have implication that patients and health care providers should consider gender difference in presenting symptoms.

Their mean ages were 21.8 for female and 23.3 for male students, 2.78.0% of the males md 46.5% of the females permitted premarital sex, 57.1% of male and 10.3% of female college students had experienced sexual intercourse, 7. l% of males and 2.4% of females had experienced pregnancy, 10.3% of males and 3.4% of females had been infected with STDs, 72.1% of male and 13.8% of female didn't use condoms at the time of infection. 3. Most of the factors related to STDs infection, such as drinking, smoking, frequency of sexual intercourse. pregnancy, knowledge of STDs, the score of STDs prevention were statistically higher in the male student group than in the female group. 4. The students' mean score of knowledge about STDs was similar between the male group(7.80) and the female group(7.84) with a possible score range from 0-18. 5. Only fifteen percent of male and 9.6% of female students expressed that they will do something to prevent STD. 6. The group having the experience of sexual intercourse(t=3.924, P=.048) and the group of having experience of contracting STDs(t=16.638, P=.000) had shown statistically higher STDs prevention score than the group not having that kind of experience. but the group having experience with pregnancy didn't show any difference from. the group not having experience with pregnancy. Considering that 57.1% of males and 10.3% of females had sexual intercourse experience, 78% of male and 46.5% of female permitted premarital sex, 10.3% of male and 3.4% of female had been infected with STDs. It could be concluded that the college students were ignorant about the prevention of STDs and had unrealistic stage of the STDs prevention. Therefore, enforcement of education for the prevention of STDs including the dynamics of the sexual intercourse and STDs infection is needed.

  • Background and Aim: Cholangiocarcinoma (CCA) is an uncommon malignancy of the bile duct, occurring in nearly 2 out of 100,000 people. It is a type of adenocarcinoma that originates in the mucous glands of the epithelium, or surface layers of the bile ducts. The aim of this study was to evaluate the clinical features, diagnostic results and factors associated with survival, morbidity and mortalityof cholangiocarcinoma cases in Iranian patients. Method: In this retrospective study the hospital medical records of 283 patients with a primary or final diagnosis of cholangiocarcinoma who had been admitted to gastroenterology ward of our hospital from 2004 to 2011 were retrospectively reviewed. Results: 283 patients (180 male, 63%, and 103 female, 38.6%) with a mean age of $59.7{\pm}14.4$ years were studied. The most frequent symptoms were painless jaundice (190, 66.9%), abdominal pain (77, 27%), pruritus 133 (46.8%) and weight loss (169, 59.5%). The most frequent associated risk factors and diseases were as follows: gallstones (72, 25.4%), diabetes (70, 24.6%), HBV infection (52 (18.3%), HCV infection 43 (15%), primary sclerosing cholangitis (16, 5.6%) and smoking (120, 42.3%). The most frequent type of cholangiocarcinoma in ERCP and MRCP was hilar. The mean survival time was $7.42{\pm}5.76$ months. Conclusion: The mean survival time in our study was lower than one year. Moreover the most frequent risk factors and associated diseases were smoking, gallstones and diabetes. Painless jaundice, abdominal pain and weight loss were the most clinical features related to cholangiocarcinoma. Additionally survival time did not correlate with risk factors, associated diseases and clinical presentations, but was linked to biliary metallic stenting and surgery.

  • The purpose of this study was to investigate the factors influencing body mass index of 618 university students located in the Gwangju area, from Nov. 20th to 30th, 2002. The subjects included 301 males(48.7%) and 317 females(51.3%). This survey was conducted using a self-administered questionnaire. The percentage of students who were underweight, normal and overweight based on their body mass index(BMI) was 3.7, 88.0 and 8.3 for the males, and 29, 71 and 0 for the females, respectively. Most students (73.8%) skipped breakfast. This was because of lack of time(65.1%) of the cases. Self-reported eating habit problems were eating irregular meal(52.4%), overeating(21%), prejudiced meal(12.4%) and skipping meal(7.4%). The type of favorite snack was biscuit(31.9%) and cup Ramyon(31.9%). The type of favorite beverage was juice(23.3%). The coffee intake was higher in males compared to females. The rates of alcohol drinking in male and female students were 88.0% and 85.8% respectively, and the rates of smoking were 35.9% and 2.8% respectively. Percentages of weight control experience were 30.2 and 51.4 in the male and female, respectively. The BMI of students living in home and eating breakfast was higher than that of the other students. The BMI of students eating cup Ramyon and water increased. The BMI of students taking regular exercise and weight control tended to be higher than that of subjects not doing so. When the amount of pocket money available and the rates of coffee intake, alchol drinking and smoking increased, the BMI was increased. Therefore, nutritional education for university students is needed so as to improve their health and to modify life habits and nutritional education program should be developed to meet the various needs of these students.
