Primary Pleural Small Cell Carcinoma - A case report- (원발성 흉막 소세포암 - 1예 보고 -)

  • A primary pleural small cell carcinoma is an extremely rare disease. Because of a newly developed metastatic lesion, we performed an operation on a patient who had undergone a right upper lobe lobectomy for adenocarcinoma 3 years previously. We resected the pleural lesion and the pathology report showed that it was not a metastatic lesion, but rather, it was a primary pleural small cell carcinoma. So we reported this case and we review the relevant literature.

Pneumatosis Coli in a Dog (개의 결장기종 1례)

  • A poodle with clinical signs of colorectal disease was diagnosed as pneumatosis coli. In this dog, hypoadrenocorticism and immune-mediated hemolytic anemia concurred. Pneumatosis coli is one of the emphysematous gastrointestinal diseases. The etiology is unknown but mechanical, pulmonary and bacterial theories have been proposed. In pneumatosis coli, gas accumulates within submucosal or subserosal layer of colon or rectum, so intramural radiolucent gas is observed on abdominal radiographs. In this dog, intramural gas accumulation occurred in cecum. Definitive diagnosis was based on radiographic findings and an underlying cause was not elicited. Pneumatosis coli should be included into differential diagnosis for nonspecific colorectal signs.

Clinical Value of Eukaryotic Elongation Factor 2 (eEF2) in Non-small Cell Lung Cancer Patients

  • Background: The purpose of this study was to evaluate a new type of tumor biomarker, eukaryotic elongation factor 2 (eEF2), in serum for the early diagnosis, confirmative diagnosis as well as assessment of treatment of non-small cell lung cancer (NSCLC). Methods: 130 patients with NSCLC and 50 healthy individuals undergoing physical examination in our hospital provided the observation and healthy control groups. An enzyme linked immune sorbent assay (ELISA) method was applied to determine serum eEF2 levels. Serum neuron specific enolase (NSE) and squamous cell carcinoma antigen (SCC) levels in the observation group were assessed with an automatic biochemical analyzer. Results: The median levels of eEF2 in the serum of NSCLC patients was found to be significantly higher than the healthy control group (p < 0.01) and it was markedly higher in stages III, IV than stages I, II (p < 0.05). eEF2 was higher with tumor size ${\geq}2$ cm than <2 cm (P< 0.01). Furthermore, two weeks after surgery patients showed a significant trend for eEF2 decrease (p < 0.05). Conclusions: The eukaryotic elongation factor 2 (eEF2) has certain clinical values for early diagnosis, verification, and prognosis as well as classification of lung cancer patients.

Clinical Efficacy of Bevacizumab Concomitant with Pemetrexed in Patients with Advanced Non-small Cell Lung Cancer

  • Objective: To observe the clinical efficacy of bevacizumab concomitant with pemetrexed in patients with advanced non-small cell lung cancer (NSCLC). Materials and Methods: A total of 72 patients were randomly divided into a combination group (pemetrexed+bevacizumab, n=36) and a pemetrexed group (n=36) and assessed for disease control (CR+PR+SD) after 4-cycles of first-line GP chemotherapy (gemcitabine+cisplatin). Clinical efficacy, progression-free survival time (PFS), overall survival time (OS), overall response rate (ORR), disease control rate (DCR) and rate of adverse responses between two groups were observed and compared. Results: ORR and DCR were 27.8% and 83.4% in combination group, and 16.7% and 69.5% in the pemetrexed group, respectively, but there were no significant differences (P>0.05). PFS in combination group and pemetrexed group were 4.6 months and 3.9 months respectively (P=0.09), whereas OS in the combination group was 14 months, evidently higher than in the pemetrexed group (11 months, P=0.004). Adverse responses in both groups included high blood pressure, bleeding, thrombocytopenia, anemia, elevated transaminase, diarrhea, vomiting and proteinuria, but there were no significant differences (P>0.05). Conclusions: Bevacizumab concomitant with pemetrexed has better clinical efficacy and safety, giving rise to prolonged survival time in patients with advanced NSCLC.

A Small-and-medium-sized Hospital's Crisis Management during 2015 MERS Outbreak: A Case of G Hospital (중소병원의 2015 MERS 위기 대응: G병원의 사례)

  • Purpose: Crisis is inevitable to every organization and therefore, successful crisis management is critical to the organizations' survival and prosperity. With the understanding, this study aims to draw propositions for successful crisis management of hospitals when facing infectious disease outbreak. For the purpose, a case of a small and medium sized hospital's experience of crisis management during 2015 Middle East Respiratory Syndrome outbreak was analyzed. Methodology/Approach: The detailed internal circumstances and experiences of the hospital during the MERS outbreak were identified by in-depth interview as well as the extensive material review, and analyzed under the view of the theories of accident, error, and crisis in relation of organization management Findings: Overall, nine propositions are drawn by the phase of crisis. In pre-crisis phase, for example, 'the hospital preparedness has positive influence on the effective responding to the crisis'. In detection phase, 'the mindfulness of the hospital organizations' as well as the individuals' has positive influence on detecting the crisis signals'. In crisis phase, for example, 'improvising naturally occurs in crisis by the unknown disease, therefore, a component site supervisor coordinating such improvision is important'. Lastly, in post-crisis phase, 'successful crisis responding experience facilitates the positive hospital culture'. Practical implication: From the experience of a small and medium size hospital, it is suggested that proactive system approach oriented by safety is beneficial for effective crisis management.

Two Cases of Gastric Metastasis from Small Cell Lung Cancer (소세포 폐암에서의 위 전이 2예)

  • This is a report of gastric metastases secondary from a primary small cell carcinoma of the lung in two men. Blood-borne metastatic involvement of the stomach by cancer is a rare entity. According to the reports in the literature the prevalence of metastasis to the stomach occurs in 0.4% and the most common cell type of the primary lung carcinoma is large cell type(3.7%) followed by adenocarcinoma(2.4%), small cell carcinoma(1.7%) and squamous cell carcinoma(0.7%). The most common tumors that spread to the stomach through the blood stream are malignant melanoma, breast carcinoma and lung carcinoma. Most of the gastrointestinal tract metastases had no specific symptoms because of its submucosal involvement. The prognosis was poor and the mean survival period from the onset of symptoms was 49 days. The first patient was a 56-year-old man who had primary lung carcinoma with brain metastasis. Gastroscopic findings showed two elevated mass lesions in the anterior wall of the mid body with central ulcer and the posterior wall of the fundus with intact surface mucosa. Pathologic examination of stomach tissue revealed small cell type tumor cells infiltrate in the stomach wall segmentally without destruction of the glands. The second patient was a 67-year-old man who had no other evidence of the distant metastasis. Gastroscopic findings showed a huge, oval shaped, ulcerofungating mass with deep penetrating central ulcer coated with dirty exudate in the anterior wall from mid to upper body of the stomach, and thickened elevated rugal folds in the posterior wall of the fundus. Pathologic examination of stomach tissues revealed the small cell type tumor cells showing small smudged nucleus infiltrate into the mucosa of the stomach and the architecture of mucosa intact. We report the two cases of metastatic gastric cancer from the primary small cell lung carcinoma with the literature review.

A Clinical Report of the Patient with Limited-Stage Small Cell Lung Cancer (제한병기 소세포폐암 환자의 치험례 임상보고)

  • Objectives : This study was designed to evaluate the effects of oriental herb medicine therapy on a limited stage small ell lung cancer(SCLS) patient. Methods : The medical record of this case of SCLS was researched. The patient had been treated with oriental herb medicine for five months continuously in OPD. Clinical data was analyzed and the mass of small cell lung cancer before and after oriental herb medicine therapy was compared using X-ray and CT scan of the whole body in order to determine metastasis. Result : After 5 months of oriental herb medical treatment, most of symptoms disappeared or improved. The small cell lung cancer patially receeded and improvements were mainly seen in sputum, coughing, insomnia, diarrhea, and performance status. Conclusion : The study suggests that oriental herb medicine therapy effects the tumor size by causing remission and Improving health and relieving symptoms, and also positively effects the quality of life as a supportive or curative therapy for SCLC patients.

Influences of Safety Attitude for Patient, and Moral Sensitivity in Practice of Standard Precaution in Small and Medium Hospital Nurses (중소병원 간호사의 환자안전에 대한 태도, 도덕적 민감성이 표준주의지침 수행에 미치는 영향)

  • This study aimed to explore the influences of safety attitude for patient and moral sensitivity in practice of standard precaution in small and medium hospital nurses. Data were collected from 150 small and medium hospital nurses in January 11, 2021 to January 22, 2021 and were analyzed using Descriptive statistics, Independent t-test, one-way ANOVA, Pearson correlation coefficients, and multiple regression. Practice of standard precaution was significantly correlated with safety attitude for patient (r=.539, p<.001) and moral sensitivity(r=.249, p=.002). Practice of standard precaution was associated safety attitude for patient, and work unit with 33.8% of the variance(F=18.47, p<.001). Based on the findings of this study, each institution needs to make efforts to strengthen the safety attitude for patient of small and medium hospital nurses for practice of standard precaution.

  • Bronchogenic carcinoma involving the carina has remained a challenging problem for thoracic surgeons. Carinal resection and reconstruction is limitedly indicated because this aggressive surgical approach has been reported to be associated with significant morbidity and mortality while long-term outcome has not been determined. Wesuccessfully performed carinal reconstruction and sleeve right upper lobectomy assisted with ECMO for a 60-year-old male with squamous cell carcinoma in the right upper lobe extending to the carina.

  • Objectives: This study examined the case of a patient with ROS1-positive recurrent non-small-cell lung cancer treated with crizotinib and traditional Korean medicine. Methods: The patient was treated with crizotinib from January 20 2021 to May 22 2021, together with Haedogyangpye-tang and Haengso-tang. The tumor size was measured using computed tomography (CT), and adverse events were evaluated according to the National Cancer Institute's Common Terminology Criteria for Adverse Events (NCI-CTCAE) version 5.0. Results: After four months of combined treatment, the sizes of the lymph nodes in the porta hepatis, hepatoduodenal, retrocrural, aortocaval, and para-aortic areas had decreased, and no lymph nodes larger than 1 cm in diameter were observed. The side effects of chemotherapy also improved. Conclusions: This case study suggests that traditional Korean medicine may alleviate the side effects of chemotherapy, improve quality of life, and complement chemotherapy itself.
