Changes in Multi-channel Electrogastrography after Acupuncture Treatmet in Patients with Functional Dyspepsia (기능성 소화불량증 환자에서 침구 처치 후 다채널 위전도 소견의 변화)

  • The aim of this study was to investigate the effects of traditional Korean medical therapy in patients with functional dyspepsia evaluted using multi-channel electrogastrography. Before the treatment, the EGG of 14 outpatients (male 3, female 11) with functional dyspepsia were recorded and their dominant frequency(DF), percentage of gastric slow waves, percentage of Power ratio, and percentage of slow wave couplings were analysed. For 8 weeks, the patients received acupuncture treatment. After 8 weeks, EGG was rechecked and compared with the previous data. After treatment, normogastria was significantly increased in channel 1 and channel 3($53.70{\pm}18.94$% vs $72.64{\pm}15.49$% in channel 1, P<0.05; $79.74{\pm}18.09$ vs $90.86{\pm}8.86$ in channel 3, P<0.05). arrythmia was significantly decreased in channel 3($14.34{\pm}15.21$ vs $5.93{\pm}6.90$, P<0.05). The patients who had less severe decrease in EGG indexes showed improvement of those indexes through 8 weeks of outpatient care. EGG channel 3 was shown to be the most adequate standard of evaluation. But there was no significant improvement in slow wave coupling rate which is a new index of multichannel EGG.

  • The second inward current $(i_{si})$ was studied by the two microelectrode voltage clamp technique in the sino-atrial node of the rabbit. The slow component $(i_{si,2})$ of the second inward current was sometimes identified and $i_{si}$ behaved as if it were a mixture of two currents. We analysed the $(i_{si,2})$ in relation to membrane potential and frequency of voltage clamp pulses. 1) Membrane was held at -40mV which was usually found to be zero current level. When depolarizing pulses were applied, the slow inward current $(i_{si})$ was activated. 2) It was shown that there are three categories of the $i_{si}$ activation by the low level of depolarizing clamp pulses. Moderately fast inward current with single component was developed in most cases in the presence of tetrodotoxin(TTX). But sometimes there was two separate components of $i_{si}$ activation in the peak level and the time course. Thirdly the only slow component of $i_{si}$ was found in rare cases. 3) The activation of $(i_{si,2})$ was dependent upon membrane potential. The $i_{si}$ shows two separate peaks during clamp depolarizations and higher clamp pulses lead to fusion of the peaks. 4) The $i_{si,2}$ activation showed that it decreased with repetitive clamp pulses and it was more evident in higher frequencies(2Hz)(negative staircase).

The inhibitory action of nitric oxide donor on the slow wave and spontaneous contraction in the guinea pig antral circular muscle (기니피그 유문부 윤상근의 서파 몇 자발적 수축에 대한 nitric oxide donor의 억제적 작용)

  • We investigated the effects of nitric oxide (NO) donors, S-nitroso-L-cysteine (Cys-NO) and 3-morpholinosydnonimine hydrochloride (SIN-1), on the contractile and electrical activity of the circular muscle of guinea pig gastric antrum by using intracellular microelectrode technique. The gastric antral circular muscle showed spontaneous phasic contraction and slow wave of membrane potential. Cys-NO ($0.001{\sim}10{\mu}M$) and SIN-1 ($0.001{\sim}100{\mu}M$) reduced not only the tonic and phasic contraction but also the amplitude of slow wave in a concentration dependent manner. NO donors were more potent to inhibit phasic contraction than to do slow wave. These inhibitory effects of NO donors were mimicked by the membrane permeable guanosine-3',5'-cyclic monophosphate (cGMP) analogue, 8-bromo-cyclic GMP (8-br-cGMP, $10{\sim}300{\mu}M$). The inhibitory effects of SIN-1 and Cys-NO were antagonized by the guanylate cyclase inhibitor, 1H[ [1,2,4]oxadiazolo[4,3,-a]quinoxalin-1-one (ODQ, $10{\mu}M$). These results suggest that the inhibitory effects of NO donors on the mechanical and electrical activity is mainly mediated by cGMP pathway.

  • Lennox-Gastaut syndrome (LGS) is an electro-clinical syndrome composed of the triad of mental retardation, multiple seizure types, and the characteristic generalized slow spike-wave complexes in the electroencephalogram. In this article, we report on two patients with LGS whose brain magnetic resonance imaging showed dysgenesis of corpus callosum (CC). We discuss the role of CC in the genesis of secondary bilateral synchrony.

  • This study was carried out to characterize nonadrenergic, noncholinergic(NANC) relaxation of porcine retractor penis(PRP) muscle induced by electrical field stimulation(EFS) and to investigate the actions of niric oxide(NO) and vasoactive intestinal polypeptide(VIP) as candidates for NANC neurotransmitters. Biphasic relaxations of PRP muscle were induced by EFS to NANC nerve. Rapid-phase relaxation was observed at low frequency(0.5-16Hz) and slow-phase relaxation followed during high frequency(8-60Hz). Both relaxations were frequency-dependent and TTX($1{\times}10^{-6}M$)-sensitive. L-NAME($2{\times}10^{-5}M$) inhibited the rapid-phase relaxation, but not the slow-phase relaxation. The inhibition of the rapid-phase relaxation with L-NAME was reversed by L-arginine ($1{\times}10^{-3}M$) but not by D-arginine($1{\times}10^{-3}M$). Methylene blue($4{\times}10^{-5}M$) reduced the rapid-phase relaxation. Exogenous No(ExoNO, $1{\times}10^{-5}-1{\times}10^{-4}M$) induced dose-dependent relaxations of PRP muscle. Oxyhemoglobin($5{\times}1^{-5}M$) blocked the relaxation induced by ExoNO and inhibited EFS-induced relaxation. Hydroquinone($1{\times}10^{-4}M$) also abolished the relaxation induced by ExoNO, but did not affect EFS-induced relaxation. L-NAME resistant slow-phase relaxation to EFS was inhibited by ${\alpha}$-chymotrypsin(2.5 U/ml). Both methylene blue($4{\times}10^{-5}M$) and Nethylmaleimide($1{\times}10^{-4}M$) reduced the slow-phase relaxation by EFS. [4-Cl-D-$Phe^6$, $Leu^{17}$]-VIP($3{\times}10^{-6}M$) inhibited the slow-phase relaxation by EFS. External applications of VIP ($1{\times}10^{-7}M$) caused relaxations that were simillar to the L-NAME resistant slow-phase relaxations induced by EFS, and relaxant effects of exogenous VIP were blocked by ${\alpha}$-chymotrypsin(2.5 U/ml).

  • A 1-year-old female Slow loris was presented with swelling of left nasal bridge. On physical examination, fracture of left upper canine tooth was found. Cytologic and radiographic examination on the lesion revealed bacterial infection and pulpitis, respectively. The bacteria were identified as Streptococcus gordonii by culture and molecular diagnosis. After removal of damaged tooth and administration of antibiotics, the abscess was resolved.

  • The aim of this article is to argue that a valuable tool to calm impatience is to listen to Sijo. The author seeks to argue that the best way to calm impatience is to listen to Sijo. Sijo refers to a slow and mellow music in the family of Han Ak (Korean music, 韓樂). The term slow is a revered keyword in our culture. "Slow" is a blank word in Han Ak (Korean music, 韓樂). The soul of Wol-ha Kim's Sijo is a beauty of space and easiness. Therefore, her voice will help relax the muscles of the listener and calm the soul. It is akin to the struggles of modern people competing for something excessively but ending up with nothing. We often find that gentle jogging is better than sprinting. Slow music is thus good for one's health. For example, we know that our skin can become beautiful and that real beauty can be obtained only when the body and mind are at rest and in comfort. Physical appearance depends on a healthy mind and body. The author believes that Sijo for music therapy is good for our mental health. If its effectiveness is confirmed after performing tests on animals and humans in an experimental study, we can use this type of music to treat patients with psychological illnesses.

  • Myositis ossificans progressiva, otherwise called fibrodysplasia ossificans progressiva is a rare hereditary disease of unknown etiology, which is characterized by progressive ectopic ossification in connective tissues of the muscles, tendons, fascia and aponeurosis. We report a case of myositis ossificans progressiva confined to hands and feet, which has an unusual clinical feature with slow progression.

  • It was investigated whether $Ca^{2+}$ and $K^+$ channels were involved in the inhibitory action of nitric oxide (NO) on the contractile and slow wave activity of guinea pig gastric antral circular muscle. The gastric antral circular muscle showed spontaneous phasic contraction and slow wave. NO donors, 3-morpholinosydnonimine hydrochloride (SIN-1, $0.01{\sim}100{\mu}M$) and S-nitroso-L-cysteine (CysNO, $0.001{\sim}10{\mu}M$), reduced not only the amplitude of phasic contraction but also that of slow wave in a concentration-dependent manner. Both the perfusion of $Ca^{2+}$-free solution and the administration of $Ni^{2+}$, a nonselective $Ca^{2+}$ channel blocker, reduced the phasic contraction as well as the amplitude and frequency of the slow wave. The effects of these treatments were similar to those of NO donors. Nifedipine ($10{\mu}M$), a specific L-type $Ca^{2+}$ channel blocker, abolished the phasic contraction and remarkably reduced the plateau of slow wave but had no profound effect on the upstroke of slow wave. In the whole-cell patch clamp mode, CysNO shifted the steady-state activation curve for L-type $Ca^{2+}$ current to the right and the steady-state inactivation curve to the left. Pretreatment of various $K^+$ channel blockers such as tetraethylammonium (1 mM), 4-aminopyridine (0.5 mM), glibenclamide (10 mM), apamin ($0.1{\mu}M$), and iberiotoxin ($0.1{\mu}M$) did not affect the inhibitory action of SIN-1. These results suggest that NO donors suppress mechanical and electrical activity of guinea pig gastric antral circular muscle by inhibition of L-type $Ca^{2+}$ channel rather than by activation of $K^+$ channels.
