Objectives : The aim of this study was to evaluate the effects of EFT-I(EFT program for insomnia) for insomnia in the elderly as a preliminary study. Methods : This study was a single group pre-post comparative study that involved 10 elderly women(mean age=$76.3{\pm}4.29$), who visited a senior welfare center, complained of insomnia symptoms. Subjects received 8 sessions(twice a week for 4 weeks and 1 hour for each session) of EFT-I group training. Insomnia severity, depression, state-anxiety, and life satisfaction of each subject were evaluated twice at pre and post of EFT-I. Korean Sleep Scale, Short form of Geriatric Depression Scale(SGDS), State-Trait Anxiety Inventory(STAI), and life satisfaction scale were used as evaluation tools. The data were analyzed using paired-samples T-test. Results : Insomnia severity, life satisfaction, depression, and state-anxiety were significantly improved by EFT-I. Conclusions : Result of this study showed that EFT-I can be a useful treatment program for elderly insomnia. Larger clinical trials are needed to verify effect of EFT-I as a community based insomnia management program for the elderly.
도시에서 교통 및 주차 관리의 효율성은 삶의 질과도 직접적으로 관련이 있는 중요한 문제이다. 거주자우선주차장은 도시의 주차공간 확보에 큰 비중을 차지하면서도 개별 주차 구역이 작고 별도의 출입구가 없어 효율적인 운영을 위한 적절한 인증시스템이 부재하였다. 본 논문에서는 무선 센서 네트워크를 이용하여 추가적인 관리인 없이 적은 비용으로 거주자우선주차장을 효율적으로 관리할 수 있는 인증시스템을 설계하고 구현하였다. 구현된 시스템의 실험을 통하여 저전력 소모를 위한 슬립 주기별 센서 노드의 수명을 측정하였고, 간단히 측정된 조도값의 변화만으로 주차 여부를 판단하기 위하여 시간대별로 주차 유무에 따른 조도값의 변화를 실험하여 그 오차범위가 최소 45 이상이면 주차 여부를 확인할 수 있음을 보였다.
본 연구는 국민건강영양조사 제6기(2013년~2015년) 자료를 토대로 건강행태 및 심혈관질환과 치주질환의 영향요인을 분석하고자 하였다. 연구대상은 심혈관질환(고혈압, 협심증, 심근경색증, 뇌졸중), 치주질환이 있는 만 19세 이상을 대상자로 선정하였고, SPSS(SPSS 23.0 for Windows)를 사용하여 14,940명을 분석하였다. 교차분석(chi-quare test) 및 복합표본 로지스틱 회귀분석을 사용하여 인구사회경제학적 특성, 건강행태 및 심혈관질환과 치주질환의 유무를 분석하였다. 결과로는 고혈압, 협심증, 심근경색증, 뇌졸중과 관련성이 있었다. 영향요인은 고혈압이 없을 경우 치주질환 유병률이 0.684배 낮았고, 남자, 연령, 학력, 교육수준, 흡연, 잠자기 전 칫솔질, 치실 치간칫솔 사용이 통계적으로 유의하였다. 결과로는 건강행태 개선과 두 질환의 체계적 관리가 요구되고, 향후 질환 간의 예방교육 프로그램 개발에 도움을 주고자 한다.

Wireless smart sensors enable new approaches to improve structural health monitoring (SHM) practices through the use of distributed data processing. Such an approach is scalable to the large number of sensor nodes required for high-fidelity modal analysis and damage detection. While much of the technology associated with smart sensors has been available for nearly a decade, there have been limited numbers of fulls-cale implementations due to the lack of critical hardware and software elements. This research develops a flexible wireless smart sensor framework for full-scale, autonomous SHM that integrates the necessary software and hardware while addressing key implementation requirements. The Imote2 smart sensor platform is employed, providing the computation and communication resources that support demanding sensor network applications such as SHM of civil infrastructure. A multi-metric Imote2 sensor board with onboard signal processing specifically designed for SHM applications has been designed and validated. The framework software is based on a service-oriented architecture that is modular, reusable and extensible, thus allowing engineers to more readily realize the potential of smart sensor technology. Flexible network management software combines a sleep/wake cycle for enhanced power efficiency with threshold detection for triggering network wide operations such as synchronized sensing or decentralized modal analysis. The framework developed in this research has been validated on a full-scale a cable-stayed bridge in South Korea.
본 연구의 목적은 치주질환자의 치주질환에 대한 인식도 및 생활 스트레스와 구강건강관련 삶의 질과의 관련성을 파악하여 치주건강의 증진에 긍정적 기여를 할 수 있는 프로그램을 모색하고자 시도하였다. 연구대상은 치과 병 의원에 내원한 치주질환 환자 201명으로 하였고, 구조화된 무기명 자기기입식 설문조사를 실시하였다. 구강건강관련 삶의 질에 영향을 미치는 유의한 변수는 생활 스트레스, 치주질환 인식도, 구강위생용품, 수면량, 연령으로 나타났다. 이에 치주질환자들의 스트레스 관리능력 및 대처법의 중요함과 올바른 치주질환 인식도를 정립하기 위해 환자 상태별 적합한 상담이 필요할 것으로 사료된다.
Anticholinergic drugs are included in the efficacy group of various antidepressants, antihistamines, antispasmodics like skeletal muscle relaxants. Elderly patients are often taking anticholinergic drugs due to various diseases such as sleep disorders and dysuria. But the use of anticholinergic drugs is restricted in guidelines such as Beers Criteria or STOPP due to the anticholinergic adverse effects including dry mouth, constipation, difficult urination, delirium, hallucinations and especially cognitive impairment. In this study, we investigated the usage of anticholinergic drugs in out-of-hospital prescription of 4,442 elderly patients. Results of the study were obtained that 32% (n=1,421) of overall patients were prescribed with 1~6 products (average 1.37) of anticholinergic drugs. 70.9% of the 1,421 patients (n=1,007) were prescribed with one drug, 22.7% (n=323) were two drugs and 4.9% (n=70) were three drugs. 27.1% of the 1,421 patients (n=430) were 70~74 years old patients who were the most commonly prescribed with anticholinergic drugs. Amitriptyline, chlorpheniramine, dimenhydrinate and quetiapine were most frequent component of ACB Score 3 drugs and amantadine, baclofen, carbamazepine, cyproheptadine and oxcarbazepine were most frequent of ACB Score 2 drugs. Anticholinergic Drug Scale (ADS) of individual patients presented one point (48.5%, n=689), 2 points (15.4%, n=219), more than 3 points (36.1%, n=513), and up to maximum 12 point (n=1). More than 2 points were more than half (51.5%, n=732). Therefore, additional prospective study in the use and adverse effects of anticholinergic drugs for elderly patients will be required. And national management such as DUR program will be required for elderly drug administration from now on.
Objectives: The purpose of this study was to analyze the factors affecting weight loss effect of Gamitaeumjowee-tang and to see if weight loss could be predicted using influence factors and weight loss progress. Methods: From September 2016 to March 2017, we retrospectively reviewed the medical records of 139 patients who were visited to the Korean Oriental Clinic for 3 months. We conducted a regression analysis to determine whether age, gender, initial weight, patient health questionnaire-9, heart rate variability (HRV), sleep quality, drinking habit and the medication history of weight loss affect weight loss. We found weight loss prediction equations using multiple regression analysis applying significant factors and weight loss progress. Results: Gender and initial weight had a significant effect on weight loss in all periods (P<0.001). HRV had a significant effect on primary weight loss (P<0.01). Other factors did not have any significant effect on weight loss. Using the significant factors, weight loss of each period could be predicted from 23.9% to 44.6%, and tertiary weight loss could be predicted with 76.6% using factors, primary weight loss and secondary weight loss (P<0.001). Conclusions: This study suggests that weight loss effect of Gamitaeumjowee-tang maybe be affected by influence factors and that weight loss prediction equations using them can be used for obesity treatment.
Purpose: The previous studies on $H_1$ antihistamine overdose have generally been limited to cases of acute doxylamine succinate (DS) poisoning, yet there have been some studies on diphenhydramine (DPH) overdosing. But many clinicians consider the two drugs to be very similar and to have similar ingredients. The purpose of this study was to clarify the toxicologic characteristics and clinical outcomes between DS and DPH poisoning/overdose. Methods: We reviewed the medical and intensive care records of the patients with acute DS or DPH poisoning and who admitted to our emergency department from January 2008 and April 2010. We collected patient information regarding the features of the poisoning and the clinical and demographic characteristics. The patients were assessed for the clinical outcomes, the GCS, the PSS (Poisoning Severity Score) and the SOFA (Sequential Organ Failure Assessment). Results: Fifty seven patients (45 cases of DS poisoning and 12 cases of DPH poisoning) were enrolled. Compared with the DS group, the DPH group had higher incidences of intubation, serious mental change, QTc prolongation and ECG conduction abnormality (p=0.041, <0.001, 0.014 and 0.044, respectively). The DPH group had a higher PSS and a longer ICU stay. The peak CPK time and the CPK normalization time were longer for the patients with rhabdomyolysis due to DS poisoning. Conclusion: Two common $H_1$ antihistamines, doxylamine and diphenhydramine, are in the same ethanolamine-structural class, but the toxico-clinical outcomes are different according to many aspects. Therefore, clinicians could take a careful approach for the differential diagnosis and management between DS and DPH poisoning.
최근의 에너지 하베스팅 무선 센서 네트워크에 대한 연구는 제한된 에너지 자원 문제를 해결하여 네트워크 수명을 효율적으로 연장시킬 수 있는 기술 개발에 집중되고 있다. 에너지 하베스팅 기술은 무선신호에 포함된 에너지를 이용하여 배터리를 충전시킴으로써 네트워크 수명을 지속적으로 연장시킬 수 있는 장점을 지니고 있으나 에너지를 수확하는 시간 동안에는 데이터를 전송할 수 없기 때문에 에너지 수확양이 증가할수록 데이터 전송지연도 증가하는 문제점을 지니고 있다. 이에 따라 에너지 하베스팅 무선 센서 네트워크를 설계할 때에는 네트워크 수명 연장뿐만 아니라 전송지연 축소 문제를 모두 고려하여야 한다. 본 논문에서는 네트워크에 유입되는 트래픽 양과 수확되는 에너지양에 따라 데이터 전송에 필요한 에너지를 계산하고 패킷데이터의 전송 시간을 조절함으로써 종단간 네트워크 지연을 최소화하는 MAC 프로토콜을 제안한다. 이를 위해 에너지 수확 시간을 측정하여 네트워크의 수면 시간을 조절하는 에너지 관리 메커니즘을 설계한다. 또한, 시뮬레이션을 이용한 성능평가를 통하여 기존의 MAC 프로토콜보다 에너지 소비량과 종단간 지연 측면에서 성능이 향상됨을 보인다.
Objective: Shift work including night work is known to be harmful to the health of nurses. This study was conducted in order to systematically review the academic literature on the health effects of working time patterns, including shift work or night work, among Korean nurses and propose a research direction for the future. Methods: We searched online academic databases to find relevant papers in domestic and international journals using the keyword terms 'shift work', 'night work', 'night shift', 'work shift', 'nurse', and 'Korea', and reviewed a total of 36 articles. Results: As a result of this literature review, more than half of the articles were found to investigate the health effects between shiftwork and non-shiftwork nurses. A few studies considered the shift type(n=6), frequency of night shift(n=4), and shiftwork duration(n=3) as working time patterns. These studies focused mainly on mental health(n=18) or sleep(n=15). Conclusions: Based on the review results obtained from this study, it is necessary to assess working time patterns of Korean nurses using a variety of variables including night-work-related factors, and various health outcomes should be examined considering the confounder.
