Predictors and Prevalence of Alcohol and Cannabis Co-use Among Filipino Adolescents: Evidence From a School-based Student Health Survey

  • Objectives: This study explored the prevalence and predictors of alcohol and cannabis co-use among 9263 Filipino adolescents, using data from the 2019 Global School-based Student Health Survey (GSHS). Methods: We conducted a cross-sectional secondary analysis of the GSHS, targeting adolescents aged 13-17 years and excluding cases with incomplete data on alcohol and cannabis use. Our analysis employed the bivariate chi-square test of independence and multivariable logistic regression using Stata version 18 to identify significant predictors of co-use, with a p-value threshold set at 0.05. Results: The weighted prevalence of co-users was 4.2% (95% confidence interval [CI], 3.4 to 5.3). Significant predictors included male sex (adjusted odds ratio [aOR], 4.50; 95% CI, 3.31 to 6.10; p<0.001) and being in a lower academic year, specifically grade 7 (aOR, 4.08; 95% CI, 2.39 to 6.99; p<0.001) and grade 8 (aOR, 2.20; 95% CI, 1.30 to 3.72; p=0.003). Poor sleep quality was also a significant predictor (aOR, 1.77; 95% CI, 1.29 to 2.44; p<0.001), as was a history of attempted suicide (aOR, 5.31; 95% CI, 4.00 to 7.06; p<0.001). Physical inactivity was associated with lower odds of co-use (aOR, 0.45; 95% CI, 0.33 to 0.62; p<0.001). Additionally, non-attendance of physical education classes (aOR, 1.48; 95% CI, 1.06 to 2.05; p=0.021), infrequent unapproved parental checks (aOR, 1.37; 95% CI, 1.04 to 1.80; p=0.024), and lower parental awareness of free-time activities (aOR, 0.63; 95% CI, 0.45 to 0.87; p=0.005) were associated with higher odds of co-use. Factors not significantly linked to co-use included age group, being in grade 9, always feeling lonely, having no close friends, being bullied outside school, and whether a parent or guardian understood the adolescent's worries. Conclusions: The findings highlight the critical need for comprehensive interventions in the Philippines, addressing not only physical inactivity and parental monitoring but also focusing on sex, academic grade, participation in physical education classes, sleep quality, and suicide attempt history, to effectively reduce alcohol and cannabis co-use among adolescents.

Study on Therapeutic Effect of KMST (Korean Medicine Steam Therapy) -Included Korean Medicine Combination Therapy about Leukorrhea Patients (熱氣熏法 및 침, 뜸, 한약 치료가 帶下를 주소로 내원한 환자의 증상변화에 미치는 영향에 대한 연구)

  • Objectives This study aimed to investigate therapeutic effect of Korean Medicine Steam Therapy (KMST) for leukorrhea patients. Methods From December 1st 2013 to Nov 31st 2014, 16 leukorrhea outpatients treated with KMST filled out questionnaire before and after treatments. The questionnaire was composed of 8 question items such as sleep, appetite & digestion, urination, defecation, emotion, cold hypersensitivity, menstrual pain, leukorrhea and patients were told to evaluate their symptoms as NRS score. We analyzed mean NRS score of each question item before and after KMST by using Wilcoxon's signed rank test of SPSS ver. 19. Results Mean age of participants was 40.8±13.0 years and mean value of treatment numbers per person was 3.8±1.8 times. 43.75% (n=7) of the patients had history of vaginitis caused by gardnella vaginalis, ureaplasma urealyticum or candida albicans. All patients were treated with acupuncture, moxibustion and Korean medicine. In addition, there was no side effect reported by participants. NRS score of quantity, odor of leukorrhea, perineal unpleasant sensation such as itching, dryness, burning sensation remarkably decreased (p<0.01, p<0.05, p<0.01 each). NRS score of symptoms including sleep, urination, cold hypersensitivity, menstrual pain also remarkably decreased (p<0.05). Conclusions Symptoms of leukorrhea patients including sleep, urination, cold hypersensitivity, menstrual pain, leukorrhea showed significant decrease after treated with concurrent KMST.

A Case of Fluoxetine-induced Mania & Suicidal Ideation in Obsessive-Compulsive Disorder (Fluoxetine 투여 후 조증과 자살 사고를 보인 강박증 환자 1례)

  • The fluoxetine is one of the most frequently prescribed drugs for the treatment of depression and obsessive-compulsive disorder(OCD). This has been known as one of the most safest medication. But since the advent of this drug, there have been several reports of side effects-the mania and suicidal ideation-encountered during coadministration of fluoxetine with or without other psychotropic drugs. We experienced a case of 20 years old male OCD patient who developed into abrupt manic state and also was preoccupied with intense suicidal ideation following fluoxetine use. He was a only child in his family and his father had a history of alcoholism about 15years ago. Our patient's obsessive-compulsive symptoms have been occured since puberty. His OCD symptoms and anxiety were aggravated since joining the army. Beside these facts, we could not find any other psychiatric history such as depressive disoder and bipolar disorder. We used the fluoxetine starting dosage of 20mg and increased to 40mg at second week. About 3 weeks after the treatment, he developed sudden manic symptom and more aggravated suicidal ideation without any OCD symptoms. He felt vitalized and energetic without having enough sleep and food. These symptoms were ceased over two weeks by stopping medication. Up to this point, the reason why fluoxetine induces mania and suicidal preoccupation is unclear. But somehow the fluoxetine has effects on serotonin receptor and serotonin-dopamine regulations, thus we could make an assumption that fluoxetine can induce mania, extrapyramidal symptoms(EPS) and suicidal ideation in some part of the serotonin unbalanced patients. We think this would be the first report to remark on fluoxetine's suicidal and manic side effects in Korea. So here we present the case with the summary of reviewed articles.

The Psychiatric Treatment Link Characteristics of Suicide Attempters Visiting Emergency Room (응급실 내원 자살시도자의 정신과 진료 연계 관련 특성)

  • Purpose: This study aimed to investigate the characteristics of patients attempting suicide and to analyze treatment determination factors for patients with mental illness who go to an emergency center for treatment. Methods: Data collected from 117 suicide attempters who visited Busan Regional Emergency Medical Center were analyzed using frequency, percentage, ${\chi}^2$-test with SPSS/WIN 15.0. Results: Only 31 cases consulted with the psychiatric department; the other attempters' refused to consult (26.5%). Among the 31 attempters who consulted, 23 cases (74.2%) were suffering from depression. The most common reason (38.6%) suicidal attempters gave to refuse psychiatric treatment was "I'm not mad. I don't need the psychiatric treatment". Treatment determination factors for mental illness were religion (p<.001), past history (p=.017), financial satisfaction (p=.048), previous history of suicidal attempts (p=.006), sleep disturbance (p<.001), expression of suicide (p=.010), and type of leaving the emergency room (p=.020, p<.001). Conclusion: Results suggest that people frequently misunderstand psychiatric treatment which leads to their refusal to accept psychiatric treatment. Therefore hospitals need to develop standard guidelines and procedures for suicidal attempters with the collaboration of emergency and psychiatry departments. In addition, medical teams need to provide attempters appropriate information and encourage them to actively seek psychiatric treatment.

  • A Study on environmental factors influencing blood pressure control of a patient with essential pressure hyper -tension was conduct to the identification of differences of demographic characteristics, socioeconomic factors and living habits. This study was performed on 219 cases who were divided into two groups from Sept. 14 to Oct. 20, 1979. One is the group with complication in the departments of internal medicine and outpatient of 7 hospitals in Seoul. The other is the group without complication in the same places as above. The instrument applied to this study consists of 19 items amended through preliminary test after consideration of related literatural review and Nursing History made by Smith (1965). The materials were analyzed by computer program. The findings were as follows : 1. The group with complication had more sleep disturbances than the group without complication 2. The group with complication used more short term coping method on stress than the group without complication. 3. The group with complication had a tendency to more intake of salt, fat, alcohols and cigretts than the group without complition, but there were no significant differences between two groups. 4. The group with complication were lower social class than the group without complication. 5. The group with complication were more single(Death of spouse, Divorce, Separation) in marital status than the group without complication.

  • Home gateway is always full-powered for ubiquitous home services, and consumes much energy yearly. Power-saving algorithm to conserve this energy must reduce the energy consumption and preserve always-on services. Our algorithm predicts current idle period using the history of the past idle period when the idle period starts, and then determines whether the power mode is changed to the saving mode or not. On the power saving mode, it processes the simple protocol data for network control using proxying with no wakeup. And it changes the power mode to active mode when user's traffic exists. As the results of the simulation using real traffic, our algorithm saves the energy consumption from 14% to 49% as compared with existing method.

  • Bruxism is nonfunctional jaw movement that includes clenching, grinding and gnashing of teeth. It usually occurs during sleep, but with functional abnormality of brain, it can be seen during consciousness. Oromandibular dystonia (OMD) can involve the masticatory, lower facial, and tongue muscles and may result in trismus, bruxism, involuntary jaw opening or closure, and involuntary tongue movement. Its prevalence in the general population is 21%, but its incidence after brain injury is unknown, Untreated, bruxism and OMD cause masseter hypertrophy, headache, temporomandibular joint destruction and total dental wear. We report a case of successful treatment of bruxism and OMD after brain injury treated with botulinum toxin A and occlusal appliance. The patient was a 59-year-old man with operation history of frontal craniotomy and removal of malformed vessel secondary to cerebral arteriovenous malfomation. We injected with a total 60 units of botulinum toxin A each masseteric muscle and took impression for occlusal appliance fabrication under general anesthesia. On follow up 2 weeks and 2 months, the patient remained almost free of bruxism. We propose that botulinum toxin A and occlusal appliances be considered as a treatment for bruxism and OMD after brain injury.

  • Chronic fatigue syndrome(CFS) is a complex, debilitating disorder characterized by at least 6 months of severe persistent of relapsing fatigue and a group of characteristic but nonspecific symptoms. Many researchers have proposed that CFS has a specific cause. However currently no evidence exists that proves either a specific cause of CFS. And there is no diagnostic test for CFS. The diagnosis of chronic fatigue syndrome is based on the patient's history, excluding other illnesses In the absence of consistent biological markers, the diagnosis of CFS arises from operational criteria that do not afford validity. The prognosis is poor and often disability and impairment of daily function and performance are prolonged. A limited understanding of the CFS has complicated the management of this disorder. Therefore, treatment of CFS may be variable and should be tailored to each patient. Therapy should include exercise, diet, good sleep hygiene, antidepressants, and other medications, depending on the patient's presentation. Regular follow-up is key to continue to exclude other medical problems and provide support for patients. Chronic fatigue syndrome is a challenging illness to manage and requires a team approach of caring providers. For the majority of patients this is a chronic illness with the goals of therapy being to improve functional status and to prevent disability. Further understanding of the etiology and pathogenesis of this illness should lead to better specific therapy.

  • Some species of traditional herbal medicine has a history of use, most traditional natural herbs have been used for various diseases such as diabetes, hypertension, and obesity. Among them, Passiflora incarnata L. is a traditional natural medicine, flowers as well as berries, roots, and leaves have been used as a medicine. It has been used as a natural medicine for the treatment of insomnia and anxiety for a longtime in Europe, and it has been used primarily for sedation tea in North America. Moreover, Passiflora incarnata L. is widely used anti-asthmatic, analgesic and sedation in Brazil. In other words, Passiflora incarnata L. has been used to treat a sedative, dysmenorrhea, insomnia, cancer, etc. in many countries. Present review of the plants showed a wide range of pharmacological activity in anxiolytic relax the clinical disease, such as anti-inflammatory, anxiety and antioxidant. In addition, Passiflora incarnata L. affects menopause symptoms such as vasomotor symptoms, insomnia, and depression. This review aims to provide the latest information on specific functional components of Passiflora incarnata L. especially the results of clinical trials will provide new insights into opportunities for the future development of natural medicines and doors will be used for purposes of analysis.

Insomnia Treated with Baekho-tang Based on the Shanghanlun Disease Pattern Identification Diagnostic System (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 백호탕(白虎湯) 투여 후 호전된 불면증 증례 1례)

  • Objective: This study aimed to report the improvement of a patient with chronic insomnia who was treated with Baekho-tang based on the Shanghanlun disease pattern identification diagnostic system (DPIDS). Methods: The patient's symptoms were checked on days 19, 37, and 52 after taking Baekho-tang. Chronic insomnia was evaluated by the degree of insomnia, hours spent asleep, and the number of times the patient woke up during the night, which were all assessed by a visual analog scale (VAS) with the Leeds Sleep Evaluation Questionnaire. In addition, new clinical implications for the interpretation of the 350th Shanghanlun provision were reviewed. Results: After administering Baekho-tang for 52 days, based on the 350th Shanghanlun provision according to the DPIDS, the VAS score decreased from 10 to 1. Conclusions: The patient recovered from chronic insomnia. This case report suggests that the words "滑," "厥," and "裏" in the 350th provision of Shanghanlun mean physical and psychological causes of insomnia. In addition, Baekho-tang had a therapeutic effect in improving the chief complaint of the patient. Therefore, the interpretation of the 350th Shanghanlun provision requires a new definition that includes the patient's lifestyle, family conflict, and past history of the patient as a cause of chronic insomnia.
