Objective: The purpose of this study is to see whether pain in cancer patients is decreased by Eriobotryae Folium Fomentation. Methods: We examined the change in pain among six cancer patients, prescribing Eriobotryae Folium Fomentation. As for evaluating methods, we used the case history of pain, the direct evaluation of pain, the indirect evaluation of pain. The direct evaluation of pain is subdivided by the language expression, the non-language expression, and Visual analog scale(VAS). The indirect evaluation of pain is subdivided by the condition of walking, mood and the condition of sleep. Results: After two weeks, any remarkable pain decrease is not observed in both of the direct evaluation and indirect evaluation.
A small number of viable tuberculosis bacilli can reside in an individual with latent tuberculosis infection (LTBI) without obvious clinical symptoms or abnormal chest radiographs. Diagnosis and treatment for LTBI are important for tuberculosis (TB) control in public and private health, especially in high-risk populations. The updated 2014 Korean guidelines for TB recommend that tuberculin skin tests, interferon-gamma release assays, or a combination of the two can be used for LTBI diagnosis according to age and immune status of the host as well as TB contact history. The regimens for LTBI treatment include isoniazid, rifampicin, or isoniazid/rifampicin. However, results of drug susceptibility test from the index case must be considered in selecting the appropriate drug for recent contacts. Standardized LTBI diagnosis and treatment based on the new 2014 guidelines will contribute to the effective TB control in Korea as well as to the establishment of updated guidelines.
Personalized Patient centered nursing care is increasingly difficult to achieve despite the fact that it remains one of our consistent goals. So, we must find away to individualized nursing care. One means, to achieve this is by use of the nursing history form, which has been developed to help the nurse make maximum use of her limited time with the patient, by obtaining systematically the information needed to plan his nursing care. The nursing history form can be used to collect data about individual nursing needs but also it lends itself to the collection of epidemiological data relevant to the needs of patient population. So this study was undertaken in an attempt to describe the general characteristics of the population studied, to find out their perceptions and expectations related to their illness and hospitalization, to find out specific basic needs and to examine the relationship between the patients nursing needs and demographic characteristics through the responses to the nursing history questions. The study population defined and selected was all the patients (70) who were admitted to Yonsei University Hospital from October 1 - 15, 1975. The direct interview method was used and the data were categorized by the investigator, according to the nature of responses to each question and were subjected to the percentile and the chi- square tests. The findings can be summarized as follows 11. General characteristics of the study population ; The population was made up mainly of urban patients who were highly educated. The age was equally distributed. 2. Patients, perceptions and expectations related to illness and hospitalization ; 88.6% of the patients knew about the reason for hospitalization but 64. 5% could state symptoms only. 37.5% recognized the cause of illness. Approximately three fourth of the patients expressed on expectation for early recovery. 60.0% of the patient responses indicated, that they expected less than 10 days of hospitalization. Of the total responses regarding hospitalization, 45.7% were negative about the post -hospitalization expectation, 45.7% expected to return to work. As a result of these findings, we can see that there is a great educational need, a psychological need and environmental need for adaptation to the hospital and a socio- economic need for the post - hospitalization period. 3. Specific basic needs : The physical needs include the problem of getting sufficient sleep (50.0%), difficulty in food intake(47.1%), problems with hospital diet (47.0%), abnormal condition of the skin(44.3%), problems with bowel elimination(35.7%), assistance with bathing(35.7%), pain(30.0%), difficulty in walking(30.0%) , difficulty in seeing(30.0%) problems in urinary elimination(20.0%) , and difficulty in hearing(10.0%), 4. Nursing needs and epidemiological characteristics ; Age distribution was related to the rendition of the mouth but no significant differences were observed statistically with the patients responses to the other nursing history questions according to the epidemiological variables of age, sex, education and residence.
For this study, the Chuna Manual Relaxation Therapy, a therapeutic procedure in Oriental Medicine was carried out on patients suffering from insomnia following hospitalization. If a patient with no prior medical history in insomnia should suffer from insomniac symptoms, the following procedure was carried out on thos patients showing 15 points on the Insomnia Severity Index starting on the day when the patient first complains of symptoms associated with insomnia, stages 1, 2 & 3 of the 'JS Cervical Shinyoun Therapy' in accordance with the Korea Chuna medical Index standard was carried out on the patient's bed from 8pm to 9pm everyday for 1 week. After the 1 week procedure, a medical examination by interview was carried out the following day, and the Insomnia Severity Index measured again. (1 being when the patient's state of sleep is at its worst, and 10 being when it is the most satisfactory. In case of 5 patients, Insomnia is disappeared, and the quality of sleep improved. In regards to insomnia suffered after hospitalization, Stage 1,2 and 3 of "JS Cervical Shinyoun Therapy" for relaxing tension showed effective results to the patient.
The clinical study was carried out the 33 patients with insomnia who were treated in Daejeon University Oriental Hospital from 17 March 1997 to 12 May 2001. The results were summarized as follows. 1. The ratio of male and female was 10:23 and sleep initiation insomnia and sleep maintenance insomnia were the highest frequence(97%), the beginning of insomnia was frequent at 30s(27.3%). 2. Living accident by mental attack was the most inducing factor and many patients came to our hospital by way of west-neuropsychiatry(55.6%). 3. In admission period most of the patients were within 15 days(78.8%) and Liver-Qi- depression was the main cause and in classification of Four Human coporeal constitution the number of patients Sho-Eum-In(少陰人) was remarked mostly and in distribution of the prescription drugs of growing heart and warming gall bladder such as GUIBIONDAMTANG(歸脾溫膽湯) and ONDAMTANGGAMI (溫膽湯加味) were many, 4. Relatively most patients were well treated(69.7%) and insomnia was mainly caused by Anxiety Disorder and Depression and Hwabyoung and in the age distribution the highest frequence was 30s but we know insomnia appeared at all ages. 5. In distribution of the period of the clinical history was various and within 15 days were all improved and the treatments group of drug and acupuncture and aid treatments etc. and hypnotics was more improved than not used hypnotics treatments group but using hypnotics was temporal. Purpose: The aim of this study was to investigate the current state of antidepressant prescriptions in breast cancer patients and factors affecting the prescription of antidepressants. Methods: This study targeted female breast cancer patients who were prescribed antidepressants by a psychiatrist at least once between August 2010 and July 2011 at the Asan Medical Center in Seoul. The prescription history of each study subject was investigated to analyze the current state of antidepressant prescriptions in breast cancer patients. Results: The analysis of the prescription histories of 136 subjects in the antidepressant group determined that escitalopram, mirtazapine, and trazodone were the three most commonly prescribed medications with an average of 1.54 antidepressants prescribed per patient. A logistic regression analysis showed a statistically significant increase in antidepressant prescriptions in patients who were divorced or widowed, had sleep disturbances, or had undergone oncologic surgery for the breast cancer (p<0.050). In contrast, the prescription rate was lower for patients with tumour sizes greater than 50 mm (p<0.050). Conclusion: The sociodemographic factor of marital status, clinical factors of sleep disorders and tumour size, and a treatment-specific factor of the use of surgical therapy were identified as affecting the prescription of antidepressants in female breast cancer patients.
Objectives: The aim of this study was to evaluate and quantify the risk of both individual and combined health behaviors on premature mortality in middle aged men in Korea. Methods: In total, 14 533 male subjects 40 to 59 years of age were recruited. At enrollment, subjects completed a baseline questionnaire, which included information about socio-demographic factors, past medical history, and life style. During the follow-up period from 1993 to 2008, we identified 990 all-cause premature deaths using national death certificates. A Cox proportional hazard regression model was used to estimate the hazard ratio (HR) of each health risk behavior, which included smoking, drinking, physical inactivity, and lack of sleep hours. Using the Cox model, each health behavior was assigned a risk score proportional to its regression coefficient value. Health risk scores were calculated for each patient and the HR of all-cause premature mortality was calculated according to risk score. Results: Current smoking and drinking, high body mass index, less sleep hours, and less education were significantly associated with all-cause premature mortality, while regular exercise was associated with a reduced risk. When combined by health risk score, there was a strong trend for increased mortality risk with increased score (p-trend < 0.01). When compared with the 1-9 score group, HRs of the 10-19 and 20-28 score groups were 2.58 (95% confidence intervals [CIs], 2.19 to 3.03) and 7.09 (95% CIs, 5.21 to 9.66), respectively. Conclusions: Modifiable risk factors, such as smoking, drinking, and regular exercise, have considerable impact on premature mortality and should be assessed in combination.
Objectives : This study was conducted to reveal the relationship between multiple factors of traditional Korean Medicine diagnosis and consider the further probabilities of treating people with physical and mental problems not defined as diseases, which is called 'Mibyeong' in traditional Korean Medicine. Methods : 40 healthy participants were included in the observational clinical trial. The participants were asked to complete health questionnaires (e.g. State-Trait Anxiety Inventory, Pittsburgh Sleep Quality Index, Stress Response Inventory) and they went through a traditional diagnosis process, including four stages of diagnosis (looking, listening/smelling, inquiring, and pulse taking), by a Korean Medicine doctor. Both the Korean Medicine doctor and an artery tonometry device performed the pulse diagnosis. Results : Although all participants were healthy people with no history of disease, more than half of participants had a problem related with severe level of fatigue (n=19), sleep disturbance (n=26) and stress (n=27) status according to the related questionnaires. Participants diagnosed with phlegm syndrome by the Korean Medicine doctor showed significantly greater score in phlegm pattern questionnaires than participants who were not. However, there was little agreement between the doctor's pulse diagnosis and radial artery tonometry results. Conclusions : We conducted a pulse diagnosis and measured health-related information along with the traditional Korean Medicine diagnose procedure, including four stages of diagnosis, and we found a linkage between diagnosis of phlegm and the phlegm pattern questionnaire score. The results suggest that a number of healthy participants, with no disease diagnosed, have Mibyoung symptoms which need further clinical management. Thus, we suggest that Mibyoung management programs based on qualified diagnosis tools and traditional Korean medicine diagnosis procedures be developed, and that future research using various diagnostic tools be carried out on a large population.
Objectives : This study was undertaken to investigate the prevalence of depressive symptoms and their related factors among a group of medical students. Method : A total of 874 (529 male and 345 female) medical students were randomly selected to participate in a survey. Depressive symptoms, satisfaction with life, health behavior including alcohol use, stress, sleep disturbance and happiness were collected using self-reported questionnaires. Results : The prevalence of depressive symptoms was 10.8%. In stepwise multiple regression analysis, lower satisfaction of life, daytime dysfunction due to sleepiness, history of suicidal attempt, stress, sleep disturbance were found to be significant relating factors of depressive symptoms. In moderated regression analysis, the result showed that the impact of life stress were moderated by satisfaction of life on depressive symptoms, but the moderating effect of happiness was not significant. Conclusion : This study showed considerably high prevalence of depressive symptoms and BDI score in medical students. The findings suggest that early detection of depressive symptoms and intensive mental health promotion program is needed in order to improve medical student's mental health status.
연구배경 : 정신분열증 환자들이 정보처리 과정의 문제가 있을 것이라는 가설을 지지해 주는 사건관련유발 전위인 P3의 이상을 보고하는 연구들이 있어 저자들은 정신분열증 환자들에게 청각사건관련유발 전위를 실시하여 P3의 이상을 확인하고자 하였다. 또한, 음성 및 양성 정신분열증환자에 P3의 차이가 있는지 비교하였으며, 가족력이나 전기충격경련요법, 밖에 임상양상과 P3와의 연관성을 알아보기 위하여 본 연구를 실시하였다. 방법 : DSM-III-R 진단기준에 맞는 정신분열증 환자 54명 (남자 31, 여자 23) 과 그 대조군 75 명 (남자 33, 여자 42) 을 대상으로 oddball paradigm 을 이용한 사건관련유발전위를 실시하여 P3의 잠복기와 진폭을 측정하였으며, 정신분열증의 인지기능이상을 알아볼 수 있는 프랑크푸르트 임상검사를 같이 실시하였다. 결과 : 정신분열증 환자의 P3 잠복기는 그 대조군에 비해 의미 있게 지연되어 있었으며, 또한 모든 진폭도 대조군에 비해 의미 있게 감소되어 있었다. 정신분열증 환자군에서 프랑크푸르트 임상검사의 10가지 임상증상 모두에서 통계적으로 유의하게 높은 점수를 보였다. PANSS의 종합점수에 따라 음성과 양성으로 나누어 비교한 결과, P3의 잠복기 및 진폭에서 의미 있는 차이가 발견되지 않았다. 또한, 정신과질환의 가족력의 유무에 따라, 전기 충격경련요법 유무에 따라 비교한 결과 의미 있는 차이를 발견할 수 없었다. 환자군에서 프랑크푸르트 임상검사의 임상증상, 그밖에 임상특성과 P3의 잠복기와 진폭 사이에 연관성을 발견할 수 없었다. 결론 : 본 연구의 결과로 정신분열증 환자들에서 선택적 주의집중 과정의 이상이 있을 것이고 추측할 수 있었으며, 임상양상이나 가족력 유무와 P3사이에 연관성이 없다는 것을 시사해 주었다.
