Treatment of obstructive sleep apnea in children

  • Obstructive sleep apnea (OSA) in children is a frequent disease for which optimal diagnostic methods are still being defined. Treatment of OSA in children should include providing space, improving craniofacial growth, resolving all symptoms, and preventing the development of the disease in the adult years. Adenotonsillectomy (T&A) has been the treatment of choice and thought to solve young patient's OSA problem, which is not the case for most adults. Recent reports showed success rates that vary from 27.2% to 82.9%. Children snoring regularly generally have a narrow maxilla compared to children who do not snore. The impairment of nasal breathing with increased nasal resistance has a well-documented negative impact on early childhood maxilla-mandibular development, making the upper airway smaller and might lead to adult OSA. Surgery in young children should be performed as early as possible to prevent the resulting morphologic changes and neurobehavioral, cardiovascular, endocrine, and metabolic complications. Close postoperative follow-up to monitor for residual disease is equally important. As the proportion of obese children has been increasing recently, parents should be informed about the weight gain after T&A. Multidisciplinary evaluation of the anatomic abnormalities in children with OSA leads to better overall treatment outcome.

신체화를 동반한 외상후 스트레스 장애 환자의 근막이완술 적용이 신체화, 우울증 및 통증에 미치는 영향 : 사례 연구 (The Effects the Application of MyoFascial Release of Depression, Anxiety, Sleep Disturbance, and Headache in Post Traumatic Stress Disorder Patients with Somatization: Case Study)

  • Background: The purpose of this study is to examine the effects of myofascial release technique (MFR) on psychological and physical symptom in somatization with post traumatic stress disorder (PTSD). Based on this, proceed to present an effective physical treatments. Methods: In this study, three subjects were applied ABA design for a total of 12 weeks. Intervention was performed three times per week, and only MFR for 6 weeks was applied to the patient for 60 minutes. General physical therapy consisted of a total of 60 minutes including hot pack, electric therapy, and ultrasound. In this study, we measured three times in the second baseline stage at the initial evaluation before the commencement of intervention, somatization, depression, anxiety, sleep disorder, and pain after 6 weeks and 12 weeks. Results: In this study, the application of MFR showed significant differences in somatization symptoms, sleep disturbance, and headache. There was no significant difference in depression and anxiety. Conclusions: As a result, the application of MFR in PTSD patients with somatization can be suggested as a useful intervention to resolve the psychosomatic problem.

Effects of sleep deprivation on coronary heart disease

  • The presence of artificial light enables humans to be active 24 h a day. Many people across the globe live in a social culture that encourages staying up late to meet the demands of various activities, such as work and school. Sleep deprivation (SD) is a severe health problem in modern society. Meanwhile, as with cardiometabolic disease, there was an obvious tendency that coronary heart disease (CHD) to become a global epidemic chronic disease. Specifically, SD can significantly increase the morbidity and mortality of CHD. However, the underlying mechanisms responsible for the effects of SD on CHD are multilayered and complex. Inflammatory response, lipid metabolism, oxidative stress, and endothelial function all contribute to cardiovascular lesions. In this review, the effects of SD on CHD development are summarized, and SD-related pathogenesis of coronary artery lesions is discussed. In general, early assessment of SD played a vital role in preventing the harmful consequences of CHD.

일 도시의 초등학교 학생의 수면습관과 행동, 정서, 주의력, 학습과의 관계 (Differences in Sleep Patterns are Related to Behavior, Emotional Problems, Attention and Academic Performance in Elementary School Students of a South Korean Metropolitan City)

  • Objectives: The aim of this study was to investigate the sleep patterns of South Korean elementary school children and whether the differences in sleep patterns were related to behavior, emotional problems, attention and academic performance. Method: This study included a community sample of 268 boys and girls from fourth-, fifth- and sixth-grade classes in a South Korean metropolitan city from November to December 2010. The primary caregivers completed a questionnaire that included information on demographic characteristics, as well as the Child's Sleep Habit Questionnaire (CSHQ), the Korean version of Child Behavior Checklist (K-CBCL), the Korean version of the Learning Disability Evaluation Scale (K-LDES), the Korean version of ADHD Rating Scale (K-ARS) and the Disruptive Behavior Disorder Scale (DBDS). We conducted analyses on the CSHQ individual items, between the subscales, on the total scores and on the K-CBCL, the K-LEDS, the K-ARS and the DBDS. Results: Based on the findings from the CHSQ, the subjects had significantly higher scores for bedtime resistance ($9.18{\pm}2.17$), delayed sleep onset ($1.32{\pm}0.62$), the sleep duration ($4.19{\pm}1.52$) and daytime sleepiness ($14.10{\pm}3.55$) than the scores from the previous reports on children from western countries. The total CHSQ score showed positive correlations to all subscales of the K-CBCL : withdrawn (r=0.24, p<.005), somatic complaint (r=0.24, p<.005) and anxious/depressive (r=0.38, p<.005). Bedtime resistance was associated with oppositional defiant disorder (r=0.15, p<.05) and a positive correlation was demonstrated between sleep anxiety and the oppositional defiant disorder score (r=0.13, p<.05), night waking and the conduct disorder score (r=0.16, p<.05). Delayed sleep onset was related with low performance on the K-LDES with respect to thinking (r=-0.17, p<.05) and mathematical calculation (r=-0.17, p<.05). Conclusion: The results of this study reconfirm Korean children's problematic sleep patterns. Taken together the results provide that the reduced sleep duration and disruption of sleep pattern can have a significant impact on emotion, behavior, performance of learning in children. Further studies concerning more diverse psychosocial factors affecting sleep pattern will be helpful to understanding of the sleep health in Korean children.

염증 반응과 수면 장애 (Inflammation and Insufficient or Disordered Sleep)

  • 수면현상은 인간의 삶의 1/3을 차지하는 중요한 생리적 현상으로 인간의 수면은 신체의 기능을 유지하기 위한 중요한 생리적 기능뿐 아니라 수면을 통한 휴식을 통하여 신체 건강을 유지하고 성장을 유지하는 중요한 역할을 한다. 최근의 급격한 산업화와 정보화로 인하여 급격한 사회 생활구조의 변화를 유발하여 성인뿐 아니라 소아에서도 평균수면 시간의 감소와 다양한 스트레스 환경으로 인한 수면의 질적 저하로 인한 다양한 수면 문제가 사회적 문제로 대두되고 있다. 단순히 수면 부족 또는 수면의 질적 저하뿐 아니라 수면 장애, 특히 폐쇄성 수면 무호흡증은 심혈관 질환뿐 아니라 비만, 대사증후군, 심혈관 질환, 뇌혈관 질환 등 의 다양한 만성 질병의 발생과 깊은 연관성이 있는 것으로 보고하고 있다. 특히 이러한 만성질환을 유발하는 핵심기전으로 염증 반응이 관여하고 있는 것으로 알려져 있으며, 따라서 수면 현상과 다양한 수면 장애에 대한 관심이 급격히 증가하고 있는 실정이다. 비록 수면 무호흡증으로 인한 이러한 질병의 이환에 대한 기전은 다양한 요인이 복합적으로 작용하는 것으로 알려져 있지만 핵심 기전으로 알려진 염증반응의 활성화가 병태 생리학적으로 중요한 역할을 하는 것으로 추측하고 있다. 따라서 최근 수면 무호흡증을 낮은 수준의 만성 염증 질환으로 인식되고 있으며, 더불어 비만과 수면무호흡증으로 인한 연관된 질병의 이환을 더욱 증가시키는 것으로 보고되고 있다. 따라서 본 종설에는 수면의 질적 또는 양적 저하뿐 아니라 폐쇄성 수면 무호흡증으로 인한 염증성 반응의 활성화가 인간의 신체 면역 과정 미치는 영향을 중심으로 최근의 연구 결과 및 수면 무호흡증과 비만의 잠재적 상호작용에 대하여 서술하고자 한다

A Dynamic Zigbee Protocol for Reducing Power Consumption

  • One of the obstacles preventing the Zigbee protocol from being widely used is the excessive power consumption of Zigbee devices in low bandwidth and low power requirement applications. This paper proposes a protocol that resolves the power efficiency problem. The proposed protocol reduces the power consumption of Zigbee devices in beacon-enabled networks without increasing the time taken by Zigbee peripherals to communicate with their coordinator. The proposed protocol utilizes a beacon control mechanism called a "sleep pattern," which is updated based on the previous event statistics. It determines exactly when Zigbee peripherals wake up or sleep. A simulation of the proposed protocol using realistic parameters and an experiment using commercial products yielded similar results, demonstrating that the protocol may be a solution to reduce the power consumption of Zigbee devices.

  • The rural area is activated and if it does to sleep, that versatility the reading is necessary from the side which is various it sees. Only expansion or improvement of namely base facility it will have and there is not a possibility becoming the basic activation countermeasure. With this with the instance investigation against the traditional market activation which is developed the Gamgok from same background has both the quality of the rural and the city in the center it joins in and the far it will be able to complement a problem point it presents. Also it presents the activation plan which considers the regional quality which from the research which it sees relates with a traditional market to sleep and it does.

여성 문제음주자의 건강행태와 건강관련 삶의 질이 우울에 미치는 영향 (The Influence of Health Behaviors and Health related Quality of Life on Depression among Korean Female Problem Drinker)

  • 본 연구의 목적은 한국 여성 문제음주자의 음주실태, 건강행태, 건강관련 삶의 질과 우울의 정도를 파악하고, 건강행태와 건강관련 삶의 질이 우울에 미치는 영향을 파악하는 것이다. 본 연구는 보건복지부와 질병관리본부에 의해 2010년 1월부터 2012년 12월까지 시행된 제 5기 국민건강영양조사의 원시자료를 사용하였다. 본 연구의 대상자는 AUDIT(Alcohol Use Disorder Identification Test)에 의해 8점 이상으로 분류된 여성 문제음주자 328명이었으며, 자료는 IBM SPSS 21.0 프로그램으로 복합표본 분석하였다. 연구결과, 높은 수준의 스트레스를 인지하는 대상자와 건강관련 삶의 질이 낮은 대상자는 우울을 경험할 위험이 증가하였다. 또한 1-4시간의 수면을 취하는 대상자는 8시간 이상 수면을 취하는 대상자에 비해 우울을 경험할 위험이 증가하는 것으로 나타났다. 그러므로 한국 여성 문제음주자의 우울 및 정신건강 향상을 위해 이에 영향을 주는 요인인 스트레스, 수면, 건강관련 삶의 질 향상에 관심을 가져야 할 것이다. 특히 위험도가 높았던, 높은 수준의 스트레스를 인지하는 여성 문제음주자의 우울 관리를 위한 치료적 지지체계가 계획되어야 할 것이다.

비만, 폐쇄성 수면무호흡증과 대사장애 (Obesity, Obstructive Sleep Apnea, and Metabolic Dysfunction)

  • 수면은 필수적인 생리적 기능일 뿐만 아니라 인간의 성장, 성숙 및 전반적인 건강을 증진시키는 데 중요한 역할을 한다. 수면과 수면 장애가 대사성 질환에 미치는 영향에 대한 관심이 높아지고 있다. 폐쇄성 수면무호흡증은 일반적인 건강 문제이며, 지난 10년 동안 비만율의 증가로 인해 더 두드러진 대사 질환과 함께 폐쇄성 수면무호흡증의 유병률이 현저하게 증가했다. 폐쇄성 수면무호흡증에 의한 대사성 질환을 유발하는 근본적인 메커니즘은 다인성일 가능성이 높으며, 완전히 밝혀지지 않고 있지만, 염증과 산화 스트레스의 활성화와 식욕 조절 호르몬의 조절 장애는 폐쇄성 수면무호흡증 환자에게 나타나는 대사 기능 장애와 비만의 중요한 병리 생리학적 성분으로 나타났다. 본 연구에서는 폐쇄성 수면무호흡증과 대사질환의 연관성에 대한 연구 현황과 폐쇄성 수면무호흡증이 이러한 질병을 유발하는 병리생리학적 메커니즘에 대해 검토하고자 한다. 이를 통해 폐쇄성 수면무호흡증과 비만, 그리고 폐쇄성 수면무호흡증과 대사 기능 장애 사이의 잠재적인 상호작용을 이해할 수 있다.

A Learning Automata-based Algorithm for Area Coverage Problem in Directional Sensor Networks

  • Coverage problem is a research hot spot in directional sensor networks (DSNs). However, the major problem affecting the performance of the current coverage-enhancing strategies is that they just optimize the coverage of networks, but ignore the maximum number of sleep sensors to save more energy. Aiming to find an approximate optimal method that can cover maximum area with minimum number of active sensors, in this paper, a new scheduling algorithm based on learning automata is proposed to enhance area coverage, and shut off redundant sensors as many as possible. To evaluate the performance of the proposed algorithm, several experiments are conducted. Simulation results indicate that the proposed algorithm have effective performance in terms of coverage enhancement and sleeping sensors compared to the existing algorithms.
