The Association between Serotonin Reuptake Inhibitors and Obstructive Sleep Apnea in People with Epilepsy-A Retrospective Analysis

  • Objectives: Obstructive sleep apnea (OSA) is common in people with epilepsy (PWE), and confers medical and seizure-related consequences when untreated. Positive airway pressure, the gold-standard for OSA management, is limited by tolerability. As serotonin is involved respiratory control and amelioration of seizure-induced respiratory events, this study aims to determine whether serotonin reuptake inhibitors (SRIs) may represent a potential therapeutic option. Methods: A retrospective study of 100 PWE and OSA ${\geq}18$ years of age was conducted. The primary outcome measure was OSA severity as function of SRI use, with rapid eye movement (REM)-related OSA as a secondary outcome. Results: Older age and depression were more common in those taking an SRI. There was no association between SRIs and OSA severity. However, the SRI group was less likely to have REM-related OSA. Conclusions: In PWE and OSA, SRI use is associated with reduced risk of REM-related OSA, and may represent a potential management strategy.

소아수면과다증과 수면무호흡 (Childhood Hypersomnia and Sleep Apnea Syndrome)

  • Natural sleep pattern and its physiology in childhood are much different from those in adulthood. Several aspects of clinical evaluation for sleepiness in childhood are more difficult than in adulthood. These difficulties are due to several factors. First, excessive sleepiness in childhood do not always develop functional impairments. Second, objective test such as MSLT may not be reliable since it is hard to be certain that the child understand instructions. Third, sleepiness in children is often obscured by irritability. paradoxical hyperactivity, or behavioral disturbances. Anseguently, careful clinical evaluation is needed for the sleepy children. Usual causes of sleepiness in children are the disorders that induce insufficient sleep such as sleep apnea syndrome, schedule disorder, underlying medical and psychiatric disorder, and so forth. After excluding such factors, we can diagnose the hypersomnic disorders such as narcolepsy, Kleine-Levin syndrome, and idiopathic central nervous system hypersomnia. Among the variety of those causes of sleepiness, I reviewed the clinical difference of narcolepsy and obstructive sleep apnea syndrome in childhood compared with in adulthood. Recognition of the childhood narcolepsy is difficult because even severely sleepy children often do not develop pathognomic cataplexy and associated REM phenomena until much later. Since childhood narcolepsy give srise to many psychological, academical problem. Practicers should be concerned about these aspects. Childhood obstructive sleep apnea syndrome is different from adult obstructive sleep apnea syndrome too. Several aspects such as pathophysiology. clinical feature, diagnostic criteria, complication, management, and prognosis differ from those in the adult syndrome. An important feature of childhood obstructive sleep apnea syndrome is the variety of severe complications such as behavioral disorders, cognitive impairment, cardiovascular symptoms, developmental delay, and ever death. Fortunately, surgical interventions like adenotosillectomy or UPPP are more effective for Childhood OSA than adult form. CPAP is a "safe, effective, and well-tolerated" treatment modality too. So if early detection and proper management of childhood OSA were done, the severe complication would be prevented or ever cured.

Efficacy of botulinum toxin in the management of temporomandibular myofascial pain and sleep bruxism

  • Objectives: This study sought to evaluate the efficacy of injecting botulinum toxin into the masseter and temporal muscles in patients with temporomandibular myofascial pain and sleep bruxism. Materials and Methods: The study was conducted based on a clinical record review of 44 patients (36 females and eight males; mean age, 35.70±12.66 years). Patients who underwent the injection of botulinum toxin into the masseter and temporal muscles for the management of temporomandibular myofascial pain and sleep bruxism were included in the study. Patients were diagnosed based on the Diagnostic Criteria for Temporomandibular Disorders. Sleep bruxism was diagnosed according to the criteria defined by the American Academy of Sleep Medicine. The values of the visual analogue scale (VAS) and range of jaw motion, including unassisted maximum mouth opening (MMO), protrusion, and right and left laterotrusion, were observed preoperatively and postoperatively at one-, three-, and six-month follow-up visits. Results: MMO, movements of the right and the left laterotrusion, and protrusion increased significantly (P<0.05), while VAS ratings decreased significantly at the three follow-up points relative to baseline values (P<0.05). Conclusion: Botulinum toxin is an effective treatment for patients with temporomandibular myofascial pain and sleep bruxism.

임베디드 응용프로그램의 동작 특성을 이용한 에너지 인식 스케쥴링 기법 (Energy-Aware Scheduling Technique to Exploit Operational Characteristic of Embedded Applications)

  • 효율적인 전력관리는 모바일 휴대용 제품 시장에서 중요한 경쟁력 중의 하나이다. 본 논문은 전력을 미리 예측하는 방법으로 실행중인 응용프로그램의 sleep 상태를 이용하는 Energy-Aware Scheduling policy(EASY)를 제안한다. 기존 대기 모드에서 전력소모를 줄이는 방법과의 차이점은 응용프로그램들이 얼마나 오랫동안 스케쥴러에서 sleep 상태에 있었는지를 검사하여 각 응용프로그램들의 동작 상태를 결정한다. EASY 기법은 측정된 sleep 시간을 기준으로 현재의 작업량에 맞는 적정한 CPU 클럭 주파수를 정하고, 다음 작업량의 적정한 CPU 클럭 주파수를 예측함으로서 동작 상태에서 전력 소모를 줄일 수 있다. 실험 결과 기존의 대기모드를 이용한 전력관리 기법과 비교하여 평균적으로 10-30%의 전력소모를 줄임으로써 제안된 기법의 우수성을 입증한다.

교대근무 간호사의 건강증진행위가 수면의 질에 미치는 영향 (Influence of Health-Promoting Behaviors on Quality of Sleep in Rotating-Shift Nurses)

  • Purpose: The purpose of this study was to identify quality of sleep and health-promoting behaviors in rotating-shift nurses and to explore the influence of health promoting behaviors on quality of sleep. Method: Participants were 161 staff nurses working in an irregular three shift system in one of three general hospitals located in Kyungpook province. Data were collected using the Pittsburgh Sleep Quality Index and the Health Promoting Lifestyle Profile-IIand were analyzed using hierarchical multiple regression analysis. Results: The majority of the participants had very poor quality of sleep and performed a low level of health promoting behaviors. In terms of health promoting behaviors, nutrition and stress management significantly influenced quality of sleep of these nurses. Conclusion: Results indicate the importance of encouraging rotating-shift nurses to enhance their health promoting behaviors in order to improve quality of sleep.

서울시 및 수도권 거주자의 침실환경과 침실 사용자그룹 세분화에 관한 연구 (A Study on Sleep Environment in Bedroom and Segmentation of Sleepers in Seoul and Metropolitan Area)

  • Sleep has big influence on our life, for example productivity, character formation of babies, mortality and the people who suffered from sleep problem are also diverse. Therefore, indoor environmental supports for good sleep, might be different with the situation and need of respective person. The objectives of this study are: 1) to investigate the current bedroom condition, sleep problem, and life pattern of Korean. 2) to classify Korean into several groups and explain their characteristics relevant to sleep, in order to understand Korean's need for good sleep. The questionnaire was sent out to Seoul and its environs in Jan.$\sim$Feb. 2008, and total of 702 questionnaires were collected. The research on the bedroom condition was conducted around perceived influence of environmental factors, opening and closing of the door, cooling and heating strategy, clo value during sleep, bedding type, lighting, sharing bedroom. People are divided into 6 groups by 8 factors, and 5 groups could be seen that they need some management for good sleep because of their sleep problem.

중환자실 환자의 수면에 영향을 미치는 요인: 체계적 고찰 (Influencing factors for Sleep Disturbance in the Intensive Care Unit Patients: A Systematic Review)

  • Purpose : Sleep disturbances in patients in the intensive care unit (ICU) are related to health problems after discharge. Therefore, active prevention and management are required. Hence, identification of the factors that affect sleep in patients who are critically ill is necessary. Methods : The PubMed, Cochrane Library, CINAHL, EMBASE, and Web of Science databases were searched. Selection criteria were observational and experimental studies that assessed sleep as an outcome, included adult patients admitted to the ICU, and published between November 2015 and April 2022. Results : A total of 21,136 articles were identified through search engines and manual searches, and 42 articles were selected. From these, 22 influencing factors and 11 interventions were identified. Individual factors included disease severity, age, pain, delirium, comorbidities, alcohol consumption, sex, sleep disturbance before hospitalization, chronic obstructive pulmonary disease (COPD), cardiovascular disease, and high diastolic blood pressure (DBP), low hemoglobin (Hb), and low respiratory rate (RR). Environmental factors included light level, noise level, and temperature. Furthermore, treatment-related factors included use of sedatives, melatonin administration, sleep management guidelines, ventilator application, nursing treatment, and length of ICU stay. Regarding sleep interventions, massage, eye mask and earplugs, quiet time and multicomponent protocols, aromatherapy, acupressure, sounds of the sea, adaptive intervention, circulation lighting, and single occupation in a room were identified. Conclusion : Based on these results, we propose the development and application of various interventions to improve sleep quality in patients who are critically ill.

건강한 임상전문가 육성을 위한 보건계열 대학생들의 수면 유형과 스트레스 수준 분석 (Promote of Healthy Clinical Expert for Analysis of Sleep Type and Stress Level on University students on Health Series)

  • Purpose . The purpose of this study was to investigate the stress level and sleep type on university students at health series. Method . This research conducts a survey of 250 university students on health series. The research tools were structured questionnaires. The questionnaire about stress was measured using psychological stress questionnaire in life stress and sleep type was measured using the Korean Translation of Composite Scale of Measuring Morningness-Evening. Result . In this study, university students 16.4% of the students were at warning. The morning sleep types were 55.2% in all health series university students but slee type was not in the most of sleep type. There were not correlation between the sleep type and stress level. Conclusion . It is seems to need to management of stress on university students at health series.
