스컬용융법에 의한 패각을 이용한 큐빅지르코니아 단결정 성장 (Cubic zirconia single crystal growth using shell by skull melting method)

  • 본 연구에서는 안정화제로, 패각으로부터 정제한 CaO를 사용하여 Skull melting법으로 큐빅 지르코니아를 성장시켰다. 꼬막 패각을 HCl을 0.1~1 mol%로 1차 처리한 후 수화반응을 거쳐 최적의 처리시간과 농도를 검토하였다. HCl을 1 mol%의 농도로 처리했을 때 가장 순도가 높았으며 수화반응시 $45^{\circ}C$의 온도로 24시간 동안 반응시킬 때 완전한 $Ca(OH)_2$ 를 얻을 수 있었다. $Ca(OH)_2$$1200^{\circ}C$에서 5시간 처리하였을 때 CaO의 순도가 가장 높게 측정되었다. 패각으로부터 얻어진 CaO를 $ZrO_2$에 첨가하여 함량을 10~30 mol% 로 변화시켜 Skull melting법으로 단결정을 성장시켰다. Skull melting법에 사용된 고주파 발진기의 주파수는 약 3.4 MHz이며 단결정의 하강속도는 시간당 3 mm로 4 cm 길이로 성장시켰다. 실험결과 CaO의 함량이 15 mol% 일 때 결정성이 가장 우수했다.

Haas 촬영법에서 선예한 영상 획득을 위한 연구 (A Study to Acquire Sharp Images in the Haas(Skull PA Axial Projection))

  • 본 연구는 머리뼈 후전축방향 촬영(Haas)에서 선예한 영상을 얻어 진단하고자 Skull -Phantom을 이용하여 자세 각도를 변경하여 영상을 획득한 후 대학병원 영상의학과에 근무한 방사선사 5명에게 주관적 영상평가한 결과 머리를 엎드린자세에서 머리 뒤쪽의 뒤통수점에서 4 cm 아래로 내리고 머리쪽을 향하여 25° 입사하여 촬영시에 뒤통수뼈와 큰구멍내에 투영된 안장등, 뒤침대돌기등 영상평가 점수가 20점으로 높았으며, 뒤통수점에서 아래로 8 cm 내려서 머리쪽으로 30° 입사 시 바위모뿔대칭의 음영, 바위능선과 시상봉합, 시웃자봉합 등 19점으로 높은 영상평가를 받았다. 또한, 점수 유의성을 검증하였으며, Cronbach Alpha 값이 0.789 신뢰도가 좋음을 평가 받았다. 영상의 관심영역(ROI)을 설정하여 신호대잡음비(SNR)를 산출한 결과 머리 뒤쪽 뒤통수점에서 4 cm 아래로 내리고 머리쪽으로 25° 입사 시 5.957로 가장 높으며, 뒤통수점에서 아래로 8 cm 내려서 입사 시는 머리쪽으로 30° 기울여 촬영하였을 때 6.430으로 가장 높았다. 연구결과에 의하면 머리 후전축방향 촬영시 뒤통수뼈와 안장, 뒤침대돌기의 선예한 영상을 얻기 위해서는 뒤통수점에서 아래로 4 cm 에서 머리쪽으로 25° 입사하여 촬영하고 바위모뿔대칭의 음영과 바위능선과 시상봉합, 시웃자봉합의 선예한 영상을 얻기 위해서는 뒤통수점에서 아래로 8 cm 내려서 머리쪽으로 30° 기울여 촬영한다면 임상에서 활용에 도움이 있으리라 사료된다.

하악과두 잘림과 경추의 하악지 겹침을 야기하는 파노라마방사선촬영 오류 (Experimental panoramic positioning errors for inducing condylar cutoff and superimposition of cervical vertebrae on the mandibular ramus)

  • Purpose: To measure the head tilting angle creating initial condylar cut-off and to find the head position inducing the superimposition of the cervical vertebrae over the mandibular ramus on panoramic radiograph. Materials and Methods: The panoramic radiographs were taken with Didactic skull on cervical spine model (Scientific GmbH, Hamburg, Germany) using Kodak 8000c Digital Panoramic radiography. For the inherent radiolucency of the plastic skull model, radiopaque 1 mm diameter lead wires were attached along the margin of the mandibular condyle, ramus, mandibular body, cervical vertebrae, and FH plane of the skull model. For measuring the head tilting angle creating the condylar head cutoff, panoramic radiographs were taken by tilting the FH plane downward in 5 degree increments. For finding the distance between transverse process of the third cervical vertebra and gonion inducing superimposition of cervical vertebrae on the mandibular ramus, panoramic radiographs were taken by decreasing the distance in 0.5 cm increments. Result and Conclusion: The condylar cutoff began to appear when the head of skull model was tilted downward by 15o. As the head tilting angle increasing, the condylar cutoff became more prominent. The superimposition of cervical vertebrae over the mandibular ramus began to appear when the distance between the gonion and third cervical vertebra was 1.0 cm. As the distance decreasing, the superimpostion became more prominent.

  • Recently, in order to satisfy the consumer's demand the life cycle and the lead-time of a product is to be shortened. It is thus important to reduce the time and cost in manufacturing trial products. Several techniques have been developed and successfully commercialized in the market RPM(Rapid Prototyping and Manufacturing). However, most commercial systems currently use resins or waxes as the raw materials. So, the limited mechanical strength for functional testing is regarded as an obstacle towards broader application of rapid prototyping techniques. To overcome this problems, high-speed machining technology is being investigated worldwide for rapid manufacturing and even for direct rapid tooling application. In this paper, some fundamental experiments and analyses are carried out to obtain the filling time, materials, method, and process parameters for HisRP process. HisRP is a combination process using high-speed machining technology with automatic filling. In filling process, Bi58-Sn alloy is chosen because of the properties of los-melting point, low coefficient of thermal expansion and enviromental friendship. Also the use of filling wire is of advantage in term of simple and flexible mechanism. Then the rapid manufacturing product, for example a skull, is machined for aluminum material by HisRP process with an automatic set-up device of 4-faces machining.

Development and validation of a clinical phantom reproducing various lesions for oral and maxillofacial radiology research

  • Purpose: The objective of this study was to propose a method for developing a clinical phantom to reproduce various diseases that are clinically prevalent in the field of dentistry. This could facilitate diverse clinical research without unnecessarily exposing patients to radiation. Materials and Methods: This study utilized a single dry skull, which was visually and radiographically examined to evaluate its condition. Existing lesions on the dry skull were preserved, and other relevant lesions were artificially created as necessary. These lesions were then documented using intraoral radiography and cone-beam computed tomography. Once all pre-existing and reproduced lesions were confirmed by the consensus of 2 oral and maxillofacial radiologists, the skull was embedded in a soft tissue substitute. To validate the process, cone-beam computed tomography scans and panoramic radiographs were obtained of the fabricated phantom. All acquired images were subsequently evaluated. Results: Most lesions could be identified on panoramic radiographs, although some sialoliths and cracked teeth were confirmed only through cone-beam computed tomographic images. A small gap was observed between the epoxy resin and the bone structures. However, 2 oral and maxillofacial radiologists agreed that this space did not meaningfully impact the interpretation process. Conclusion: The newly developed phantom has potential for use as a standardized phantom within the dental field. It may be utilized for a variety of imaging studies, not only for optimization purposes, but also for addressing other experimental issues related to both 2- and 3-dimensional diagnostic radiography.

  • 흥미있는 이물 5례를 치험하였기에 보고하는 바이다. 증례 1 : 7세남아에세 Tracheostomy outer cannula관이 삭아 부러져서 Rt. main bronchus례 들어있었던 예. 증례 2 : 51세 남자 trachea에 걸쳐 있었던 한의 침구용 침. 증례 3 : 10개월 여아의 식도제이협착부에, 입밖까지 실 달린채로 걸려 있었던 낚시 바늘. 증례 4 : 16세 남자의 식도제삼협착부에 끝(첨단)이 위로 향해 열려있었던 안전 pin. 증례 5 : 20세남자에서 큰 엽총총알이 Rt. mastoid process 밑을 통하여 Rt. mastoid process와 foramen magnum(skull babe) 중간점에 있는 것을 관혈적으로 제거하였다.

Application of 3D Simulation Surgery to Orthognathic Aurgery : A Preliminary Case Study

  • The aim of this report is to evaluate accuracy using3D surgical simulationand digitally printedwafer in orthognathic surgery. 22-year-old female was diagnosed with mandibular prognathism and apertognathia based on 3D diagnosis using CT. Digital dentition images were taken by laser scanning from dental cast, and each STL images were integrated into one virtual skull using simulation software. Digitalized intermediate wafer was manufactured using CAD/CAM software and 3D printer, and used to move maxillary segment in real patient. Constructed virtual skull from 1 month postoperative CT scan was superimposedinto simulated virtual model to reveal accuracy. Almost maxillo-mandibular landmarks were placed in simulated position within 1 mm differences except right coronoid process. Thus 3D diagnosis, surgical simulation, and digitalized wafer could be useful method to orthognathic surgery in terms of accuracy.

CAD for Detection of Brain Tumor Using the Symmetry Contribution From MR Image Applying Unsharp Mask Filter

  • Automatic detection of disease helps medical institutions that are introducing digital images to read images rapidly and accurately, and is thus applicable to lesion diagnosis and treatment. The aim of this study was to apply a symmetry contribution algorithm to unsharp mask filter-applied MR images and propose an analysis technique to automatically recognize brain tumor and edema. We extracted the skull region and drawed outline of the skull in database of images obtained at P University Hospital and detected an axis of symmetry with cerebral characteristics. A symmetry contribution algorithm was then applied to the images around the axis of symmetry to observe intensity changes in pixels and detect disease areas. When we did not use the unsharp mask filter, a brain tumor was detected in 60 of a total of 95 MR images. The disease detection rate for the brain was 63.16%. However, when we used the unsharp mask filter, the tumor was detected in 87 of a total of 95 MR images, with a disease detection rate of 91.58%. When the unsharp mask filter was used in the pre-process stage, the disease detection rate for the brain was higher than when it was not used. We confirmed that unsharp mask filter can be used to rapidly and accurately to read many MR images stored in a database.

Rapid Spontaneous Redistribution of Acute Epidural Hematoma : Case Report and Literature Review

  • Acute epidural hematoma (AEDH) occurring as a result of traumatic head injury constitutes one of the most critical emergencies in neurosurgery. However, there are only several reports that show the rapid disappearance of AEDH without surgical intervention. We suggest redistribution of hematoma through the overlying skull fractures as the mechanism of rapid disappearance of AEDH. A 13-year-old female fell from a height of about 2 m and presented with mild headache. A computed tomography (CT) scan performed 4 hours after the injury revealed an AEDH with an overlying fracture in the right temporal region and acute small hemorrhagic contusion in the left frontal region. A repeat CT scan 16 hours after injury revealed that the AEDH had almost completely disappeared and showed an increase in the epicranial hematoma. The patient was discharged 10 days after injury with no neurological deficits. This case is characterized by the rapid disappearance of an AEDH associated with an overlying skull fracture. We believe that the rapid disappearance of the AEDH is due to the redistribution of the hematoma, rather than its resolution or absorption, and fracture plays a key role in this process.

상악골 확장이 안면골에 미치는 영향에 관한 Laser Holography연구 (LASER HOLOGRAPHIC STUDY ON THE EFFECT OF FACIAL SKELETON TO MAXILLARY EXPANSION)

  • The highly accurate laser holographic interferemotry method was used to determine in what way low-magnitude forces during maxillary expansion are transmitted to the entire maxillary complex and its surrounding structures. The experiments were carried out on a dryed human skull which had a perfectly preserved, normally aligned maxillary dental arch and intact alveolar process. The skull was fixed within a constructed metal frame which ensured maximal stability of the object. The optical equipment and the object were mounted on antivibration table. Interferograms were taken on the lateral and frontal sides of the maxillary complex, using the 10mW He-Ne laser and the double-exposure method. Analysis of the fringe pattern on the recorded object surface was performed by graphically determining the deformation curves related to the bony surface in selected horizontal and vertical planes. On the basis of this study, the following conclusions can be drawn: 1. The density of the interference fringes was gradually increased with the degree of expansion force. 2. Mechanical reactions on the maxillary complex, circummaxillary sutures, and surrounding bones were clearly visible, even with the lowest loading degree. 3. The amount of bone displacement was greater in application of the force after $90^{\circ}$ turn than in initial application of the same force. 4. The direction of interference fringes on the bony surface was similar at all loading degrees.
