Experience with 78 Cases with Preauricular Sinus and 28 Cases with Preauricular Skin Tag (전이동 78예 및 전이 피부 부속기 28예에 대한 임상 경험)

  • Preauricular sinus and preauricular skin tag are common childhood congenital anomalies. It is important for the pediatric surgeon to be familiar with the embryology and differentiation of head and neck structure to accurately diagnose and treat these lesions. Seventy eight patients with preauricular sinus and twenty-eight with preauricular skin tag treated in the Department of Pediatric Surgery at Hanyang University Hospital from January 1981 to May 2002 were reviewed to determine relative frequency, clinical classification and appropriate treatment. The male to female ratio of preauricular sinus was 1:1.2, and preauricular skin tag was 1:1. The most commonly presenting age of sinus and skin tag was before 5 year (62.8%) and before 1 year (53.6%). Twenty nine of 78 cases of preauricular sinuses were on the left, 25 on the right and 24 bilateral. Signs of infection were seen in 73.0% of patients with preauricular sinus at operation. Only 31.3% of lesions were infected in patients less than one year of age, but 89.5% between 3-5 years and 100% between 5-8 years. Cartilage was present in five patients with preauricular skin tag. Although re-operation due to wound infection was necessary in four cases, no recurrences were found. The preauricular sinus is a common anomaly in childhood, and has had a relatively high recurrence rate. But most of the recurrences were due to incomplete resection because of combined infection. Initial proper diagnosis and early operation are very important. Identification of the exact anatomical location of sinus tract is necessary because total excision of the lesions including those tracts is the only way to prevent recurrence.

  • This paper introduces UHF RFID tag antenna which can be inserted between the skin and muscle of animal. Animal Information Management System (AIMS) using the developed tag has been developed. Since the general and commercial tags are not working in the body of any animal, the new tag for flash body has been developed with the consideration of dielectric constant of the muscle and fat of animal. The tag shape is flat and sharp end to make it easy to insert the tag just under the skin. The performance of the installed tag under the skin and in the muscle has been verified. The AIMS can manage the animal, history of illness, information of owner and address. This program can be applied to the management of military and police dogs.

  • We investigated the effects of substrate, metal-line, and surface material on the performance of radio frequency identification(RFID) tag antenna using a tag antenna with a meander line radiator and T-matching network. The results showed that readability of the tag antenna with a thin high-loss substrate could be increased so that it was similar to that of a low-loss substrate if the substrate was very thin. The readability of the tag antenna decreased significantly when the metal line was thinner than the skin depth. The readability of the tag also decreased drastically when the tag was attached to high-permittivity high-loss target objects.

  • Congenital anomalies of the head and neck region such as preauricular sinus and skin tag, thyroglossal duct cyst, branchial anomaly, cystic hygroma and dermoid cyst are common in pediatric population. It is important for pediatricians and pediatric surgeons to be familiar with the embryology and the anatomical characteristic of these lesions in order to diagnose and treat them properly. Three hundred and nineteen patients with congenital head and neck anomalies treated at Hanyang University Hospital between 1980 and 1999 were reviewed to determine the relative frequency of the anomalies and to analyze the method of management. Eight-four (25.1 %) of 335 lesions were preauricular sinus and skin tag, 81 (24.2 %) were thyroglossal duct cyst, 81 (24.2 %) branchial anomaly, 58 (17.3 %) cystic hygroma and 31 were (9.2 %) dermoid cyst. The male-to-female ratio was 1.4:1. Thyroglossal duct cyst most commonly present at 3-5years, however branchial anomalies commonly are diagnosed in children younger than 1 year. Preauricular sinus showed familial tendency in three patients and was bilateral is 33.8 %. Most head and neck anomalies in children have specific clinical and anatomical characterics. A careful history and physical examination is very useful for diagnosis and proper management. Experienced pediatric surgeons should do the initial surgery since the recurrence rate after incomplete surgical excision can be high.

  • This paper is proposed the hardware structure and signal processing method of the RF-Tag based on heart sound to develop the mobile biomedical information device for the u-healthcare system. The RF-Tag in this study is consisted of a skin temperature sensor, a dynamic microphone for heart sound detection, Bluetooth communication to transmute healthcare data, and pulse period detection algorithm with adaptive gain controller. We experimented to evaluate performance of the RF-Tag in noisy environments. In addition, the RF-Tag has shown the good performance in the results of experiment. If the proposed methods in this study apply to design the u-healthcare device, we will be able to get the exact health data on real time in mobile environments.

  • Knowledge regarding lipid catabolism has been of great interest in the field of animal sciences. In the livestock industry, excess fat accretion in meat is costly to the producer and undesirable to the consumer. However, intramuscular fat (marbling) is desirable to enhance carcass and product quality. The manipulation of lipid content to meet the goals of animal production requires an understanding of the detailed mechanisms of lipid catabolism to help meticulously design nutritional, pharmacological, and physiological approaches to regulate fat accretion. The concept of a basic system of lipases and their co-regulators has been identified. The major lipases cleave triacylglycerol (TAG) stored in lipid droplets in a sequential manner. In adipose tissue, adipose triglyceride lipase (ATGL) performs the first and rate-limiting step of TAG breakdown through hydrolysis at the sn-1 position of TAG to release a non-esterified fatty acid (NEFA) and diacylglycerol (DAG). Subsequently, cleavage of DAG occurs via the rate-limiting enzyme hormone-sensitive lipase (HSL) for DAG catabolism, which is followed by monoglyceride lipase (MGL) for monoacylglycerol (MAG) hydrolysis. Recent identification of the co-activator (Comparative Gene Identification-58) and inhibitor [G(0)/G(1) Switch Gene 2] of ATGL have helped elucidate this important initial step of TAG breakdown, while also generating more questions. Additionally, the roles of these lipolysis-related enzymes in muscle, liver and skin tissue have also been found to be of great importance for the investigation of systemic lipolytic regulation.

  • BIPV or double skin applied to the surface of the building, power and thermal load cannot both be increased. In the case of BIPV, because it is applied to the facade, incident solar energy decreases and efficiency drops off. The system in this paper complements these disadvantages and aims to decrease the heating & cooling load by transforming solar energy to electronic and thermal energy. The research in this paper is about the applicability of the clear PV attached double-skin system. And the PV electronic generation and the factors that affect the heating & cooling load such as the daily radiation, sun shading ratio, heating & cooling load, daylight luminance and glare distributions in the building are simulated.

Congenital midline cervical cleft: An easily misdiagnosed disease

  • Congenital midline cervical cleft is a rare congenital disease. The disease is often misdiagnosed as a branchial cleft deformity, thyroglossal duct cyst, or other skin diseases. It has the following characteristics: skin defect at the midline of the anterior neck, a skin tag at the upper end of the lesion, and a blind sinus tract at the caudal aspect with or without mucoid discharge. Treatment is usually for aesthetic purposes; therefore, early surgical en bloc resection with Z-plasty or W-plasty is recommended to reduce recurrence and scar formation.

  • Background: Compared to the frequency of occurrence of pre-auricular skin tag, cervical chondrocutaneous branchial remnants is one of congenital, benign neck masses that is very rare all over the world. Most of these rare anomalies are reported in case reports and especially, rare cases of unilateral cervical chondrocutaneous branchial remnants have been reported in Korean. Materials & Methods: A 9-year-old male patient visited the hospital on September 2017 for a rod-shaped mass. As a simultaneous diagnosis and treatment method, complete surgical excision was executed. Results: Excised mass was 0.5cm in diameter, 1.2cm in. Histologically, a hyaline and elastic cartilage was found in the core. As a family history, the same remnant was found in the right Anterior neck area of his mother. Conclusion: In this case, it is possible to diagnose and treat simply at the same time and even an optimal aesthetic result can be obtained.

Development of Livestock Traceability System Based on Implantable RFID Sensor Tag with MFAN (MFAN/RFID 생체 삽입형 센서 태그 기반 가축 이력 관리 시스템 개발)

  • With the recent increased risk of livestock disease spread and human infection, livestock disease control has become very important. Consequently, there has been an increased attention on an implantable real-time monitoring and traceability system for individual cattle. Therefore, we have developed a robust monitoring and traceability system based on an implantable MFAN/RFID sensor tag. Our design combines the MFAN technology that is capable of robust wireless communication within cattle sheds and the 900MHz RFID technology that is capable of wireless communication without battery. In MFAN/RFID implantable sensor tag monitoring system, UHF sensor tag is implanted under the skin and accurately monitors the body temperature and biological changes without being affected by external environment. In order to acquire power needed by the tag, we install a MFAN/RFID tranceiver on the neck of cattle. The MFAN coordinator passes through the MFAN node and the RFID-reader-combined MFAN/RFID transceiver and transmits/receives the data and power for the sensor tag. The data stored in the MFAN coordinator is transmitted via the internet to the livestock history monitoring system, where it is stored and managed. By developing this system, we hope to alleviate the problems related to livestock disease control.
