Background Little is known about the sensate recovery of skin grafts over free non-neurotized muscle flaps. The aim of this study was to evaluate the sensitivity of free gracilis muscle flaps and meshed skin grafts without nerve coaptation. Methods Thirteen consecutive patients with a median age of 55 years (range, 21-70 years) who underwent lower extremity reconstruction between September 2014 and October 2016 were included. Complications, flap contour, skin perception, and sensate recovery were assessed. Results All flaps survived completely. In one patient, wound dehiscence and infection occurred 1 month after surgery. After a median follow-up of 14 months (range, 10-51 months), a satisfactory contour and skin perception were achieved. The Semmes-Weinstein (SW) monofilament test (154.8±22 g) and static two-point discrimination (2-PD) (12.6±0.7 mm) showed intermediate recovery compared to the surrounding site (41% and 76%, respectively). There was an intermediate correlation between flap size and sensate recovery (2-PD: r=0.27, P=0.36; SW test: r=0.45, P=0.12). Vibration sensation recovered to 60%, whereas thermal sensation remained poor (19% at 5℃ and 25% at 25℃). Conclusions Finer sensation could be partially restored. However, thermal sensation remained poor.
콜라겐 펩타이드(collagen peptide)는 단백질의 가수 분해물로서 주름 완화, 보습력 증대, 탄력 개선 등의 특정 피부 효능을 나타낼 수 있어 화장품 혹은 피부 개선 기능식품으로서 활용되고 있다. ${\gamma}-aminobutyric$ acid (GABA)는 척추 동물의 뇌, 척수에 존재하는 신경전달 물질로서 수면의 질과 양을 개선해 준다고 알려져 있다. 본 연구에서는 콜라겐 펩타이드와 GABA의 복합물이 수면 장애가 있는 여성에게 8주 경구 섭취를 통해서 수면 및 피부 상태를 개선할 수 있는가에 관하여 확인하였다. 복합물(J85091900)은 8주간 연속적으로 섭취 시, 수면장애지수(PSQI)가 유의적으로 감소하였으며, 수면 시간을 7% 증가시켰다. 또한, 피부 거칠기, 눈가 주름 및 피부 수분량(capacitance)을 유의적으로 개선하였다. 이상의 결과에서 콜라겐과 GABA의 복합물은 복합 수면 장애에 따른 피부의 노화 현상으로부터 피부를 보호할 수 있음을 확인하여 먹는 화장품의 핵심 소재로 활용 가능함을 확인하였다.
잎새버섯 균사체 배양액으로부터 분리된 세포외 다당체(exopolysaccharide, GF-glucan)의 피부보습과 SLS에 의한 피부손상에 대한 자극완화 효과를 연구하였다. 사람을 이용한 피부수분 보유 효과 및 피부 수분 손실량을 평가한 결과, 피부의 수분함량을 유지하는 효과가 우수하였으며 표피 수분손실량 또한 전어서 피부 보습제로서 매우 우수하였다. GF-glucan을 함유한 화장품의 자극완화 효과를 평가하기 위하여 사람 피부에 $0.5\%$ SLS 첩포로 유발된 자극성 피부염의 항염증 효과를 평가한 결과, SLS에 의해 유발되는 자극정도가 피검자의 피부상태에 따라 다양하게 나타났으며, GF-glucan이 함유된 제품의 도포 부위에서 대조 부위에 비해 홍반이 유의하게 감소하였다. 또한 자극유발 후 피부에 대한 수분유지와 피부 수분 손실량을 감소시킴으로서 우수한 항염증 효과를 보였다. 결론적으로 잎새버섯의 균사체를 배양하여 생산한 배양액으로부터 분리한 세포외 다당체인 GF-glucan을 화장품에 응용하면 우수한 보습효과와 자극 완화 효과를 동시에 가지는 화장품을 개발 할 수 있다고 사료된다.
녹조식물 옥덩굴의 자연 개체군 동태 및 영양번식성을 파악하므로써 이를 옥덩굴의 양식에 적용하기 위하여, 1998년 5월부터 1999년 4월까지 부산 용호동 인근의 조하대에서 옥덩굴 엽체를 skin diving에 의해 매월 채집하였다. 염장의 변동은 수온의 변화와 밀접한 관계를 가지고 변동하였으며, 7월에 13.4${\pm}$1.5 cm로 길이생장의 최고치를 보였고 3월에 5.1${\pm}$0.5 cm로 최저치를 나타내었다. 또한 생체량은 7월에 2.2${\pm}$0.4 g으로 최고치를 보였고 10월에 가장 낮은0.7${\pm}$0.1 g이였다. 이 조사기간동안 옥덩굴의 배우자낭 형성은 관찰되지 않았다. 옥덩굴 말단가지 (ramuli)의 재생률은 적정 수온 조건하에서는 광량에 따라 차이를 나타내었으며 50 ${\mu}$molm$^{-2}$s $^{-1}$에서 최대 5.6%였으며, 20 및 70 ${\mu}$molm$^{-2}$s $^{-1}$에서 각각 3.0 및 2.6%를 보였다. 옥덩굴 직립지 (erect branch)를 절단하여 5개의 온도 구간 (18, 22, 25, 28, 31$^{\circ}C$) 및 5개의 조도 구간 (10, 20, 40, 60,100 ${\mu}$molm$^{-2}s$$^{-1}$)의 조합인 총 25개의 각 배양조건별로 직립지의 재생을 조사한 결과 배양 15일 후 25$^{\circ}C$ 와 20 ${\mu}$molm$^{-2}s$^{-1}$) 실험 구에서 엽장 4.5${\pm}$1.0 cm, 중량 1.2${\pm}$0.7 g으로 가장 높은 길이생장 및 엽중량의 증가를 나타내었다. 지금까지의 연구 결과로 옥덩굴의 인공채묘는 자연에서 성숙된 개체를 얻기 어려우므로 재생에 의한 영양생장 효과를 이용하는 것이 보다 효과적이라고 보여진다.
최근에는 콜라겐 펩타이드가 항상화효과를 통해 피부의 재생을 촉진시키는 효과를 한다는 연구가 보고되고 있다. 본 구는 저분자 경구 콜라겐 펩타이드가 1550 nm fractional photothermolysis 레이저 치료 후 피부재생에 미치는 영향을 알아보고자 한다. 본 실험에 참여한 환자는 총 10명으로 실험군과 대조군 각 5명씩 무작위배정되었다. 대조군과 실험군 모두 동일하게 fractional photothermolysis 레이저를 시술받았다. 실험군에서는 경구 콜라겐 펩타이드를 하루에 1,000 mg씩 총 8주간 복용하였다. 치료 전후 모든 환자에서 피부의 탄성도, 경표피수분손실도, 홍반지수를 측정하였다. 피부탄성도는 실험군에서 대조군에 비해 통계적으로 유의하게 피부의 탄력성이 증가하였음을 확인하였다(p < 0.05). 경표피수분손실도는 두군간에 유의한 차이가 없었고, 홍반지수는 실험군에서 더 빨리 감소하는 결과를 보였다. 환자의 주관적 만족도평가에서 시술 4주 후 환자의 만족도는 실험군에서 평균 2.0점, 대조군에서 1.2점으로 나타났다. 저분자 콜라겐 펩타이드는 1550 nm fractional photothermolysis 레이저 치료 후 피부회복 촉진에 도움이 된다.
In vivo에서 10주간의 UV 조사에 의해 유발되는 피부 손상 및 2주간의 아세톤 도포에 의해 유발되는 급성 피부장벽손상에 대한 콜라겐 펩타이드의 보호 효능을 관찰한 결과, 주름 증가, 비정상적 각질 세포 증식에 의한 피부 두께 증가, 염증성 사이토카인의 증가 등이 콜라겐 펩타이드의 경구 섭취에 의해 개선됨을 확인하여, 콜라겐 펩타이드가 피부 손상을 방어하고, 피부 장벽회복기능이 정상적으로 작용할 수 있도록 도움을 주는 것을 알 수 있었다. 콜라겐 펩타이드의 피부 장벽 회복 기전을 살펴보기 위하여 사람 각질세포를 이용하여 평가한 결과, 콜라겐 펩타이드는 SPT 발현을 농도 의존적으로 증가시킴을 확인하였으며, 이를 통하여 콜라겐 펩타이드가 광노화 및 급성 피부장벽 손상에 의해 유발되는 피부 진피 및 표피 층의 손상을 회복 혹은 보호하는 효능이 있음을 알 수 있었다. 이상의 결과로부터 콜라겐 펩타이드가 광노화 보호 또는 피부장벽 개선 효능을 갖는 새로운 미용 식품 소재로써 이용 가능성이 높음을 확인할 수 있다.
For proper wound healing, dermal contraction and remodeling are critical; during the natural healing process, differentiated fibroblasts called "myofibroblasts" typically undertake these functions. For severe wounds, however, a critical mass of dermal matrix and fibroblasts are lost, making self-regeneration impossible. To overcome this impairment, synthetic wound patches with embedded functional cells can be used to promote healing. In this study, we developed a polydioxanone (PDO)-based cell-embedded sheet on which dermal fibroblasts were cultured and induced for differentiation into myofibroblasts, whereby the following combinatorial physicochemical stimuli were also applied: aligned topology, electric field (EF), and growth factor. The results show that both the aligned topology and EF synergistically enhanced the expression of alpha smooth-muscle actin (${\alpha}$-SMA), a key myofibroblast marker. Our proof-of-concept (POC) experiments demonstrated the potential applicability of a myofibroblast-embedded PDO sheet as a wound patch.
Platelet-rich fibrin (PRF) is a strong but flexible fibrin including a enrich platelet which contain growth factors and cytokines. PRF can be made very simply and requires no artificial additives unlike platelet-rich plasma. While PRF is remodeled and released in the tissue, this induces cell growth, vascularization, collagen synthesis, osteoblast differentiation and an anti-inflammatory reaction. Taking advantage of these functions, PRF can stimulate regeneration of bone and soft tissue in a diverse number of ways during the course of hemostasis, wound coverage, preservation, and reconstruction of alveolar bone. Moreover, the use of PRF to improve bone regeneration has become a recent technique in implantology. In this study, through a literature review of PRF's existing clinical applications, we classified a range of potential PRF oral and maxillofacial surgery applications including preservation of extraction sockets, guided bone graft, sinus lift, dressing and periodontal treatment. This trial gave us chance to confirm the usefulness of PRF. Recently, updated clinical studies results concerning skin and tendon wound healing have become available. These results suggest that the usage of RPF will gradually expand.
The purpose of this study was to regenerate human dental pulp tissues similar to native pulp tissues. Using the mixture of type I collagen solution, primary cells collected from the different tissues (pulp, gingiva, and skin) and NIH 3T3 ($1{\;}{\times}{\;}10^5{\;}cells/ml/well$) were cultured at 12-well plate at $37^{\circ}C$ for 14 days. Standardized photographs were taken with digital camera during 14 days and the diameter of the contracted collagen gel matrix was measured and statistically analyzed with student t-test. As one of the pulp tissue engineering, normal human dental pulp tissue and collagen gel matrix cultured with dental pulp cells for 14 days were fixed and stained with Hematoxyline & Eosin. According to this study, the results were as follows: 1. The contraction of collagen gel matrix cultured with pulp cells for 14 days was significantly higher than other fibroblasts (gingiva, skin) (p < 0.05), 2. The diameter of collagen gel matrix cultured with pulp cells was reduced to 70.4% after 7 days, and 57.1% after 14 days. 3. The collagen gel without any cells did not contract, whereas the collagen gel cultured with gingiva and skin showed mild contraction after 14 days (88.1% and 87.6% respectively). 4. The contraction of the collagen gel cultured with NIH 3T3 cells after 14 days was higher than those cultured with gingival and skin fibroblasts, but it was not statistically significant (72.1%, p > 0.05). 5. The collagen gel matrix cultured with pulp cells for 14 days showed similar shape with native pulp tissue without blood vessels. This approach may provide a means of engineering a variety of other oral tissue as well and these cell behaviors may provide information needed to establish pulp tissue engineering protocols.
Burns are one of the most devastating forms of trauma and wound healing is a complex and multicellular process, which is executed and regulated by signaling networks involving numerous growth factors, cytokines, and chemokines. Recombinant human granulocyte macrophage colony-stimulating factor (rhGM-CSF) was specifically produced from rice cell culture through use of a recombinant technique in our laboratory. The effect of rhGM-CSF on promotion of deep second-degree burn wound healing on the back skin of a hamster model was evaluated through a randomized and double-blind trial. As macroscopic results, hamster skins of the experimental groups showed earlier recovery by new epidermis than the control groups. Immunohistochemical reactions of proliferating cell nuclear antigen and transforming growth factor-b1, which are indicators of cell proliferation, were more active in the experimental group, compared with the control group. On electron microscopy, basal cells in the epidermis of the experimental group showed oval nuclei, prominent nucleoli, numerous mitochondria and abundant free ribosomes. In addition, fibroblasts contained well-developed rough endoplasmic reticulum with dilated cisternae. Bundles of collagen fibrils filled the extracellular spaces. Particularly, ultrastructural features indicating active metabolism for regeneration of injured skin at 15 days after burn injury, including abundant euchromatin, plentiful free ribosomes, and numerous mitochondria, were observed. These findings suggest that use of rhGM-CSF could result in accelerated deep second-degree burn wound healing in animal models.
