  • Exfoliative dermatitis is a name of a group of symptoms which is characterized by red rash, itching and scale in almost whole body. The patients are suffered from hypoalbuminemia, general weakness and fever sign in addition to characteristic skin lesions. So in the treatment of this disease, adequate symptomatic treatment is important with adequate topical skin management. We experienced a 67 year old Korean male patient diagnosed as exfoliative dermatitis based on clinical symptoms. We concluded that the symptoms of this patient are Sun-symptoms(癬症) and gave him Sunpunghwangi-san(宣風換肌散) and Chungdae-san(靑黛散). After taking herbal medicine, clinical improvement was noticed. Based on this experience, both internal and external use of oriental medicine have a good effect on exfoliative dermatitis.

  • Despite the high prevalence of chronic dermatitis and the accompanied intractable itch, therapeutics that specifically target itching have low efficacy. Increasing evidence suggests that TLRs contribute to immune activation and neural sensitization; however, their roles in chronic itch remain elusive. Here, we show that the RBL-2H3 mast cell line expresses TLR4 and that treatment with a TLR4 antagonist opposes the LPS dependent increase in mRNA levels of Th2 and innate cytokines. The pathological role of TLR4 activation in itching was studied in neonate rats that developed chronic itch due to neuronal damage after receiving subcutaneous capsaicin injections. Treatment with a TLR4 antagonist protected these rats with chronic itch against scratching behavior and chronic dermatitis. TLR4 antagonist treatment also restored the density of cutaneous nerve fibers and inhibited the histopathological changes that are associated with mast cell activation after capsaicin injection. Additionally, the expression of IL-1β, IL-4, IL-5, IL-10, and IL-13 mRNA in the lesional skin decreased after TLR4 antagonist treatment. Based on these data, we propose that inhibiting TLR4 alleviated itch in a rat model of chronic relapsing itch, and the reduction in the itch was associated with TLR4 signaling in mast cells and nerve fibers.

  • Objective : The purpose of this study is to report about a mid-40's female patient with Prurigo Nodularis, whose symptoms were relieved after Korean medicine treatment alone with Whidam's Su-Gi Therapy. Methods : the patient in this study received only Korean medicine treatment such as herbal medication, acupuncture treatment, cupping therapy and specially Whidam's Su-Gi therapy. Results : After the treatment, the symptoms of Prurigo Nodularis were relieved. Contact Itching Index fell from 10 to 5, and Non-Contact Itching Index fell from 10 to 4. On the whole the Body Heat has risen and skin condition has improved visually. The significance of Meridian Function Check(based on HRV) and skin syndrome is remarkable., Conclusions : This study showed that Whidam's Su-Gi therapy added with Korean Medicine treatment can be an effective choice for Prurigo Nodularis. So, wasting disease, especially immune-reduced syndrome such as Prurigo Nodularis, may also be treated with Korean Medicine alone without side effects.

  • Objective : The purpose of this study is to report the Korean medicine treatment on Pustulosis Palmaris et Plataris with Extra-Palmoplantar lesions. Method : We adopted herbal medicine(Seungchung-tang-1, Eunjin-1), acupuncture and vapor treatment to treat Pustulosis Palmaris et Plantaris with Extra-Palmoplantar lesions. Steroids were administered for preventing aggravation. We observed patient's hot sensation, itching and pain of both hands and patient's itching and pain of both feet with the Visual Analogue scale(VAS). Also photographs were taken for judging inflammatory skin. Result : After the treatment, the grade of VAS was decreased from 10 to 0 and clinical symptoms were resolved. By judging photographs patient's inflammatory skin was cured. Administered steroid were also reduced from 15 mg per day to 0. Conclusion : This study shows that the Korean medicine treatment was effective to treat Pustulosis Palmaris et Plataris with Extra-Palmoplantar lesions and enable to stop steroid therapy.

  • Objectives : Seborrheic dermatitis is common abnormal skin condition characterized by flaking and itch. In seborrheic dermatitis, the flakes are greasy and yellowish. Inflammation is also observed. Through this paper we can understand the process of seborrheic dermatitis of the scalp and confirm the scalp changes in the treatment process. The purpose of this study was to report the clinical effects of herbal medicine on Seborrheic dermatitis Methods : We employed oriental medical treatments; herbal-medication (Hyeongbangpaedoksangamibang), infant acupuncture, moxibustion and blood therapy. We treated her three times a week. She took medicine three times a day after a meal. During taking medicine, we let her avoid fatty food, flour based food. Her father took a picture the patient's head parts and we compared the symptom with previous symptom. With the picture, we evaluated the patient's scalp condition. Results : After taking treatment, the scalp condition of the patient was much improved. At first, yellowish to reddish scaly pimples appear along the hairline and itching was severe. There were thick and black crusts on the scalp, red, greasy skin covered with flaky white or yellow scales. • Anticonvulsant hypersensitivity syndrome includes fever, skin eruptions, lymphadenopathy, hematologic abnormality and hepatitis, but its mechanism remains unknown. Anticonvulsants including phenytoin, carbamazepine can cause hypersensitivity reaction. We treated a patient who had severe itching sensation and insomnia: he had undergone an operation for cerebral hemorrhage and was administered anti-convulsant agents to prevent convulsions. We administered the anti-convulsant, Dangkwieumja(Dangguiyinzi). After the treatment, clinical symptoms caused by hypersensitivity were improved.

  • In order to compare the therapeutic effect as well as side effects between antithyroid therapy and radioiodine therapy in hyperthyroidism, the author evaluated 111 cases of hyperthyroidism which were composed of 57 patients with antithyroid treatment, 23 patients with combined treatment comprising of antithyroid and radioactive iodine ($^{131}I$) and 31 patients with treatment of $^{131}I$ alone. The $^{131}I$ treatment was limited to older age, above 20 years of age, and not employed in patients with pregnancy and lactation. The patients treated with antithyroid belonged to relatively younger age, and also milder in symptoms and thyroid function tests. The remission rate of antithyroid therapy group was 97.8% within 16 months. Among them the early remission rate occured within 4 months was found to be 21.7%. The combined therapy group, having prescribed more severe cases, showed the remission rate to be 75% within 16 months. The remission rate of $^{131}I$ therapy group revealed 90.9% within 16 months. Among them the early remission rate taking place within 4 months disclosed 16.7%. The side effects of antithyroid drug, propylthiouracil, were noted as follows: itching (2.7%) skin rashes (1.3%), and adverse enlargement of goiter (10.7%). The side effects $^{131}I$ therapy were transient hypothyroidism(3.9%) permanent hypothyroidism (7.8%) and itching (2.0%).
