  • Idiopathic hypereosinophilic syndrome (HES) is a disorder characterized by prolonged eosinophilia without an identifiable cause and eosinophil related tissue damage in multiple organs including heart, lung, skin, gastrointestinal tract, liver, and the nervous systems. Pulmonary involvement occurs in about 40% of HES cases, but pleural effusion due to pleuritis and bilateral pneumothoraces are very rare manifestations. We report a case of hypereosinophilic syndrome presented with bilateral pleural effusions and recurrent bilateral pneumothoraces in a 44 year-old male with brief review of the literature.

Performance Evaluation of Sintered Metal Filter in LILW Vitrification Facility (중.저준위 방사성폐기물 유리화설비에서 금속필터 적용성평가)

  • A performance test of the stainless steel based sintered metal filter was conducted on the low and intermediate level radioactive waste (LILW) vitrification process. The applicability of the metal filter was based on the test results as well. The baseline pressure drop of the metal filter was evaluated similar to the ceramic filter. During the test, when the flow rate of off-gas was $110Nm^{3}/h$, the total baseline pressure drop was shown as $92mmH_{2}O$. The total pressure drop was attributed to the filter media and the residual dust layer and the value of each was $25mmH_{2}O\;and\;67mmH_{2}O$ respectively. The SEM-EDS spectrum and micrograph of the metal filter specimen showed, no corrosion and no physical damage both at the skin membrane and at the support layer. And most of the baseline pressure drop was caused by the deposition of dust on the surface of the membrane. In conclusion, even though the filter exposure time was short at the test, the performance of the stainless steel based metal filter was acceptable for the treatment of LILW vitrification process.

  • Purpose : Our clinical experiences in distally based sural artery island flap is presented to show the usefulness and the reliability as an alternative to flaps currently used for defect in lower extremity. Materials and Methods : From February 1998 to September 2001, nine cases of soft tissue defects in the lower leg, the foot, and around the ankle were treated with distally based sural artery island flap. The cause of the wound was trauma in 6 cases, and osteomyelitis in 3 cases. Defects were located at the lower leg in 2 cases, at the foot in 3 cases and around the ankle in 4 cases. The results were retrospectively analyzed. Results : The defect size ranged from $3{\times}3cm\;to\;20{\times}3cm$. Among 9 cases, 7 cases survived and 2 cases were failed. Flap failure was due to not including the deep fascia in one case and due to extensive soft tissue damage in the other case. Both failed cases were reoperated with the split thickness skin graft. Conclusion : The advantages of distally based sural artery island flap follows (1) reliable blood supply, (2) ease of flap elevation, (3) preservation of the major arteries, (4) less donor site morbidity. Owing to the advantages of this flap, we think it is useful for the soft tissue coverage of the lower leg, the foot and around the ankle. Also we believe it will continue to gain acceptance and use in the majority of lower leg reconstruction.

Effect of Porcine Placenta Extract from Subcritical Water Extraction on Photodamage in Human Keratinocytes

  • The objective of this study was to evaluated the photoprotective effects of porcine placenta extract (PPE) on ultraviolet B (UVB)-induced oxidative stress in human keratinocytes (HaCaT) to evaluate its functional activities as a skin food ingredient. PPE prepared by subcritical water extraction was termed SPE, and subsequently digested by enzymes to prepare E-SPE. Increased intracellular reactive oxygen species (ROS) levels (192.0%) induced by UVB were decreased by SPE and E-SPE. SPE had more effective ROS scavenging activity than E-SPE treatment. UVB treatment increased expression of tissue inhibitor of metalloproteinase 1 (TIMP-1), and this elevated expression was decreased by E-SPE treatment. High-dose treatment with E-SPE (50 and 100 µg/mL) reduced TIMP-1 expression levels of UVB-C (control) to 33.5 and 34.6%, respectively. In contrast, at low SPE doses (1 and 10 µg/mL), the treatment slightly decreased TIMP- 1 expression levels to 73.3% and 71.3% of UVB-C, respectively. In conclusion, the present study demonstrated the protective effect of SPE and E-SPE against UVB damage in keratinocytes via ROS scavenging, down-regulating MMP-2 expression and up-regulating TIMP- 1 expression. This highlights the potential for SPE as an ingredient in the preparation of functional food against photoaging.

Changes in Ceramide in Stratum Corneum and Anti-inflamatory Effects of Sopungdojeok-tang on Atopic Dermatitis (소풍도적탕(消風導赤湯)이 아토피 피부염의 항염증효과와 각질층 ceramide변화에 미치는 영향)

  • Objective : This study was designed to identify lipid protection formation in stratum corneum and anti-inflammatory effects of Sopungdojeok-tang(SD) on atopic dermatitis(AD). Materials and Methods : In Vivo, SD extract was orally administered to BALB/c mice at $2.5m{\ell}/kg/day$ for 2 days after 5% sodium dodecyl sulfate evoked atopic dermatitis in abdominal skin. Morphological changes were observed by immunohistochemical stain using monoclonal antibodies(BrdU, ceramide, MIP-2, $NF-{\kappa}B$ p50, IL-4, and STAT6) and TUNEL method. In vitro, the alterations of IL-4 mRNA expression were detected by RT-PCT in SD extract treated EL4 cells after phorbol-12-myristate-13-acetate and 4-tert-Octylphenol induce Th2 skewed condition. Results : SD is used in Oriental Medicine for its potential curative for atopic dermatitis. In this study, we have investigated the anti-inflammatory and lipid lamella repair effects of SD were investigated. SD decreased the number of eosinophil in atopic dermatitis induced mice. In the histological properties, the hyperplasia, edema, infiltration of lymphocytes, damage of intercellular space of stratum corneum, BrdU positive reacted cells in stratum basal, and degranulated mast cells and capillaries in dermal papillae decreased in mice with SD. Treatment of SD also decreased MIP-2, STAT6 and IL-4 in dermal papillae. The IL-4 mRNA expression decreased in a dose-dependant manner in SD treated EL4 cells. In addition, decrease of $NF-{\kappa}B$ p50 and increase of apoptotic cells in dermis were observed in SD treated mice. These data suggest that SD may beneficial for atopic dermatitis. Conclusions : These data suggest that SD is beneficial in treatment of atopic dermatitis, and that SD provides lipid protection in stratum corneum and anti-inflammatory effects on atopic dermatitis.

Finite Element Analysis of Wrist Orthosis with 3D Printing (3D 프린트를 통해 제작된 손목 보조기의 유한요소해석)

  • The purpose of this study was to manufacture a wrist brace using a computerized tomography system, clinical design software (MediACE 3D Program), and 3D printer. After acquiring the Dicom file of the upper limb with a computed tomography, the wrist brace was designed using the MediACE 3D Program to create a "stereolithography" file. The designed wrist brace was printed using a 3D printer. To verify the effectiveness of wrist assistive device manufactured by 3D printing technology, the stress distribution of the pressure and orthosis applied to bone and skin is represented by finite element analysis. It is expected that the wrist brace can be manufactured by reinforcing the part where the damage caused by pressure and breakage of the brace frequently occurs with the result of finite element analysis when producing the wrist brace.

  • This study presents a new system that can make up for the decrease in identification rate due to the environmental or physical factors of a fingerprint identification system, which is most actively used among existing biometric systems. Considered most usable in life among many types of biometrics due to the convenience of use, smaller initial investment, and the smaller size, fingerprint identification is widely used in diverse applications. However, a fingerprint identification system has several defects, including environmental factors, physical skin damage, and variable sensor contact, lowering the identification rate in comparison to other biometric systems. On the basis of the existing fingerprint system, this study presents a more efficient system that can improve the identification rate by getting additional biometric information and three-dimensional finger type information through the use of simple and inexpensive additional equipment, which can be used in diverse areas.

Removal of Skin from Filefish Using Enzymes (효소를 이용한 말쥐치의 탈피)

  • Collagenase existed in the internal organs of filefish Novoden modestrus was isolated with ammonium sulfate and was purified by ion exchange column chromatography with DEAE-Sephadex A-50 and gel filtration with Sephadex G-150. The activity of the purified enzyme was increased 92.4 folds than that of the crude one and the yield of the purified one was $10.9\%$. The optimum conditions showing the maximum activity of the crude enzyme to digest insoluble collagen(Type I) were $55^{\circ}C$ and pH 8.0, while those showing the maximum activity of the purified one were $55^{\circ}C$ and pH 7.75. However, the use of the crude enzyme for skinning of filefish was more profitable because the yield was 800 folds higher than that of the purified one and the cost was also able to economy. When hydrolysis for skinning of filefish was conducted with $0.3\%$(w/w) crude collagenase at $50^{\circ}C$ and pH 8.0 for 3hrs, there was some problem to cause a damage on muscle of the fish by heat. To solve such problem for the skinning, the hydrolysis at $18^{\circ}C$ for 4hrs with $0.3\%$ (w/w) crude enzyme after pretreated with 0.5M acetic acid for 10 min provided a good result for skinning of filefish.

The Behavior of Bearing Capacity for the Precast files (기성말뚝의 지지거동)

  • Dynamic and static load tests are conducted in four construction sites by using steel pipe piles(SPP) and concrete piles to compare differences of load bearing mechanism. Steel pipe piles are instrumented with electric strain gages and are subject to dynamic load tests during driving. The damage of strain gages attached is checked simultaneously. Static load test is also conducted on the same piles after two to seven days' elapse. Then load-settlement behavior and shaft and/or tip resistances are measured. As a result, the allowable bearing capacity calculated by the Davisson's offset method of CAPWAP analysis shows 2~33% larger than that of static load test. The average value of allowable bearing capacity of static load test is closer to the allowable capacity obtained at the safety factor of 2.5 applied on ultimate bearing capacity than to the one obtained from the Davisson's offset method. The analysis of strain gage readings shows that unit skin friction increases with depth. Furthermore, the friction mobilized around the 1~2m above the pile tip considerably contributes to the total shaft resistance.

AN ELECTRON MICROSCOPIC STUDY ON THE EFFECTS OF IRRADIATION ON THE ACINAR CELLS OF RAT PAROTID GLAND (방사선조사가 백서 이하선의 선세포에 미치는 영향에 관한 전자현미경적 연구)

  • The author studied the histopathologic changes according to a single or a split dose and the time after irradiation on the acinar cells of rat parotid gland. 99 Sprague Dawley rats, weighing about l20gm, were divided into control and 3 experimental groups. In experimental groups, GroupⅠ and Ⅱ were delivered a single dose of l5Gy, 18Gy and Group Ⅲ and Ⅳ were delivered two equal split doses of 9Gy, 10.5Gy for a 4 hours interval, respectively. The experimental groups were delivered by a cobalt-60 teletherapy unit with a dose rate of 222cGy/min, source-skin distance of 50㎝, depth of l㎝ and a field size of l2×5㎝. The animals were sacrificed at 1, 2, 3, 6, 12 hours, 1, 3, 7 days after irradiation and examined by light and electron microscopy. The results were as follows: 1. As the radiation dose increased and the acinar cells delivered a single dose exposure were more damaged, and the change of acinar cells appeared faster than those of a split dose exposure. 2. The histopathologic change of acinar cells appeared at 1 hour after irradiation. The recovery from damaged acinar cells appeared at 1 day after irradiation and there was a tendency that the recovery from damage of a split dose exposure was somewhat later than that of a single dose exposure. 3. Light microscope showed atrophic change of acinar cells and nucleus, degeneration and vesicle formation of cytoplasm, widening of intercellular space and interlobular space. 4. Electron microscope showed loss of nuclear membrane, degeneration of nucleus and nucleoli, clumping of cytoplasm, widening and degeneration of rough endoplasmic reticulum, loss of cristae of mitochondria, lysosome, autophagosome and lipid droplet. 5. Electron microscopically, the change of rough endoplasmic reticulum was the most prominent and this appeared at 1 hour after irradiation as early changes of acinar cells. The nuclear change appeared at 2 hours after irradiation and the loss of cristae of mitochondria was observed at 2 hours after irradiation in all experimental groups.
