Application of Human Dermal Fibroblast and Keratinocyte on Allogenic Dermis(AlloDerm®) (동종진피에 사람진피 섬유모세포와 각질세포를 적용한 인공피부의 실험적 제작)

  • Purpose: Large skin defect by various causes, should be covered by autologous skin graft. But, the donor site of autologous skin graft is limited and leaves permanent donor scar and contracture. There have been our trial to engineer artificial skin using allogenic dermis (AlloDerm) with basement membrane. Methods: Dermal and epidermal layer were separated by immersing in dipase solution for 30 minutes, and the separated layers were treated with 0.05% trypsin for 10 minutes. And then each layer was cultivated to fibroblasts and keratinocytes on a culture medium. Fibroblasts were first penetrated into basement membrane of allogenic dermis facing down, then allogenic dermis was flipped over to face up and keratinocytes were transplanted to allogenic dermis. Results: Observing artificial skin fabricated in vitro, we found following: 1) The artificial skin opened in air for 5 days formed epidermal layer. In dermal layer, fibroblast was distributed evenly among all. 2) The artificial skin opened in air for 30 days formed thicker and thicker, and it formed basement membrane, spinous and granular layers. PAS stain to confirm existence of basement membrane showed positive reaction. 3) Cytokeratin 10 stain to confirm the formation of epidermal layer showed positive reaction. 4) The formation of thick keratin, lamellar body and desmosome similar to human skin were observed in result of an electron micrograph. Conclusion: As a result of research, the structure seen in normal skin such as rete ridge, is found in reproduced artificial skin. This type of artificial skin can be used as a useful model for investigating skin disease and for clinical application also.

  • Objectives : Conclusion : The purpose of this study is to examine effect that KDKEJ used to atopic dermatitis disease patient get in atopy eruption control experimentally. Methods : We analyzed the expression of IgE, IL-6, II-4, IL-5 and IL-13's level in serum, and $IFN-{\gamma}'$ production by KDKEJ extract. We also analyzed KDKEJ extract get to NC/Nga mine's skin establishment analyzes neck-back skin after biopsy, and H&E method measured about epidermis and dermis part in comparison with control group. Results : In this research KDKEJ extract as treatment result to a NC/Nga mice, IgE and IL-6 content in serum decreased remarkably than control group. And decreased than result control group which measure II-4, IL-5 and IL-13's level in serum and $IFN-{\gamma}'$ production secreted in Th1 cell displayed increase by KDKEJ extract. IL-4 and $IFN-{\gamma}'$ gene revelation amount displayed marked decrease than control group in result that observe effect that get in skin of a NC/Nga mice's skin establishment analyzes neck-back skin after biposy, and dye by H&E method decreased about epidermis and inflammation of dermis part remarkable than control group. Conclusions : These results suggest that Th1 cell and Th2 cell observe to be shifted by secretion amount of IL-4 and $IFN-{\gamma}$ by KDKEJ extract could know that KDKEJ extract can use usefully in allergy autoimmune disease.

Usefulness of V-Y Advancement Flap for Defects after Skin Tumor Excision

  • Background After skin tumor excision on the face, extremities, or trunk, the choice of treatment for a skin defect is highly variable. Many surgeons prefer to use a local flap rather than a skin graft or free flap for small- or moderately-sized circular defects. We have used unilateral or bilateral V-Y advancement flaps, especially on the face. Here we evaluated the functional and aesthetic results of this technique. Methods All of the patients were pathologically diagnosed with squamous cell carcinoma (SCC), basal cell carcinoma (BCC), or malignant melanoma or premalignant lesion (Bowen's disease). Thirty-two patients underwent V-Y advancement flap repair (11 unilateral and 21 bilateral) from January 2007 to June 2011. We analyzed the patients' age and satisfaction, and location and size of defect. The patients were followed up for 6 months or more. Results There were 22 women and 10 men. The ages ranged from 47 to 93 years with a mean age of 66 years. The causes were SCC in 15 cases, BCC in 13 cases, malignant melanoma in 1 case, Bowen's disease in 2 cases, and another cause in 1 case. The tumor locations were the face in 28 patients, and the scalp, upper limb, and flank each in one patient. All of the flaps survived and the aesthetic results were good. Postoperative recovery was usually rapid, and no complication or tumor recurrence was observed. Conclusions The V-Y advancement flap is often used not only for facial circular defects but also for defects of the trunk and extremities. Its advantages are less scarring and superior aesthetic results as compared with other local flap methods, because of less scarification of adjacent tissue and because it is an easy surgical technique.

Muscle-Sparing Blepharoplasty: A Prospective Left-Right Comparative Study

  • Background Standard upper blepharoplasty involves removal of both the skin and a portion of the underlying orbicularis oculi muscle. The senior author had observed sluggishness of eyelid closure, lagophthalmos as well as varying degrees of eye irritation in certain patients during the early postoperative period. He postulated that these findings could be due to orbicularis muscle excision. He therefore undertook a prospective study 27 years ago comparing standard blepharoplasty on one eyelid to skin-only excision on the fellow eyelid. Methods A randomized, prospective, single-blinded study was designed using the fellow eye as an internal control. 22 patients undergoing upper blepharoplasty procedure requiring greater than 5 mm of skin resection and with no history of ophthalmologic disease, dry eye, or previous eyelid surgery were selected. Upper blepharoplasty was performed with skin-only removal on one side, and combined skin-muscle removal on the other side. Patients were evaluated until six months after surgery except for two patients who were lost to follow-up after three months. Sluggish eyelid closure, lagophthalmos, dry eye and aesthetic result were outcome measures scored by patient survey, the operating surgeon, and a blinded expert panel. Results There were comparable aesthetic outcomes in both eyelids. The incidence of sluggish eyelid closure, lagophthalmos and dry eye syndrome were significantly higher in eyelids where wide segments of muscle had been resected. Conclusions Muscle-sparing upper blepharoplasty produces similar aesthetic outcomes as conventional blepharoplasty, while significantly reducing the complications of sluggish eyelid closure, lagophthalmos and dry eye disease. The authors therefore recommend muscle-sparing upper blepharoplasty.

Anti-inflammatory Effects of Effective Microorganism Fermentation Substance on Atopic Dermatitis-like NC/Nga Mouse Model (아토피 피부염 유사 NC/Nga 마우스 모델에서 유용 미생물 발효물질의 항염증효과)

  • Atopic dermatitis (AD) is a chronically relapsing pruritic inflammatory skin disease. To find new anti-inflammatory products for skin inflammatory disease such as AD and contact dermatitis, we produced the effective microorganism fermentation substance (EM-S) by fermentation of medicinal plants with effective microorganisms including photosynthetic bacteria, lactic acid bacteria and yeast, screened the effects of EM-S on NC/Nga model mice. Murine AD-like skin lesions were made by painting Dermatophagoides farinae (Df) extract. Topically applied EM-S significantly reduced clinical severity score, ear thickness and histological grade in AD-like NC/Nga mouse model by Df antigen sensitization. In addition, the serum IgE and Th2 chemokine levels (TARC/CCL17, MDC/CCL22 and CTACK/CCL27) were significantly reduced by EM-S. Futhermore, skin tissue expressions of Th2 chemokines were significantly reduced by EM-S. These results demonstrate that topical application of EM-S may be improve the AD-like skin lesion by suppressing IgE and Th2 chemokines.

  • Behcet's disease is a chronic, multisystemic disorder which is more frequently seen in the Mediterranean basin, Middle East, and Far East. The mean age at the first onset is third decades. In large series of patients, men predominate over women. Infectious agents, immune mechanisms, and genetic factors are implicated in the etiopathogenesis of the disease. Eyes, skin, joints, the oral cavity, blood vessels, and central nervous system are usually involved, although less frequently the heart, lung, kidney may be affected. The prognosis of the disease has been improved because of early diagnosis and suitable treatment. Local remedies and systemic administration of colchicine, corticosteoids, immunosuppressives, and other agents have been applied. Pathergy, or skin hypersensitivity to needle puncture has been reported as a diagnostic test for Behcet's disease. We have managed a Behcet's disease patient with pathergy test & corticosteroids therapy. We have obtained good result and report this case with review of literatures.

Evaluation on Skin Irritation of Bee Venom in Human (봉독의 인체 피부자극성 평가)

  • The purpose of this study was to investigate the skin irritation of bee venom (melittin) in human. Skin irritation test was performed at 0.05% bee venom by 48 h single patch test in 30 healthy volunteers with no skin disease and skin sensitization test was performed by local lymph node assay in animal. In single patch test, 28 human subjects showed no reaction and 2 subjects showed 1+ or 3+ grade skin reaction. In a skin sensitization test of bee venom conducted using mice. no erythema was observed on the dorsal side of mice up to 8 days after application of bee venom on the skin. The results of the average stimulation index by ATP values showed that there was no irritation to the mice skin at 0.005% and 0.01% of bee venom. Therefore, bee venom is not likely to induce a significant skin irritation under 0.05% concentration.

The Effect of Lumbar Sympathectomy Using Radiofrequency Thermocoagulation in Patients with Buerger's Disease -A case report- (Buerger병 환자에서 고주파 열응고술을 이용한 요부 교감신경절 절제술의 효과 -증례 보고-)

  • Buerger's disease is a nonatherosclerotic occlusive inflammatory disease of the small and medium arteries, and veins of the distal leg or arm. Percutaneous lumbar sympathectomy is used to lower extremity occlusive vascular disease as well as Buerger's disease. Lumbar sympathectomy improves blood flow and provides pain relief in the lower extremity. We report two cases of lumbar sympathectomy using radiofrequency thermocoagulation in patients with Buerger's disease. After no paresthesia and muscle contracture at 50 Hz, 1 volt and 2 Hz, 3 volts, respectively, radiofrequency lesioning was performed for 90 sec at $80^{\circ}C$. After the procedure, both patients showed skin temperature increases greater than $2^{\circ}C$ on the affected extremity. Both patients received relief from pain and symptoms without complications. We consider that lumbar sympathectomy using radiofrequency thermocoagulation is a safe and effective procedure that can relieve pain in patients with Buerger's disease.

  • Psoriasis is a common inflammatory disease of the skin marked by excessive scaling associated with inflammation, affecting 1-2% of the population. Although their etiologies remain unknown, the finding of CD4+ and CD8+ lymphocytes at sites of psoriasis has led many reports to hypothesize that psoriasis is a T lymphocyte-mediated disease directed against unknown autoantigens. The oriental medicine understands the cause of psoriasis as low humidity of skin tissues and functional deteriorations of the liver and lung. Gami-Yunbueum (GY) is a mixture of two previous prescription, which were used to the protection of liver and lung. We had prescribed the GY to the three psoriasis patients for 11 weeks and examined the effectiveness of the GY by PASI (Psoriasis Area and Severity Index) score. In our experiment, GY improved the symptoms of psoriasis in all cases. These results indicated that GY can apply to the treatment of psoriasis. Furthermore, the process for treatment of psoriasis is possible to provide humidity to skin and control the functions of liver and lung.

Two Cases of Toxic Epidermal Necrolysis Associated with Deflazacort Therapy in Nephrotic Syndrome: Successfully Treated with Cyclosporine A

  • Toxic epidermal necrolysis (TEN) is a rare, acute, serious, and potentially fatal skin disease, in which cell death causes the epidermis to separate from the dermis. It is thought to be a hypersensitivity complex that affects the skin and mucous membranes, and is caused by certain medications, infections, genetic factors, underlying immunologic disease, or more rarely, cancers. We report two cases of TEN associated with deflazacort (DFZ), a derivative of prednisolone, used in the first episode of nephrotic syndrome (NS). The skin eruption appeared on the $4^{th}$ and $5^{th}$ weeks after DFZ administration, while NS was in remission. The widespread lesions were managed by intensive supportive treatment, discontinuation of DFZ, and oral administration of cyclosporine. Both patients showed a rapid improvement in symptoms of TEN without any complications or relapse of NS.
