Herpes zoster is a viral disease of the posterior root ganglion and sensory nerve fiber, which presents clinically with vesicular eruption of the skin, radicular pain and sensory changes in the distribution of the affected ganglion. However, involvement of the motor neurons can be seen as well. If classic cutaneous lesions are present, herpes zoster-related motor paresis is easily diagnosed. Otherwise, the diagnosis may be more difficult and suspicious, especially if weakness occurs as a symptom before cutaneous lesions appear, or abnormal findings on the MRI are consistent with the signs. There have been few reports of sciatica with motor loss preceding skin lesions. Here, we report a patient with herpes zoster-related motor paresis preceding skin lesions. In the preliminary diagnosis, the herpes zoster-related motor paresis was confused for some structural disorder.
Objectives: This study attempted to investigate the allergic histories and the distribution of offending allergens in the general public and further to help their diagnosis and treatment with Oriental Medicine. Methods: Allergic skin tests (AST) were performed and allergic histories were taken of 359 members of the general public who visited the International Exhibition on Oriental Medicine from Sept. 1 to Sept. 5, 2000. The allergen reagents for AST were three (House dust, D. farinae, Dog hair) and the control reagent was histamine solution. Results: 1.50.1 % of the subjects (n=359) were positive to AST. The ratio between males' positivity and females' was 1.06:1. 2. The younger the subjects were, the higher the positivity was. 3. The positive subjects' (n=180) positivity to three allergens was as follows: D. farinae 98.9%, House dust 30.0%, Dog hair 1.7%. 4. The younger the positive subjects were, the higher the positivity to House dust was. In contrast, the positivity to D. farinae was high in all age groups. 5.71.1 % of the positive subjects reacted positively to monotype allergen and 28.9% reacted positively to multiple allergens (2.46:1). The most common monotype allergen was D. farinae (98.4%) and the most common combination of multiple allergens was House dust and D. farinae (94.2%). 6.52.8% of the positive subjects (n=180) and 51.4% of the negative subjects (n=179) represented the history of allergic diseases. There was no relationship between allergic skin test and allergic diseases. Conclusion: To help in diagnosis and treatment with oriental medicine, research to analyze the relationship between allergic skin test and allergic diseases should be continued on the basis of Oriental medical theories.
Purpose: Pyoderma gangrenosum is a rare inflammatory, reactive dermatosis marked by painful cutaneous ulcer. The causes of pyoderma gangrenosum remain unclear. Gastrointestinal, hematological, rheumatological, and immmune disorders may be associated with pyoderma gangrenosum. The appearance of this disease may range from mild skin ulcers to life-threatening conditions. Generalized multiple ulcerative pyoderma gangrenosum is very rare. Here we report our experience with a case of multiple ulcerative pyoderma gangrenosum accompanied by ulcerative colitis. Methods: A 67-year-old man had cutaneous ulcers at multiple sites including the scalp, face, chest, abdomen, hands, and buttocks. He also developed gastrointestinal symptoms such as intermittent dyspepsia and bloody excrement. Debridement and irritation aggravated the disease progress. We gave a diagnosis of pyoderma gangrenosum with ulcerative colitis based on the clinical appearance and biopsy. The patient was treated with systemic intravenous steroid therapies and careful wound cares. Ulcers of the scalp and buttocks were treated with split thickness skin grafts. Results: Most of the multiple cutaneous ulcers were treated by systemic intravenous steroid therapies and wound cares. The rest of the ulcers were treated with skin grafts. Systemic intravenous steroid therapy was used to treat the ulcerative colitis. Conclusion: Generalized multiple ulcerative pyoderma gangrenosum is very rare. Without making an accurate diagnosis, hasty surgical treatments could aggravate the progression of the disease. Additionally, care should be taken to systemically treat underlying disease as well as administrating local treatments for the skin lesions. Intravenous systemic steroid therapy and skin grafts are useful treatments for generalized pyoderma gangrenosum.

Chronic burn scars often cause various skin malignancies at rates of up to 2%. These lesions are usually squamous cell carcinomas, but rarely, malignant melanoma is reported. We report a 67-year-old male with a malignant melanoma on a burn scar with regional metastasis. This patient presented an ulcerative lesion only in 2 weeks. After histopathological diagnosis, we performed only palliative surgery on patient's demand, and followed up the subsequent deterioration course. Our case reemphasizes the need for rapid diagnosis and treatment when suspect lesions are present on chronic burn scar. Also, physician should be in mind and inform the patient about malignant melanoma and its aggressive course.
Atopic dermatitis is a chronic inflammatory skin disease caused by a variety of genetic and environmental factors, dysregulation of immunological response, as well as dysfunction of the skin barrier proteins. The purpose of this study is to develop an ELISA kit suitable for evaluating the expression of skin barrier proteins. Proteins were obtained from the skin via AriNo and D-Squame patches. The efficiency of protein collection from the skin, using the Arino patch, was shown to be more effective than using D-Squame; while the efficiency of lysis using 0.1% Triton-X100 was higher than that of other lysis solutions, including 0.1 M Tris-HCL, 0.1% Tween-20, and 5 mM KOH. Recombinant skin barrier proteins, such as filaggrin and involucrin, were produced by molecular biological methods. Monoclonal antibodies against filaggrin and involucrin were produced by immunization of mice, fusion of spleen cells and myeloma cells, as well as a selection of antibody-producing hybridoma cells. The filaggrin expression in the skin of subjects suffering from atopic dermatitis was lower than that in normal mice. Involucrin expression was not altered between normal individuals and subjects with atopic dermatitis. These findings contribute to an elucidation of the importance of the skin barrier protein expression in atopic dermatitis and the development of a diagnostic kit for atopic dermatitis.
Hemagglutinating antigen of Toxoplasma gondii was prepared and purified by the method of a slight modification of Tsunematsu, and the preparation of the skin test antigen (toxoplasmin) was made by means of acetone-ether treatment described by Nobute et al. With these antigens the passive hemagglutionation and skin tests were performed for the diagnosis of swine toxoplasmosis by using artificially infected pigs. The results obtained were summarized as follows: 1. The hemagglutinating antibody and the skin test antibody were demonstrated one and three weeks after infection, respectively. And these antibodies were maintained over nine weeks after infection. 2. The antigenicity of hemagglutinating antigen was stable when it was kept in frozen state, while was unstable in a liquid state. 3. Freeze-dried skin test antigen (toxoplasmin) was stable for two months or more if it was kept at $5^{\circ}C$ and room temperature, but in the liquid or reconstituted state it was unstable. 4. Freeze-dried skin test antigen could be preserved without loss of antigenicity for more than two months. 5. Passive hemagglutination test could be applied effectively at the early phase of the disease process and skin test at later phase, mainly for epidemiological survey. However, by combiniation of these methods, the more accurate results could be obtained.

The methodology of constitution diagnosis is main concern in sasang constitutional medicine, but highly reliable method is not yet established. So we statistically analyzed the relationship between the constitution and characteristic factors of skin such as thickness, stiffness, roughness, and touch-feeling, using 1079 clinical data registered to SCIB(Sasang constitution Information Bank), and the following results are obtained: The skin of Taeeumin are more thick or more stiff or more rough than other constitutions. The skin of Soeumin are more soft or more feeble or more feeble-refined than other constitutions. The skin of Soyangin are more slippery or more thin or more slippery-refined than other constitutions. The differences written above were proved to be meaningful enough by Chi-square test.

Eosinophilic granuloma is a common hypersensitive inflammatory skin disease in cats, and rare in dogs and horses. The skin biopsies of 5 years old female Cocker spaniel and 2 years old female mixed dog had the clinical signs of skin nodules with alopecia were submitted for diagnosis. Solitary skin nodules and papillary nodules were presented on the left external ear and back of Cocker spaniel and on the external ear of mixed dog, respectively. Histopathologically, epidermis of skin showed mild to severe hyperplasia with multifocal ulceration. Small to large irregular, brightly eosinophilic foci with degenerating eosinophils and homogeneous degenerated collagens were existed in the dermis of both ear and back skin. Typical 'flame figures', a mixture of degenerated collagen and degranulated eosinophils, were observed in both cases. Based on the histopathologic findings and special staining characters, 2 cases were diagnosed as canine eosinophilic granuloma. This is the first report for the eosinophilic granuloma of dogs in Korea.
Three neonatal pigs from the same litter in a domestic farm were born with skin lesions. Grossly, multiple well-circumscribed, round papules distributed over the skin of the three piglets. Two piglets were submitted for a diagnosis of skin disease. Microscopically, epidermal hyperplasia with ballooning degeneration of stratum spinosum keratinocytes was observed. Some keratinocytes contained eosinophilic intracytoplasmic inclusions and a central nuclear vacuole and chromatin margination. Swinepox (SWP) virus was detected by polymerase chain reaction and nucleotide sequencing, and Staphylococcus hyicus was isolated in skin lesions. Based on the gross findings and laboratory results, these piglets were diagnosed with congenital SWP with a secondary staphylococcal infection.

Small fiber neuropathy (SFN) mainly affects thinly myelinated $A{\delta}$-fibers and unmyelinated C-fibers presented with neuropathic pain like burning feet or numbness. Many conditions are known as a causes of SFN, metabolic derangement, especially glucose intolerance, is the most frequent cause of SFN. It has been hard to diagnose SFN because there has been lack of specialized test for small nerve fiber. Quantification of intraepidermal nerve fiber density using skin biopsy is promising method to diagnose SFN. A skin biopsy also could give helps to research pathophysiology of SFN by specialized stain method.
