Renal Manifestations and Imaging Studies of Kawasaki Disease (가와사키 질환에서의 신증상과 영상 검사 소견)

  • Purpose: The aim of this study was to verify renal inflammation following Kawasaki disease (KD) using single photon emission computed tomography along with Technetium-99m dimercaptosuccinic acid scintigraphy (DMSA renal SPECT). Methods: From March 2011 to October 2011, 15 patients diagnosed with KD at the National Health Insurance System Ilsan Hospital were enrolled in the study. All patients underwent DMSA renal SPECT to evaluate renal involvement during the acute phase of KD. Urine ${\beta}2$-microglobulin (${\beta}2$-MG), a marker of renal proximal tubular dysfunction, was also measured to assess renal damage. Results: All 15 patients had normal renal function test results. However, microscopic hematuria and pyuria were observed in 13% and 33% of the patients, respectively. Moreover, urine ${\beta}2$-MG was elevated in 46% of the patients. In addition, patients were divided into two groups based on ${\beta}2$-MG level: those with an increased ${\beta}2$-MG level, and those with a normal ${\beta}2$-MG level. No significant differences were found between these two groups in clinical characteristics, laboratory, sonography, and echocardiography findings. All patients' DMSA renal SPECT scans were normal. Conclusion: Our study showed that mild abnormalities in the urinalysis and elevated urine ${\beta}2$-MG were the only findings of renal involvement in KD. However, no aggressive renal manifestations were detected on DMSA renal SPECT.

  • 단일광자방출전산화 단층촬영술을 이용한 영상정보를 효과적으로 얻기 위하여 고려되어야 할 사항으로는 1) 조준기(collimator)의 선택, 2) 기질(matrix)의 크기, 3) 회전각의 수 (number of angles), 4) 360도 또는 180도 획득(acquisition), 5) continuous 또는 step& shoot, 6) 원형 또는 비원형회전 등이 있다. 저자들은 비원형회전으로 검체와 검출기 사이의 거리를 단축시킴으로써 직선성, 균일성, 대조도, 해상력에 미치는 영향을 알아보기 위하여 원형회전 방법과 비교하여 다음과 같은 결과를 얻었다. (1) 비원형회전을 하여도 균일성(uniformity)과 직선성(linearlity)을 유지한다. (2) 균일성, 대조도(contrast), 해상력(resolution)들이 비원형 회전을 한 경우에 보다 더 개선되었다. (3) 영상 획득시간은 비원형회전인 경우에 더 소요되었다. (매스캔 당 10분) 따라서 검사자는 영상 화질의 개선효과와 상반되는 보정(calibration)과 설치(set-up)에 소요되는 시간(매스캔당 10분이상)을 비교하여 자료획득(data acquisition) 회전방법을 선택하여야한다.

Fusiform Aneurysm on the Basilar Artery Trunk Treated with Intra-Aneurysmal Embolization with Parent Vessel Occlusion after Complete Preoperative Occlusion Test

  • Fusiform aneurysms on the basilar artery (BA) trunk are rare. The microsurgical management of these aneurysms is difficult because of their deep location, dense collection of vital cranial nerves, and perforating arteries to the brain stem. Endovascular treatment is relatively easier and safer compared with microsurgical treatment. Selective occlusion of the aneurysmal sac with preservation of the parent artery is the endovascular treatment of choice. But, some cases, particularly giant or fusiform aneurysms, are unsuitable for selective sac occlusion. Therefore, endovascular coiling of the aneurysm with parent vessel occlusion is an alternative treatment option. In this situation, it is important to determine whether a patient can tolerate parent vessel occlusion without developing neurological deficits. We report a rare case of fusiform aneurysms in the BA trunk. An 18-year-old female suffered a headache for 2 weeks. Computed tomography and magnetic resonance image revealed a fusiform aneurysm of the lower basilar artery trunk. Digital subtraction angiography revealed a $7.1{\times}11.0$ mm-sized fusiform aneurysm located between vertebrovasilar junction and the anterior inferior cerebellar arteries. We had good clinical result using endovascular coiling of unruptured fusiform aneurysm on the lower BA trunk with parent vessel occlusion after confirming the tolerance of the patient by balloon test occlusion with induced hypotension and accompanied by neurophysiologic monitoring, transcranial Doppler and single photon emission computed tomography. In this study, we discuss the importance of preoperative meticulous studies for avoidance of delayed neurological deficit in the patient with fusiform aneurysm on lower basilar trunk.

Diagnosing Symptomatic Accessory Tarsal Bones Using SPECT/CT (SPECT/CT를 이용한 증후성 족부 부골의 진단)

  • Purpose: This study was designed to analyze the usefulness of Single Photon Emission Computed Tomography/Computed Tomography (SPECT/CT) in diagnosing symptomatic accessory tarsal bones. Materials and Methods: Twenty four feet (16 patients) with symptomatic accessory navicular and/or os trigonum, who agreed to take SPECT/CT, were included in this study. Fifteen feet had accessory navicular, five had os trigonum, and four had both. According to the uptake in the SPECT/CT, 11 feet were classified into high and 13 into low uptake groups. The low uptake group was treated non-operatively, while the high uptake group received operations when initial conservative management failed. A modified Kidner procedure was performed for accessory navicular and arthroscopic excision was done for os trigonum. After a mean follow-up of 6.8 (range, 3~13) months, the American Orthopaedic Foot and Ankle Society (AOFAS) score and the Visual Analogue Scale (VAS) for pain were compared. Results: Patients in the high uptake group had a higher initial mean VAS score ($7.0{\pm}0.8$ vs $2.2{\pm}0.9$, p<0.05) and a lower initial mean AOFAS score ($45.9{\pm}9.2$ vs $83.9{\pm}4.2$, p<0.05) compared to the low uptake group. All patients in the low uptake group improved after non-operative treatment. Seven patients underwent operations and had a decreased VAS ($1.6{\pm}0.5$) and an increased AOFAS score ($88.3{\pm}1.8$) at the last follow-up. Four patients in the high uptake group demonstrated erratic symptoms. Conclusion: SPECT/CT can be a useful diagnostic tool and helpful in designing treatment plans for symptomatic accessory navicular and os trigonum.

  • Background and Objetive : Acupuncture is a valuable method of oriental medicine with broad application in many disease. It is based on the experiences of traditional oriental medicine as well as on experimentally proven biological (biochemical and neurophysiological) effects. Acupuncture theory has been explained by the meridian system that is thought to be linked with particular organs. However, in western medicine it is held that many disorders are either controlled or affected by the brain. Material and Method : In order to review the studies concerned with the mechanism related brain, we have referred to the Pubmed site and the Journal of Korean acupuncture and moxibustion Result and Conculsion : Among the 12 studies in the Journal of Korean acupuncture and moxibustion, 8 papers related neurotransmitters were done by experimental study, 4 papers related brain mapping were done by clinical study. Among the 8 studies related brain mapping in the Pubmed site, 6 clinical studies using functional magnetic resonance imaging(fMRI) were done and I clinical study using single-photon emission computed tomography(SPECT) was done, I paper was review article. By the above result, it would be needed further research on the acupuncture mechanism related brain using SPECT, fMRI, positron emission tomography(PET) etc.

  • This review introduces advances in clinical and pre-clinical single photon emission computed tomography (SPECT) and positron emission tomography (PET) providing noninvasive functional images of biological processes. Development of new collimation techniques such as multi-pinhole and slit-slat collimators permits the improvement of system spatial resolution and sensitivity of SPECT. Application specific SPECT systems using smaller and compact solid-state detector have been customized for myocardial perfusion imaging with higher performance. Combined SPECT/CT providing improved diagnostic and functional capabilities has been introduced. Advances in PET and CT instrumentation have been incorporated in the PET/CT design that provide the metabolic information from PET superimposed on the anatomic information from CT. Improvements in the sensitivity of PET have achieved by the fully 3D acquisition with no septa and the extension of axial field-of-view. With the development of faster scintillation crystals and electronics, time-of-flight (TOF) PET is now commercially available allowing the increase in the signal-to-noise ratio by incorporation of TOF information into the PET reconstruction process. Hybrid PET/SPECT/CT systems has become commercially available for molecular imaging in small animal models. The pre-clinical systems have improved spatial resolution using depth-of-interaction measurement and new collimators. The recent works on solid state detector and dual modality nuclear medicine instrumentations incorporating MRI and optical imagers will also be discussed.

  • Nuclear Medicine Images have comparatively poor spatial resolution, making it difficult to relate the functional information which they contain to precise anatomical structures. Anatomical structures useful in the interpretation of SPECT /PET Images were radiolabelled. PET/SPECT Images Provide functional information, whereas MRI mainly demonstrate morphology and anatomical. Fusion or Image Registration improves the information obtained by correlating images from various modalities. Brain Scan were studied on one or more occations using MRI and SPECT. The data were aligned using a point pair methods and surface matching. SPECT and MR Images was tested using a three dimensional water fillable Hoffman Brain Phantom with small marker and PET and MR Image was tested using a patient data. Registration of SPECT and MR Images is feasible and allows more accurate anatomic assessment of sites of abnormal uptake in radiolabeled studies. Point based registration was accurate and easily implemented three dimensional registration of multimodality data set for fusion of clinical anatomic and functional imaging modalities. Accuracy of a surface matching algorithm and homologous feature pair matching for three dimensional image registration of Single Photon Emission Computed Tomography Emission Computed Tomography (SPECT), Positron Emission Tomography (PET) and Magnetic Resonance Images(MRD was tested using a three dimensional water fill able brain phantom and Patients data. Transformation parameter for translation and scaling were determined by homologous feature point pair to match each SPECT and PET scan with MR images.

Radio-Iodinated arbutin for tumor imaging

  • Arbutin is a hydroquinone derivative with a glucose moiety. As a tyrosinase inhibitor, it is widely used as a skin-whitening cosmetic agent for the treatment of cutaneous hyperpigmentary disorders, such as melasma and freckles. In the medical field, many studies have addressed the use of arbutin in various tumors, but the mechanism for tumor uptake of arbutin is still unclear. In this paper, we radiolabeled arbutin using radioiodine and studied its pharmacokinetics and tumor uptake via biodistribution experiments and single-photon emission computed tomography (SPECT) imaging. Radiolabeled $^{131}I-arbutin$ was stable for up to 24 h in PBS and serum. Biodistribution studies and SPECT imaging indicated high uptake of the compound in the bladder and kidneys shortly after injection. Twenty-four hours post-injection, significant deiodination was observed. Apart from high thyroid uptake, selective tumor uptake was clearly observed. The tumor-to-muscle and tumor-to-blood ratios were 26 and 9, respectively.

Comparison of planar scintigraphy and bone SPECT with clinical findings and other imaging modalities in temporomandibular disorder patients (측두하악관절장애 환자에서의 평면 골 스캔 및 Bone SPECT 소견과 임상적 및 영상학적 소견과의 비교)

  • Purpose : This study was performed to evaluate the diagnostic value of both planar and SPECT imagings by comparing the results with the clinical and other imaging modalities findings. Materials and Methods: A total of 578 joints with TMJ complaints were examined using planar scintigraphy and SPECT. The planar scintigraphy and SPECT findings were analyzed and compared to the clinical findings of pain, clicking, crepitus and limitation of mouth opening. Moreover we compared the accuracy of the planar scintigraphy and SPECT methods with the one of the panoramic, transcranial, tomographic and MR imaging methods. Results: The planar scintigraphy and SPECT methods showed a high sensitivity of 0.76-0.84 and low specificity of 0.25-0.45 toward the clinical findings: pain, clicking, crepitus and mouth opening restriction. Simple radionuclide uptake ratio was high in each group of patients with pain, crepitus, limitation of mouth opening (p < 0.05) complaints, in each group with positive bone changes on panoramic, transcranial or tomographic images, and in disc displaced group. Conclusion: Although both planar and SPECT imagings have limited specificity, these techniques are sensitive for detection of internal derangement of the TMJ.

A case of tacrolimus-induced encephalopathy after kidney transplantation

  • We present a case of tacrolimus-induced encephalopathy after successful kidney transplantation. An 11-year-old girl presented with sudden onset of neurologic symptoms, hypertension, and psychiatric symptoms, with normal kidney function, after kidney transplantation. The symptoms improved after cessation of tacrolimus. Magnetic resonance imaging (MRI) showed acute infarction of the middle cerebral artery (MCA) territory in the right frontal lobe. Three days later, she had normal mental function and maintained normal blood pressure with left hemiparesis. Follow-up MRI was performed on D19, showing new infarct lesions at both cerebral hemispheres. Ten days later, MRI showed further improvement, but brain single photon emission computed tomography (SPECT) showed mild reduction of uptake in both the anterior cingulate gyrus and the left thalamus. One month after onset of symptoms, angiography showed complete resolution of stenosis. However, presenting as a mild fine motor disability of both hands and mild dysarthria, what had been atrophy at both centrum semiovale at 4 months now showed progression to encephalomalacia. There are two points of interest in this case. First, encephalopathy occurred after administration of tacrolimus and improved after discontinuation of the drug. Second, the development of right-side hemiplegia could not be explained by conventional MRI; but through diffusion tensor imaging (DTI) and diffusion tensor tractography (DTT) of white matter tract, visualization was possible.
