  • 견관절의 ABER position에서 posterosuperior rotator cuff 가 posterior glenoid에 contact 하여 internal impingement가 일어나는 현상은 운동선수가 아닌 일반인에게도 발생하는 physiologic phenomenon이며 과도한 반복 동작으로 인하여 증상이 발현한다. Internal impingement는 증상이 모호할 뿐만 아니라 견관절 내 여러 병소가 함께 존재하기 때문에 각각의 pathology에 대한 자세하고 정확한 evaluation이 필수적이다. 아직 controversy가 있지만 internal impingement의 pathologic factors로서는 anterior capsular laxity (true anterior laxity), posteroinferior capsular tightness (pseudo anterior laxity)와 이로 인한 glenohumeral internal rotation deficit (GIRD), 그리고 SICK scapula syndrome (scapular dyskinesis) 등으로 요약할 수 있다. 진단은 주로 병력 청취와 신체 검사로 내려지며 ultrasonogram, MRA등으로 확진한다. Sleeper stretching exercise 등과 같은 보존적 치료에 반응이 좋지만 6개월간의 재활 치료에도 호전이 없다면 증상과 직접 관련된 주 병소(appropriate pathology)를 찾아 이에 대한 적절한 수술이 이루어져야 한다.

The Effect on Appendage muscle strength due to Increase in Occlusal Vertical Dimension (수직적 교합고경의 증가가 사지 근력에 미치는 영향에 관한 연구)

  • This study was conducted to observe the effect on appendage muscle strength according to increase in occlusal vertical dimension. For this study, ten males with a mean age of 21 were selected. The subjects had a complete or almost complete set of natural teeth and reported no subjected symptoms of pain or dysfunction in the masticatory system. The tested occlusal splints were made at the position of increased occlusal vertical dimension of 2mm, 3.5mm, and 5mm from the ICP. Before and after wearing occlusal splints, the appendage muscle strength were tested by CybexII Dynamometer in each subject. The results were as follows : 1. When occlusal vertical dimension was increased, most of mean muscular strength values were increased except for those of supination and pronation of forearm at the position of 5mm increased occlusal vertical dimension. 2. The statistical analyses demonstrated that the increased occlusal vertical dimension position to be significantly stronger than intercuspal position for the muscle strength of the flexion and extension of hip, supination of forearm, external and internal rotation of knee, dorsiflexion and plantarflexion of ankle (p<0.05). 3. At the position of 3.5mm increased vertical dimension displayed the highest mean muscluar strength value than other positions. 4. Statistically demonstrated values, except for supination of forearm, internal rotation of shoulder, were related to lower appendage. Therefore splint was more effective on lower appendage than upper appendage to make muscle strength increased. 5. The mean increased rate of muscular strength tested on knee(57%), ankle(42%), and wrist(20%) were higher than hip(31%), elbow(14%), and shoulder(17%).

Comparison of the Effects of Closed Kinetic Chain Exercise and Open Kinetic Chain Exercise According to the Shoulder Flexion Angle on Muscle Activation of Serratus Anterior and Upper Trapezius Muscles During Scapular Protraction

  • Background: Methods for exercising serratus anterior (SA) and upper trapezius (UT) muscles are important for the recovery of patients with various shoulder disorders, yet the efficacy of closed or open kinetic chain exercises have not yet been evaluated. Objects: The purpose of this study was to compare the activation of the SA and UT muscles during scapular protraction considering both closed and open kinetic chain exercises. Methods: Thirty subjects were randomly divided into experimental groups (closed kinetic chain exercise) and control groups (open kinetic chain exercise) in which scapular protraction was performed at $90^{\circ}$ or $125^{\circ}$ shoulder flexion. Electromyographic activity data were collected from the SA and UT muscles per position and exercise method. Results: Separate mixed 2-way analysis of variance showed significant differences in the activation of the SA (F1,28=6.447, p=.017) and the UT (F1,28=35.450, p=.001) muscles between the groups at $90^{\circ}$ and $125^{\circ}$ shoulder flexion. Also, the SA/UT ratio measures at $90^{\circ}$ and $125^{\circ}$ shoulder flexion significantly differed between the groups (F1,28=15.457, p=.001). That is, the closed chain exercise was more effective than open chain exercise for strengthening the SA muscle and controlling the UT muscle, $125^{\circ}$ of shoulder joint was more effective than $90^{\circ}$. Conclusion: The findings suggest that scapular protraction with shoulder $125^{\circ}$ flexion at the closed kinetic chain exercise may be more effective in increasing SA muscle activation and decreasing UT muscle activation as well as increasing the SA/UT ratio than open kinetic chain exercise.

A Comparative Analysis of Horizontal Rotation Movements for Different Ball Course during Two-handed Backhand Drive Stroke in Tennis (테니스 양손 백핸드 드라이브 스트로크 시 볼 방향성에 따른 수평회전운동 비교분석)

  • Objective : The purpose of this study was to compare the kinematic data of the horizontal rotation movements of shoulder, hip, knee during two-handed backhand drive stroke according to two different ball directions. Methods : The kinematic variables were analyzed such as the joint angles of the lower body, horizontal rotation angles of the shoulder, hip, inter-knee segment, body twist angle and difference in angle of forward swing. Two-handed backhand drive stroke was analyzed through a three-dimensional motion analysis. The collected data were analyzed by a paired t-test, and the statistical significant value was set at ${\alpha}=.05$. Results : The findings of this study were as follows; First, there was no difference in the total angles of lower limb joints from the forward swing position to impact posterior. Second, there was no difference in the horizontal rotation angles of E1 shoulder, hip, and E2 shoulder but the horizontal rotation angles of E1 knee, E2 hip, knee, E3, and E4 shoulder, hip, and knee were different in all events. Third, there was no difference in the body twist angle of the maximum horizontal rotation. In addition, there was no difference in the angle of the body twist by the ball direction in the shoulder-hip, the hip-knee and the shoulder-knee. Conclusion : Horizontal rotation angle determines ball directions.

Assessment of Capsular Insertion Type and of Capsular Elongation in Patients with Anterior Shoulder Instability and It's Correlation with Surgical Outcome: A Quantitative Assessment with Computed Tomography Arthrography

  • Background: The study aimed to determine the type of capsular insertion and the extent of capsular elongation in anterior shoulder instability by quantitatively evaluating their computed tomography arthrographic (CTA) findings, and to investigate the correlation of these parameters with surgical outcomes. Methods: We retrospectively reviewed 71 patients who underwent CTA and arthroscopic capsulolabral reconstruction for anterior shoulder instability between April 2004 and August 2008. The control group comprised 72 patients diagnosed as isolated type II superior labrum anterior to posterior (SLAP) lesion during the period. Among the 143 patients, 71 were examined with follow-up CTA at an average 13.8 months after surgery. It was measured the capsular length and cross-sectional area at two distinct capsular regions: the 4 and 5 o'clock position of the capsule. Results: With regards to the incidence of the type of anterior capsular insertion, type I was more common in the control group, whereas type III more common than in the instability group. Anterior capsular length and cross-sectional area were significantly greater in the instability group than in the control group. Among patients of the instability group, the number of dislocations and the presence of anterior labroligamentous periosteal sleeve avulsion lesion were significantly associated with anterior capsular redundancy. Postoperatively, recurrence was found in 3 patients (4.2%) and their postoperative capsular length and cross-sectional area were greater than those of patients without recurrence. Conclusions: Capsular insertion type and capsular redundancy derived through CTA may serve as important parameters for the management of anterior shoulder instability.

A Comparison of EMG Activity for the Middle and Lower Trapezius Muscle in the Frontal and Scapular Plane According to Shoulder Abduction Angles (어깨관절의 이마면과 어깨면에서 벌림각도에 따른 중간 등세모근과 아래 등세모근의 근 활성도 비교)

  • Purpose: The purpose of this study was to compare muscle activities in the frontal plane and scapular plane of the middle fiber and lower fiber of the trapezius muscle at different shoulder abduction angles. Methods: Twenty male and female students in their 20s participated in this study. Each subject maintained shoulder abduction at $75^{\circ}$, $90^{\circ}$, $125^{\circ}$, and $160^{\circ}$ in a standing position and repeated motions three times each in the frontal plane and the scapular plane. While maintaining the motions for 10 seconds in each posture, surface electromyography (EMG) was used to measure muscle activity of the middle fiber and lower fiber of the trapezius muscle. The collected EMG data were normalized using maximal voluntary isometric contraction (MVIC). Differences in muscle activity of the middle fiber and lower fiber of the trapezius muscles according to the angles at each plane were statistically processed using repeated measured analysis of variance, and an independent t-test was used to examine the differences between the two planes at each angle. Results: Muscle activity of the middle and lower trapezius during shoulder abduction in the frontal plane and scapular plane significantly increased as the angles increased (p<.05). However, muscle activity of the middle trapezius was significantly lower in the scapular plane than in the frontal plane for all shoulder abduction angles (p<.05). Conclusion: The results of this study suggest that during shoulder abduction, angles should be different according to the goals, and for training during an acute phase or early phase for functional recovery, it is more efficient to perform the training in the scapular plane than in the frontal plane.

A Kinematical Analysis of Belle Motion on Parallel Bars (평행봉 Belle 기술동작의 운동학적 분석)

  • This study is to define how the difference of athletic change influence on the last regrasp after somersault in Belle movement of parallel bars. For his study, the following conclusion was produced by analysis of athletic change by means of three dimensional visual image in three athlete of nation. 1. As the picture of S1, there are total used time(2.01 sec), S3(2.17 sec) and S2(2.19 sec). In case of a short needed time, it is difficult for them to perform the remaining movement of the vertical elevating flight easily and comfortably, it is judged as performing the small movement with restrict swing. 2 In the change of body center sped by each event, it is calculated as $-89.1^{\circ}$ the narrowest in S1, $-81.96^{\circ}$ the widest and then $86.34^{\circ}$ in S3. In E3 event, average compound speed is 4.07m/s, S2 showed the fastest speed of 4.14m/s whereas S1 the narrowest angle of 3.95m/s. 3. A shoulder joint and coxa are the period of mention in E3. In E4 which was pointed out the longest vertical distance, S2 that is indicated the highest vertical height as the period of detach in parallel bars. showed -3.91m. This is regarded as a preparatory movement for dynamic performance after using effectively elastic movement of shoulder joint and coxa while easily going up with turning back movement. In the 5th phrase, long airborne time and vertical change position is showed as the start while regrasping securely air flight movement from high position. 4. In E5, a long flight time and a long vertical displacement were shown as the regrasp after somersault efficiently in high position with stability from the point of the highest peak of the center of the body. Especially, S2 is marked as a little bit long position, while S1 is reversely indicated as performing somersault and unstable motion in a low position. 5. In E3, at the point of the largest extension of the shoulder joint and hip joint the shoulder joint is largely marked in $182^{\circ}$ and the hip point $182^{\circ}$ in S2. The shoulder joint is marked at the smallest angle in $177^{\circ}$ and the hip point $176^{\circ}$ in S1. And S1 is being judged by its performance of the less self - confident motion with lessening a breath of swing. S2 makes the most use of flexion and extension of the shoulder joint and the hip joint effectively. It was performed greatly with swinging and dropping the rotary movement and the rotary inertia naturally. 6. In E6, as the point of regrasp of the upper arm in parallel bars it is recognized by the that of components of vertical and horizontal velocity stably. During this study, the insufficient thing and the study on the parallel bars at a real game later are more activated than now. If it is really used as the basic materials by means of Belle Picked Study of Super E level after Bell movement, you may perceive the technique movement previously and perform without difficulty. Especially, such technique as crucifix is quite advantageous for oriental people thanks to small body shape condition. In conclusion we will nicely prepare for our suitable environment to gradually lessen trials and errors by analyzing and studying kinematically this movement.

The Effect of Human Appendage Muscle Strength on Increase in Vertical Dimension from Intercuspal Position of Mandible (교두감합위로부터 출발한 수직교합고경의 단계적 증가가 사지 근력에 미치는 영향에 관한 연구)

  • According to our recent investigation that the increase in the occlusal vertical dimension made the appendage muscle strength got higher, the occlusal appliances were made by increasing the occlusal vertical dimension "from the centric relation" position of the mandible. In this experiment, the authors tried to study the change in the appendage muscle strength due to increase in occlusal vertical dimension from intercuspal position(ICP) of mandible with the same subjects and manner as the former experiment. For this study, ten male athletes in a mean age of 23 year who were joined the former study were selected. All the subjects had a complete or almost complete set of natural teeth and reported no subjective symptoms of temporomandibular disorders. Upper and lower casts were mounted on the semi-adjustable articulator at the intercuspal position and a point was marked on the attached gingival area between the right canine and the right 1st. premolar in each upper and lower cast. From the points, the occlusal vertical dimension was increased by 2mm, 3.5mm and 5mm, and then each 10 maxillary type occlusal splint at each 3-increased position were fabricated with heat curing clear acrylic resin. Including the intercuspal position, the 3 kinds of occlusal splints were placed on the subjects individually, and then isokinetic muscle strength on 7 parts of the human appendage which are shoulder, knee, ankle, wrist, forearm, elbow and hip was measured with the CYBEX 6000 SYSTEM (Lumex, NewYork, USA). The results were as follows: The highest mean value in muscular strength was shown at the position of 2mm-increased vertical dimension. The muscle strength during internal/external rotation of shoulder and knee, plantarflexion of ankle, flexion of elbow, and flexion and extension of hip at the increased occlusal vertical dimension position were significantly higher than them at the intercuspal position (p<0.05). Only in view of the increase in the appendage muscle strength, regardless of the way of making the occlusal splints by elevating the occlusal vertical dimension from the centric relation position or intercuspal position, the occlusal splints had an effect on the increase of isokinetic muscle strength at the occlusal vertical dimension which increased within the proper range on the habitual arc of closure.

Comparison of the Maximal Muscular Activities of the Deltoid During Shoulder Abductions in Patients with Adhesive Capsulitis (유착성 관절낭염 환자의 어깨벌림 각도에 따른 중간어깨세모근의 최대근활성도 비교)

  • PURPOSE: This study aimed to compare the electromyography (EMG) activity for the middle deltoid (MD) and upper trapezius (UT) muscles in various shoulder abduction angles in patients with adhesive capsulitis (AC). METHODS: A total of 15 subjects participated in the study: 6 without AC (control group) and 9 with AC (AC group). The muscle activities of the UT and MD were measured using EMG during maximal static shoulder abductions in both groups. Each subject performed three repetitions of horizontal abduction at $0^{\circ}$, $30^{\circ}$, $60^{\circ}$, and $90^{\circ}$ of the shoulder abduction angle in a sitting position. The measurement data from the middle 3-seconds of the 5-second periods were used. The mean value of three separate sets of measurements was used in the data analysis. For each muscle, independent t-tests were performed to determine group differences. A measured repeated-measures ANOVA was performed using Bonferroni's post-hoc test. RESULTS: The muscle activity of the UT was significantly greater in the AC group; than in the control group (p<.05). The muscle activity of the MD was significantly lower in the AC group; than in the control group (p<.05). The greatest level of muscle activity for both the MD and UT was demonstrated at the $0-60^{\circ}$ and $0-90^{\circ}$ of shoulder abduction angles in the AC group. CONCLUSION: These findings showed that low muscle activation of the MD may contribute to hyperactivity of the UT during shoulder abduction in AC patients.

Effect of Motor Functions of Ipsilateral Upper Limb Induced by Long-Term Cane Usage in Chronic Stroke Patients (장기간 지팡이의 사용이 뇌졸중 환자의 건측 상지 기능에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the adverse effects of sensorimotor function at the shoulder joint according to long-term cane usage in stroke patients without apraxic behavior, in terms of the presence of shoulder joint pain, accuracy of tracking task, proprioceptive joint position sense, and nine-hole pegboard. Methods: Nineteen stroke patients with long-term cane usage (cane usage group) and nineteen stroke patients without cane usage (non-cane usage group) were recruited. All subjects were tested in pain presence, a tracking task for visuomotor function, joint reposition, and nine-hole pegboard in the shoulder joint regarding the non-affected side. Results: In the accuracy index for tracking task and the nine-hole pegboard test, significant differences were observed between the cane usage group and the non-cane usage group. However, although a higher emergence of shoulder pain and a lower accuracy for joint reposition sense were detected in the cane usage group in comparison to the non-cane usage group, there were no significant differences between the two groups. Conclusion: Our findings suggest that long-term cane usage could induce to decrease in delicate movement and coordination in the non-affected upper arm in stroke patients. In addition, they could experience high frequency of shoulder pain and poor joint reposition sense. Therefore, careful evaluation and observation will be required concerning stroke patients with long-term cane usage.
