Analysis of connecting joint anglle and moment in arm landing action in Sports Aerobics (스포츠에어로빅스 팔착지 동작의 연계관절 각도와 모멘트분석)

  • A relation between the movement range of arms and arising moment has been studied to find out efficient movement range to minimize impact concerning arm landing in sports aerobics. Four male athletes who won top three in national-level sports aerobics competition were chosen for the experiment. They were allowed to jump in between two force platform so that the right hand and the right leg could land onto the front and rear force platform, respectively. The sampling frequency was 200 Hz. The main conclusions based on the analysis of the angle and joint moment parameters of wrist, elbow, and shoulder are as follows: 1. The wrist moment was small when its angle was small, indicating that the dorsi-flexion of the wrist joint offered a positive influence to reduce wrist moment. 2. The elbow angle increased as wrist angle decreased and vice versa. This means that the movement range of the wrist joint affects that of the elbow joint. The darsi-flexion of the wrist is the position to absorb the impact of the elbow effectively rather than to absorb the impact of the wrist itself. The impact is absorbed by the flexion of wrist joint rather than the wrist. 3. The degree of moment transfer of the shoulder joint, having absorbed the impact from the elbow and elbow joint, became dependent on the efficiency of the fore-joints impact absorption.

Arthroscopic Anterior Capsular Release of a Post-traumatic Flexion Contracture in the Elbow (외상성 주관절 굴곡 구축에 시행한 관절경하 전방 피막 유리술)

  • We treated 5 elbows with post-traumatic flexion contractures (mean contracture: 51 degrees) by arthroscopic anterior capsular release and post-operative alternative flexion and extension splint. Each elbow had been resistant to at least six months of conservative therapy. An incongruent ulno-humeral articulation was considered to be a contraindication to this procedure. All patients complained of the residual deformity and some functional deficit. All patients, after failure of non-operative therapy, desired a operative treatment. At follow-up(mean 21.8 months), there was a mean post-operative contracture of 19 degrees, which is within a functional range of motion. So we conclude that arthroscopic capsular release in selected patients is reasonable alternative to open release.

Generation of 3-dimensional isocomfort workspace using the robot kinematics (로보트 기구학을 이용한 3차원 등편의 작업영역의 생성)

  • The purpose of this study is to obtain 3-dimensional isocomfort workspace using the robot kinematics, which is based on perceived discomfort in varying postures for manipulating four types of controls. Fifteen healthy male subjects participated in the experiment where their perceived discomfort in the given postures was measured, in which L32 orthogonal array was adopted. The shoulder flexion and adduction-abduction, elbow flexion, types of controls, and right/left hands were selected as experimental variables. The results showed that the shoulder flexion and adduction-abduction, elbow flexion, and types of controls significantly affected the perceived discomfort at .alpha. =0.01. Depending upon the types of control used, regression equations predicting perceived dis- comfort and three dimensional isocomfort workspace were suggested based on the experiemntal cata. Using the equations, driver's isocomfort workspace in his/her cabin for pushing operation was illustrated, in which the robot kinematics was employed to describe the translational relationships between the upper arm and the lower arm/hand. It was ecpected that isocomfort workspace could be used as a valuable guideline to design workplaces ergonomically.

  • For an effective acupuncture treatment, the location of muscles around Sutaeumkyongkun and Suyangmyongkyongkun were researched and they were made contracted. The conclusion is as follows; 1. The contraction of muscles around Sutaeumkyongkun gives appearance of the postures ; free movement of thumb, abduction of extension of wrist, flexion and pronation of elbow, depression and abduction of girdle of superior limb, flexion, internal rotation and horizontal flexion of shoulder joint These postures all together consequently produces the action 'holding something in arms'. 2. The contraction of muscles around Suyangmyongkyongkun gives appearance of the postures; extension of metacarpophalangeal and interphalangeal joint of index finger extension and abduction of thumb, extension of wrist, extension and supination of elbow, adduction, elevation and upward rotation of girdle of superior limb, extension, abduction, adduction, internal rotation, external rotation, horizontal extension of shoulder joint, flexion and opposite rotation of neck. These postures all together consequently produces the action 'raising arms'.

  • Background: Individuals with spinal cord injury (SCI) rely on their upper limbs for body-lifting activity (BLA). While studies have examined the electromyography (EMG) and kinematics of the shoulder joints during BLA, no studies have considered foot position during BLA. Objects: This study compared the effects of different foot positions during BLA on the shoulder muscle activities, peak plantar pressure, knee flexion angle, and rating perceived exertion in individuals with SCI. Methods: The study enrolled 13 mens with motor-complete paraplegic SCI, ASIA (American Spinal Injury Association) A or B. All subjects performed BLA with the feet positioned on the wheelchair footrest and on the floor independently. Surface EMG was used to collect data from the latissimus dorsi, pectoralis major, serratus anterior, and triceps brachii. The peak plantar pressure was measured using pedar-X and the knee flexion angle with Image J. Borg's rating perceived exertion scale was used to measure the physical activity intensity level. The paired t-test was used to compare the shoulder muscle activities, peak plantar pressure, knee flexion angle, and rating perceived exertion between the two feet positions during BLA. Results: The activity of the latissimus dorsi, pectoralis major, serratus anterior, and triceps brachii and rating perceived exertion decreased significantly and the peak plantar pressure and knee flexion angle increased significantly when performing BLA with the feet positioned on the wheelchair footrest compared with on the floor (p<.05). Conclusion: These findings suggest that individuals with SCI may perform BLA with the feet positioned on the wheelchair footrest for weight-relief lifting to decrease the shoulder muscle activities and the rating perceived exertion and to increase the peak plantar pressure and the knee flexion angle.

The Reliability and Validity of Korean Version of the Wheelchair User's Shoulder Pain Index in Wheelchair Users (휠체어 사용자를 위한 한국어판 WUSPI의 신뢰도와 타당도)

  • PURPOSE: The purpose of this study was to establish the reliability and validity of the Wheelchair User's Shoulder Pain Index (WUSPI), which was translated into Korean for long-term wheelchair users. This index measured 15 functional activities, including transfer, self-care, wheelchair mobility and general activities. METHODS: To assess test-retest reliability, 23 long-term wheelchair users completed this self-administered index twice within the same day. Reliability was determined by the intraclass correlation coefficient (ICC), and Cronbach's alpha was used to measure internal consistency. To examine concurrent validity, 21 long-term wheelchair users completed the questionnaire, and we examined the correlation between the index score and the shoulder range of motion measurements. RESULT: The results showed that the intraclass correlation for test-retest reliability of the total index score ranging from .88 to .99 was good to excellent. Additionally, Cronbach's alpha was .96. The internal consistency indicated excellent. Concurrent validity showed negative correlations of total index score to range of motion measurements of shoulder flexion (rho=-.58), extension (rho=-.09), abduction (rho=-.59), external rotation (rho=-.07) and internal rotation (rho=-.3), suggesting a relationship of total index score to loss of shoulder range of motion. CONCLUSION: The Korean WUSPI shows not only high reliability and internal consistency, but also concurrent validity with loss of shoulder flexion and abduction.

The Effect on grasp and pinch strength according to degree of elbow flexion in normal adult (성인의 주관절 굴곡 각도가 파악력과 핀치력에 미치는 영향)

  • Purpose : This study was investigated the effect on grasp and pinch strength according to degree of flexion in elbow joint. Methods : Thirty male and thirty female students, aged 20 to 31 years were tested for using opened positioning with their shoulder abduction at $55^{\circ}$ and shoulder horizontal adduction at $30^{\circ}$ in four elbow flexion($0^{\circ}$, $45^{\circ}$, $90^{\circ}$, $135^{\circ}$). An electronic dynamometer(E-LINK V900s Evaluation System)and goniometer were used to measure grasp and pinch strength. Results : The average grasp strength of the dominant hand was strongest at elbow $45^{\circ}$ however, weakest at elbow $135^{\circ}$ flexion in both of male and female. The average pinch strength of the dominant hand was strongest at elbow $135^{\circ}$ flexion in both of male and female. The average pinch strength of dominant hand was weakest at elbow $45^{\circ}$ flexion in both of male and female. According to degree of elbow flexion in both of male and female, grasp and pinch strength was no significant statistically. In analyzing correlations, the grasp strength of male showed the most significant difference at elbow $45^{\circ}$ & $90^{\circ}$ flexion, and the pinch strength was most significant difference at elbow $0^{\circ}$ & $45^{\circ}$ flexion. Conclusion : In analyzing correlations, the grasp strength of female showed the most significant difference at elbow $90^{\circ}$ & $135^{\circ}$ flexion, and the pinch strength was most significant difference at elbow $45^{\circ}$ & $90^{\circ}$ flexion.

  • $\cdot$ Arthroscopic management is the effective method with acceptable results for coronoid impingement of stiff elbow contributing to the functional improvement and pain relief. $\cdot$ The functional improvement and pain relief seem to be affected by the severity of a degenerative change of the elbow joint. $\cdot$ Excision of coronoid process is required in a marked limitation of further flexion in addition to deeping of the coronoid fossa and anterior capsular release. $\cdot$ Excision of olecranon tip or posterior capsular release are effective method in severe flexion contracture.

Evaluation of Varying Shoulder Postures with External Loads using a Psychophysical Method (외부 부하가 부과된 어깨 자세의 심물리학적 불편도 평가)

  • The purpose of this study is to quantitatively investigate perceived discomfort of complex shoulder postures with external loads and to propose a preliminary evaluation scheme of shoulder postures. Twelve healthy male adults participated in an experiment to rate their perceived discomfort of shoulder postures. The independent variables were shoulder flexion angle(45, 90 and 150$^{\circ}$), adduction/abduction angle(-30, -10, 0, 30 and 60$^{\circ}$), and external load(0, 1.5 and 3.0kg). The results revealed that the flexion angle, external load and their interaction significantly affected the perceived discomfort(p$<$0.05) but the effect of adduction/abduction angle on the discomfort was not significant(p$>$0.05). The effect of external load was much larger than that of any other factor with explaining about 81% of the total variation of discomfort scores. Based on the experimental results a preliminary scheme was presented to evaluate the stress of shoulder postures with external loads.

The Effect of Electro-acupuncture Stimulation of Tiaoko(S38)-Chengsan(B57) on Shoulder ROM with Frozen Shoulder Patient (조구(S38)-승산(B57)혈의 전침 자극이 동결견 환자의 관절가동범위에 미치는 영향)

  • Objectives : The purpose of this study was to identify the effect of electro-acupuncture stimulation regarding restoration of range of motion on patients with frozen shoulder. Methods : Experimental group measured with shoulder flexion and abduction angle at pre and post electro-acupuncture stimulation. Results : There were statistically significant difference regarding change of flexion angle of electro-acupuncture stimulation(p<0.01) and the testing value of all levels had a significant difference with the value of final post-test(p<0.01). There were statistically significant difference regarding change of abduction angle of electro-acupuncture stimulation(p<0.01). Conclusions : Electro-acupuncture stimulation of distal part increased range of motion of frozen shoulder. Therefore, application of electro-acupuncture should be developed as a therapeutic method in treatment of frozen shoulder. Also, it can contribute as one of good therapeutic methods offered to patients through approach of oriental rehabilitation medicine.
